Description
제1도는 피검자 1에서 재조합 IGF I을 체중 1kg 및 1시간당 32.0㎍의 속도로 연속 피하 주입한 후 주입 첫날의 혈당 및 유리 IGF I의 혈청내 농도의 비교도이다.
제2도는 두 피검자에서 체중 1kg 및 1시간당 20.0㎍ 용량의 재조합 IGF I을 일정하게 피하 주입하기 6일 전, 주입한 6일 동안 및 주입한지 6일 후의 총 IGF I의 혈청내 농도의 비교도이다.

본 발명은 인슐린 유사성 성장인자 I(insulin-like growth factor I; IGF I)을 투여함으로써 부작용(secondary effect)인 과인슐린증(hyperinsulinemia)을 치료 및 예방하는 방법, 및 이러한 목적에 유용한 IGF I을 함유하는 약제학적 조성물에 관한 것이다.
인슐린은 a) 간으로부터의 글루코스 유출을 감소시키고, b) 말초 조직, 예를 들어, 근육 및 지방 조직의 글루코스 흡수를 증가시킴으로써 혈당을 조절한다.
인슐린은 대부분의 세포에 존재하는 인슐린 수용체와 상호작용함으로써 효과를 발휘한다. 인슐린에 대한 민감도는 각 세포에 존재하는 인슐린 수용체의 수에 좌우된다. 이들 수용체의 수는 인슐린에 의하여 감소되거나 하향조절(downregulated)되는데, 즉 고농도의 인슐린은 2차적으로 상대적인 인슐린 내성을 초래한다. 내인성 인슐린 과분비를 특징으로 하는 증상은 비만증, 타입(type) 2당뇨병, 프레드릭센(Fredricksen)의 과지질혈증 타입 IV이다. 타입 1 당뇨병(유년기 진성 당뇨병, 인슐린 의존성)에서, 인슐린의 말초 투여 결과, 인슐린 내성이 유도됨으로써 간의 글루코스 항상성 기능이 손상되고 말초성 글루코스가 과도하게 흡수된다. 비만증, 타입 2 당뇨병(인슐린 비의존성) 및 과지질혈증은 주로 식이요법, 즉 칼로리를 제한함으로써 치료된다. 이러한 치료법에 대한 환자의 순응성은 매우 불량하므로, 더 우수한 신규의 치료법이 요구된다. 인슐린으로 치료되는 타입 1 당뇨병에서, 인슐린이 문맥내 보다는 피하로 운반됨으로써, 투여된 인슐린이 간을 통해 통과한 후 말초 조직에 도달하기 보다는, 먼저 말초 조직에 도달함으로써 과인슐린증이 유발된다. 인슐린과분비 및 과인슐린증으로 인한 장애를 극복하는 것이 요구된다.
인슐린 유사성 성장 인자 I(IGF I)은 사람에서 정맥내 거환(bolus) 주사된 후 혈당을 감소시키는 것으로 밝혀졌다(참조 문헌 1). IGF I의 성장 촉진 작용은 통상적으로 IGF I의 농도가 낮은 수개의 대사 조건, 예를 들어, 뇌하수체 절개된 래트(참조 문헌 2 및 5), 당뇨병에 걸린 래트(참조 문헌 3) 및 스넬 왜소마우스(Snell dwarf mice)(참조 문헌 4)에서 증명된다.

IGF I을 주입하는 동안 트리글리세라이드, 콜레스테롤 및 LDL-콜레스테롤은 감소되는 반면에 HDL-콜레스테롤은 증가하는 경향이 있다. 이러한 관찰 결과는 IGF I이 주입되는 동안 인슐린 분비가 감소하며, IGF I이 상당히 소량의 인슐린으로 글루코스 항상성을 유지하는데 도움을 줌으로써 과인슐린증으로 인한 장애가 극복됨을 나타낸다. IGF I의 작용 기작에는 2가지 이상이 존재한다 : 첫번째로, 거환 주사 시험에서 나타난 바와 같이, IGF I은 주로 근육의 글루코스 흡수를 증가시킴으로써 저혈당증을 초래하므로 IGF I을 주입함으로써 근육에 의한 글루코스 흡수를 또한 촉진시킬 수 있을 것이며 이러한 효과는 용량과 관계되는 것으로 추측된다. 두번째로는, 아직까지 알려지지 않은 어떤 효과가 인슐린 분해와 관계가 있다는 것이다.
현재까지는 IGF I 투여 동안 인슐린 분비 억제에 대하여 보고된 바가 없다. IGF I의 투여에 의하여 인슐린의 필요량이 감소됨으로써 부작용인 과인슐린증이 예방되는 것으로 예상된다.
본 발명의 목적은 당뇨병 환자에 투여된 인슐린의 부작용을 방지하고 비만증, 타입 2의 당뇨병 및 과지질혈증에서 유발되는 과량의 내인성 인슐린의 효과를 최소화하는 것이다.
본 발명의 다른 목적은 용량 단위 형태로 및 전술한 유익한 효과를 달성하기 위한 양으로 IGF I을 함유하는 약제학적 조성물을 제공하는 것이다.
본 발명은 유효량의 IGF I 및 정상량 미만의 인슐린을 투여함을 특징으로 하여, 인슐린으로 치료받는 당뇨병 환자에서 유발되는 부작용인 과인슐린증을 치료 및 예방하는 방법에 관한 것이다.
본 발명은 또한 유효량의 IGF I을 단독으로 투여함을 특징으로 하여, 타입 2 당뇨병 환자, 비만증 환자 및 과지질혈증 환자를 치료하는 방법에 관한 것이다.
IGF I의 공급원으로서 자연의 공급원 또는 합성적으로 생산된 공급원이 사용될 수 있다. 예를 들어, 유립 특허 제123 228호에 따라 제조된 재조합 사람 IGF I(rhIGF I)이 바람직하다.
돼지 또는 소의 인슐린, 또는 바람직하게는 사람 재조합 인슐린과 같은 어떠한 유형의 인슐린이라도 사용될 수 있다.
유효량은 계획된 치료 조건에 대하여 치료 효과를 갖는 양으로 정의한다.
IGF I은 약 24 내지 약 720㎍/kg/일의 용량으로 정맥내, 피하 또는 근육내 투여될 수 있거나, 약 1 내지 약 30㎍/kg/h의 용량으로 연속 투여하는 경우, 각각 미니 펌프를 통하여 2일마다 주사하거나 피하 주입한다. 6일 동안 1일 용량 500㎍/kg에 대등한 총 약 3000㎍/kg이 투여될 수 있다.
IGF I을 인슐린과 함께 투여하는 경우, 인슐린은 혈당에 거의 영향을 미치지 않는 양 미만인, 단독으로 사용될 경우보다 더 적은 용량, 즉 약 0.1 내지 약 0.5IU/kg/24h의 양으로 사용된다.
용량은 물론 환자의 특정 질환, 투여 경로, 치료되는 환자의 개인별 체중 및 일반적인 증상에 따라 조절되어야 하며 최종적으로는 의사의 판단에 따라야 한다. 혈당을 검사하고 저혈당증이 되지 않도록 주의해야 한다.
당뇨병 환자에서 유도되는 부작용을 예방하기 위한 약제학적 조성물은 유효량, 즉 약 20 내지 약 300mg의 양의 IGF I을 함유한다.
IGF I과 인슐린과의 배합물의 상승작용 효과에 기인하여, 인슐린은 배양물 중에 0.2 내지 10.0mg의 양으로 존재한다.
일반적으로, 약제학적 제제는 유효량의 활성성분(들)을 바람직하게는 비경구 투여에 적합한 고체 또는 액체인 약제학적으로 허용되는 무기 또는 유기 담체와 함께 함유하거나 이들과의 혼합물로서 함유한다.
본 발명의 활성 화합물은, 단독으로 또는 인슐린과의 배합물 형태로, 바람직하게는 비경구 투여, 예를 들어 피하, 근육내 또는 정맥내 투여용 주입 용제와 같은 약제학적 제제 형태로 사용된다. 이러한 용제는 바람직하게는 사용 전에, 예를 들어, 활성성분을 단독으로 함유하거나 약제학적으로 허용되는 담체와 함께 함유하는 동결건조된 제제로부터 제조될 수 있는 등장성 수성 용액 또는 현탁액이다. 약제학적 제제는 멸균될수 있고/있거나 보조제, 예를 들어, 방부제, 안정화제, 습윤제 및/또는 유화제, 가용화제, 삼투압 조절용 염 및/또는 완충제를 함유한다. 본 발명의 약제학적 제제는 필요한 경우, 추가로 약리학적으로 가치 있는 물질을 함유할 수 있고, 공지된 방법, 예를 들어, 통상적인 용해 과정 또는 동결 건조 과정을 통하여 제조되며, 활성성분(들)을 약 0.1 내지 100%, 특히 1 내지 약 50%, 및 동결 건조제의 경우 100% 이하로 함유한다.

본 발명은 추가로 인슐린으로 치료받는 당뇨병 환자에서 유발되는 부작용인 과인슐린증을 예방하기 위한 약제학적 배합물의 용도에 관한 것이다.
본 발명은 또한 IGF I을 통상적인 방법에 따라 약제학적으로 가공함을 특징으로 하여, 타입 2 당뇨병 환자, 비만증 환자 또는 과지질혈증 환자를 치료하기 위한 약제학적 제제를 제조하는 방법에 관한 것이다.
본 발명은 사용 지시서를 포함할 수 있고, 타입 2 당뇨병 환자, 비만증 환자 및 과지질혈증 환자에서 유발되는 부작용을 예방하기 위한 약제학적 제제를 제조하기 위한 단독 IGF I의 용도, 및 IGF I과 인슐린을 함유하는 상승 작용성 배합물을 제조하기 위한 IGF I의 용도에 관한 것이다.
본 발명은 또한 IGF I 또는 IGF I과 인슐린과의 배합물을 함유하며, 사용 지시서를 포함할 수 있는 약제학적 제제 또는 팩(pack)에 관한 것이다.
하기는 본 발명에 따르는 약제학적 제제의 실시예이지만, 이에 제한되는 것은 아니다. 실시예에서 용어 IGF I은 달리 언급되지 않는한, 재조합 사람 IGF I(rhIGF I)이며, 이는 유럽 특허 제123 228호에 따라 제조된다. IGF I은 화학적으로 및 생물학적으로 특성화되었고 고도로 정제되고 추출된 사람 IGF I과 동일한 것으로 판명되었다. 사람에 대한 선행의 연구(참조 문헌 1)에서와 동일한 물질이 사용되었다.
1. IGF I을 단독으로 함유하는 약제학적 제제에 대한 실시예
50mg 또는 300mg의 IGF I을 함유하는 건조 앰플 :
각각 5ml 또는 50ml 용량의 앰플을 각각 IGF I의 멸균 여과된 10%(w/v) 수용액 5ml 또는 30ml로 충전시키고 동결건조시킨다. 각각의 용적(5ml 또는 30ml)의 멸균수, 생리식염수 또는 0.1M 아세트산을 가하여 주입 용액을 제조한다.
치료학적 배합물은 한번의 치료 기간, 예를 들어, 6일 동안 필요한 목적하는 수의 앰플, 및 약제가 주입되어야 하는 시간을 규정하는 사용에 대한 임의의 지시서를 포함한다.
2. IGF I 및 인슐린을 함유하는 약제학적 제제에 대한 실시예
50mg 또는 300mg의 IGF I을 함유하는 건조 앰플 :
5ml 또는 50ml 용량의 앰플 각각에 IGF I의 멸균 여과된 10%(w/v) 수용액 5ml 또는 30ml로 각각 충전시킨다. 인슐린, 바람직하게는 사람 재조합 인슐린을 0.2mg 또는 10mg의 양으로 가하고, 바이알을 동결건조시킨다. 각각의 용적(5ml 또는 30ml)의 멸균수, 생리 식염수 또는 0.1M 아세트산을 가하여 주입 용액을 제조한다.
치료학적 배합물은 치료 기간, 예를 들어, 6일 동안 필요한 목적하는 수의 앰플을 함유하며 약제가 주입되어야 하는 시간을 규정하는 사용상의 임의 지시사항을 함유한다.
1. 사람의 치료에 대한 실시예
피검자
당해 임상 시험을 위한 정상 피검자로서 두명의 남자(연령/체중/신장; 1. : 38/65/172; 2. : 34/61/172)를 대상으로 한다. 이들의 체중은 이상적이고, 질환의 임상적 징후를 나타내지 않으며 어떠한 약제도 복용하지 않았다. 통상적인 혈액병학, 혈액화학 및 내분비 매개변수는 정상 범위내이다.
실험 계획안
초기 대조 시험 기간 동안 IGF I을 6일 동안 연속적으로 피하 주사로 투여한 후, 기준값을 수득한다. 당해 방법 및 투여 지속 기간은 혈청중의 IGF I이 일정한 농도에 도달하도록 하기 위하여 선택한다. 연구 결과는 2차 대조 기간에 의해 결론지워진다. 시험 전 기간 동안 음식물 섭취는 2500kcal/일(단백질 25%, 즉 체중 1kg당 단백질 1.9g, 지방 20% 및 탄수화물 55%)로 엄격히 통제한다.
피검자 1
대조 시험 기간은 3일 동안 지속한다. 치료 첫날, IGF I을 체중 1kg 및 1시간당 32.0㎍의 임의의 용량으로 주입한다. 이러한 용량의 IGF I로 인해 저혈당증이 유발된다(하기 결과 참조). 이후 5일 동안 체중 1kg 및 1시간당 20㎍을 투여하는 경우 안전하며 혈당은 정상인 것으로 나타났다. 6일 동안 주입된 IGF I의 총량은 184mg이다.
피검자 2
대조 시험 기간은 5일이다. 총 6일 동안 피검자 1에서와 동일한 투여량(체중 1kg 및 1시간당 20.0μg)으로 IGF I을 주입한다. 주입된 IGF I의 총량은 167mg이다.
주입 장치
소형의 인슐린 주입 장치(MRS 1 Infusor/Disetronic AG, Burgdorf, Switzerland)를 사용한다. IGF I을 0.1M 아세트산에 용해시킨다. 1시간당 25μl를 주입한다. 3일 후 IGF I을 함유하는 주입기 카트리지를 재충전시킨다. 복부 피부 밑에 마이크로카테테르를 설치한다. 3일 후 교환하는데 첫번째 것으로부터 떨어진 위치에 설치한다.
인슐린 및 C-펩타이드의 혈청내 농도를 절식 그룹의 혈청 샘플에서 매일 아침 측정하며 피검자 2에서는 IGF I을 주입한지 6일째 밤 동안 뿐만 아니라 주입한지 수주 후 특정일의 밤 동안에도 측정한다.
정맥혈을 매일 아침 6시 내지 7시에 채혈하여 즉시 얼음 위에 놓고 1시간 후에 원심분리한다. 혈청 또는 혈장 1ml를 -20℃에서 저장한다. 모든 분석은 한번도 해동된 적이 없는 샘플로 수행한다.
연구 기간 내내 24시간 동안(오전 6시에서 다음날 오전 6시)의 소변을 채취한다. 수개의 분취량을 -20℃에서 저장한다.
분석
이미 기재되어 있는 방사선 면역분석(참조 문헌 6 및 7)을 통해 총 IGF I 및 유리 IGF I을 측정한다. 혈당량은 YSI 23A 글루코스 분석기로 측정한다. 시판되는 키트(kt)(hiGH-RIA-kit, Medipro, Teufen, Switzerland; RIA-GNOST Insulin, Behringwerke AG, Marburg, Germany 및 RIA kit for human C-peptide, Medigenix, Fleurus, Belgium)를 사용하여 성장 호르몬, 인슐린 및 C-펩타이드를 측정한다. 기타 모든 분석 방법은 취리히 대학 부속 병원 임상화학과에서 수행한다.
결과
피검자 1에의 IGF I 용량(제1도) :
3일 동안의 대조일 후(어떠한 호르몬도 없음), 오전 6시 30분에 체중 1kg 및 1시간당 32.0μg의 속도로 IGF I의 주입을 개시한다. 혈당량은 4.4mmol/l이고, 혈청 중의 총 IGF I의 농도는 120ng/ml이며 유리 IGF I의 농도는 20ng/ml이다. 총 28.1mg의 IGF I을 주입하고 13.5시간 후, 최종 식이 후 8시간 만에 저혈당증의 임상적 징후는 없지만 혈당량을 2.6mmol/l로 감소된다. 이때, 혈청 중의 총 IGF I의 농도는 683ng/ml이고, 혈청 중의 유리 IGF I의 농도는 123ng/ml이다. 밤 사이에는 주입을 중단하고 다음날 아침 오전 6시 30분에 체중 1kg 및 1시간당 20.0μg의 속도로 다시 주입한다. 피검자 1에 있어서는, 이후 5일 동안 이 투여량을 일정하게 유지하며 피검자 2에 전체 6일 동안의 주입 기간 동안에도 이 투여량을 사용한다. 혈당량이 2.6mmol/l로 저하되는 경우, 오전 6시 30분에 주입을 개시하여 오전 8시에 중단한다. 이때의 혈청중의 유리 IGF I의 농도는 기준값 20ng/ml에서 123ng/ml로 증가한다. 제1도에서 M은 식이 시간을 나타낸다.
임상적 관찰
IGF I 주입 첫날에 피검자 1에서의 저혈당 증상을 제외하고는 어떠한 현상도 기록되지 않는다. 두 피검자는 연구 전반에 걸쳐서 정상으로 생각되어진다. 혈압, 맥박수, 체온 및 체중은 안정하게 유지된다.
혈당량
밤새 절식(12시간 이상)한 후의 매일 혈당량을 검색한 결과 연구 전반에 걸쳐 3.7 내지 4.4mmol/l로 유지 된다. 피검자 2의 경우, IGF I을 하룻밤 주입하는 동안 매시간 측정한 혈당량은 3.6 내지 4.4mmol/l이다.
혈청 중의 트리글리세라이드의 농도
피검자 2의 혈청 중의 트리글리세라이드의 농도는 밤새 절식한 후 아침에 수득한 혈액 샘플을 히타치 737 다중 분석기(Hitachi 737 Multi Analyser)를 사용하여 통상의 효소학적 색체 시험(GPO-PAP)으로 측정한다.
유사한 방법으로 3명의 추가의 피검자를 체중 1kg 및 1시간당 IGF I 20μg으로 처리하고 혈청 중의 트리글리세라이드의 농도를 측정한다.
결과는 표 1에 나타낸다.
1 내지 6일 동안 연속 주입. 제1일의 값은 주입 개시 전에 측정한 것이다.
혈청 중의 총 IGF I의 농도(제2도)
주입 개시 후 2 내지 4시간 내에 IGF I의 농도가 상승하여 13 내지 14시간 후에는 700ng/ml의 농도에 이른다. 두명의 피검자의 피크 농도는 각각 980ng/ml 및 920ng/ml이다. 주입을 중단하는 경우, IGF I의 농도는 하루만에 정상범위로 떨어진다.
혈청 중의 유리 IGF I의 농도
대조 시험 기간 동안 유리 IGF I의 농도는 15 내지 20ng/ml이고 연속적인 IGF I의 주입 동안의 농도는 50 내지 80ng/ml이다.
절식 동안의 혈청 중의 인슐린의 농도(제3도)
절식 동안 IGF I의 주입 전, 주입 동안, 주입 후의 모든 인슐린 값은 17내지 77pmol/ml이다. 주입 후 6일째 밤 및 5주 동안 피검자 2에서 매시간마다 채혈한 혈액 샘플에서도 유사한 값이 나타난다(51 내지 77pmol/ml). 제3도에서 점선은 분석의 검출 한계를 나타낸다.
혈청 중의 C-펩타이드의 농도(제3도 및 제4도)
절식 동안 혈청 중의 C-펩타이드의 농도를 IGF I의 주입 전 및 주입 후에 매일 아침 측정한 결과, 450±80pmol/ml인 반면에, 주입하는 동안 2개의 값(각각 95pmol /ml 및 69pmol/ml)을 제외하고는 모두 검정의 검출 한계 미만이다(50pmol/ml)(제3도). 혈청 중의 C-펩타이드의 농도를 피검자 2에서 주입한 후 6일째 밤 동안에 측정한 결과, 오후 10시에 측정한 값(98pmol/l)을 제외하고는 모두 50pmol/l미만이다. 주입한 후 5주 후의 대조용 밤 동안의 C-펩타이드의 농도는 650±440pmol/l이다(제4도). 제4도에서 점선은 분석의 검출 한계를 나타낸다.
결과에 대한 논의
체중 1kg 및 1시간당 20.0μg 용량의 IGF I을 건강한 성인에 피하 주입하는 경우 안전하며, 일반적인 안녕, 혈압, 맥박수 및 체온에 영향을 주지 않으며 저혈당증을 초래하지 않는다. IGF I을 주입하면 혈청 중의 트리글리세라이드의 농도는 현저히 감소한다. 아침에 절식하는 경우 혈청 중의 인슐린의 농도는 정상 범위인 반면, C-펩타이드의 농도는 50pmol/l 미만이다(제3도).
본 연구에서 관찰된 바와 같이, 감소된 C-펩타이드 농도의 존재하에 나타나는 정상 인슐린 농도는 인슐린의 분해가 감소되고 연속하여 이의 반감기가 연장되었음을 시사한다.
IGF I은 유기체가 인슐린에 더욱 민감해지도록 하며, 이러한 점으로 인하여 타입 2 당뇨병, 비만증, 과지질혈증, 및, 또한, 타입 1 당뇨병과 같은 인슐린 내성 치료에 대한 잠정적으로 매우 각광 받는 치료 수단이 되고 있다.

- 유효량 IGF I이 투여됨을 특징으로 하는, 타입 2 당뇨병, 비만증 또는 과지질혈증의 치료 및 예방용 약제학적 제제.
- 제1항에 또는 제2항에 있어서, 재조합 IGF I이 사용됨을 특징으로 하는 약제학적 제제.
- 제1항에 있어서, IGF I 이외에, 인슐린이 단독으로 사용되는 경우보다 적은 양으로 함유되는 약제학적 조성물.
- 제1항에 있어서, IGF I 이외에, 인슐린이 자체적으로 혈당에 거의 영향을 미치지 않는 양으로 함유되는 약제학적 제제.
