Abstract
본 발명은 종래에 콜레스테롤 흡수 저해제로 알려진 에제티미브(Ezetimibe)를 유효성분으로 사용하여 인플라마좀 매개 염증성 질환을 예방 또는 치료용 약학 조성물에 관한 것이다.
Description
본 발명은 인플라마좀 매개 염증성 질환의 예방 또는 치료용 약학 조성물에 관한 것이다.
염증 반응은 손상이나 박테리아, 곰팡이, 바이러스 등 외부 물질에 의해 자극되어 각종 염증 매개 인자 및 면역 세포에 의한 효소의 활성화, 염증 매개 물질 분비, 체액 침윤, 세포의 이동, 조직 파괴 등 일련의 복합적인 생리적 반응이 일어나는 것을 말하며, 이로 인해 홍반, 부종, 발열, 통증 등과 같은 증상이 수반된다. 염증 반응은 외부 감염원을 제거하고 손상된 조직을 재생하여 생명체의 기능을 회복시키는 작용을 하지만, 항원이 제거되지 않거나, 내부 물질이 원인이 되는 등 염증 반응이 과도하거나 지속적으로 일어나면 오히려 점막 손상, 조직 파괴 등이 일어나고, 암, 염증성 피부 질환, 관절염 등을 초래하기도 한다.
IL-lβ는 병원균 감염에 의해 유도되는 사이토카인으로서 면역과 염증반응을 조절하여 숙주 방어에 관여한다. IL-lβ는 먼저 병원체 유래의 물질의 자극으로 전사수준의 발현 유도에 의해 비활성형 (pro-IL-lβ)으로 생산된 후 단백분해의 숙성 과정을 통해 활성형으로 전환되어야 세포 밖으로 분비되며, 이 두 과정이 엄격히 분리되어 조절됨으로써 IL-lβ가 과잉으로 분비되는 것이 통제된다.
인플라마좀은 골수성 세포(Myeloid cells)의 세포질에 존재하는 단백질 복합체로서, 선천적 면역 수용체(NOD-like receptor), 어뎁터 단백질(ASC) 및 카스파제-1(caspase-l)로 이루어져 있다. 인플라마좀은 세포의 감염이나 스트레스 등 선천성 면역 방어체계(innate immune defenses)와 관련된 염증성 사이토카인의 전구체인 pro-IL-lβ를 활성형인 IL-lβ(p17)로 전환시키는데 관여한다. 대표적으로 NLRPl, NLRP3, NLRC4, AIM2 단백질 등으로 구성된 인플라마좀이 있고, 인플라마좀-매개형 공정은 세균 감염 시 또는 대사 공정이나 점막 면역반응 조절 모두에서 중요하다. 예를 들어, NLRP3 유전자의 변이는 크리오피린 관련 주기적 증후군 (cryopyrin-associated periodic syndromes)에 해당하는 자가면역 질환들을 유발하는 것으로 밝혀졌다. 따라서, 잘못 조절된 인플라마좀 활성은 심각한 염증성 질환을 유발할 수 있어, 인플라마좀의 작용과 선천적 및 후천적 염증성 질환의 관련성이 면역반응 조절기작에서 중요하게 부각되고 있다.
현재까지는 상기 염증성 질환의 치료를 위하여 주로 항 히스타민제, 비타민 연고, 부신피질 호르몬제가 사용되어왔으나, 이러한 약물은 그 효과가 일시적인 것이 대부분이고, 부작용이 심하여 염증성 질환의 치료 효과가 있는 새로운 물질의 개발이 요구되고 있다. 이러한 기술의 일 예가 대한민국 공개특허공보 제10-2006-0034544호(2006.04.24 공개) 등에 개시되어 있다.
본 발명의 일 목적은 인플라마좀 매개 염증성 질환을 예방 또는 치료할 수 있는 약학 조성물을 제공하고자 한다.
그러나 본 발명이 이루고자 하는 기술적 과제는 이상에서 언급한 과제에 제한되지 않으며, 언급되지 않은 또 다른 과제들은 아래의 기재로부터 당업계에서 통상의 지식을 가진 자에게 명확하게 이해될 수 있을 것이다.
본 발명의 일 구현 예에 따르면, 에제티미브(Ezetimibe)를 유효성분으로 포함하는 인플라마좀 매개 염증성 질환의 예방 또는 치료용 약학 조성물에 관한 것이다.
본 발명의 다른 구현 예에 따르면 에제티미브를 유효성분으로 포함하는 인플라마좀 매개 염증성 질환의 예방 또는 개선용 식품 조성물에 관한 것이다.
본 발명에서 상기 인플라마좀은 NLRP3 인플라마좀인 것이 바람직하다. 본 발명에서 상기 에제티미브는 NLRP3 인플라마좀을 효과적으로 억제하여 NLRP3 매개 염증성 질환을 예방, 개선 또는 치료할 수 있다.
인플라마좀은 선천적 면역수용체(NOD-like receptor), 어템터 단백질(ASC) 및 caspase-1로 이루어진 단백질 복합체로서, 세포의 감염이나 스트레스 등 선천성 면역 방어처체계(innate immune defenses)와 관련된 염증성 사이토카인의 전구체인 pro-Interleukin-1β(pro-IL-1β)를 활성형인 IL-1β(p17)으로 전환시키는데 관여한다.
본 발명에서 상기 NLRP3 인플라마좀은 세균 감염 시 중요하며 대사 공정이나 점막 면역반응 조절 모두에 중요하다. 예를 들어, NLRP3 유전자의 변이에 의하여 크리오피린 관련 주기적 증후군(cryopyrin-associated periodic syndromes)에 해당하는 자가면역질환들을 유발하는 것으로 밝혀졌다. 또한 증가된 IL-1β는 인간의 선천적 또는 후천적 질병과 관계 있으며, IL-1β의 길항제나 수용체가 일부 질병의 성공적인 치료에 도움이 된다는 사실이 밝혀졌다. 따라서, 부적절한 인플라마좀의 작용과 선천적 및 후천적 염증성 질환의 관련성이 면역반응 조절기작에서 중요하게 부각되고 있다.
보다 상세하게, 상기 NLRP3 인플라마좀 단백질의 유전적 변이로 인하여 NLRP3 인플라마좀의 활성이 상향 조절되고 IL-1β의 과발현이 유도되면 머클-웰스 증후군(Muckle-Wells syndrome) 및 가족성 한랭 자가 염증 증후군(familial cold autoinflammatory syndrome)을 발생시키는 것으로 널리 알려져 있다(Rheumatology Advance Access published September 5, 2010; Nat Genet. 2001 Nov;29(3):301-5).
또한, 상기 NLRP3 인플라마좀의 과활성화는 아세트아미노펜 간독성(acetaminophen hepatotoxicity)을 유발할 수 있는 것으로 널리 알려져 있다. (Toxicol Appl Pharmacol. 2011 May 1;252(3):289-97; J Clin Invest. 2009 Feb;119(2):305-14.).
본 발명에서는 상기 에제티미브가 NLRP3 인플라마좀의 활성을 억제함으로써, 염증성 사이토카인 IL-1β의 분비 양을 효과적으로 조절할 수 있어 염증성 질환으로, 머클-웰스 증후군 및 가족성 한랭 자가 염증 증후군과 같은 크리오피린 관련 주기적 증후군이나 아세트아미노펜 간독성을 예방, 개선 또는 치료할 수 있다.
본 발명에서 상기 "에제티미브(Ezetimibe)"는 일반적으로 플라즈마 콜레스테롤 수준을 낮춰주는 약으로, 소장에서 콜레스테롤의 흡수를 억제하는 것으로 알려져 있다.
한편, 본 발명에서, "예방"은 본 발명의 약학 조성물을 이용하여 인플라마좀 매개 염증성 질환의 증상을 차단하거나, 그 증상의 억제 또는 지연시키는 모든 행위라면 제한없이 포함할 수 있다.
또한, 본 발명에서, "치료"는 본 발명의 약학 조성물을 이용하여 인플라마좀 매개 염증성 질환의 증상이 호전되거나 이롭게 되는 모든 행위라면 제한없이 포함할 수 있다.
또한, 본 발명에서, "개선"은 본 발명의 약학 조성물을 이용하여 인플라마좀 매개 염증성 질환의 증상이 호전 또는 이롭게 변경되는 모든 행위라면 제한없이 포함할 수 있다.
본 발명에 있어서, 상기 약학 조성물은 캡슐, 정제, 과립, 주사제, 연고제, 분말 또는 음료 형태임을 특징으로 할 수 있으며, 상기 약학 조성물은 인간을 대상으로 하는 것을 특징으로 할 수 있다.
본 발명의 약학 조성물은 이들로 한정되는 것은 아니지만, 각각 통상의 방법에 따라 산제, 과립제, 캡슐, 정제, 수성 현탁액 등의 경구형 제형, 외용제, 좌제 및 멸균 주사용액의 형태로 제형화하여 사용될 수 있다. 본 발명의 약학 조성물은 약제적으로 허용 가능한 담체를 포함할 수 있다. 약제학적으로 허용되는 담체는 경구 투여 시에는 결합제, 활탁제, 붕해제, 부형제, 가용화제, 분산제, 안정화제, 현탁화제, 색소, 향료 등을 사용할 수 있으며, 주사제의 경우에는 완충제, 보존제, 무통화제, 가용화제, 등장제, 안정화제 등을 혼합하여 사용할 수 있으며, 국소투여용의 경우에는 기제, 부형제, 윤활제, 보존제 등을 사용할 수 있다. 본 발명의 약제학적 조성물의 제형은 상술한 바와 같은 약제학적으로 허용되는 담체와 혼합하여 다양하게 제조될 수 있다. 예를 들어, 경구 투여시에는 정제, 트로키, 캡슐, 엘릭서(elixir), 서스펜션, 시럽, 웨이퍼 등의 형태로 제조할 수 있으며, 주사제의 경우에는 단위 투약 앰플 또는 다수회 투약 형태로 제조할 수 있다. 기타, 용액, 현탁액, 정제, 캡슐, 서방형 제제 등으로 제형할 수 있다.
한편, 제제화에 적합한 담체, 부형제 및 희석제의 예로는, 락토즈, 덱스트로즈, 수크로즈, 솔비톨, 만니톨, 자일리톨, 에리스리톨, 말디톨, 전분, 아카시아 고무, 알지네이트, 젤라틴, 칼슘 포스페이트, 칼슘 실리케이트, 셀룰로즈, 메틸 셀룰로즈, 미정질 셀룰로즈, 폴리비닐피롤리돈, 물, 메틸하이드록시벤조에이트, 프로필하이드록시벤조에이트, 탈크, 마그네슘 스테아레이트 또는 광물유 등이 사용될 수 있다. 또한, 충진제, 항응집제, 윤활제, 습윤제, 향료, 유화제, 방부제 등을 추가로 포함할 수 있다.
본 발명에 따른 약학 조성물의 투여 경로는 이들로 한정되는 것은 아니지만 구강, 정맥내, 근육내, 동맥내, 골수내, 경막내, 심장내, 경피, 피하, 복강내, 비강내, 장관, 국소, 설하 또는 직장이 포함된다. 경구 또는 비경구 투하가 바람직하다.
본 발명에서, "비경구"는 피하, 피내, 정맥내, 근육내, 관절내, 활액낭내, 흉골내, 경막내, 병소내 및 두개골내 주사 또는 주입기술을 포함한다. 본 발명의 약학 조성물은 또한 직장 투여를 위한 좌제의 형태로 투여될 수 있다.
본 발명의 약학 조성물은 사용된 특정 화합물의 활성, 연령, 체중, 일반적인 건강, 성별, 정식, 투여시간, 투여경로, 배출율, 약물 배합 및 예방 또는 치료될 특정 질환의 중증을 포함한 여러 요인에 따라 다양하게 변할 수 있고, 상기 약학 조성물의 투여량은 환자의 상태, 체중, 질병의 정도, 약무형태, 투여경로 및 기간에 따라 다르지만 당업자에 의해 적절하게 선택될 수 있고, 1일 0.0001 내지 50mg/kg 또는 0.001 내지 50mg/kg으로 투여할 수 있다. 투여는 하루에 한번 투여할 수도 있고, 수회 나누어 투여할 수도 있다. 상기 투여량은 어떠한 면으로든 본 발명의 범위를 한정하는 것은 아니다. 본 발명에 따른 의약 조성물은 환제, 당의정, 캡슐, 액제, 겔, 시럽, 슬러리, 현탁제로 제형될 수 있다.
또한, 본 발명의 식품 조성물은 각종 식품류, 예를 들어, 음료, 껌, 차, 비타민 복합제, 분말, 과립, 정제, 캡슐, 과자, 떡, 빵 등의 형태로 제조될 수 있다. 본 발명의 식품 조성물은 독성 및 부작용이 거의 없는 예방 목적으로 장기간 복용 시에도 안심하고 사용할 수 있다.
본 발명의 에제티미브가 식품 조성물에 포함될 때 그 양은 전체 중량의 0.1 내지 50%의 비율로 첨가할 수 있다.
여기서, 상기 식품 조성물이 음료 형태로 제조되는 경우 지시된 비율로 상기 식품 조성물을 함유하는 것 외에 특별한 제한점은 없으며 통상의 음료와 같이 여러가지 향미제 또는 천연 탄수화물 등을 추가 성분으로서 함유할 수 있다. 즉, 천연 탄수화물로서 포도당 등의 모노사카라이드, 과당 등의 디사카라이드, 슈크로스 등의 및 폴리사카라이드, 덱스트린, 시클로덱스트린 등과 같은 통상적인 당 및 자일리톨, 소르비톨, 에리트리톨 등의 당알콜 등을 포함할 수 있다. 상기 향미제로서는 천연 향미제(타우마틴, 스테비아 추출물(예를 들어 레바우디오시드 A, 글리시르히진등) 및 합성 향미제(사카린, 아스파르탐 등) 등을 들 수 있다.
그 외 본 발명의 식품 조성물은 여러 가지 영양제, 비타민, 광물(전해질), 합성 풍미제 및 천연 풍미제 등의 풍미제, 착색제, 펙트산 및 그의 염, 알긴산 및 그의 염, 유기산, 보호성 콜로이드 증점제, pH 조절제, 안정화제, 방부제, 글리세린, 알콜, 탄산 음료에 사용되는 탄산화제 등을 함유할 수 있다.
이러한 성분은 독립적으로 또는 조합하여 사용할 수 있다. 이러한 첨가제의 비율은 그렇게 중요하진 않지만 본 발명의 조성물 100 중량부 당 0.1 내지 약 50 중량부의 범위에서 선택되는 것이 일반적이다.
본 발명에서 제공하는 약학 조성물은 인플라마좀의 활성을 억제하여 인플라마좀 활성에 의한 각종 염증성을 효과적으로 예방, 개선 및 치료할 수 있다.
도 1의 (a) 및 (b)는 본 발명의 일 실시예에서 에제티미브 또는 바필로마이신의 존재 하에서 팔미트산염의 반응에 따른 THP-1 세포의 IL-1β 및 TNF-α의 유전자 발현량을 나타낸 것이다(그룹당 n=5~7).
도 2는 본 발명의 일 실시예에서 THP-1 세포를 에제티미브 또는 바필로마이신 존재 하에서 0.1ug/mL 리포폴리사카라이드(LPS) 및 팔미트산염과 함께 배양하였을 때 인플라마좀 관련 단백질의 발현 정도를 면역 블롯으로 분석한 결과를 나타낸 것이다(n=3~5).
도 3은 본 발명의 일 실시예에서 THP-1 세포에 에제티미브 또는 바필로마이신으로 선택적으로 전처리를 한 후 팔미트산염과 함께 배양한 후 IL-1β 수준을 ELISA 분석한 결과를 나타낸 것이다(그룹당 n=5~7).
도 4는 본 발명의 일 실시예에서 대조군, 팔미트산염, 또는 팔미트산염과 에제티미브로 처리한 HepG2 세포로부터 분리한 세포 외 소포체의 투과 전자 현미경 사진을 나타낸 것이다(그룹당 n=3~5).

이하, 본 발명을 하기의 실시예에 의해 상세히 설명한다. 단, 하기 실시예는 본 발명을 예시하는 것일 뿐, 본 발명의 내용이 하기 실시예에 의해 한정되는 것은 아니다.
실시예
항체 및 시료의 준비
본 실험에서는 하기의 상업적으로 구입 가능한 항체를 이용하였다: anti-pNF-κB 및 anti-NF-κB는 Cell Signaling Technology (Danvers,MA)에서 구입하였다. 또한, anti-NLRP3 및 anti-IL-1b는 Santa Cruz Biotechnology(Santa Cruz, CA)에서 구입하였다. 또한, anti-Flag는 Sigma (St. Louis, MO)에서 구입하였다. CD63에 대한 항체는 연세대학교 의료원, 환경의생물학교실, 열대의학연구소로부터 공급받았다.
본 실험에서는 하기 화학 물질들을 이용하였다: 팔미트산염(palmitate), 올레산염(oleate, OA), 리포폴리사카라이드(lipopolysaccharide, LPS), 바필로마이신 A1(bafilomycin A1) 및 클로로퀸(chloroquine, CQ)은 Sigma로부터 구입하였고, 에제티미브(Ezetimibe)는 Cayman Chemical (Ann Arbor, MI)에서 구입하였다.
마우스의 준비
10주된 수컷 C57BL/6J-배경 간 특이 ATGT 야생형 타입, 반가불충분성(haploinsufficient) 또는 동형(homozygous) KO 마우스에 대하여 메티오닌- 및 콜린-결핍 식이(MCD)를 수행하였다. 마우스는 식이 및 음용수에 자유로이 접근이 가능하도록 하고, 23±2℃의 온도 및 60±10%의 습도에서 12시간 주기로 낮과 밤 사이클을 변경해 주면서 유지하였다. 음식 섭취와 몸무게를 매주 측정하였다. 임의로 마우스를 구분하여 베히클(증류슈) 또는 에제티미브(10mg/kg)을 4주 동안 매일 경구투여 하였다. 마우스를 안락사시킨 뒤 심장 천자를 통해 혈액을 회수하고, 조직을 확보하였다. 자가 소화 작용의 실험을 위하여, 경쇄 3(LC3)-초록 형광 단백질(GFP) 이식 마우스에 에제티미브(10mg/kg)을 3일 동안 매일 처리하였고, GFP 점을 형광 현미경을 이용하여 관찰하였다.
1차 간세포(primary hepatocytes) 및 복막 대식세포(peritoneal macrophages)의 분리 및 세포 배양
8주된 수컷 C57BL/6J 야생형 마우스(Jackson Laboratory, Bar Harbor, ME)의 간에 콜라게나아제를 처리하여 간세포(hepatocytes)를 분리하였다. 간세포를 4X105 cells/ml의 양으로 6-웰 플레이트의 콜라겐 1형 코팅된 웰에 플레이팅한 뒤, 5% 소 태아 혈청, 1% 페니실린, 1% 스트렙토 마이신(Gibco), 100nM 덱사메타손 및 100nM 인슐린(Invitrogen, Carlsbad, CA)로 보충된 윌리암스 배지(Gibco, Grand Island, NY)에서 4시간 동안 배양하였다. 이후, 배지를 혈청-포함 또는 무혈청 배지(HepatoZYME-SFM, Invitrogen)로 교체하여 추가로 배양하였다. 인산 완충액을 이용하여 복막 관류(peritoneal lavage)로부터 복막 대식세포(peritoneal macrophages)를 통해 확보한 뒤 원심 분리하고, RPMI 1640 배지(Gibco)를 포함하는 6-웰플레이트에 웰당 106 마이크로파지로 플레이팅하였다.
인간 간세포(HepG2)를 둘베코 변형 이글스 배지(Dulbecco's modified Eagle's medium, Gibco)에서 배양하였고, 인간 백혈구 세포(THP-1)는 RPMI 1640 배지(Gibco)에서 배양하였다. 상기 세포들은 10% 소 태아 혈청, 1% 페니실린 및 1% 스트렙토 마이신으로 보충된 배지에서 37℃, 5% CO2 인큐베이터에서 배양하였다.
세포 외 소포체(extracellular vesicles)의 분리
총 엑소좀 분리 키트(Total Exosome Purification Kit)를 이용하여 HepG2 세포로부터 세포 외 소포체를 분리하였다. 세포 잔해를 제거하기 위하여 배양 배지를 2,000 xg로 20분 동안 원심 분리하였다. 상층액을 새로운 마이크로퓨즈 튜브(microfuge tube)로 옮긴 뒤, Invitrogen 시약 0.5 볼륨을 첨가한 후 혼합을 위하여 볼텍싱하였다. 이후, 시료를 4℃에서 밤새 배양하고 4℃에서 10,000xg로 1시간 동안 원심 분리하였다. 상층액을 제거한 뒤, 펠렛을 100 μL PBS에 재부유시켰다.
면역 블롯 분석(Immunoblot analyses)
1% 프로테아제 억제제 칵테일(Pierce Biotechnology, Rockford, IL)을 포함하는 RIPA 버퍼(20 mM Tris-HCl, 150 mM NaCl, 1% NP-40, 1% sodium deoxycholate, 2.5 mM sodium pyrophosphate)를 이용하여 1차 간세포 및 HepG2 세포를 용해하여 총 세포 용해물을 얻은 뒤, 브래드포드 어쎄이(Bio-Rad, Hercules, CA)를 이용하여 그에 포함된 단백질 함량을 측정하였다. 각 단백질 추출물의 등가량을 5x 샘플 버퍼(2% sodium dodecyl sulfate, 62.5 mM Tris (pH 6.8), 0.01% bromophenol blue, 1.43 mM mercaptoethanol, and 0.1% glycerol)에서 열-변형 시킨 뒤 10% 폴리아크릴아마이드 겔에서 분리시키고 폴리비닐리덴 플로오라이드 막(Bio-Rad)에서 전기 영동적으로 이송시켰다. 블러킹 후 막을 적절한 항체로 처리하였다. 화학 발광제(SuperSignal West Pico Luminol/Enhancer solution, Thermo Scientific, Rockford, IL) 및 Agfa 의학 X-ray 필름(Mortsel, Belgium)을 이용하여 면역 염색을 수행하였다. β-액틴 단백질 수준을 로딩 대조군으로 사용하였다.
인터루킨-1β(IL-1β)에 대한 ELISA 및 생화학적 분석
ELISA 키트(eBioscience, San Diego, CA)를 사용하여 THP-1 및 1차 복막 마크로파아지 배양된 세포 상층액 및 혈청에서 IL-1β의 양을 측정하였다. 상층액 부분과 기준으로서 재조합 인간/마우스 IL-1β를 인간/마우스 IL-1β 포획 항체와 혼합한 뒤 96-웰 ELISA 플레이트에 코팅하였다. 4℃에서 12시간 동안 배양시킨 뒤 플레이트를 세척 버퍼(0.05% Tween 20 in PBS; pH 7.4)로 3번 세척하고, 인간/마우스 IL-1β 감지 항체 용액을 첨가하였다. 상온에서 2시간 동안 배양한 후, 플레에트를 세척 버퍼로 5번 세척한 뒤 각 웰에 스트렙타비딘 HRP를 첨가하였다. 상온에서 30분 배양하고 HRP와 반응을 위한 기질 용액과 450nm로 캘리브레이트된 미량정량판 리더(Molecular Devices, Sunnyvale, CA)를 사용하여 암흑에서 플레이트를 시각화하였다.
RNA 분리 및 RT-PCR 분석
TRIzol 시약(Invitrogen)을 사용하여 세포로부터 총 RNA를 분리한 뒤 High Capacity cDNA Reverse Transcription kit (Applied Biosystems, Foster City, CA)를 이용하여 총 RNA 2μg을 cDNA로 역전사시켰다. Power SYBR(R) Green PCR Master Mix (Applied Biosystems)를 이용하여 ABI 7500 시퀀스 감지 시스템(Applied Biosystems)에서 cDNA를 증폭시켰다(40 cycles of 95℃ for 5 s, 58℃ for 10 s, and 72℃ for 20 s). 타겟 유전자 발현은 글리세라이드 3-포스페이트 디하이드로제네이즈(Gapdh) 또는 β-액틴으로 정규화하였다. 정량적 분석은 컴cycle 역치 방법 및 StepOne Software version 2.2.2을 이용하여 수행하였다. RT-PCR에 사용된 프라이머 세트는 하기 표 1과 같다. 모든 반응들은 최소 3회 이상 반복하여 수행하였다.

통계적 분석
데이터는 적어도 3회 독립된 실험들에서 평균±평균의 표준 오차로 나타내었다. 데이터는, 두 그룹의 비교를 위해서는 양측 꼬리 스튜던트 테스트로 분석하였고, 2 그룹을 초과하는 다그룹 비교를 위해서는 post-hoc Bonferroni 다중 분석 테스트를 이용한 일원 변량 분석(ANOVA)로 분석하였다. 통계적 분석은 PRISM 5.0 software (GraphPad Software, La Jolla, CA)를 이용하여 수행되었으며, 0.05 미만의 P 값(P values < 0.05)을 통계적으로 유의한 것으로 해석하였다.
에제티미브의 염증 개선 효과 및 세포 외 세포체를 통한 세포-세포 상호작용
면역 반응에서 중요한 역할을 하는 대식 세포에서 에제티미브의 역할을 확인한 결과, THP-1 세포에 에제티미브를 처리한 경우 NF-κB-의존성 사이토카인인 IL-1β 및 TNF-α의 mRNA 발현량이 현저히 감소한 것을 볼 수 있었다(도 1(a) 및 도 1(b)). 하지만, 바필로마이신을 함께 처리한 경우 이러한 억제 효과가 차단되는 것을 볼 수 있었다. 바필로마이신을 에제티미브와 함께 처리하는 경우 대식세포에서 NLRP3 인플라마좀-IL-1β 경로에 의해 중재되는 염증 반응에 대한 에제티미브의 보호 효과를 약화시키는 것을 알 수 있었다(도 2). 또한, 에제티미브를 처리한 경우 대식세포에서 방출되는 IL-1β의 양이 감소하지만, 바필로마이신을 함께 처리하는 경우 다시 증가하는 것을 볼 수 있었다(도 3).
최근 염증성 세포 외 소포체가 NASH에서 간세포 및 대식세포 사이에 세포간 조절자로 작용하는 것이 밝혀진 바 있었다. 에제티미브가 간세포-유래 세포외 소포체를 통하여 인플라마좀 경로에 관련되는지 확인하기 위하여, HepG2 세포로부터 세포 외 소포체를 분리하여 전자 현미경을 통해 관찰하였고(도 4), 면역 블롯을 이용하여 CD63 단백질의 발현 및 엑소좀 마커(exosomal marker)를 확인하였다(도 5). 대식세포에서 IL-1β mRNA 및 단백질의 발현량을 확인한 결과, 에제티미브 처리된 세포 외 소포체에서 그 발현량이 현저히 감소하는 것을 확인할 수 있었다(도 6(a) 및 도 6(b)).
또한, ATG7 야생형 마우스에 에제티미브를 처리하자 염증성 마커인 TNF-α, NLRP3, 및 IL-1β의 유전자 발현량이 현저하게 감소한 것을 확인할 수 있었다(도 7(a), 도 7(b) 및 도 7(c)). 또한, 에제티미브를 처리한 경우 pNF-κB, NLRP3, 및 성숙한 IL-1β 단백질 발현량 역시 현저히 감소하였으며(도 8), 혈청 내 IL-1β 수준 또한 감소하였다(도 9).
이를 통하여, 본 발명과 같이 에제티미브를 처리하는 경우 NLRP3 인플라마좀 활성을 억제하여 IL-1β 및 TNF-α 발현량을 감소시킴에 따라 염증 반응을 개선시키는 것을 알 수 있다.
