静脈循環をシュミレーションする装置と方法
発明の分野
この発明は、下肢で静脈循環のシュミレーションする装置およびそのようなシ
ュミレーションに対する方法に関する。
発明の背景
長期間座っていると血液が局部的に溜まることを示す下肢の悪化した静脈循環
が生じるが知られている。これは、例えば、靴を履いている場合、足首の周りが
膨らむ形状の不快や種々の困難に出くわす。しかし、長期間座っている効果は、
特に年長者にとて相当深刻である。或る場合には、血液の停滞(血行不良)は人
が足を動かし始める場合、静脈の血液と共に進行し、生命に脅威となる条件であ
る肺塞栓症となる。人が長期間座っていることを強られる状況の例は、人を車椅
子に一時的あるいは恒久的に結び付ける飛行機の旅、病気回復期あるいは病弱で
ある。
この血液の停滞は、脚の筋肉の動きが一方向弁として働く静脈弁と一緒になっ
てポンプ作用をするので、適当な期間に下肢を動かすことにより生じる。
しかし、座った位置で適当な助けなしに脚を動かすことはやっかいである。そ
のような運動をより「自然」に、従ってより容易に、しかも楽に行うため、説明
した多数の装置が提案されいる。例えば、米国特許第 4 669 744号明細書、ドイ
ツ公開特許第 27 44 996号明細書、および WO 84/00695号明細書に開示されてい
るものである。これ等の装置は何れも大抵足を逆に向けたりや裏返したり、内向
きや外向きにすることと組み合わせて、足の背部や裏を曲げるタイプの多かれ少
なかれ「自然」な動きのパターンを脚や足が行うようにしている。
しかし、これ等の周知の装置にはそのデザインや動きの生理機能に関連した多
数の難点がある。製造や運搬が容易で、同時に自然な感じの運動を与える装置で
のみ、利用者は、例えば長旅でこの装置を携帯し、そして循環を促進し血行停滞
を防止する効果を達成するように動機付けられる。
発明の目的
それ故、この発明の目的は、従来の技術で改善され、上に述べた問題を解決す
る下肢の静脈循環をシュミレーションする装置、特に脚や足の動きの生理機能に
関して「自然」な運動パターンを提供し、動きをほぼそのような動きに限定する
装置を提供することにある。この発明の目的は、簡単に作製でき、その目的に良
く適合し、運搬の容易な装置に対してキーとなる技術を提供することにもある。
この発明の他の目的は、静脈循環をシュミレーションする方法を提供することで
ある。
発明の要約
この発明によれば、簡単で調整された手法で脚や足の可能な動きを三つの期間
、即ち背の屈曲期間、展開期間、および循環期間で行う下肢の静脈循環をシュミ
レーションする装置を開示している。背の屈曲期間と展開期間を組み合わせると
、「自然」な感じとなり、静脈循環を効果的で適当な方法でシュミレーションす
る足の動きとなる。
この発明による静脈循環をシュミレーションする装置は、フレームと、前足部
分および後あるいは裏足部分を有する足支持板と、後足部分に配置された手段と
を備えている。前記手段は、フレームに足支持板を回転可能(旋回可能に)支持
するため後足部分あるいは後足部分と足支持板の間に配置された手段と、フレー
ムおよび足支持板の間に配置された第一圧縮バネ手段と、フレームあるいは足支
持板あるいは両者に配置された足支持板を傾ける揺動部材あるいは傾斜手段とを
有する。後足部分は踵骨の下の足裏部分を受け止めるためにあり、前足部分は足
裏の前(前部)領域を受け止めるためにある。従って、足支持板はフレームに旋
回可能に支持されている。つまり対応する理論的あるいは実際の回転軸は傾斜あ
るいは揺動手段の軸に関して後に(背後に)配置されている。旋回あるいは回転
手段の軸の周りに足支持板を旋回させることは、曲がった(円、楕円等)の表面
(例えば円筒カバーの一部の表面等)の上で展開させることに置き換えることも
できる。この場合、「傾斜」は半径(表面が円形断面なら)あるいは近似的な半
径(円形断面以外の表面なら)が足板の長さに比べて小さいような面の上の傾斜
もどきの展開運動を含む。
この発明によれば、下肢の静脈循環をシュミレーションする方法も開示してい
る。この方法は、二つの連続する期間にあって各期間で一つの軸周りに回転可能
な足板を踏みつけることから成り、第二期間では第一期間で足板が回転する軸に
平行で、この軸に対して前にある軸の周りで足板が回転する。
この発明の方法が下記の構成を備えているいれば好ましい。即ち、
・静脈循環をシュミレーションすべき人物が座位置にあるとする、
・静脈循環をシュミレーションする装置の足板の上側にある隣接領域に対し
て前記人物の足の一方を足裏と共に置き、前記足板が垂線に対して 25 °から 7
5 °傾き、後あるいは裏足部分と前足部分を持っている、
・第一期間で、足支持板をバネ力に逆らって踏みつけ、この足支持板と足は
組み合わせて足の長手軸の方向にほぼ垂直で、隣接部にほぼ平行に配置され、踵
骨の下あるいはその背面の後のところに延びている第一回転軸の周りに回転する
、
・これに続く第二期間で、足支持板をバネ力に逆らって踏みつけ、足支持板
を足と組み合わせて第一回転軸にほぼ平行で、その前(前方)に配置され、隣接
部の面内あるいはその下に延びる第二回転軸の周り揺動する、
・足板とこの足板に載せた足をバネ力の作用により初期位置へ実質上受動的
に戻す。
この発明の方法は、完全にあるいは部分的に横たわった位置の人物でも実行さ
れる。これは装置を正常の位置から約 90 °まで傾けることを要求する。これは
、例えば、ベッドの壁あるいは内端に固定される。この場合、「垂線」はフレー
ムの底の万に垂直であることを意味する。
この発明による装置の他の特徴は、請求の範囲に開示され、この発明の好適で
あるがそれに限定しない多数の実施例の説明から明らかになる。模式的な図面に
関連してこの発明を以下に説明する。

第1図に示すこの発明の静脈循環をシュミレーションする装置は、折り畳んだ
横部分や終端部分と平坦な底を有するプレスされたスチール板の雨樋状のフレー
ム1を含む。フレーム1は、支持、例えば床あるいは足台に対して底の外側で止
めるようにされている。使用時に装置が動くことを防止するため、フレーム1の
下側を波形にするか、あるいは一部または全部を滑り止め材料で被覆すると良い
。しかし、この装置はその底や側壁で支持体に固定することもできる。
後部あるいは後端と呼ばれるフレームの一端の近くに、側壁は垂直スリット7
を有する。このスリットには足支持板2が位置決めされている。足支持板2には
足支持板の後端に固定された横方向に突出するピン8(つまり、足支持板2の面
に平行にただ一つのみ示すが、それ等は足支持板を通過する軸の形状を有する)
がある。ピン8はスリット7内で回転し、スリット内で垂直方向に自由に移動す
る。
プレスされたスチール板(他の材料もフレームと足支持板に考慮でき、例えば
高分子材料、恐らく強化樹脂材料)が下側に固定された二つのバネ部材によって
フレーム1の底の上側に固定される。この上側では足の滑りを防止する材料によ
り足支持板が被覆されている。この装置は、もちろん、素足あるいは靴下または
靴を履いた足に使用できる。後端部では足支持板2に踵支持部(図示せず)が適
当に設けてある。例えば、足を板に保持するためスキーを付ける踵支持部に似て
いるものである。足を足支持板に保持することを助けるベルト、穴あるいは類似
の部材を配置してもよく、足支持板2をスリッパーのように形成してもよい。
第一バネ部材3は後端部から見て足支持板2の長さの約 2/3の点のところで足
支持板に固定されている。この固定点は足支持板の前半分内にあると好ましい。
この第一バネ部材3は圧縮バネ要素である。このため、種々のタイプのバネ部材
を使用できる。例えば、強力なコイルバネ、バネ板、発泡ゴム部材あるいは空気
クッションである。
第二バネ部材4は足支持板2の後端部に近い点で、好ましくは足支持板の後の
5分の1以内でこの板に固定されているとよい。この第二バネ部材4は膨張ある
いは引張バネ要素であり、このバネ要素の目的は足支持板4の後端部をフレーム
に引っ張るためにある。バネ部材4の代わりに、足支持板2の後端部をフレーム
1から外れないようにするのに適した他の手段を配置してもよい。例えば、後端
部(前止めピン8)のことろで上向き(上側へ)の方向へ足支持板2に直接的あ
るいは間接的に働く圧縮バネ(図示せず)を配置してもよい。
足支持板2に対する揺動ヨークあるいは傾斜ヨークとして働く横向きに配置さ
れた棒6はフレーム1の両側の壁の間に延びていて、フレーム1の両側の壁の縦
方向に延びるスリット9(一つのみ示す)内に多数の個所でその端部を移動可能
でロック可能に位置決めされている。スリット9中の棒6の位置決めを、以下に
装置の機能の説明に結び付けて説明する。例えば、スリット9を多数の穴で置き
換えることができ、これ等の穴の対に棒6を挿入できる。各対の穴は棒6に対す
る一つの位置に相当する。棒6をスリット9に固定することは第1図に示してい
ない。しかし、この固定は、例えば棒6の両端にネジ山を付け、フレーム1の両
側の壁に隣接する二つのネジ付きナットで適所に保持することにより達成できる
。以下では、第1図の装置の機能を説明する。
第1図に示す装置の足支持板は、動作サイクルの開始時の位置Aとして実線で
示してある。このサイクルの初期期間、つまり背部を膨らませる期間では、前足
と後足の間にほぼ一様に分配された接触圧力で足を足支持板2に載せる。足支持
板2の前足部分を2a とし、その後あるいは後部足部分を2b とする。足支持板
の前部分2a をその人の前足で下に押して運動が始まる。その場合、前記足支持
板はピン8の周りに旋回し、フレーム1に近接する。この動きは足支持板2の下
側が(場所5で)棒6に接触して位置Bに到達するまで行われる。その点で、足
支持板の負荷を前部分2a に移す。前足で前部分2a に圧力を加え続けると、棒
6の周りに足支持板2を傾けることになり、この棒は第一期間で旋回運動に対す
るストッパーとして働き、第二期間、つまり展開期間で傾斜軸として働く。この
展開期間は徒歩での足裏の展開に似せている。足板2の傾きがピン8をスリット
7内でバネ4の力に逆らって持ち上げる。スリット7の上端で決まる最上位置に
達すると、足支持板2を更に下に押圧することはできない。これは、明らかに利
用者が脚の筋肉を緩めることを知らせ、これは圧縮バネ4が載っている足で足支
持板2を初期位置へ戻すように押すことを可能にする。利用者の視点からこの戻
し期間は一般に受動的である。棒6(傾斜あるいは揺動ヨーク)の適当な位置は
利用者により望むように調整できる。フレームの後端部に棒6がより近く移動す
ると展開期間がより早く始まり、展開角度β(傾斜角度)がより大きくなる。第
一期間の背部の膨らみ角度はそれに応じて減少する。傾斜角度βが 5〜 20 °で
あると好ましい。後足部分2b の長さを前足部分2a の 25 〜 40 %の間で可変
するように、棒6の位置の傾斜を選択するため、スリット9を適当に配置する。
傾斜した棒の隣接点5は前足部分2a を後足部分2b から区別する。
第2図に示すこの発明の装置の実施例は、第一実施例の第二バネ部材4とスリ
ット7を欠いている。ピン8′(機能的に対応する部材を同じ番号で指定する)
はフレーム1の両側部の壁に回転可能に支承されている。傾斜ヨークとして機能
する第一実施例の棒6はフレーム1′の下側に配置されている断面が半円の傾斜
ヨーク6′で置き替わっている。第2図では、これはフレーム1′の下側に固定
されているが、例えば、底板の上側から、底板にあり一列あるいは複数の並列に
配置された一つまたは複数の穴に挿入された一つまたは複数のネジで脱着可能に
固定することにより、この傾斜ヨークをフレーム1′の前または後端部の方向に
移動可能に配設してもよい。そして、フレーム1′の底板に隣接する揺動ヨーク
6′の上側に設けてあるネジ付き穴にネジを螺入させることに固定される。前ス
トッパー10は傾斜動作を制限するために配置されている。
第2図に示す装置の足支持板2′は動作サイクルの初期位置A′として実線で
示してある。このサイクルの第一期間、つまり背部の膨張期間では、前足と後足
の間にほぼ一様に配分された接触圧で足を足支持板2′に載せる。足板2′の前
足部分を2a′とし、後足部分を2b′とする。第一実施例と同じように、動きは
足支持板2′の前部分2a′を押し下げる前足で始まり、足支持板2′をピン8
′の周りに旋回させてフレーム1′に近づける。この動きは、足の接触圧の重心
が十分足支持板2′の前端部へ移動するか、あるいは足支持板2′がフレーム1
′の長い側部に固定されたストッパー11に当たるまで(この方が第2図に示し
あり、ストッパーを第一実施例の棒6と同じようにフレーム1′の縦方向に移動
可能に配置してもよく、これは第2図に示されていない)。こうして、位置B′
に達し、背部の膨張期間が展開期間となる。足支持板2′の前部分2a′に対し
て前足による継続する圧力は傾斜ヨーク6′の周りで足支持板2′を傾けること
になる。この傾斜はフレーム1の前端部あるいは従順なストッパー10が支持体
に接触するまで、傾斜角度β′にわたり、好ましくは 5から 20 °まで継続する
。このストッパーは柔らかい柔軟な材料であって、傾斜動作を減衰させるため圧
縮バネとして機能すると適切である。足支持板2′を更に傾斜できない場合(位
置C′),これは脚の筋肉を緩める時を信号化する。圧縮バネは足支持板2′を
接触する足で初期位置へ押す。利用者の視点からこの戻し期間は再びほぼ受動的
である。
説明したこれ等の実施例では、支持部材、つまりフレームと足支持板はスチー
ル板の中で作製されている。他の材料も使用できる。例えば射出成形されたポリ
スチレンあるいはポリカーボネートあるいは強化高分子材料である。このような
材料は装置の重量を低減するが、経済性のため大きな一貫生産を要求する。この
発明の装置は圧縮バネを多かれ少なかれ完全に圧縮し、足支持板をフレームに平
行でそれに近い位置にロックするように設計すると適切である。そのような折り
畳み可能な設計は、棒6とストッパー11がそれぞれ脱着可能に配置されること
必要とし、この効果のために配置された足支持板2;2′の穴(図示せず)に挿
入可能に設計することにより、足支持板2;2′をフレーム1;1′に対して保
持するように配置されることを必要とする。
第3図と第4図に示す第三実施例には、足支持板22がある。この足支持板は
H字型の板突張23を介してフレーム21に回転可能に支承されている。一方で
、突張23は一対の保持板27で足支持板22の下側に固定された足支持板軸3
5で突張の横棒の一方の対の穴28,29によりヒンジ状に固定され、他方で、
一対の似た保持板26によりフレーム26の上側に固定されたフレーム軸36に
突張23の横棒の他方の対の穴24,30により固定されている。軸35と36
は互いに平行である。保持板27,26は平坦で半円であり、足支持板22の下
側とフレーム21の上側からそれぞれ垂直延び、板22とフレーム21の長い側
面に平行である。突張23は軸36と35にそれぞれ組み込まれたスペーサー2
4,25により突張23と保持板26,27の間に中心合わせされている。
フレーム21への足支持板22の動きを妨げる圧縮バネの機能は、先端で丸め
られたほぼV字状の内部あるいは基礎部分から成る強いバネ線部材を配置するこ
とにより得られ、前記基礎部分は幾分曲がった棒部分33,34と、それぞれ曲
がった棒部分33,34に結び付く直線状の端部39,40とから成る。端部3
9,40は33と34の間の遷移領域と33と39の間および34と40の間の
遷移領域で決まる面に対して約 45 °の角度を成す。バネ線部材の曲がった棒部
分33,34は第一開口と第二開口(挿入方向に見るように)を通過する。第一
開口は軸36,突張23の横棒および開口30,37を有する横棒の半分で形成
されている。第二開口は軸35,突張23の横棒および半分が穴28,29を担
持する横棒で形成されている。曲がった棒部分33,34は足支持板22に対向
する側から第一開口を通過し、次いで足支持板22から離れて対向する側部で突
張23の横棒を通過し、次いで同じ側から第二開口を通過し、次いで同じ側から
第二開口を通過し、その丸い先端(縦方向に曲がった棒部分33,34がつなが
る遷移領域)で、足支持板22の下側に対して接触し、足支持板22で固定され
た目釘31により保持される。V字形に曲がった先端は目釘31を取り巻き、そ
の内側で目釘に接合し、バネ部材が挿入方向とは反対の方向に引き込まれること
を防止する。バネ部材は目釘31の自由端にネジ山を付け、バネ部材を止めるク
ランプ溝をネジ込み、足支持板22に対してバネ部材を挟持して目釘31に固定
することもできる。しかし、原理的には足支持板22にバネ部材を固定すること
は必ずしも必要ではない。このバネ部材が前記板に接触することさえも必要では
ない。バネ部材の自由端部分39,40はフレーム21の上側に接触する。これ
等の部分はフレームに固定ないしは無拘束に配置される。バネ部材はその柔軟性
により軸35,36と突張23の横棒の間に挟持されて保持されるので、足支持
板22と横棒の縁部分38(足の方向に見て後縁部分)および32(前縁部分)
に対向する軸36,35の側に接する。
この図面では、第3図と第4図の実施例は無負荷状態で示してある。圧力が足
支持板22に加わると、この板は先ずフレーム21に接する下後縁部分で形成さ
れる軸の周りに旋回し、足支持板22が第一軸36,あるいはより正確には、板
突張と間隔部材24の穴30,37に合い、前記穴と間隔に対する隣接線である
新しい軸の周りに傾斜する、つまり回転する。正確には、これは穴の円筒状表面
上での展開であるが、実際にはこの動きも穴30,37の直径が小さいため軸の
周りの回転と見なせる。バネ力は傾斜期間中に抵抗を増加させ、適当に設計する
と、足支持板の(過剰な動きに対する)ストッパー部材を取り付ける必要のない
ように調整できる。もちろん、このバネ部材はバネ鋼鉄の曲がった板バネであっ
てもよい。第3図には、間隔部材25は図面を見やすくするため示していない。
例えば、軸36,35はホルダー板26,27に設けた開口の摩擦セットであっ
てもよい。
第5図と第6図には、第3図と第4図に極めて類似しているが、更により少な
い部品を必要とするように単純な利点を持つ第四実施例が示してある。従って、
この機能を詳細に説明するため、第三実施例を参照する。二つの実施例の間の主
要な相違は、第3図と第4図に示した実施例の板突張が第四実施例では排除され
、その機能をバネ部材、より詳しくは、各「横棒」の二つの穴、つまり第一軸5
8と通す穴59と第二軸55を通す穴59を形成するために曲がっているV字型
の内部分のバネ部材が引き受けている点にある。軸55,58を曲げにより形成
さ
れる穴54,59に挿入するため、ホルダー板57と56の開口(図示せず)に
それぞれ脱着可能に組み込むと適切である。フレーム51に関連して足支持板5
2の横変位は足支持板52の後部分のところにあるフレーム51に配置されてい
るガイド60により制限される。参照符号61のところでバネ部材の膨らみは装
置の機能に何ら寄与しない。
第五実施例は第7〜11図に示してある。足支持板72とフレーム71は足裏
の形状に似た形状の点でほぼ同一である。これ等の部材には高さが 20 mmの上に
折り畳んだ外周壁91,92があり、これにより両者はコップ状になっている。
組み立てた状態では、内側のカップの底は互いに対向する。足支持板72の横壁
92はロックのようにフレーム71の横壁91を包む。横壁91の後部分は(参
照符号91のところで)切断されているので、横壁がそこで低くなっている。同
様に、横壁92の後部分も切断されている。強化するため、フレーム71と足支
持板72には、対にして配置された多数の強化リブ73,74,90が設けてあ
る。
第7〜9図では、装置が組み立てた無負荷状態(シュミレーションサイクルの
初期位置)にして示してある。足支持板72とフレーム71は閉じた二重のU字
型のスプリングワイヤループでテンターされている。フレーム71に対して載っ
ているU字部分(ベース84,アーム81)および足支持板72に対して載って
いるU字部分(ベース85,アーム82)の端部はそれぞれ回転軸79のところ
で繋がり、この回転軸の周りに両者は二つの穴83を形成して11/2 回ほど回転
している。スプリングワイヤ81,82,83,84,85の配置は、第9図で
最も良く分かる。部材71,72の前端(91,92のところ)の間の最大のテ
ンターあるいは間隔は足支持板72の下側に対してフレーム71の斜めの後端部
(89のところ)を隣接して決まる。
穴83の間には、突張75は軸79の後端部で旋回可能に組み込まれている。
この軸79の両端はリブ74に配置された穴の仲に保持されている。前端部では
、突張75が軸98に旋回可能に組み込まれている。この軸98の両端はリブ9
0に配置されている穴に支持されている。軸79,98は互いに平行であり、足
支持板72とフレーム71とに平行で、分割面に対して垂直である。
板バネ86はZ状に曲がった一端で突張75の穴87を貫通し、その穴の中に
Z形状にして保持されている。無負荷状態では、板バネ86の主要部は足支持板
72の下側と突張75の上側の間に平坦に保持されている。この主要部は突張よ
り相当長く、板バネの前部分を前記下側に自由に隣接する。この前端部(前側9
3)には、突張75が足支持部材72に対向する側で楔(ある程度の丸みもある
)のように斜にされ、力が突張75の後端部に作用し、支持板72に対する接触
を終わらせるなら、軸98の周りの回転のためにある。
足支持板72をシュミレーションサイクルの初めで押下すると、軸79の周り
に旋回し、突張75と足支持板72は相対位置を不変に維持する。この期間の終
わりは第10図に示してある。足支持板72はそれ以上軸79の周りに回転しな
い。何故なら、突張75の下側がフレーム71の上側のリブの対74の間に着脱
可能に固定された突張ストッパー76の丸い縁部分88により阻止されるからで
ある。
足支持板72の前部分に更に荷重を加えると下に動き続けるが、今度は軸98
の周りになる。足支持板72の踵部分が持ち上がり89での接触が終わる。今度
は、揺動は板バネ86(95のところ)とワイヤーバネ部材のバネ力に逆らって
生じる。この実施例では、前記展開期間には固定終点がないが、この期間は湾曲
に対して増加した抵抗力と足で加えた力の間の平衡により個々に決まる。
穴94(第7図)は軸79,98の組み込みを容易にするためフレーム71の
横壁に形成されている。
足で加える力が減少すると、この装置は第7〜9図に示す初期位置に戻る傾向
がある。
突張ストッパー76は、フレーム71の縦方向に延びるスリット80を通して
延びるネジ77によりフレーム71の上側に固定することにより、シュミレーシ
ョン装置の縦方向に移動可能に配置されている。ネジ77はスリット80の両側
に配置された肩部78に対して頭部分で固定される。こうして、第一期間中の足
支持板の回転角に関して多数の設定が可能になる。突張ストッパー76の後端部
の方向への移動はこの角度を減少させる。
第11図に示すように、突張ストッパー76と突張75には肩部97,96が
設けてある。これ等の肩部97,96は装置をここに示していない輸送位置に固
定する働きをする。この目的を持って、突張ストッパー76はフレーム71に移
動した時に突張75を最早止めないまで、矢印Bの方向に前進する。適当な圧縮
力を加えると、突張75は同時にフレーム71と足支持板72に接触する。この
位置では、フレーム71と足支持板72は互いにほぼ平行であり、上に被せる錠
前(72)と共に缶(71)のように折り畳まれる。突張ストッパー76が突張
