X線造影剤としてのキレートの使用

本発明は、水溶性の金属含有キレートの使用に関するものである。
現在、尿路・血管造影およびコンピュータ断層撮影に使用できるX線造影剤は
、もっぱらトリヨード芳香族化合物系に属する化合物である。例えば、アミドト
リゾエート(イオンモノマー)、ヨーヘクソール、ヨーパミドール、ヨープロミ
ド、ヨーペントール、ヨーヴァソール(非イオンモノマー)、ヨークサグレート
(イオンダイマー)、ヨートロランおよびヨージクサノール(非イオンダイマー
)がそうである。
ヨウ素を含有しない非経口X線造影剤は確かに従来公知であるが、かかる化合
物に対する需要がきわめて大きいにも拘らず、これまで実際上の意義を獲得して
こなかった。ヨウ素含有造影剤の欠点は、組成物中に必ず、甲状腺に合併症を引
き起こしかねない遊離ヨウ素イオンが出現することにある。ヨウ素(ヨウ化物)
含有X線造影剤の注入によってヨウ化物投与が著しく増大すると、甲状腺の機能
過剰の結果、甲状腺機能亢進症的な代謝状態に至る可能性がある。結果として、
例えばB.Gl bel『造影剤の実際』Springer-Verlag 刊(1993)、80〜82ページに
述べられている甲状腺発症が起こり得る。
並びに、アレルギー性の副作用が慣用の造影剤のヨウ素分と結び付けられるこ
とはよくある。C.Zwicker、M.Langer、V.Urich、R.Felix の、ランタノイド含有
物質(Gd-DTPA 等)を使った実験『C
Tにおけるヨウ素、ガドリニウムおよびイッテルビウムの造影;生
ら、かかる化合物をもってしてはX線診断法に汎用的に使える溶液は生成できな
いことが判明した。このことでは、上述の化合物の使用も失敗に終わっている。
X線診断法にランタノイド含有造影剤を使用できるようにするには、分量をM
R診断法で通常使われる分量の約10倍に増やさなければならないが、これはとも
かく、この種の造影剤がもはや患者にとって耐えられなくなるという不利な結果
となる。

』1990,A291(1〜2),67〜73ページには、ガドリニウムまたはイッテルビウムを
含有する造影剤が、シンクロトロン放射を適用した上で使用できると述べられて
いるが、選定された濃度に関するデータが明らかにされていない。
本発明の課題は、ヨウ素含有造影剤をX線診断法に使用できるようにすること
であり、詳記すれば、かかる造影剤が人間において相応の適合性を示す点でもす
ぐれているものとすることである。
通常のX線装置またはCT装置で使われているX線放射の代わりにシンクロト
ロン放射を採用すれば、水溶性の金属含有キレートが、ヨウ素を含有しない原子
番号40〜42、50、51、56〜78、80、82および83の元素と共にX線造影剤として使
用できることが分かった。できれば、この造影剤をMR診断法にとって普通の組
成、普通の用量で使用するのが望ましい。シンクロトロン放射の代わりに、単色
のX線放射、ほぼ単色のX線放射または限定されたエネルギー領域より上、すな
わちそれぞれキレート中に含有される金属原子のK境界より上のX線放射も採用
することができる。
シンクロトロンCTにとって新しいこの方法の利点は、第一にヨ
ウ素含有造影剤の代わりにより少ない用量、より高い適合性のMR造影剤を使用
できることで、第二に甲状腺疾患の併発につながりかねない遊離ヨウ素イオンが
介在しないことである。
もうひとつの利点は、高エネルギー放射(K境界33keV のヨウ素の代わりに51
keV のガドリニウムを使用)のもとで作業できることである。放射線エネルギー
がより高いということは、イオン化作用が明らかに低下していて、結果的に患者
の放射線被曝が少なくなることを意味する。
シンクロトロン放射が単色であることによって、それぞれの造影剤のK境界よ
り下とK境界より上とで撮影が行われる減色法を適用すれば、放射線のエネルギ
ーは、従来のX線放射をもって波長分布に基づき可能となるより高い密度でK境
界付近に存在するように設定することができる。これによって、コントラストを
かなり強めることができる。このコントラストの強まる度合いは、意外にも、き
わめて高い浸透度と比較的低い水溶解度を有する造影剤や、溶液中で著しく希釈
した形でしか十分に流動できないほど高い粘稠度を展開する物質さえも使用でき
るほど大きい。その上、これまでX線では獲得できなかった、もしくはいずれに
しても耐えられる用量では達成できなかったほどの新種のコントラスト効果が得
られるのである。
金属含有造影剤として問題となるのは、特に下記の物質である。
キレート化剤DTPA、DOTA、HP-DO3A、EOB-DTPA、BOPTA およびDTPAポリリジン
のランタノイド錯体、およびその他の高分子錯化剤、または錯化剤を含有する高
分子(例えばEP 043863 を参照)。
できれば、高分子化合物の分子量は10000 Dより大きいのが望ましい。
ここでは、例えばGd-EOB-DTPA 等の変性錯体が特に肝臓の造影に
好適であることが判明した。
その後、ビスマス、鉛、タンタル等のより重い元素も同じX線造影剤として有
利であることが明らかとなった。確かに、過去において例えばタンタル含有造影
剤がDE-OS 28 31 524 で検討されはしたが、これも、従来の多色X線放射だけを
対象とし、大抵は水不溶性のタンタル粉末としてでしかなかった。この種の造影
剤であっても、シンクロトロン放射を適用すれば、体内の特殊な構造とその疾患
を検出する上で大いに有利であることが明らかとなった。
I. 細胞外腔の描出
1. GD-DTPA
GD-DTPA は、MR診断法にとって普通の組成物0.5mol/lにおいて、かつMR診
断法にとって普通の用量0.1 〜0.3mmol/体重kgにおいて静脈注射する。
電子ストレージリング(例えばドイツ・ハンブルクにあるDESY、日本・茨城の
筑波大学またはその他の研究所にある5.8GeVストレージリング等)のシンクロト
ロン放射を使って、血管、器官および組織を、その血液含有量、局所潅流および
細胞外腔の割合に応じて、従来可能であったよりはるかに豊かなコントラストで
描出することができた。
撮影装置は、光源としての2.5GeVストレージリング(260mA、5T Wiggler)、
モノクロメータとしての可動シリコンプレート、2エネルギー描出用のGdフィル
タおよびディテクタとしての増幅器から構成される。写真は、32msec間隔でGdの
K境界(51keV)より上で1回、K境界より下で1回、撮影する。両方の写真を
減色した後、血管が描出されることになる。
2. Gd-GOTA
Gd-GOTA は、MR診断法にとって普通の組成物0.5mol/lにおいて
、かつMR診断法にとって普通の用量0.1 〜0.3mmol/kgにおいて静脈注射する。
撮影は、Gd-GTPA について述べた方式に準じて行う。
3. Gd-HP-DO3A
Gd-HP-DO3Aは、MR診断法にとって普通の組成物0.5mol/lにおいて、かつMR
診断法にとって普通の用量0.1 〜0.3mmol/体重kgにおいて静脈注射する。撮影は
、Gd-GTPA について述べた方式に準じて行う。代わりに、1mmol/kgまで注射する
こともできる。この場合は、腫瘍および梗塞について特に高い検出感度が得られ
る。
II. 肝臓の描出
1. Gd-EOB-DTPA
Gd-EOB-DTPA は、MR診断法にとって普通の組成物1mol/lにおいて、かつMR
診断法にとって普通の用量0.01〜0.3mmol/体重kgにおいて静脈注射する。
Gd-EOB-DTPA は肝細胞に吸収されるので、この物質をもってすれば、シンクロ
トロン放射を使った上で肝臓の描出が可能である。これについて、古典的な水溶
性X線造影剤の範囲内、および従来のX線放射の使用下では、肝臓において十分
な造影剤濃度が得られる例は皆無である。
撮影は、注射後10分〜1 時間以内にGd-GTPA について述べた方式で行う。ここ
では、二波長撮影の代わりに唯一のエネルギー(>52keV)だけでも作業するこ
とができる。
2. Er-EOB-DTPA
Er-EOB-DTPA は、MR診断法にとって普通の組成物において、かつMR診断法
にとって普通の用量0.01〜0.3mmol/体重kgにおいて静脈注射する。
撮影は、注射後30分〜1 時間以内にGd-GTPA について述べた方式で行う。ここ
では、二波長撮影の代わりに唯一のエネルギー(>52
keV)だけでも作業することができる。
III. 脈管内腔の描出
1. Dy-DTPA ポリリジン
Dy-DTPA ポリリジンは、MR診断法におけるGd-DTPA にとって普通の組成物お
よび分量において静脈注射する(G.Schuhmann-Giampieri,H.Schmitt-Willich,
