Abstract

瘍効果に著しい効果があることは明らかであったが、同
時に全身的副作用の問題があり、両面を克服できるシス
テムの開発が急務となっていた。そこで本発明者らは、
肝動脈内に大量に投与された抗癌剤を血液ごと肝静脈よ
り選択的に脱血し、活性炭濾過した後に患者へ送血する
システムにより前記抗腫瘍効果および全身的副作用の防
止効果の両面で著しい効果を有することを明らかにし
た。しかしながら本法は図4のように一方の鼡径部より
バルーンカテーテル(脱血管)を肝静脈分離付近へ挿入
し、更に下肢の脱血を行うためにもう一方の鼡径部より
カテーテル(脱血管)を挿入し、これらの血液を活性炭
濾過器およびポンプを介して腋下より挿入したカテーテ
ル(送血管)で心臓へ返血するシステムのため、両鼡径
部、腋下の計3箇所の静脈切開が必要であり、更に肝静
脈血を選択的に脱血するため閉鎖バルーン等で血流を遮
断する操作も加わり、手技が煩雑なばかりでなく患者の
侵襲も大きいため今後本システムの臨床的普及や反復治
療を推進する上で、より簡便なシステム化が望まれてい
た。

ーテルチューブ(1)の気道ルーメン(11)に通じて
いて、気道ルーメン(11)の後端部には導入管(3)
が付設され、その後端のコネクター(4)に接続したシ
リンジから、カテーテルチューブ(1)の気道ルーメン
(11)を通じてバルーン(2)内部に生理食塩水等を
注入できる構造となっている。カテーテルチューブ
(1)には更に先端が封止された脱血ルーメン(9)
が、また先端が解放された送血ルーメン(10)がそれ
ぞれ後端部には分岐チューブ(5)が付設され、その後
端にコネクター(6)が接続されており、脱血ルーメン
(9)は2個のバルーン(2)の間に、送血ルーメン
(10)は後側のバルーン(2)の後ろにそれぞれ1〜
5個の側孔(7)、(8)がチューブ外に開口している
が、側孔(7)、(8)は目的に合わせて位置を変えて
も、また付設せずとも特に制限はない。当然のことなが
ら、本発明は本実施例に限定されるものではなく、選択
的な送脱血を行うためにバルーンにより区画を分離し、
ルーメン数及び側孔位置をコントロールして、様々なア
レンジが可能である。例えば、図2は本発明の他の実施
例を示すもので、2個のバルーン(2)により3区画に
分けられ、チューブ先端へ通ずる送血ルーメン(10)
と、2個のバルーン(2)の間の側孔(7)に通ずる脱
血ルーメン(9)と、後側ぼバルーン(2)の後ろの側
孔(8)に通ずる脱血ルーメン(12)とを備えた5ル
ーメンチューブであり、このようにチューブ外径と必要
血流量の許す限りルーメン数を増やすことができる。次
に図3により本発明による2バルーンカテーテルの使用
方法を説明する。基本的には従来の図4に示した回路と
同様であるが、患者の体内へ挿入されているのは肝静脈
分離付近の上下をバルーン(2)で閉鎖した2バルーン
カテーテルが鼡径部より1本挿入されているのみであ
り、肝動脈(32)から大量投与された抗癌剤(40)
は肝静脈血(41)ごとバルーン間の側孔(7)及び脱
血ルーメン(9)より脱血され、活性炭濾過器(26)
およびポンプ(25)を介した後送血ルーメン(10)
へ再度戻し、側孔(8)からの下肢静脈血(42)と一
緒にチューブ先端より心臓(30)へ送血する。このよ
うに本2バルーンカテーテルを使用すると肝臓癌にとど
まらず流出静脈の分離が容易な腎臓、骨盤内臓器などの
腹部臓器の悪性腫瘍の治療にも本システムを行うことが
可能である。
