Description
본 발명은 아미노산 및 히알루론산 나트륨에 기반한 멸균 분말 제형의 상처-치료용 약학적 조성물에 관한 것이다.
장기간 마비되거나 또는 누워있는 환자는 적절한 예방 조치의 부재시, 돌출된 뼈, 특히 천골(sacral), 좌골(ischial), 복사뼈(malleolar), 뒷꿈치(heel) 및 큰 돌기(trochanter) 부분을 덮고 있는 조직의 허혈성 괴사(ischaemic necrosis) 및 궤양화(ulceration)에 걸리기 쉽다.
욕창 및 만성 궤양성 상처는 대개 순환을 회복시켜 가능하게는, 괴사 조직의 기계적인 제거와 비누(부종 또는 탈수를 야기할 수 있음), 또는 친수성 중합체, 과산화수소 또는 알코올 천(alcohol rub)(피부 조직의 지방 제거가 피부를 건조시키고 갈라지게 하여 상처를 야기할 수 있음)을 이용한 세척과 함께, 가벼운 마사지로 치료되고 있다.
또한 심한 화상은 영향받은 부위의 절제 및 괴사 조직의 제거를 요구한다.
현재 일부 아미노산과 히알루론산 나트륨의 조합물이 결합 조직의 재구성 및 상피 세포의 결과적인 재생산을 보조하여, 신속한 상처-치료를 위한 기반을 형성하는 세포 복원 과정을 촉진하는데 특히 효과적인 것으로 알려져 있다.
따라서 본 발명은 활성 성분으로서,
a) 글리신 및 프롤린,
b) 히알루론산 나트륨, 및 가능하게는,
c) 라이신 및 루이신의 조합물을 포함하는 멸균 분말 제형의 상처-치료용 약학적 조성물에 관한 것이다.
더 구체적으로는, 본 발명에 따른 조성물은 글리신, L-프롤린 및 히알루론산 나트륨, 및 가능하게는 히드로클로라이드 형태의 L-라이신, 및 L-루이신을 포함한다.
본 발명에 따른 조성물은 만성 궤양성 상처, 심한 화상 및 욕창과 같이, 피부 물질의 손실을 포함하는, 봉합될 수 없고 진피를 심하게 손상시킨 상처의 치유를 촉진하는 것에 있어서 놀라운 보조(adjuvant) 효과를 입증했다.
본 발명의 조성물은 세균의 확산을 방지함과 동시에, 괴사 조직의 제거를 촉진하여, 상기 조직의 더 신속한 재생을 촉진하고, 피부 병변의 재-상피화를 보조하는 이상적인 습도 상태를 유지하게 한다.
본 발명에 따르면, 상기 분말 조성물은 프롤린만을 포함하는 하나, 및 나머지 활성 성분을 포함하는 다른 하나인, 두 개의 개별적인 캡슐 제형를 가질 것이다.
저습도 조건 하에서, 프롤린의 -NH2기가 아미노산의 분해에 의한 상기 분말의 색을 상당히 어둡게 하는 Maillard 반응을 일으키면서, 히알루론산의 히드록실기와 반응하기 때문에 안정성을 위해 분말 제형의 다른 활성 성분과 프롤린의 분리가 필요하다.

본 발명에 따른 조성물은 하기 중량 기준 백분율 범위 이내에서 다양한 활성 성분을 포함할 것이다.
- 글리신 : 0.5 내지 2%;
- L-프롤린 : 0.2 내지 1.5%;
- 히알루론산 나트륨 : 0.5 내지 3%;
및 가능하게는
- L-라이신 히드로클로라이드 : 0.05 내지 1%;
- L-루이신 : 0.05 내지 0.3%.
바람직한 양태에 따르면, 본 발명에 의한 조성물은 하기 중량 기준 백분율 범위로 다양한 활성 성분을 포함할 것이다.
- 글리신 : 1%;
- L-프롤린 : 0.75%;
- 히알루론산 나트륨 : 1.33%;
및 가능하게는
- L-라이신 히드로클로라이드 : 0.1%;
- L-루이신 : 0.15%.
본 발명에 따른 조성물은 국소용 투여를 위하여 멸균 분말 제형으로 적절하게 제제화될 수 있고, 그의 최종 용도에 부합하는 부형제, 가용화제(solubiliser), 유연제, 안정제, 유화제, pH 조절제, 및 방부제를 사용하여, Remington's Pharmaceutical Handbook, Mack Publishing Co., N.Y., USA에 기술된 방법과 같은, 약학 기술 분야에서 널리 알려진 통상적인 방법에 의해 제조될 수 있다.
약리학적인 실험
본 발명에 따른 조성물이 고령 환자, 당뇨병 환자 및 혈관 질환을 갖는 환자의 만성 염증을 치료하기 위한 효능을 평가했다.
구체적으로, 욕창을 앓고 있는 40명의 고령 환자, 하지까지 전개된 궤양을 갖는 36명의 타입 II 당뇨병 환자, 및 정맥혈전-후 궤양(post-phlebitic ulcer)을 갖는 32명의 환자를 평가했다.
병변의 심각성에 따라, 상처에 일주일에 3/4회, 상기 멸균 분말을 도포하는 치료를 적용했다.
욕창은 명백한 결과 없이, 4개월 이상 동안 통상적인 수단에 의해 이미 치료가 적용된 10cm2 이상의 탈-상피화된 부위를 가져야 했다. 붕대(bandage) 유형은 무관했다.
욕창을 상기 실험의 4주차 및 8주차에 치료 전에 임상적으로 평가하고 촬영했다. "치료(Healing)"는 상기 상처의 폐쇄(closing)로 정의했고, "개선(improvement)"는 최초 부위의 70%를 초과하는 치료 부위의 크기의 감소로 정의했다.
4주차까지, 18명의 환자가 개선, 즉 70%를 초과하는 욕창 크기의 감소를 보였고, 2명의 환자는 완치되었고, 관찰 기간의 종료까지(8주차), 24명의 환자가 치료되고, 10명은 개선되고 3명의 환자는 탈-상피화 면적의 50% 미만의 감소를 보였다. 2명의 환자는 동시발생한 병상으로 입원했고 1명은 중복감염으로 악화되었다.
본 연구 전 이미 4개월 이상 동안 치료를 받았으나 효과가 없었던, 다양한 면적 및 깊이의 궤양을 갖는 36명의 당뇨병 환자 중, 12명은 4주의 치료 후 치료되었고, 다른 30명은 치료 기간의 종료시까지 더 이상의 궤양화된 부위가 관찰되지 않았다. 특히 심한 4명의 경우 개선은 있었지만, 욕창이 8주차 치료의 종료시까지 여전히 관찰되었다.
이미 어떠한 결과도 없이 2개월 이상 통상적인 치료를 받아온 정맥혈전-후 궤양을 갖는 환자의 경우, 본 발명에 따른 멸균 분말 조성물의 투여로 18명의 환자는 1개월 이내에 치료되었고 다른 10명의 환자는 치료 종료시까지 치료되어, 치료받은 32명 중 총 28명이 치료되었다.
결론적으로, 욕창, 당뇨병 및 정맥혈전-후 피부 궤양의 경우에 있어서, 본 발명에 따른 멸균 분말 조성물은 통상적인 치료와 비교하여 80%를 초과하는 치료지수(healing index)(% 개선으로 표현)를 얻었다.
하기에서 본 발명에 따른 멸균 분말 제제의 실시예를 개시한다.
각 박스는 2개의 경질 젤라틴(hard gelatin) 캡슐(캡슐 A 및 캡슐 B)을 각각 포함하는 5개의 블리스터 팩(blister pack)을 포함했다.
각 캡슐은 멸균 분말 438 mg (캡슐 A) 및 150 mg (캡슐 B)을 포함한다.
국소 적용 전에, 하나의 캡슐 A 및 하나의 캡슐 B를 개봉하고, 그 안에 포함된 분말을 상처에 부었다.
캡슐 A 및 캡슐 B의 조성물은 하기와 같다.

Claims (4)
- 활성 성분으로서a) 글리신 및 프롤린,b) 히알루론산 나트륨, 및 가능하게는c) 라이신 및 루이신의 조합물(combination)을 포함하는 멸균 분말 제형의 국소용 약학적 조성물.
- 제1항에 있어서, 하기 중량 기준 백분율 범위 이내의 다양한 활성 성분을 포함하는 것인 조성물.- 글리신 : 0.5 내지 2%;- L-프롤린 : 0.2 내지 1.5%;- 히알루론산 나트륨 : 0.5 내지 3%;및 가능하게는- L-라이신 히드로클로라이드 : 0.05 내지 1%;- L-루이신 : 0.05 내지 0.3%.
- 제2항에 있어서, 하기 중량 기준 백분율로 다양한 활성 성분을 포함하는 것인 조성물.- 글리신 : 1%;- L-프롤린 : 0.75%;- 히알루론산 나트륨 : 1.33%;및 가능하게는- L-라이신 히드로클로라이드 : 0.1%;- L-루이신 0.15%.
- 만성 궤양성 상처, 심한 화상, 욕창, 수술 상처의 치료를 위한 멸균 분말 제형의 국소용 약물의 제조를 위한a) 글리신 및 프롤린,b) 히알루론산 나트륨, 및 가능하게는c) 라이신 및 루이신의 조합물의 용도.
