Abstract

본 발명은 견관절용 내부 보철에 관한 것으로, 이는 관절 헤드 및 보철 헤드(2)를 갖는 상완골 내에 고정된 핀 부분(1)으로 이루어져 있다. 이때 상기 보철 헤드(2)의 외측 표면에는 다수의 구멍(3)들이 제공되며, 상기 구멍 내로 결절 단편(tubercula fragments; 4)을 고정하는 핀(5)이 압입되어져 형태상 고정되도록 삽입되어진다.

Description

견관절용 내부 보철 {SCHULTERGELENK-ENDOPROTHESE}
본 발명은 견관절용 내부 보철에 관한 것으로, 이는 관절 헤드(articulation head) 및 상완골 내에 고정되며 보철 헤드를 갖는 핀 부분으로 이루어져 있다.
이와 같은 종류의 견관절용 내부 보철은 독일 특허 DE 43 10 200 C1에 개시되어 있으며, 그 목적은 인조 회선 건판(rotator cuff)을 갖는 완전 내부 보철을 만들어내는데 있다.
상완골 소두의 분쇄 골절(comminuted fracture of the capitellum) 시에, 결절에 의해 해제된 회선 건판의 힘줄은 상기 보철에 단단히 고정되어야 한다. 이때 상기 고정 상태는 지속적으로 유지되고 부하에 견딜 수 있어야 되며, 뿐만 아니라 자연스러운 동작을 재생시킬 수 있어야 한다.
본 발명의 목적은 적절한 견관골용 내부 보철을 제공하는 것으로, 이로 인해 이와 같은 고정 상태가 간단한 수단에 의해 달성될 수 있다.
상기 목적은, 보철 헤드의 외측 표면에 다수의 구멍들을 제공함으로써 달성되어진다. 여기서 상기 구멍 내로 결절 단편을 고정하는 핀이 압입되어져 형태상 고정되도록 삽입되어질 수 있다.
바람직한 실시 형태들은 종속항에 기술되어 있다.
상기 개구부 또는 구멍들은 보철 영역에 배열된다. 여기서 상기 보철 영역은 상완골 상부 영역 전체에 해당된다. 다수의 개구부 또는 구멍에 의해, 상기 결절 단편은 아주 정확한 방위 또는 위치를 가짐으로써 팔의 동작 능력이 완전 회복될 수 있다.
상기 결절 단편은 우선적으로 대응되는 보철 헤드의 부분 상에 놓여진다. 다음으로, 핀은 이를 통해 일정한 충격에 의해 그 아래쪽에 배치된 구멍 내로 삽입된다. 하부 또는 전방의 핀 영역 및 구멍들이 예를 들어 원뿔형으로 형성되어 있기 때문에, 상기 핀이 압입되어져 형태상 고정된다. 상기 돌출되어 있는 핀의 단부는 벤딩된다. 본 발명에 따른 해결 방안에 의해, 회선 건판이 보호되며 본래의 상태로 복구시킬 수 있다.
본 발명은 상기 도면에 의해 보다 상세히 설명된다.

상기 내부 보철은 전체적으로 도시되어 있지 않으며, 이유인 즉 본 발명을 설명하는데 있어서 이는 불필요하기 때문이다.
상기 보철의 핀 부분(1)은 보철 헤드(2) 및 실질적으로 상기 핀을 형성하는 핀 세그먼트(6)들로 이루어져 있다. 상기 핀 세그먼트는 환형으로 형성되어 있으며, 이때 그 단부만이 보다 양호하게 고정되기 위해서 원형에서 이탈하는 형태를 갖는다. 상호 대면하는 세그먼트의 환형 표면(ring surface)들은 상호 맞물리는, 즉 상호 보완적인 톱니 방식의 리드(lead)들을 구비한다.
상기 보철 헤드 및 핀 세그먼트는 종 방향 축과 동축 상으로 향하는 고정 나사(7)에 의해 형태상 고정됨으로써 움직이지 않게 된다. 한편, 상기 고정 나사를 해제한 다음 상기 부분들은 기준 치수에 따라 상호 비례적으로 이동시킴으로써, 상기 보철 헤드가 최적의 방위를 가질 수 있게 된다. 필요한 핀 길이에 따라 핀 세그먼트의 수가 결정된다.
도 2는 상기 핀(5)을 통해 보철 헤드의 외부 표면 상에 고정되어지는 상기 결절 단편(4)의 고정 방식을 도시하고 있다. 이때 상기 핀이 삽입된 다음 돌출되어 있는 단부는 벤딩 된다.
본 발명에 따른 견관절용 내부 보철에 의해, 상완골 소두의 분쇄 골절(comminuted fracture of the capitellum) 시, 결절에 의해 해제된 회선 건판의 힘줄은 상기 보철에 단단히 고정될 수 있으며, 상기 고정 상태를 지속적으로 유지시키며 자연스러운 동작을 재생시켜준다.

Claims (5)

  1. 관절 헤드 및 보철 헤드(2)를 가지며 상완골 내에 고정된 핀 부분으로 이루어진 견관절용 내부 보철에 있어서,
    상기 보철 헤드(2)의 외측 표면에 다수의 구멍(3)들이 제공되며, 상기 구멍 내로 결절 단편(4)을 고정하는 핀(5)이 압입되어져 형태상 고정되도록 삽입될 수 있는
    견관절용 내부 보철.
  2. 제1항에 있어서,
    상기 핀(5) 및 구멍(3)이 원뿔형 단부 영역을 구비하는 견관절용 내부 보철.
  3. 제1항에 있어서,
    상기 핀(5) 및 구멍(3)이 나사를 갖는 단부 영역을 구비하는 견관절용 내부 보철.
  4. 제1항 내지 제3항 중 어느 한 항에 있어서,
    상기 핀 부분(1)이 기본 치수로 제조되며, 개별적인 환형 핀 세그먼트(6)들로 이루어진 핀 부분(1)을 갖는 상기 보철 헤드(2)가 핀의 종축과 동일한 축을 갖는 고정 나사(7)를 통해 상호 결합되어 있는 견관절용 내부 보철.
  5. 제4항에 있어서,
    상호 대면하는 상기 핀 세그메트(6)의 링 표면들이 톱니 모양으로 형성되어 있음으로써, 상기 고정 나사(7)를 해제한 다음, 상기 핀 세그먼트는 기준 치수에 따라 상호 이동시킬 수 있는 반면, 고정 나사(7)가 조여질 경우에 형태상 고정되도록 결합되는 견관절용 내부 보철.
