Abstract
본 발명은 만성창상 치료용 양막 함유 창상피복재 조성물에 관한 것으로서, 가교처리된 양막, 글리세린, 히알루론산, CMC의 최적 적합비를 적용하여 제조된 상기 창상피복재가 당뇨병으로 유발된 만성창상을 신속하게 회복시키는 효능이 있음을 제시한다.
Description
본 발명은 만성창상 치료용 양막 함유 창상피복재 조성물에 관한 것이다.
양막은 태반의 가장 안쪽에 있는 얇고 반투명한 막으로, 단순 입방상피(simple cuboidal epithelium)와 두꺼운 기저막, 무혈관성의 간엽성 간질(mesenchymal stroma)로 구성되어 있으며 조직적합성항원이 표현되지 않아서 이식을 하여도 거부반응이 없는 생체막이다(Tseng SC et al., 1997; Houlihan JM et al., 1995). 양막 이식술은 1940년에 결막 결손부위의 치료에 처음으로 사용하여 그 효과를 보고하였지만(de Rotth A, 1940) 이후 주목 받지 못하다가 1995년 토끼 각막 손상 모델에서 양막을 이용한 성공적 안구 표면 재형성을 보고한 후(Kim JC et al., 1995), 지속적인 각결막 결손 및 각막 궤양(Prabhasawat P et al., 2001; Shin KS et al., 2003), 안구 화상(Meller D et al., 2000; Tamhane A et al., 2005), 공막 연화증(Jeoung JW et al., 2004), 수포각막병증(Pires RTF et al., 1999; Espana EM et al., 2003) 등 다양한 안과적 질환에서 양막 이식을 이용한 성공적인 치료들이 보고되고 있으며 실제 임상에서도 현재 활발히 사용되고 있다. 또한 안과적 질환 이외에 피부 화상(Bose B, 1979)이나 관절 질환(Vishwakarma GK et al., 1986), 이비인후과적 질환(Zahar Y et al., 1987)에서도 양막을 이용한 효과적 치료결과들이 보고된 바 있다. 양막에는 상피의 증식과 분화를 촉진시켜 창상 치유에 중요한 역할을 하는 염기성 섬유아세포 성장인자(bFGF), 상피세포 성장인자(EGF), 케라티노사이트 성장인자(KGF), 헤파토사이트 성장인자(HGF) 등의 성장 인자들이 존재한다고 보고되고 있다(Shinozaki M et al., 1995; Koizumi NJ et al., 2000). 특히, bFGF는 각막 상피세포의 상처 치유 촉진과 함께 각막의 기질세포와 내피세포에 대해서도 상처 치유 촉진, 항상성 유지에 관여하고(Hecquet C et al., 1990; Gospodarowicz D et al., 1977), 세포외 기질에서도 글라이코사미노글라이칸(GAG)과 헤파란 설페이트 등에 결합하여 존재하는 것으로 알려져 있는 성장인자이다(Vlodavsky I et al., 1987; Bashkin P et al., 1989; Vlodavsky I et al., 1991).
이에 본 발명자들은 인체 양막의 적용기술에 대한 연구를 하던 중, 양막의 제형화를 위해 이를 가교화시키고, 다양한 원료를 통해 창상 피복재로서 사용하기에 적합한 제품을 제조함으로써 본 발명을 완성하게 되었다.
본 발명의 목적은 만성창상 치료용 양막 함유 창상피복재 조성물을 제공하는 데에 있다.
본 발명은 만성창상 치료용 양막 함유 창상피복재 조성물에 관한 것이다.
상기 창상피복재는 가교처리된 양막, 글리세린, 히알루론산 및 카르복시메틸 셀룰로오즈를 포함하는 것일 수 있다. 바람직하게는 상기 창상피복재는 가교처리된 양막 100 중량부 기준으로, 글리세린 30~45 중량부, 히알루론산 14~23 중량부 및 카르복시메틸 셀룰로오즈 2~8 중량부를 포함할 수 있다.
상기 가교처리된 양막은 SDS(Sodium Dodecyl Sulfate), EDTA(ethylene-diamine-tetraacetic acid) 및 아프로티닌(Aprotinin)이 순차적으로 처리되어 양막의 상피세포가 제거되고 1,3 카보디이미드(carbodiimide)로 가교처리된 것일 수 있다.
상기 창상피복재 조성물은 메티실린 내성 스태피로코커스 아우레우스(methicillin resistance Staphylococcus aureus) 감염에 대한 저항성이 있는 것을 특징으로 한다.
상기 창상피복재의 제조방법은 다음과 같다.
가교처리된 양막, 글리세린, 히알루론산, 카르복시메틸 셀룰로오즈에 정제수를 혼합하여 혼합 조성물을 제조하는 단계; 겔 형태로 경화된 혼합 조성물을 3중 레이어 상태가 되도록 적층하는 단계; 3중 레이어 겔을 동결건조하는 단계;를 통해 제조될 수 있다. 상기 총 혼합 조성물은 100 중량% 기준, 가교처리된 양막 0.4~0.5 중량%, 글리세린 0.17~0.21 중량%, 히알루론산 0.07~0.09 중량%, 카르복시메틸 셀룰로오즈 0.01~0.04 중량%에 정제수를 혼합하여 제조가능하다. 상기 혼합 조성물은 20~35℃의 상온에서 경화되며, 겔의 두께는 필요에 따라 조절가능하다. 이 때 반드시 3중 레이어 겔이어야 할 필요는 없으나 적어도 3중 이상이 되어야 창상피복에 적절한 강도를 가질 수 있다.
상기 가교처리된 양막은 다음의 과정을 통해 제조가능하다.
(A단계) 태반으로부터 양막을 분리하는 단계;
(B단계) 양막을 멸균된 PBS(Phosphate-buffered saline)에 세척하고, PBS에 10~15시간 동안 양막을 침지한 후 정제수로 세척하는 단계;
(C단계) 세척한 양막을 SDS(Sodium Dodecyl Sulfate) 수용액에 처리하는 단계;
(D단계) SDS 처리된 양막을 EDTA(ethylene-diamine-tetraacetic acid) 수용액에 처리하는 단계;
(E단계) EDTA 처리된 양막을 아프로티닌(Aprotinin) 수용액에 반응하고, PBS로 세척하는 단계; 및
(F단계) 아프로티닌 처리후 세척한 양막을 동결건조하고, 동결건조한 양막을 1,3 카보디이미드(carbodiimide) 수용액에 반응하여 가교처리하고, Na2HPO4(pH 9.0~9.2)로 세척하는 단계;
를 통해 제조된다.
또한, 보다 바람직하게는 다음과 같이 수행할 수 있다.
(A단계) 태반으로부터 양막을 분리하는 단계;
(B단계) 양막을 멸균된 PBS(Phosphate-buffered saline)에 2~5회 세척하고, PBS에 10~15시간 동안 양막을 침지한 후 5~10회 정제수로 세척하는 단계;
(C단계) 세척한 양막을 30~90분 동안 0.4~0.6(v/w)% SDS(Sodium Dodecyl Sulfate) 수용액에 36~38℃에서 30~90분 동안 처리하는 단계;
(D단계) SDS 처리된 양막을 0.4~0.6(v/w)% EDTA(ethylene-diamine-tetraacetic acid) 수용액에 36~38℃에서 20~60분간 처리하는 단계;
(E단계) EDTA 처리된 양막을 0.4~0.6(v/w)% 아프로티닌(Aprotinin) 수용액에 10~50분간 반응하고, PBS로 1~3시간 동안 세척하는 단계; 및
(F단계) 아프로티닌 처리후 세척한 양막을 동결건조하고, 동결건조한 양막을 30~40mM 1,3 카보디이미드(carbodiimide) 수용액에 1~5분간 반응하여 가교처리 하고, 0.08~0.2M Na2HPO4(pH 9.0~9.2)로 1~3시간 동안 세척하는 단계.
를 통해 제조된다.
상기 A단계에서 양막을 트립틱 소이빈 카제인 다이제스트 배지에 5~10일간 35~38℃에 반응시켜 미생물 오염이 되지 않음을 확인하는 단계가 더 추가될 수 있다.
본 발명은 만성창상 치료용 양막 함유 창상피복재 조성물에 관한 것으로서, 가교처리된 양막, 글리세린, 히알루론산, CMC의 최적 적합비를 적용하여 제조된 상기 창상피복재가 당뇨병으로 유발된 만성창상을 신속하게 회복시키는 효능이 있음을 제시한다.
도 1a는 본 발명의 창상 피복재의 구조를 나타내는 사시도이며, 도 1b는 실시예 1의 조성대로 제조한 창상 피복재의 사진이다.

이하 본 발명의 바람직한 실시예를 상세히 설명하기로 한다. 그러나, 본 발명은 여기서 설명되는 실시예에 한정되지 않고 다른 형태로 구체화될 수도 있다. 오히려, 여기서 소개되는 내용이 철저하고 완전해지도록, 당업자에게 본 발명의 사상을 충분히 전달하기 위해 제공하는 것이다.
<실시예 1 내지 5. 양막 함유 창상피복재의 제조>
하기 표 1의 함량으로 창상피복재 원료를 혼합하고 동결건조하였다. 이 때 창상피복재는 같은 조성물을 3개의 층을 이루는 형상으로 성형하고 그대로 동결하여 3중 레이어 형태(100)가 되게 하였다. 가교처리 양막은 실험 후반부에 기재된 제조예 1의 양막을 이용하였다.

<비교예 1 내지 7. 양막 함유 창상피복재의 제조>
하기 표 3의 함량으로 창상피복재 원료를 혼합하고 동결건조하였다. 이 때 창상피복재는 같은 조성물을 3개의 층을 이루는 형상으로 성형하고 그대로 동결하여 도 1a의 3중 레이어 형태(100)가 되게 하였고 실제 제조 형태는 도 1b와 같다.
비가교처리 양막은 인체에서 얻은 상태로서 별도의 가공없이 태반에서 추출된 그대로의 것을 동결건조후 액체질소하에서 분말로 제조한 것을 사용하였다.

실험예
1. 만성 창상 동물모델 제작>
만성 창상 치료에 효과적인 인체 양막의 농도를 확인하기 위해 도 2와 같이 당뇨병성 만성 창상 동물모델을 제작하였다. 당뇨 동물모델을 제작하기 위해 8주 된 SD-rat (200-250g)에 sodium-citrate (0.1M, PH 4.5)로 희석된 Streptozotocin (60mg/kg)을 복강주사 하여 24시간 이후 쥐 꼬리 정맥에서 채혈하였다.
본 실험에서는 혈당치 300mg/dl 이상인 당뇨 동물을 만성 창상 모델로 사용하였다. 이 동물들은 평상시 (공복 전) 혈당은 약 100~150mg/dl, 몸무게는 300~350g을 유지하다가 공복 후 혈당 100mg/dl, 몸무게 300~350g으로 혈당만 감소하였으며, Streptozotocin을 투여한 다음 날 혈당이 400mg/dl 이상, 몸무게는 점차 감소하면서 도 3에서처럼 점차 실험에 사용하기에 적합한 당뇨병 동물이 되어갔다.
이 후, 도 4와 같이 당뇨가 유도된 동물모델에 1x1cm 크기로 창상을 유발하고, 창상이 지속적으로 유지되도록 감염균주로서 표준균주인 methicillin resistance Staphylococcus aureus (MRSA ATCC 43300)에 감염시켰다. 대조군으로는 바셀린 거즈, 처치군으로 1.5x1.5cm 크기로 제조예에서 제조한 인체 양막 제제를 부착하였다. 각 드레싱은 3일 간격으로 교체하였다.
도면에서 실험에 사용한 것들은 다음의 창상 피복재를 사용한 것이다.

<실험예 2. 실험동물의 창상 면적크기 관찰>
창상 유발 후 10일차 때 육안으로 창상 크기를 확인한 결과, 도 5와 같이 상처 면적이 시간에 따라 줄어드는 것이 확인되며 이를 도 6 및 표 5에 환산값으로 나타난 바와 같이 대조군과 비교하여 실시예군들이 가장 우수한 창상 치료 효과를 나타내는 것으로 확인된다.

실험예
3. 일반 창상 모델에서의 상처 치료 확인>
8주 된 SD-rat (200-250g)을 대조군과 처치군으로 나누어 도 7과 같이 창상을 내어 드레싱 처치를 하였다. 대조군에는 바셀린 거즈만, 처치군에는 실시예 1의 양막 3중 제형을 부착 후 바셀린 거즈를 부착하여 10일간 상처를 살폈다. 각 결과는 결과 도 8에 나타냈고 이를 환산하여 도 9와 표 6에 나타내었다.

확인 결과 도 9 및 표 6에서 일반 창상에서도 본 발명의 실시예 1의 양막 3중 제형의 창상 개선 효능이 매우 우수함을 확인할 수 있다.
<제조예 1. 양막의 가교 처리>
양막을 태반으로부터 분리하여 트립틱 소이빈 카제인 다이제스트 배지 (Tryptic soybean casein digest medium, Difco사)에 7일간 37℃간 두었다. 혈액과 혈흔을 씻어내기 위하여 멸균된 생리 식염수 500㎖이 함유된 병에서 진탕기 (shaker)를 이용하여 회당 10분 동안 세척하였다. 3회 세척 후에 얇은 양막 만을 얻기 위해 12시간 동안 침지시켰으며, 침지 후 4회 세척을 하였다. 마지막 8회 세척시에 증류수로 세척을 하였다.
이 후 양막을 0.5(w/v)% SDS(Sodium Dodecyl Sulfate) 수용액에 침지하여 37℃에서 30분간 처리하였다. 다음으로 상기 양막을 60분 동안 0.5(w/v)% EDTA(ethylene-diamine-tetraacetic acid) 수용액에 침지하였다. 다음으로 0.5(w/v)%의 아프로티닌(Aprotinin) 수용액에 양막을 60분 동안 침지하였다. 다시 PBS로 세척하여 양막 내 세포들을 제거하여, 양막을 -196℃ 초저온 냉동고에서 동결시킨 다음, 다시 36시간 이상 동결 건조시켰다. 동결 건조된 양막을 33mM 1,3 카보디이미드(carbodiimide)를 녹인 시약에 담궈 최종적으로 가교를 완료한 후, 0.1M Na2HPO4(pH 9.1)로 2시간 동안 세척하였다. 이 후 막자사발에 액체질소를 넣고 가교된 양막을 분쇄하여 드레싱제로 이용할 양막 분말을 제조하였다. 이렇게 가교처리된 양막은 탈세포화과정까지 마쳤기에 조직내 DNA가 20ng/mg으로서 거의 무세포 상태인 것으로 확인되었다.
<비교제조예 1 내지 7>
제조예 1의 조건에서 다음 과정을 제외하고 1,3 카보디이미드 가교만 수행하여 비교제조예들의 양막을 얻었다. 이 때 제외한 각 단계 대신 총 침지 처리 시간을 동일하게 하였다.

제조예 1의 과정에서 가교처리 방법의 순서를 다르게 하였다. 즉, 양막 분리 후, 아프로티닌, SDS 및 EDTA의 순서대로 진행하였다.
<비교제조예 9>
양막을 분리해 낸 후 SDS(Sodium Dodecyl Sulfate), EDTA(ethylene-diamine-tetraacetic acid) 및 아프로티닌 처리도, 1,3 카보디이미드 가교도 하지 않고, freeze-thaw cycle 방법을 수행하여 탈세포화만 하였다.
<비교제조예 10>
양막을 분리해 낸 후 SDS, EDTA 및 아프로티닌 처리 대신 같은 농도로 Triton X-100을 총 3회 처리하였다.
<비교제조예 11>
SDS(Sodium Dodecyl Sulfate), EDTA(ethylene-diamine-tetraacetic acid) 및 아프로티닌(Aprotinin)만 처리하고 1,3 카보디이미드 가교 처리는 수행하지 않았다.
<실험예 3. 가교처리된 양막의 만성창상 비교 효과 비교>
실험예 2의 방법대로 만성창상 비교실험을 실시하였을 때 최종 10일째의 창상의 치료 효과를 비교하였다. 이 때 드레싱제는 실시예 1의 방법에서 양막만 각각 제조예 1 대신 비교제조예 1~11로 대체하여 사용하였다.

Claims (5)
- 가교처리된 양막, 글리세린, 히알루론산 및 카르복시메틸 셀룰로오즈를 포함하는 양막 함유 창상피복재 조성물로서,
상기 가교처리된 양막은 SDS(Sodium Dodecyl Sulfate), EDTA(ethylene-diamine-tetraacetic acid) 및 아프로티닌(Aprotinin)이 순차적으로 처리되고 1,3 카보디이미드(carbodiimide)로 가교처리된 것을 특징으로 하는 양막 함유 창상피복재 조성물. - 삭제
- 제1항에 있어서,
상기 조성물은 메티실린 내성 스태피로코커스 아우레우스(methicillin resistance Staphylococcus aureus) 감염에 대한 저항성이 있는 것을 특징으로 하는 양막 함유 창상피복재 조성물. - (제1단계) 가교처리된 양막, 글리세린, 히알루론산, 카르복시메틸 셀룰로오즈에 정제수를 혼합하여 혼합 조성물을 제조하는 단계;
(제2단계) 겔 형태로 경화된 혼합 조성물로 레이어(layer)를 제조하여, 상기 레이어를 3개 이상 적층하는 단계; 및,
(제3단계) 적층된 레이어 겔을 동결건조하는 단계;
를 통해 제조되는 제1항의 양막 함유 창상피복재 조성물의 제조방법. - 제4항에 있어서,
상기 가교처리된 양막은, (A단계) 태반으로부터 양막을 분리하는 단계;
(B단계) 양막을 멸균된 PBS(Phosphate-buffered saline)에 세척하고, PBS에 10~15시간 동안 양막을 침지한 후 정제수로 세척하는 단계;
(C단계) 세척한 양막을 SDS(Sodium Dodecyl Sulfate) 수용액에 처리하는 단계;
(D단계) SDS 처리된 양막을 EDTA(ethylene-diamine-tetraacetic acid) 수용액에 처리하는 단계; 및
(E단계) EDTA 처리된 양막을 아프로티닌(Aprotinin) 수용액에 반응하고, PBS로 세척하는 단계; 및
(F단계) 아프로티닌 처리후 세척한 양막을 동결건조하고, 동결건조한 양막을 1,3 카보디이미드(carbodiimide) 수용액에 반응하여 가교처리하고, Na2HPO4(pH 9.0~9.2)로 세척하는 단계;
를 포함하여 제조되는 것을 특징으로 하는 양막 함유 창상피복재 조성물의 제조방법.
