Claims (1)
- 【特許請求の範囲】 1.(a)後部表面と前部表面を有するプレートと、 (b)前記プレートを頭蓋の後頭部に結合する結合手段と、 (c)脊椎の少なくとも2つの椎骨の後部表面に固定的に取り付けられた縦 方向脊椎安定化装置に前記プレートを載置するための載置手段とを備え、後頭部 脊椎後部の固定に使用するための固定装置。 2.前記プレートがさらに、前記プレートを通って互いに左右対称に置かれた少 なくとも2つの傾斜した第一の孔を備え、前記結合手段が前記後頭部に対する前 記プレートの傾斜した単一皮層の係合のために前記第一の各孔に埋め込まれた第 一のねじである請求項1記載の固定装置。 3.前記プレートの前記前部表面が、凹状である請求項2記載の装置。 4.前記プレートが、可鍛性の材料でできている請求項2記載の装置。 5.前記部材の前記後部表面がさらに、前記後部表面の上にあり前記傾斜した孔 7.前記載置台が、前記縦方向安定化装置を前記載置台に結合するための第二の 結合手段によって前記縦方向脊椎安定化装置と係合させるための少なくとも1つ の第二の孔の少なくとも2つの組を有する請求項2記載の固定装置。 8.前記第二の結合手段が、前記載置台を前記縦方向脊椎安定化装置に留めるた め、前記少なくとも1つの第二の孔の各々を貫通して係合する第二のねじを備え る請求項6記載の固定装置。 9.前記載置台が、それを通る2つの第二の孔の少なくとも1つの組を備え、か つ前記結合手段が、前記縦方向脊椎安定化装置を前記載置台に留めるため、前記 第二の孔の少なくとも1つの組の各々を貫いて通した少なくとも1つのワイヤを 備える請求項6記載の固定装置。 10.前記縦方向脊椎安定化装置がロッドである請求項9記載の固定装置。 11.(a)少なくとも1つのプレートを後頭部の最適な場所に結合する工程を も1つの下方向に傾斜した孔の各々を通って第一のねじを挿入し、次に第一椎骨 の外部塊の皮層後部と、前記第一椎骨の内部関節表面と、前記第一の椎骨の下方 に隣接する第二の椎骨の上方関節表面とを、前記第一のねじで貫通することと、 (b)前記縦方向脊椎安定化装置の下端を貫通して含まれる1つ以上の上 方向に傾斜した孔の各々を通って第二のねじを挿入し、次に前記最下端の椎骨の 外部塊の皮層後部を前記第二のねじで貫通することとからなる請求項11記載の 方法。 20.前記椎骨群が、前記脊椎の頸部椎骨である請求項19記載の方法。 21.前記載置手段が、前記プレートの前記後方表面と一体でかつ2つの第二の 孔の少なくとも1つの組を有する隆起した載 置台であり、少なくとも1つの組の各々がそれを通る少なくとも1つのワイヤを 有し、かつ前記結合が前記縦方向安定化装置の上端に少なくとも1つのワイヤを 取り付けることからなる請求項12記載の方法。 22.(a)脊椎の第一椎骨の外部塊の皮層後部と、脊椎の前記第一椎骨の下部 関節表面を貫通して、下向きの角度で少なくとも1つの第一の孔の各々を穿孔す ることと、 (b)前記第一椎骨の下に隣接する第二の椎骨の上部関節表面を貫通して 、下向きの角度で少なくとも1つの第二の孔の各々を穿孔することと、 (c)前記最下端の椎骨の外部塊の皮層後部を貫通して、上向きの角度で 少なくとも1つの第三の孔の各々を穿孔することと、 (d)脊椎の前記第一及び第二の椎骨群を安定化するために、前記少なく とも1つの第一の孔の各々および前記第二の孔を貫通して第一のねじを挿入する こととからなる脊椎後方内部の固定のための方法。 23.前記下向きの角度および前記上向きの角度が、約35度から約60度まで である請求項22記載の方法。 24.前記第一及び第二の椎骨群が、前記脊椎の椎骨群C−2、C−3、C−4 、C−5、C−6、およびC−7からなる群か ら選ばれる一連の少なくとも2つの隣接した椎骨群をなす頸椎椎骨群である請求 項22記載の方法。 25.前記下向きの角度および前記上向きの角度が、約35度から約60度まで である請求項24記載の方法。 26.前記穿孔工程が、2つの第一の孔、2つの第二の孔、および2つの第三の 孔を穿孔することからなり、前記方法が、さらに (e)少なくとも前記第一及び第二の椎骨群の後部表面に沿って、前記2 つの第一の孔の間に、縦方向脊椎安定化装置を置くことと、 (f)前記縦方向脊椎安定化装置を前記椎骨群に留めるために、前記2つ の第一の孔の各々および前記2つの第二の孔の各々を貫通して第一のねじを挿入 することと、 (g)前記2つの第一の孔の各々および前記2つの第二の孔の各々を貫通 して第一のねじを、そして前記第三の孔の各々を貫通して第二のねじを挿入する こととからなる22記載の方法。 27.(A)(a)上端と、(b)下端と、(c)前部表面と、(d)後部表面 と、(e)それを貫通して含まれる、少なくとも1つの下向きに傾斜した孔と、 (f)それを貫通して含まれ、かつ前記部材の下端に位置する、少なくとも1つ の上向きに傾 斜した孔とを備える縦方向脊椎安定化部材と、 (B)1つが最下端の椎骨である前記脊椎の2つの椎骨群に係合させるた め、1つ以上の前記下向きに傾斜した孔の各々の中へ係合する第一のねじと、 (C)最下端の椎骨に係合させるため、1つ以上の前記上向きに傾斜した 孔の各々の中へ係合する第二のねじとからなる脊椎用の脊椎後部安定化装置。 28.前記上向きに傾斜した孔と前記下向きに傾斜した孔が、約30度の角度か ら約60度の角度で置かれている請求項27記載の装置。 29.前記上向きに傾斜した孔と前記下向きに傾斜した孔が、約40度の角度か ら約50度の角度で置かれている請求項27記載の装置。 30.前記部材の前記後部表面がさらに、前記後部表面の上方にありかつ前記傾 斜した孔から延びる高くしたリップを有し、各々の前記ねじが、ヘッドと、前記 ヘッドと一体となったねじ山をつけた縦方向円柱状貫通部材を備える締結ねじで あり、前記ヘッドが (a)前記ヘッドの周囲を規定する外部表面と、 (b)前記高くしたリップの中でのかしめ係合のため、前記外部表面の上 に置かれた少なくとも1つのくぼみと、 (c)前記外部表面と一体となった頂上部分とを備え、前記頂上部分は、 前記縦方向脊椎固定部材および前記椎骨群の中へ前記締結ねじを係合させるため の道具を挿入するソケットを有する請求項27記載の装置。 31.前記縦方向脊椎安定化部材の前記上端がさらに、それを貫通して含まれる 少なくとも1つの中間の(neutral)孔と、前記部材を後頭部に載置するための 載置手段を備える請求項27記載の装置。 32.前記載置手段が、前記縦方向脊椎安定化部材の前記上端を前記後頭部に載 置するため、1つ以上の前記すくなくとも1つの中間の孔の各々の中で係合する 第三のねじである請求項31記載の装置。 33.前記載置手段がさらに、前記後頭部に載置されたプレートを有し、前記縦 方向脊椎安定化部材の前記上端が、1つ以上の前記すくなくとも1つの中間の孔 の各々の中で係合する前記第三のねじによって前記プレートに取り付けられる請 求項32記載の装置。 34.(a)前記縦方向脊椎安定化部材を貫通して含まれる1つ以上の少なくと も1つの下方向に傾斜した孔の各々を通って第一のねじを挿入し、次に第一椎骨 の外部塊の皮層後部と、前 記第一椎骨の内部関節表面と、前記第一の椎骨の下方に隣接する第二の椎骨の上 方関節表面とを、前記第一のねじで貫通することと、 (b)前記縦方向脊椎安定化部材の下端を貫通して含まれる1つ以上の少 なくとも1つの上方向に傾斜した孔の各々を通って第二のねじを挿入し、次に前 記最下端の椎骨の外部塊の皮層後部を貫通することとにより、縦方向脊椎安定化 部材を少なくとも2つのねじで最下端の椎骨を含む前記脊椎の少なくとも2つの 椎骨群に載置することからなる脊椎の後部内部脊椎の安定化のための方法。 35.前記椎骨群が、前記脊椎の椎骨群C−2、C−3、C−4、C−5、C− 6、およびC−7からなる群から選ばれる一連の少なくとも2つの隣接した椎骨 群をなす頸椎椎骨群である請求項34記載の方法。 36.前記椎骨群が前記脊椎の胸部椎骨群である請求項34記載の方法。 37.ヘッドと、前記ヘッドと一体となった縦方向円柱状貫通部材を備え、前記 38.(a)頭蓋の後部表面上の骨折線の上方にプレートを整合させることを備 え、前記プレートは前記プレートの中を通ってもう1つと左右対称に置かれた少 なくとも2つの傾斜した孔を備え、前記骨折線は前記少なくとも2つの傾斜した 孔の間に置かれており、さらに(b)少なくとも2つの傾斜した孔の各々を通っ てねじを挿入し、単一皮層係合のため前記ねじで前記頭蓋の皮層後部を貫通する 開を修復する方法。 41.前記ねじが、互いに集中する角度で係合している請求項40記載の方法。 42.(a)上部壁と、 (b)下部壁と、 (c)前記上部及び下部壁と一体となった後部壁と、 (d)前記上部及び下部壁と一体となった前部壁と、 (e)前記上部、下部、後部、および前部壁と一体となった2つの側部壁 とを備え、前記前部及び後部壁はその中を通って下向きの角度で置かれた少なく
