唾液の分析方法
本発明は、唾液の処理方法に関するものである。特に、本発明はヘリコバクタ
ー ピロリ(H.pylori)に対する抗原の存在に関する診断試験に使用される唾液
の処理方法に関するものである。
唾液は、口腔内の様々な腺由来の分泌物の複雑な混合物である。多くの構成成
分の中には、様々な酵素、免疫グロブリン及びムチンが細胞破壊片や細菌叢の宿
主と共にある。特に、ムチンは唾液の粘弾性に応答し、保護及び抗細菌特性を有
する潤滑剤として作用する。これらの糖タンパク質の粘弾性は合成中に得られる
タンパク質及び炭水化物組成物に依存するが、唾液の他の成分との相互作用の程

。AU−A−9067676号には、哺乳動物におけるヘリコバクター ピロリ
(H.pylori)微生物による感染の検出手段となる、ヘリコバクター ピロリ(H.p
ylori)
抗原に特異的な粘液分泌物のIgGの検出方法が記載される。相当する文献とし
ては、ウィット(Witt)ら、フロンティアーズ イン ムコサル イムノロジー(F
rontiers in Mucosal Immunology)、1巻、頁693〜696(1991年)が
ある。
ヘリコバクター ピロリ(H.pylori) (カンピロバクター ピロリ(Campylob
acter pylori))陽性患者の唾液中のIgG抗体の存在は、アメリカン ガスト
ロエンテロロジカル アソシエーション(American Gastroenterological Associ
ation)の年次総会の会報でいくらか注意を引いた。1989年の会報でこのよう
な抗体の存在がチン(Czinn)らによって開示された後、ラーセン(Larsen)らが、
1991年5月の会報で唾液中のIgGレベルは抗生物質による治療期間中の治
療の応答の実用的な非侵襲性のマーカーであると結論付けた。1992年4月の
会報では、ランデス(Landes)らは、先の観察結果を確認し、ヘリコバクター ピ
ロリ(Helicobacter pylori)に対する唾液のIgGの測定は、特に他の試験が実
用的でない広範なまたは小児の集団において、ヘリコバクター ピロリ(H.pylo

WO−A−9322682号は、ヘリコバクター ピロリ(H.pylori)に関す
る簡便で信頼性の高いインビトロ試験法を開示する。この試験は、試験される哺
乳動物の粘液分泌物中のIgG抗体と反応する抗原調製物を利用するものである
。
しかしながら、このような診断方法における唾液の使用もまた問題を伴う。こ
れらの診断方法は、唾液サンプルを膜若しくはパッドに結合した抗原、または他
の同様の材料と接触させるものであることが多い。したがって、このような装置
では唾液の流動性(flow-through)が試験の必須要素である。また、実用的な流動
時間が正確で有意義な結果を得るのに必須である。しかしながら、唾液の複雑な
性質、特に微粒子及びムチンの存在のため、個々の唾液サンプルの流動性は、患
者内及び患者間でかなり変動する。
唾液中に存在する微粒子は、表面に簡単に沈着することにより膜に結合し、流
れ時間(flow time)を減少させる。ムチンは、粘弾性を有するため、ゲル様マト
リッ
クスを膜上に沈着させ、これにより液体の流れが抑制されるであろう。唾液の流
動時間は抗原へのIgGの正確な結合に対する必要性と均衡がとれていなければ
ならず、これは、流動時間は早すぎてはならず、また、試験が実用的であるため
の必要性、即ち、流動時間は遅すぎてもいけないことを意味する。抗原が結合す
る膜またはパッドの表面への微粒子やムチンの結合により、流動時間がかなり抑
制されてしまう。
膜へのムチン及び微粒子の沈着に関連する他の問題は、シグナルの発達に関す
るものである。シグナルの発達の一つの好ましい方法は、沈着物によってトラッ
プされるIgG−コロイド金複合体を利用するものである。この方法では、偽陽
性結果が生じるため、最悪の場合には、複合体が自由に通過できず、全膜が複合
体と結合してしまう。
理論的には、ムチンを除去することは可能である。しかしながら、必要な試薬
及び固相媒質では、このような方法は商業的に使用できない。したがって、他の
より経済的な方法が沈着を抑制するのに、即ち、流動時間を向上させるのに必要
である。
現在、唾液は、唾液を綿パッド上に収集した後、緩衝液中に抽出し、多孔性の
プラスチックフィルターで濾過するオムニサル(商標)(OmnisalR)として既知の
システムを用いて処理できる。しかしながら、このようにして製造される液体は
依然として流れ特性が良くない。
驚くべきことに、唾液サンプルの性質、例えば、粘弾性、を変化させることに
よって、流動時間が向上でき、これによりより正確で感受性の高い試験が得られ
ることが発見された。これは、例えば、唾液中に存在するムチンを除去する必要

本発明の第二の実施態様によると、唾液サンプルの性質は、界面活性剤の添加
及び唾液サンプルのイオン強度および/またはpHを調節することにより操作で
きる。これは、唾液サンプルを適当な緩衝溶液中に通過させることにより達成で
きる。当然、唾液の性質がこのようにして調製される際には、粒状物質を除去す
る別の瀘過段階を含むことも有用である。
したがって、本発明の特に好ましい実施態様によると、唾液サンプルを、唾液
の性質を変える瀘過媒質に通すだけでなく、適当な緩衝溶液に通過させることに
よってそのpHおよび/またはイオン強度を調節し、界面活性剤を添加する。
本発明の方法は、唾液中に見付かる特異的な抗体に関する迅速な診断試験に特
に使用できる。したがって、さらなる概念によると、本発明は、唾液の性質、例
えば、粘弾性を変えることにより流れ特性を改善する段階からなる唾液のサンプ
ルにおける抗体の検出方法を提供するものである。本明細書中に記載されている

好ましくは、試験は、ヘリコバクター ピロリ(H.pylori)に特異的な抗体、
通常、IgGの検出を目的とするものである。
このような診断方法は、一般的に、抗原が結合する膜またはパッドからなる診

唾液サンプルは、オムニサル(商標)(OmnisalR)(サリヴァ ダイアグノステ
