Abstract
Description
[1] 血液凝固関連疾患の治療のための組換えアデノ随伴ウイルスベクター(rAAV)であって、
該ウイルスベクターは、肝臓特異的プロモーター配列および該プロモーター配列と作動可能に連結されたゲノム編集手段をコードするポリヌクレオチド配列を含むウイルスゲノムを含み、
該ゲノム編集手段が、
(a) Cas9タンパク質とガイドRNA(gRNA)とから構成されるCRISPR/Cas9、および修復用遺伝子を含む手段、または
(b) Cas9タンパク質とgRNAとから構成されるCRISPR/Cas9を含む手段
であり、
該gRNAが、患者のゲノム上にある疾患関連タンパク質の発現と関係する領域の一部に対して相補的なヌクレオチド領域、およびCas9タンパク質と相互作用する領域を含む、
組換えアデノ随伴ウイルスベクター。
[2] AAV3、AAV3B、AAV8、またはAAV9のアデノ随伴ウイルスに由来するカプシドタンパク質を含む、[1]に記載のアデノ随伴ウイルスベクター。
[3] 前記肝臓特異的プロモーター配列が、ApoEプロモーター、アンチトリプシンプロモーター、cKitプロモーター、肝臓特異的転写因子(HNF-1、HNF-2、HNF-3、HNF-6、C/ERP、DBP)のプロモーター、アルブミン、サイロキシン結合グロブリン(TBG)のプロモーター、および配列番号1のポリヌクレオチド配列からなる群から選択されるポリヌクレオチド配列と90%以上の相同性を有するポリヌクレオチドを含み、肝臓特異的なプロモーターとして機能する、[1]又は[2]に記載の組換えアデノ随伴ウイルスベクター。
[4] 前記Cas9タンパク質が、配列番号2又は6に記載のアミノ酸配列を含むか、または配列番号2又は6のアミノ酸配列と90%以上の相同性を有するアミノ酸配列を含み、配列番号3〜5又は配列番号7〜15のいずれかのgRNAと一緒になってCRISPR/Cas9複合体を形成可能である、[1]〜[3]のいずれか1項に記載の組換えアデノ随伴ウイルスベクター。
[5] 前記gRNAが、配列番号3〜5又は配列番号7〜15に記載のポリヌクレオチド配列を有するか、または配列番号3〜5又は配列番号7〜15に記載のポリヌクレオチド配列(crRNA部分及びPAM部分を除いた3’側の配列)と90%以上の相同性を有するポリヌクレオチド配列を含み、配列番号2又は6のCas9タンパク質と一緒になってCRISPR/Cas9複合体を形成可能である、[1]〜[4]のいずれか1項に記載の組換えアデノ随伴ウイルスベクター。
[6] 前記疾患関連タンパク質が、血液凝固因子または抗血液凝固因子である、[1]〜[5]のいずれか1項に記載の組換えアデノ随伴ウイルスベクター。
[7] 前記疾患関連タンパク質が、血液凝固因子IX、アンチトリプシン、血液凝固因子VIII、血液凝固因子VII、血液凝固因子XI、アンチトロンビン、プロテインS、またはプロテインCである、[6]に記載の組換えアデノ随伴ウイルスベクター。

血友病は、代表的な先天性の出血性疾患であり、血友病に罹患した患者は幼小児期から出血を繰り返す。血友病には血友病Aおよび血友病Bの2タイプが存在し、血友病Aは血液凝固VIII因子(FVIIIともいう)の欠乏や異常に起因し、血友病Bは血液凝固IX因子(FIXともいう)の欠乏や異常に起因する。血友病Aと血友病Bとは外観的な症状に基づき区別することができず、特定するには遺伝子診断を必要とする。血友病の原因遺伝子は共にX染色体上にあり、劣性遺伝型であるので、大部分の患者は男性である。治療法としては、通常は血漿由来でまたは遺伝子組換え生産由来のFVIIIまたはFIXを補充する薬物療法が行われている。
2.1.本発明に用いる組換えアデノ随伴ウイルス(rAAV)ベクター
本発明において、ゲノム編集手段のためのポリヌクレオチド、または特定の血液凝固因子をコードするポリヌクレオチドを対象の臓器胞に送達するためのベクターとしては、WO2012/057363に記載される、末梢投与の場合にも治療効果を示すのに十分な量の遺伝子送達可能な組換えアデノ随伴ウイルスベクター、あるいはWO 2008/124724などに記載されるものを用いることができる。
(1)rAAVウイルスゲノムについて
本発明のrAAVベクター中にパッケージングされるポリヌクレオチド(すなわち、ポリヌクレオチド)は、野生型ゲノムの5'側および3'側に位置するITRの間に位置する内部領域(すなわち、rep遺伝子及びcap遺伝子の一方または両方)のポリヌクレオチドを、目的のタンパク質をコードするポリヌクレオチド(ゲノム編集手段、及び/又は修復用遺伝子)およびこのポリヌクレオチドを転写するためのプロモーター配列などを含む遺伝子カセットによって置換することにより作製できる。好ましくは、5'側および3'側に位置するITRは、それぞれAAVゲノムの5’末端および3’末端に位置する。好ましくは、本発明のrAAVゲノムは、5’末端および3’末端に位置するITRは、AAV1、AAV2、AAV3、AAV6またはAAV9のゲノムに含まれる5'側ITRおよび3'側ITRを含むが、これら特定のAAVに限定されない。
一般的に、ITR部分は容易に相補配列が入れ替わった配列(flip and flop structure)をとるため、本発明のrAAVゲノムに含まれるITRは、5’と3’の方向が逆転していてもよい。本発明のrAAVゲノムにおいて、内部領域と置き換えられるポリヌクレオチド(すなわち、ゲノム編集手段及び/又は修復用遺伝子)の長さは、元のポリヌクレオチドの長さと同程度が実用上好ましい。すなわち、本発明のrAAVゲノムは、全長が野生型の全長である5kbと同程度、例えば約2〜6kb、好ましくは約4〜6kbであることが好ましい。
本発明のrAAVゲノムに組込まれる治療用遺伝子の長さは、プロモーター、ポリアデニレーションなどを含めた転写調節領域の長さ(例えば、約1〜1.5kbと仮定する場合)を差し引くと、好ましくは長さが約0.01〜3.7kb、より好ましくは長さが約0.01〜2.5kb、さらに好ましく約0.01〜2kbであるが、これに限定されない。
さらに、公知のinternal ribosome entry site (IRES)配列を介在させるなどの公知の手法を用いて、rAAVゲノムの全長が上記の範囲内である限り、二種類以上の複数の治療用遺伝子を同時に組み込むことが可能である。本発明のrAAVが二種以上の複数のタンパク質を発現する場合、各々のタンパク質をコードする遺伝子は、同じ方向に配置されても、異なる方向に配置されてもよい。
1実施形態において、本発明のrAAVベクターは、好ましくは、肝臓細胞特異的なプロモーター配列およびそのプロモーター配列と作動可能に連結されたゲノム編集手段及び/又は修復用遺伝子を含むポリヌクレオチドを含む(すなわち、そのようなポリヌクレオチドがパッケージングされている)。本発明に用いるプロモーター配列としては、肝臓に含まれる細胞に特異的なプロモーター配列、例えば、肝実質細胞、肝非実質細胞(星状細胞等)などに特異的なプロモーター配列を用いることができる。このようなプロモーター配列としては、具体的には、ApoEプロモーター、アンチトリプシンプロモーター、cKitプロモーター、肝臓特異的転写因子(HNF-1、HNF-2、HNF-3、HNF-6、C/ERP、DBPなど)のプロモーター、サイロキシン結合グロブリン(TBG)プロモーター(Nature. 2015 Apr 9;520(7546):186-91)(配列番号34)、ヒトSEPP1プロモーター領域(肝臓特異的プロモーター)(配列番号35)、その他の肝臓特異的タンパク質(アルブミンなど)のプロモーター、これらプロモーターを組み合わせた合成プロモーターなどが挙げられるが、これらに限定されない。また、これらのプロモーターはさらに、公知のエンハンサー配列、好ましくは肝臓細胞特異的なエンハンサー配列と組み合わせることができる。本発明において使用できるエンハンサー配列としては、ApoEエンハンサー配列、ヒトTIE2エンハンサー(内皮細胞特異的エンハンサー)(配列番号36)などが挙げられる。これらプロモーター配列およびエンハンサー配列は、単独または任意の複数を組み合わせて用いることができる。さらにまた、上記の肝臓細胞特異的なプロモーター及びエンハンサーを利用した合成プロモーターを用いることもできる。このような合成プロモーター/エンハンサー配列としては、HCRhAAT合成プロモーター(配列番号1)が挙げられるがこれに限定されない。本発明のrAAVベクターは、好ましくは、肝臓特異的、より好ましくは肝実質細胞特異的なApoEプロモーター、アンチトリプシンプロモーター、TBGプロモーター、またはHCRhAAT合成プロモーターの配列を含み得る。
アデノ随伴ウイルスベクター内に含まれるウイルスゲノムは、通常は宿主染色体中に取り込まれる。しかしながら、細胞内で新たに取り込まれた外来遺伝子がより安定して存在し続けるために、細胞内ゲノム中の特定に位置に外来遺伝子を組み込むことが必要とされ得る。
本発明のウイルスベクターは、ゲノム編集手段として、ゲノム上の標的部位を特異的に切断可能な手段としてCRISPR/Cas9を発現するポリヌクレオチド、および肝臓に特異的なプロモーター配列を含み得る。本明細書において、ゲノム編集手段とは、ゲノム切断手段のみを含むこと、またはゲノム切断手段とゲノム修復手段の両方を含むことを指す。本発明のウイルスベクターを利用することによって、ゲノム編集手段が肝臓に送達されて、肝臓細胞において特異的なゲノム編集が生じる。より好ましくは、本発明のウイルスベクターは肝臓に対してより指向性および/または発現特異性を備える。

B群:アスパラギン酸、グルタミン酸、イソアスパラギン酸、イソグルタミン酸、2-アミノアジピン酸、2-アミノスベリン酸;
C群:アスパラギン、グルタミン;
D群:リジン、アルギニン、オルニチン、2,4-ジアミノブタン酸、2,3-ジアミノプロピオン酸;
E群:プロリン、3-ヒドロキシプロリン、4-ヒドロキシプロリン;
F群:セリン、スレオニン、ホモセリン;
G群:フェニルアラニン、チロシン。
アミノ酸残基が置換されたニューロリギンタンパク質は、通常の遺伝子操作技術など、当業者に公知の方法に従って作製することができる。このような遺伝子操作手順については、例えば、Molecular Cloning 3rd Edition, J. Sambrook et al.,Cold Spring Harbor Lab. Press. 2001、Current Protocols in Molecular Biology, John Wiley & Sons 1987-1997などを参照することができる。
本発明のウイルスベクターは、出血性疾患を治療するために、ゲノム編集手段として、標的のゲノムの切断手段のみを有することができる。例えば、出血性疾患において機能低下または欠損している血液凝固因子の遺伝子を上記のゲノム編集手段によって修復する代替として、異なる代替機能もしくは補助機能を発揮させて出血性疾患を改善させもよい。より具体的には、部位突然変異または欠損が生じて血液凝固因子の機能が低下するなどによって血液凝固における正の制御よりも負の制御が強い場合、血液凝固の負の制御を阻害もしくは低下させることによって、最終的に正の制御を発揮させる手段を提供する。
本発明のウイルスベクターは、ゲノム編集手段として、ゲノム切断手段と併せて、出血性疾患を治療するために、好ましくは、機能低下または欠損している血液凝固因子を修復するための修復用遺伝子(修復手段)を含むことができる。本発明のウイルスベクターは、1種類のウイルスベクターにおいてゲノム編集手段および修復手段の両方を有してもよいし、代替として、ゲノム編集手段を含むベクター(a1)と修復手段を含むウイルスベクター(a2)とを複数のベクターにおいて別個に含んでもよい。よって、本発明のゲノム編集のためのrAAVは、1種または2種以上のrAAVを含み得る。本発明のウイルスベクターが2種以上の複数のrAAVベクターを含む場合、2種以上の複数のrAAVは同時に投与されても、連続して投与されてもよい。好ましくは、本発明の2種以上のrAAVベクターは、同時に投与される。また、各rAAVのウイルスゲノム量比(virus genome: vg)は、修復手段/ゲノム編集手段が0.3〜10、好ましくは0.5〜8、さらに好ましくは1〜5の範囲にあり得る。
提供し得るレベルで発現させる。すなわち、本発明の修復遺伝子が導入された結果、好ましくは、対象において血液凝固機能の向上が達成される。また、修復手段のポリヌクレオチドは、内在性遺伝子との差別化のためにコドン最適化がなされ得る。
そのようなアミノ酸配列としては、正常なFIXのアミノ酸配列に、例えば、1〜50個、1〜40個、1〜39個、1〜38個、1〜37個、1〜36個、1〜35個、1〜34個、1〜33個、1〜32個、1〜31個、1〜30個、1〜29個、1〜28個、1〜27個、1〜26個、1〜25個、1〜24個、1〜23個、1〜22個、1〜21個、1〜20個、1〜19個、1〜18個、1〜17個、1〜16個、1〜15個、1〜14個、1〜13個、1〜12個、1〜11個、1〜10個、1〜9個(1〜数個)、1〜8個、1〜7個、1〜6個、1〜5個、1〜4個、1〜3個、1〜2個または1個のアミノ酸残基が欠失、置換、挿入および/または付加されたアミノ酸配列を含み、かつ生理条件下でFIXと同等の比活性を示すものが挙げられる。
さらに、本願発明において用いられるFIXは、正常なFIXのアミノ酸配列または上記同一性を有するアミノ酸配列に対して、約90%以上、91%以上、92%以上、93%以上、94%以上、95%以上、96%以上、97%以上、98%以上、99%以上、99.1%以上、99.2%以上、99.3%以上、99.4%以上、99.5%以上、99.6%以上、99.7%以上、99.8%以上、99.9%以上の同一性を有するアミノ酸配列を有するものであって、かつ生理条件下でFIXと同等の比活性を示すものが挙げられる。
修復手段に用いる遺伝子の設計において、従来のノックアウトやノックインで用いる遺伝子の設計方法を利用できる。
本発明において、血液凝固機能は公知の方法を用いて確認して測定できる。例えば、通常凝固一段法と呼ばれる方法で凝固因子活性を測定することができる(Eur J Haematol. 2015 Feb;94 Suppl 77:38-44)。あるいは、本明細書中の実施例「実験の材料及び手順」「(9)出血時間」に記載の手順を用いる方法が利用できる。
また、本明細書において、血液凝固機能の回復とは、好ましくは、治療の結果、血友病の出血時間が改善すること、すなわちFIXに関して上述したような、健常な個体と同等の血液凝固時間を示すことを意味する。
本発明のrAAVベクターを調製する方法としては一般的なものを利用することができ、例えば、(A)カプシドタンパク質をコードする第1のポリヌクレオチド(一般に、AAVヘルパープラスミドと称される)、および(B)本発明のrAAVベクター内にパッケージングされる第2のポリヌクレオチド(目的の治療用遺伝子を含む)を、培養細胞にトランスフェクトする工程を含むことができる。本発明における調製方法はさらに、(C)アデノウイルス(AdV)ヘルパープラスミドと称されるアデノウイルス由来因子をコードするプラスミドを培養細胞にトランフェクトする工程、またはアデノウイルスを培養細胞に感染させる工程も含むことができる。さらに、上記のトランスフェクトされた培養細胞を培養する工程、および培養上清より組換えアデノ随伴ウイルスベクターを収集する工程を含むこともできる。このような方法は既に公知であり、本明細書の実施例においても利用される。
本発明のさらなる実施形態において、本発明のrAAVベクター(rAAVビリオン)を含む医薬組成物が提供される。本発明のrAAVベクターを含む医薬組成物(以下、本発明の医薬組成物という)利用することによって、被検体の肝臓細胞に高い効率で治療用遺伝子を導入可能であり、導入される治療用遺伝子が発現することによって目的の疾患を治療できる医薬組成物を提供する。このような治療用遺伝子を含むrAAVベクターは、本発明の医薬組成物に含められる。このような治療用遺伝子としては、例えば上記のようなゲノム編集手段、ゲノム修復手段などが挙げられるが、これらに限定されない。
本発明のウイルスベクターは、好ましくは、対象に末梢投与によってより安全かつ容易に投与される。本明細書において末梢投与とは、静脈内投与、動脈内投与、腹腔内投与、心腔内投与、筋肉内投与など、当業者に末梢投与として通常理解される投与経路をいう。
本発明は別の実施形態において、本発明のrAAVを作製するためのキットを提供する。このようなキットは、例えば、(a)カプシドタンパク質VP1等を発現するための第1のポリヌクレオチド、および(b)rAAVベクター内にパッケージングされる第2のポリヌクレオチドを含むことができる。例えば、第1のポリヌクレオチドは、配列番号:のアミノ酸をコードするポリヌクレオチドを含む。例えば、第2のポリヌクレオチドは、目的の治療用遺伝子を含んでも含まなくてもよいが、好ましくは、そのような目的の治療用遺伝子を組込むための種々の制限酵素切断部位を含むことができる。
本明細書中に用いられる各用語が示す意味は以下のとおりである。
特に述べられない限り、本明細書中、rAAVゲノムがコードするプロモーター、目的遺伝子、ポリアデニレーションシグナルなどの遺伝子上の配置について説明される場合、rAAVゲノムがセンス鎖である場合についてはその鎖自体について、アンチセンス鎖である場合はその相補鎖について記載される。また、本明細書中、文脈より明らかな場合、組換えを表す「r」は省略されることもある。
(1)実験動物
すべての動物のプロトコルは、自治医科大学における動物のケア及び関連の制度委員会により承認され、動物のケアは当委員会のガイドラインに準じた。C57BL/6JマウスはジャパンSLCから購入した。凝固因子IX(FIX)欠損マウス(B6.129P2-F9tm1Dws)は、ジャクソンラボラトリー(米国)より入手した。
sgRNA配列を、Thermo Fisher Scientific(Waltham, MA, USA)またはBenchling (https://benchling.com)より提供されるオンラインソフトウェアを使用して設計した(Curr Issues Mol Biol. 2016;20:1-12)。設計した各gRNAを以下の表に示す(配列番号3〜5及び7〜15)。U6プロモーター制御下のgRNAコードDNA(トランス作用性RNA−クリスパーRNAキメラ)はGenScript社(USA)により合成された。U6プロモーターを含まないsgRNA配列のPCT断片をプラスミドベクターpCR2.1 TOPOに挿入し、sgRNAをインビトロ転写させて生成した(CUGA7 in vitro Transcription Kit (日本ジーン:カタログ番号304-14641/307-13531)。
コドン最適化したStreptococcus pyogenes Cas9 (SpCas9)のmRNA(20 ng/μl; Thermo Fisher Scientific)及びsgRNA(5 ng/μl)を受精卵の細胞質に注入した。この受精卵を2細胞期まで培養し、偽妊娠させた雌性マウスに導入した。
遺伝子の突然変異を、Surveyor(登録商標) Mutation Detection Kit (Integrated DNA Technologies)を用いて決定した。簡潔に述べると、PCR断片をExTaq DNAポリメラーゼ(タカラバイオ)を用いて増幅した。等量の試験PCR産物及び参照PCR産物をサーマルサイクラーで変性及び再アニーリングさせて、Surveyor(登録商標)ヌクレアーゼを用いて処置した。DNA断片をアガロースゲル電気泳動により分析した。突然変異の決定に用いたオリゴヌクレオチドプライマー配列は、以下の表(及び配列番号17〜33)に示される。
コドン最適化したStaphylococcus aureus Cas9 (SaCas9)のcDNA(Ranら、2015)の提供を東京大学のNureki博士から受けた。キメラプロモーター(HCRhAAT:Apo E/C1遺伝子の肝臓性制御領域のエンハンサーエレメント及びヒトアンチトリプシンプロモーター) (Mimuroら、2013)、SaCas9 cDNA、SV40ポリアデニル化シグナル、及びU6プロモーター制御下の上記sgRNA配列を、pAAV2プラスミドのインバーテッドターミナルリピートの間に導入した(図2A)。この遺伝子を、以前に記載された方法(Mimuroら、2013)によって、ヒト胚性腎臓293細胞(HEK293)のトリプルプラスミドトランスフェクションによってパッケージングさせて、組換えAAV8ベクターを作出した(ヘルパーフリーの系)。組換えAAVベクターの秤量を定量的PCRによって行った。
ヌクレアーゼ標的部位を含むゲノムDNA断片を、KAPA HiFi(登録商標) HotStart PCRキット(KAPA Biosystems、Wilmington, MA, USA)を用いて増幅した。PCR産物をAMPure(登録商標) XPビーズ(Beckman Coulter, Brea, CA, USA)を用いて精製した。標的領域を増幅して、アンプリコンの標的に対してイルミナシーケンシングアダプタ及びデュアルインデックスバーコードを付加するためにライブラリを調製した。Illumina MiSeq (100,000の読取)を使用して、PCRアンプリコンを300ペアの末端読取の配列決定に供した。標的配列付近の60種の配列を抽出して、各配列の頻度を計算した。
血漿中のFIX:C(血液凝固因子IX活性)を、FIX欠損血漿(シスメックス社)を用いる自動化した凝固分析器(Sysmex CA-510アナライザー:シスメックス社)によって、1ステージ型凝固時間アッセイ(one-stage clotting-time assay)にて測定した。AT:Cを、Testzym(登録商標)S ATIII (積水メディカル株式会社)を使用して測定した。トロンビン生成を、以前に報告されたように評価した(Matsumotoら、2009)。簡単に述べると、80μlのマウス血漿を20μlのトリガー試薬(5 pMの組換えヒト組織因子および40 μMのリン脂質の混合物)と一緒に37℃で10分間インキュベートした。このアッセイを、20 μlの100 mM CaCl2および5 mMのトロンビン基質Z-Gly-Gly-Arg-AMC (和光純薬株式会社)を添加して開始させた。蛍光シグナル(励起:390nmおよび放射:460nm)を、Spark 10M multimode microplate reader (Tecan, Maennedorf, Switzerland)を用いて10秒間隔でモニタリングした。
イソフルランで麻酔したマウスを50 mlのリン酸緩衝化生理食塩水で灌流し、次いで組織を10%のホルマリンで固定した。パラフィン包埋した組織サンプルから切片を得て、へまトキシン−エオシン染色または免疫染色のために処理した。この研究で用いたSaCas9はヘマグルチニン(HA)と結合させたものである。切片を5%ロバ血清で前処理し、抗HAポリクローナル抗体(MBL社)で処理した。免疫活性をSimple Stain Mouse MAX-PO(ニチレイバイオサイエンス社)及びDAB(Agilent Technologies)を用い、その後Myerヘマトキシンで検出した。組織切片を倍率400倍でオールインワン(all-in-one)顕微鏡で観察した。
麻酔したマウスの遠位尾部先端(5mm)をクリップ止めし、尾部を速やかに37℃のリン酸緩衝化生理食塩水50mlに浸漬させた。尾部の出血時間を止血に要する時間として定義した。出血時間が10分を超える場合、動物が死なないように電気焼灼して実験を終わらせた。
インビボでのフィブリン形成を分析するために生体内顕微鏡観察を行った。簡潔に述べると、ローダミンBイソチオシアネート−デキストラン(5 mg/body; Sigma Aldrich)、ヘキスト33342(3 mg/body; Thermo Fisher Scientific)、およびAlexa488結合体化フィブリノーゲン(300 μg/body; Thermo Fisher Scientific)を麻酔したマウスに注入した。精巣静脈(少なくとも直径80μm)の連続画像を、レーザー照射(波長700nm)により誘発した局所的内皮破壊の後に共鳴走査型共焦点顕微鏡(ニコンA1RNP、株式会社ニコン)を用いて取得した。フィブリン形成のシグナル強度(Alexa-488結合体化フィブリノーゲンのシグナルにより示されるもの)を、NIS-Elements AR 3.2 (Nikon)により定量化した。
本明細書中に特に明記されない場合、数値は平均±標準偏差(SEM)を表す。統計学的解析は、ステューデントt検定、または事後のボンフェローニ検定(post hoc Bonferroni test)を用いる一方向反復測定ANOVAにより行った。
(1)FIX欠損マウスの作出
最初に、CRISPR/Cas9を用いて血友病Bを発症するマウスを作出した。血友病B患者の変異の大部分が位置する、F9のエキソン8の遺伝子を破壊した(Liら、2013)。Streptococcus pyogenes Cas9 (SpCas9)のmRNAおよびマウスF9のエキソン8に特異的なsgRNAを一緒に受精卵細胞に注入した(図1A及び1B)。用いたsgRNAの配列(tracrRNA部分、crRNA部分及びprotospacer adjacent motif (PAM)配列を含む)を配列番号3〜5及び7〜9に示す。
新生仔マウスをF9変異に関してSurveyor(登録商標)ヌクレアーゼアッセイを用いて調査した。図1に示すように、2種類のsgRNAによって変異マウスが生じた。sgRNA3が標的DNA切断を最も効率よく誘発した(6/7:約86%)(図1C)。変異を保持する全てのマウスが血漿中のFIX:Cの低減を示した(図1D)。血漿中のFIX:Cは、X染色体アリルが2本とも切断された雌マウスにおいても大きく低減していた。これらのマウスの一部を育成して子孫のマウスを得た。FIXの活性(FIX:C)を示さない2系統のF2の雄マウスのDNA配列決定を行ったところ、sgRNA配列中の1〜12塩基が欠損していることが明らかになった(図1E)。
次いで、ウイルスベクターの投与によって成体マウスのF9を破壊することによって血友病を有するマウスを作出した。FIXは、肝臓において産生されるビタミンK依存性凝固因子である。AAV8ベクターは、肝臓細胞に遺伝子移入するのに高効率であり、ヒト血友病Bの遺伝子治療に使用されている(Gaoら、2002;Nathwaniら、2014;Nathwaniら、2011)。
通常のAAVベクターに組み込む長さはプロモーター領域を含めて最大で5kbと報告されているので(Wuら、2010)、約4.5kbの長さのSpCas9 cDNAをAAVベクターに取り込むのは一般に困難である。したがって、本発明者らは、サイズが3.2kbとより小さいStaphylococcus aureus Cas9 (SaCas9)を使用した(Ranら、2015)。本発明者らは、キメラプロモーター(HCRhAAT:Apo E/C1遺伝子の肝臓型制御領域のエンハンサーエレメントとヒトアンチトリプシンプロモーターとを含む)の制御下で肝臓特異的に発現する様式でSaCas9を発現し、同時にU6プロモーター制御下でsgRNAを発現する、組換えAAV8ベクターを調製した(図2A)。SaCas9のPAM配列はSpCas9のものとは異なるため(Ranら、2015)、sgRNA配列を独自に調整したものを用いた。
本発明者らは、3種類のsgRNA配列を発現するAAV8ベクターを調製し、個別に7週齢のC57BL/6Jマウスに静脈注射した(図2A)。sgRNA2を発現するAAV8ベクターは、ベクターの投与量依存的に血漿中のFIX:Cを低減させた(図2B)。FIX:Cの濃度はベクターの高用量(1体あたり1×1012個)投与した後に2〜5%低下した。このFIX:Cの低下は、少なくとも4ヶ月間続いた(図2B)。他の2つのsgRNA配列は、血漿中のFIX:C濃度を低下させなかった。肝臓組織由来DNA中の変異をSurveyor(登録商標)ヌクレアーゼアッセイを用いて確認した(図2C)。
静脈内投与したAAV8ベクターの存在を、種々の器官において試験した。器官内のAAV由来DNAの量を、リアルタイムPCTによって評価した。AAVゲノムは主として肝臓で検出された(図2D)。AAV DNAはまた、心臓において低レベルで確認された。しかしながら、標的ゲノムDNAの切断は、肝臓以外のいずれの器官でも認められなかった。肝臓中のゲノムDNAは、sgRNA2を発現する高用量のAAV8ベクターを受け、次世代配列決定に供して配列中の変異の頻度を計算した。最も多い変異としてはPAM配列近辺における数残基の欠損または置換が認められた(表3)。正常な配列を検出したのはPCRアンプリコン中の33.4%であった(表3)。上記のFIX:Cの結果(2〜5%)と比較すると、野生型アリルの頻度は比較的高かった。このことは、おそらくは肝臓が肝実質細胞に加えて、AAV8では遺伝子導入されにくい血管上皮細胞やマクロファージを含むためと考えられる。肝臓でのSaCas9発現についての組織染色によると、ベクターの高投与量群のほぼすべての肝臓細胞においてSaCas9が発現したことを示した。SaCas9は主に肝臓細胞の核内で認められたが、血管上皮細胞の核内で認められた。ヘマトキシリン染色やエオシン染色の場合、組織学的な異常は見られなかった。
CRISPR/Cas9による肝臓細胞中のF9の高効率の切断を確認した後、その切断と同時に変異を修復するためのドナー配列の送達が、CRISPR/Cas9によって作出した血友病Bマウス(図1)において、HDRによる遺伝子修復によってFIX:Cの増加をもたらすかどうかを試験した。これらマウスのゲノムは、AAV8ベクターを用いるDSBを高効率で作出するsgRNA2によって認識される無傷の部位を有している(図3A)。sgRNA2配列の5’側及び3’側の両方にある350〜550bpの相補性アームを有するドナー配列をAAV8ベクターに組み込んだ(図3A)。相同組換されるF9がSaCas9によって再切断されないように、サイレント変異をドナー配列のsgRNA2中に組み込んだ。ドナー配列を含みsgRNA2を発現するAAV8ベクター及びSaCas9を同時に幼仔マウス又は成体マウス(2週齢または26〜46週齢)に注射した。ベクター注入後に検出されたFIX:Cは、わずかに増加していた(図3B)。Surveyor(登録商標)ヌクレアーゼアッセイによって、AAVベクター投与後に肝臓のゲノムDNA中に突然変異を検出したので、SaCas9による遺伝子切断部位では、主としてNHEJによって遺伝子修復が生じたことを確認した。
ジンクフィンガーヌクレアーゼ(ZFN)を用いたこれまでの研究から、HDRおよびNHEJによって、安全な遺伝子座での二本鎖切断(DSB)内にDNA断片を挿入できること(Rosa26またはA1bにおけるヒトF9の人工的挿入)が提案された(Anguelaら、2013;Sharmaら、2015)。内在性F9遺伝子座へのCRISPR/Cas9システムによるcDNA断片の挿入のために(図4)、本発明者らは、F9のイントロン1内に3種のsgRNAを設計し、イントロン1に対するsgRNA3がDSBを効率的に生じたことを確認した。そこで、SaCas9を発現するAAV8ベクター系にイントロン1に対するsgRNA3を組み込み(図4:AAV8-SaCas9-sgRNA3 for F9 intron)、このAAV8ベクター投与後にsgRNA3が肝臓DNAにおいてDSBを効率的に誘発したことを見出した(図5A)。次いで、両サイドに1kbの相補性アームを含むキメラDNA配列(ヒトF9スプライシングアクセプター部位+コドン最適化したマウスF9 cDNA(エキソン2〜8))を設計して、これをAAV8ベクターに組み込んだ(図4:AAV8-Targeting)。SaCas9によって生成されたDSB部位にDNA配列を組み込むために、CRISPR/Cas9により作出した8週齢の血友病B成体マウスに、AAV8-SaCas9-sgRNA3 for F9 intron(1×1012gc/body)とAAV8-Targeting(3×1012gc/body)とを同時に投与した。図5Bに示されるように、FIX:Cは11.7〜39.6%のレベルまで徐々に増加した。ゲノム編集形態を検査するために、挿入したcDNAと3’側アームの外側との間のDNA配列(図4の矢印部分)を増幅させ、F9 cDNA配列がDSB中にHDRだけでなくNHEJによっても挿入されたことを見出した(図5C)。また、ベクター投与後にエキソン1とコドン最適化したエキソン2〜8からなるFIX mRNAの発現を確認した(図5D)。AAV8-Targetingのみの投与では、FIX:Cはほどんど増加しなかった(データは示さず)。このことは、治療効果にはSaCas9によるDSBが必要であることを示している。さらに、AAVベクターの全身投与の時期を比較して、AAV媒介性ゲノム編集の最適期間を識別した。その結果、図4Eに示すように、生後0日(P0)、8日(P8)、28日(P28)、及び42日(P42)で類似の治療効果を確認した。ベクター注入の8〜16週後の肝臓中のAAVゲノムのコピー数は、生後に処置したマウスのものよりも有意に低かった(P28: 604.5 ± 58.73; P0: 1.177 ±0.3437:図5F)。これらの結果は、肝臓細胞の増殖の間の細胞分裂がAAVゲノムを希釈するとしてもゲノム編集の治療効果が維持されることを示唆している。
単独のAAVベクター系を用いて血友病の出血性傾向を治すために、CRISPR/Cas9によるAT遺伝子(Serpinc1)の破壊を使用して血友病マウスの出血を治療できるかどうかを試験した。Serpinc1に対する3種のsgRNAを設計し、Serping1を効率的に切断する配列(sgRNA1 for mAT)を選択した(データは示さず)。このsgRNAとSaCas9とを運ぶAAV8を、FIX欠損マウス(血友病Bマウス)に静注した。血漿中アンチトロンビン(AT)活性(AT:C)は、ベクター注射後に明らかに低下し(図6A)、ベクター投与後の肝臓DNAのDSBを確認した(図6B)。このAT低下は、エキソビボでの血漿トロンビン生成、インビボでの内皮破壊後のフィブリン形成、及び尾部クリップ止後の出血時間を改善させた(図6C〜6F)。このことは、CRISPR/Cas9を用いてATなどの抗凝固物質を阻害することが血友病を治療するための代替的な方針となり得ることを示している。

黄色ぶどう球菌由来のCas9を肝臓特異的プロモーターの下流に搭載し、gRNAを同時に発現するAAVを作製し、肝臓ゲノムの凝固因子・抗凝固因子に効率的に二本鎖DNAの破壊を引き起こすことに成功した。血友病Bの原因遺伝子である第IX因子を破壊すると凝固因子レベルが1-5%まで低下するため、ほぼ全ての肝臓細胞のゲノムに変異導入が可能である。本手法を用いて、変異のあるF9の第一イントロンにcDNAを挿入することによって、血友病Bの表現形を改善できることを確認した。また、血液凝固のブレーキとなるアンチトロンビンを破壊し、血友病の出血傾向を改善できることを確認した。

Claims (16)
- 血液凝固関連疾患の治療のための組換えアデノ随伴ウイルスベクター(rAAV)であって、
該ウイルスベクターは、肝臓特異的プロモーター配列および該プロモーター配列と作動可能に連結されたゲノム編集手段をコードするポリヌクレオチド配列を含むウイルスゲノムを含み、
該ゲノム編集手段が、
(a) Cas9タンパク質とガイドRNA(gRNA)とから構成されるCRISPR/Cas9、および修復用遺伝子を含む手段、または
(b) Cas9タンパク質とgRNAとから構成されるCRISPR/Cas9を含む手段
であり、
該gRNAが、患者のゲノム上にある疾患関連タンパク質の発現と関係する領域の一部に対して相補的なヌクレオチド領域、およびCas9タンパク質と相互作用する領域を含む、組換えアデノ随伴ウイルスベクター。 - AAV3、AAV3B、AAV8、またはAAV9のアデノ随伴ウイルスに由来するカプシドタンパク質を含む、請求項1に記載のアデノ随伴ウイルスベクター。
- 前記肝臓特異的プロモーター配列が、ApoEプロモーター、アンチトリプシンプロモーター、cKitプロモーター、肝臓特異的転写因子(HNF-1、HNF-2、HNF-3、HNF-6、C/ERP、DBP)のプロモーター、アルブミン、サイロキシン結合グロブリン(TBG)のプロモーター、お
よび配列番号1のポリヌクレオチド配列からなる群から選択されるポリヌクレオチド配列と90%以上の相同性を有するポリヌクレオチドを含み、肝臓特異的なプロモーターとして機能する、請求項1又は2に記載の組換えアデノ随伴ウイルスベクター。 - 前記Cas9タンパク質が、配列番号2又は6に記載のアミノ酸配列を含むか、または配列番号2又は6のアミノ酸配列と90%以上の相同性を有するアミノ酸配列を含み、配列番号3〜5又は配列番号7〜15のいずれかのgRNAと一緒になってCRISPR/Cas9複合体を形成可能である、請求項1〜3のいずれか1項に記載の組換えアデノ随伴ウイルスベクター。
- 前記gRNAが、配列番号3〜5又は配列番号7〜15に記載のポリヌクレオチド配列を有するか、または配列番号3〜5又は配列番号7〜15に記載のポリヌクレオチド配列(crRNA部分及びPAM部分を除いた3’側の配列)と90%以上の相同性を有するポリヌクレオチド配列を含み、配列番号2又は6のCas9タンパク質と一緒になってCRISPR/Cas9複合体を形成可能である、請求項1〜4のいずれか1項に記載の組換えアデノ随伴ウイルスベクター。
- 前記疾患関連タンパク質が、血液凝固因子または抗血液凝固因子である、請求項1〜5のいずれか1項に記載の組換えアデノ随伴ウイルスベクター。
- 前記疾患関連タンパク質が、血液凝固因子IX、アンチトリプシン、血液凝固因子VIII、血液凝固因子VII、血液凝固因子XI、アンチトロンビン、プロテインS、またはプロテインCである、請求項6に記載の組換えアデノ随伴ウイルスベクター。
- 前記(a)が、(a1)Cas9タンパク質とgRNAとから構成されるCRISPR/Cas9をコードするアデノ随伴ウイルスベクター、および(a2)修復用遺伝子を含むアデノ随伴ウイルスベクターを含む、請求項1〜7のいずれか1項に記載のアデノ随伴ウイルスベクター。
- 前記(a2)が、修復用遺伝子の5’側及び3’側に、ゲノム上の切断標的部位の5’側及び3’側と相同な領域を含む、請求項8に記載のアデノ随伴ウイルスベクター。
- 前記修復用遺伝子が、前記5’側及び3’側の相同な領域の内側に、正常な疾患関連タンパク質をコードするポリヌクレオチドまたはその一部を含む、請求項9に記載のアデノ随伴ウイルスベクター。
- 前記血液凝固関連疾患が、血友病、第VII因子欠損症、第XI因子欠損症、アンチトロンビン欠乏症、プロテインS異常症・欠乏症、およびプロテインC異常症からなる群より選択される、請求項1〜10のいずれか1項に記載の組換えアデノ随伴ウイルスベクター。
- 前記ウイルスゲノムがさらに、AAV1、AAV2、AAV3、AAV4、AAV6、およびAAV9からなる群より選択されるインバーテッドターミナルリピート(ITR)のヌクレオチド配列を含む、請求項1〜11のいずれか1項に記載の組換えアデノ随伴ウイルスベクター。
- 請求項1〜12のいずれに記載の組換えアデノ随伴ウイルスベクターを含む、肝臓関連疾患の治療のための医薬組成物。
- 2種以上の組換えアデノ随伴ウイルスベクターの組合せを含む、請求項13に記載の医薬組成物。
- 血友病の治療剤と併用される、請求項13または14に記載の医薬組成物。
- 請求項13〜15に記載の医薬組成物を含む、肝臓関連疾患の治療のための医療キット。
