血液から自由ヘモグロビンを除去する装置と方法
発明の分野
本発明は、全血液又は血液分画から自由ヘモグロビンを除去するためのフィル
タ又は中空繊維膜と、上記フィルタ及び膜を作製する方法と、この膜及びフィル
タを用いて全血液又は血液分画からヘモグロビンを除去する方法とに関するもの
である。
発明の背景
血液製品、特に輸血を目的とした血液製品は、異なった生物学的活性を有する
種々の分子成分のみならず数種のタイプの細胞が含まれる点において不均質であ
る。輸血しようとする患者は、しばしば、1成分(例えば、気体輸送のための赤
血球)のみ必要であって、血液製品中に存在する他の成分は不要なばかりでなく
不都合あるいは有害でさえある場合がある。
特に、自己輸血の分野では、手術の前、間又は後に患者自身の血液を取り出し
、同一の患者に戻す前に純化するに当たって、自己血液を取り戻すことに関する
技術的な挑戦課題がある。提供された血液を用いることに伴う多くの疫学的免疫
学的問題を回避できるので、自己輸血は医学的観点から有用である。しかしなが
ら、開胸手術の処置においては、重要な量の患者の血液(1ないし3リットル)
が、人工心肺と連結した体外回路中に存在することとなる。この血液は、人工心
肺を満たすのに用いる晶質溶液により希釈され、血餅の形成を防止するためのヘ
パリンで十分に凝固防止がなされ、外科的処置の間に損傷された赤血球から引き
出された自由ヘモグロビンとカリウムとを含んでいる。この血液を自己輸血に用
いるためには、望ましくない成分(水、ヘパリン、自由ヘモグロビン及びカリウ
ム)を取り除かなければならない。胸郭手術及び整形手術などの手術中に血液を
取り戻す場合には、典型的には、患者の血液は開かれた創傷から吸引され洗浄処
理さ
れる。この方法で回収された血液は、それが上述した成分に加えて手術により生
成された組織の断片を含むことから、人工心肺から取り戻した血液よりもかなり
多く汚染されている。
汚染物を取り除くのに現在2つの技術が用いられている。すなわち、第1の技
術は、洗浄液やそれに関連する遠心分離器などの機器によるものである。遠心分
離を行うシステムが機械的に複雑であるのに加えて、この技術では、後で取り除
かなければならない緩衝剤溶液をつけ加えることが必要であり、かつ、全ての血
漿蛋白が除去された赤血球分画をもたらす。第2の技術は、一端の閉じた濾過に
基づくものであるが、広く血液ふるいと血液成分フィルタの2つの階層に分割で
きる。血液ふるいは、穴の大きさが非常に粗く、穴の大きさが最小のものよりよ
り細かい場合には、急速に詰ってしまう強い傾向を持っている。血液ふるいは、
骨片、組織片、凝塊、血液からの同様なものなど大きな粒子を取り除くのに主と
して用いられる。血液成分フィルタは、白血球などの選択された自然の血液成分
を取り除くのに主として用いられる。血液成分フィルタではヘモグロビンのよう
な小さな分子を取り除くのは不可能である。
手術のための自己血を用いた輸血に付け加えて、蓄えた血液を再構成すること
に、小さな分子を取り除くことのできる血液フィルタの第2の大きな効用が見出
される。パックした赤血球を保存するのに通常用いる方法の1つは、−80℃で
冷凍した赤血球を貯蔵するものである。この処理には、凍結阻止剤(通常グリセ
リン)の添加、赤血球の冷凍、解凍、そして最後に凍結防止剤の除去のための赤
血球の洗浄とが含まれる。冷凍と解凍とにより、赤血球のリーシスと自由ヘモグ
ロビンの開放とが惹起される。
全血液と血液分画中の自由ヘモグロビンは、その毒性、特に腎臓に対する毒性
による重篤な合併症故に、望ましくはないものである。したがって、全血液及び
血液分画中から自由ヘモグロビンを除去する装置と方法とを得ることには利点が
あるであろう。
血液フィルタの第3の効用は、自然の人間のヘモグロビンのみならず、血液分
析に先立って血液の試料から化学的に改変したヘモグロビンをも取り除くことに
ある。
アフィニティクロマトグラフィによりヘモグロビンを溶液から得ることができ
ることが知られている。例えば、シア(Hsia)に付与されたアメリカ合衆国
特許第4,925,574号には、種々のヘモグロビンリガンドが付着したアフ
ィニティクロマトグラフィのためのアガロースゲルが開示されている。このアガ
ロースゲルは、ヘモグロビンを得ることが目的であるときには有用な技術である
ように思われる。(a)ゲルは機械的に不安定であり、(b)ゲルは細胞様成分
に浸透することができず、(c)多くのゲル基質は、リーシスを誘発しヘモグロ
ビンを解放する(下記参照)ので、血液を得ることが目的である場合に全血液又
は血液分画からヘモグロビンを取り除くのに用いることはできない。
それ故、全血液又は血液分画から迅速にかつ効率的に他の破片とともにヘモグ
ロビンを除去する方法を得ることは極めて望まれることである。
〔発明の概要〕
本発明の目的は、全血液または血液分画から、速く安全に、かつ溶血を起こさ
ずにヘモグロビンを取り除くためのフィルタを提供することにある。
さらなる目的は、血液または血液分画から白血球、ヘモグロビンおよび屑を取
り除くためのフィルタを提供することにある。
さらなる目的は、商品化可能なフィルタにヘモグロビン・リガンドを付着させ
るための方法を提供することにある。
さらなる目的は、全血液または血液分画から遊離ヘモグロビンを取り除くため
の方法を提供することにある。本発明の優れている点は、前述の目的が速く安全
に、かつ安価に達成されることである。さらなる優れている点は、RBC分画か
ら血漿蛋白質が約25%しか失われないことである。
これらの目的、特徴および優れた点は、
全血液または血液分画からヘモグロビンを取り除くためのフィルタが、厚さが
1mmから8mmで、かつかさ密度が0.05g/cm3から0.4g/cm3である形状
を保持して置かれた織物ウェブを含み、そのウェブが、
(a)平均0.05デニールから0.75デニールで、かつ平均長さが3mmか
ら15mmの複数のかみ合った織物用繊維を含み、その織物用繊維は、かみ合った
繊維の隣接するすき間のスペースに織物用繊維のマトリックスを形成するように
前記ウェブにほぼ均等に分布され、かつ
(b)前記マトリックスのスペース内にほぼ処理されてグラム当たり5m2か
ら60m2の表面積を有する表面改修されたポリマー材料の複数の原繊維からな
る粒子を含み、その原繊維粒子は、それらが血液のろ過中に前記ウェブからほと
んど移動しないように、前記マトリックスのスペースの隣接の織物用繊維とかみ
合わされた複数の微細な原繊維を有し、
原繊維粒子の織物用繊維に対する重量比が1:99から40:60の範囲にあ
り、かつ前記表面改修された材料が、ヘモグロビンのリガンドに共有結合的に連
結された溶血性不活性なポリマーであり、さらに前記織物用繊維が溶血性不活性
で、かつアルカリ加水分解に対し安定であることを特徴とするフィルター、に関
する一面を有する本発明により実現される。
好ましい実施例によれば、表面改修されたポリマー材料は、セルロース・エス
テルであり、最も好ましいのはセルロース・アセテートである。ヘモグロビンの
リガンドは、イノシトール・ヘキサホスフェート、アデノシン・トリホスフェー
ト(ATP)、ピリドキサール・ホスフェート、2,3−ジホスホグリセレート
(DPG)、アデノシン・ジホスフェート(ADP)またはアデノシン・ホスフ
ェートでもよい。好ましくは、リガンドはATPまたはDPGである。リガンド
は、ジヒドラジドまたはジアミン連鎖、好ましくはアジピック・ジヒドラジドを
介してセルロース・エステルに付着されてもよい。フィルタの織物用繊維は、ポ
リオレフィン、ポリアミド、ポリスルホン、ポリエステル、ポリビニル・アルコ
ールおよびポリ(エチレン−ビニル・アルコール)コーポリマー繊維のうち少な
くとも1個でよく、ポリオレフィンまたはポリオレフィンに包まれた繊維が好ま
しい。
さらなる一面によれば、本発明は、ヘモグロビンフィルタを製造する方法に関
し、その方法は、
(a)(1)アルカリ劣化に強い複数の繊維と、(2)複数のセルロース・ア
セテート繊維とを含む形状保持ウェブを準備するステップと、
(b)セルロース・アセテート・エステルの一部を対応する遊離ヒドロキシル
に加水分解するために、前記ウェブを水性基により処理するステップと、
(c)前記遊離ヒドロキシルを、アミン反応残留物を生成するために、活性化
するステップと、
(d)前記アミン反応残留物を、ジアミンまたはジヒドラジドと反応させるス
テップと、
(e)生成したアミンまたはヒドラジドを、アデノシン・トリホスフェート(
ATP)または2,3−ジホスホグリセレート(DGP)の活性化された誘導体
と反応させるステップと、
を含む。
一般的な方法の独特な実施例によりヘモグロビンフィルタが製造され、その方
法は、
(a)厚さが1mmから8mmで、かつかさ密度が0.05g/cm3から0.4g/c
m3である形状を保持して置かれた織物ウェブを準備し、そのウェブが、
(1)平均0.05デニールから0.75デニールで、かつ平均長さが3mmから
15mmである複数のかみ合ったポリオレフィン繊維またはポリオレフィン包み繊
維とポリエチレン・テレフタレート(PET)繊維の混合繊維を含み、その繊維
はかみ合った繊維の隣接するすき間のスペースに繊維のマトリックスを形成する
ように前記ウェブにほぼ均等に分布され、かつ
(2)前記マトリックスのスペース内にほぼ処理されてグラム当たり5m2から
60m2の表面積を有する複数の原繊維化されたアセテート繊維粒子を含み、該
原繊維粒子はそれらが血液のろ過中に前記ウェブからほとんど移動しないように
、前記マトリックスのスペースの隣接のポリオレフィンまたはPET/ポリオレ
フィン繊維とかみ合わされた複数の微細な原繊維を有し、さらに原繊維粒子のポ
リオレフィンまたはPET/ポリオレフィン繊維に対する重量比が1:99から
40:60の範囲にある、
織物ウェブを準備するステップと、
(b)アセテート・エステル繊維の一部を対応する遊離ヒドロキシルに加水分解
するために、前記ウェブを水性基により処理するステップと、
(c)前記遊離ヒドロキシルを、アミン反応残留物を生成するために活性化する
ステップと、
(d)前記アミン反応残留物を、ジアミンまたはジヒドラジドと反応させるステ
ップと、
(e)生成したアミンまたはヒドラジドを、アデノシン・トリホスフェート(A
TP)または2,3−ジホスホグリセレート(DGP)の活性化された誘導体と
反応させるステップと、
を含むものである。
一実施例によれば、ウェブは、1N水性のナトリウムまたはカリウムの水酸化
物により、20℃から40℃で8時間から312時間処理される。変化するパラ
メータは、例えば、水酸化ナトリウム溶液の濃度、塩基の性質(例えば水酸化ナ
トリウム)、温度および時間で、ポリマー材料の必要な改修を行うために用いる
。
遊離ヒドロキシルを直接または間接的に活性化する方法は、過ヨウ素酸塩、臭
化シアン、カルボニル・ジイミダゾール、ジビニル・スルホン、アズラクトン、
塩化スルホニル、ジエポキシ樹脂、ジハロゲン化物、ハロエポキシ樹脂、2,4
,6−トリクロル・トリアジン、2−フルオロ−1−メチルピリジウム塩、ジス
ルホニル塩化物、二酸化塩化物、三酸化塩化物、2−エトキシ−1−エトキシカ
ルボニル−1,2−ジハイドロキノール、ジイソシアン酸塩、N−ヒドロキシス
クシンイミドおよびカルボジイミドによって導かれたハロアセテート酸との反応
や、ほかの同様なプロセスを含む。遊離ヒドロキシルを活性化する好ましい方法
は、臭化シアンとの反応および過ヨウ素酸塩との反応を含むものである。アミン
反応残留物は、過ヨウ素酸塩−酸化ATPまたはカルボジイミド活性DPGとの
反応による、複数のN−1置換体のヒドラジドまたはジアミンを生成するために
、アジピン酸ジヒドラジドまたはエチレンジアミンと反応させることが好ましい
。
本発明のさらなる一面は、血液を前述のフィルタを通して通過させるステップ
を含む、全血液または血液分画からヘモグロビンを取り除く方法に関するもので
ある。
さらなる一面によれば、本発明は、
血液分画または全血液からヘモグロビンを取り除くための中空繊維または平らな
シート状膜であって、
(a)基本疎水性ポリマー成分として、機能しうるフェノール連鎖の末端を有す
るポリエーテルスルホン(PES)と、
(b)エチレン・グリコール・ジグリシディル・エーテル、1,4−ブタンジオ
ル・ジグリシディル・エーテルおよびエピクロロヒドリンからなるグループから
選択された1個のオキシレンから誘導された第1の連鎖であって、複数の前記フ
ェノール連鎖端に付着された第1連鎖の半分と、
(c)複数のヘモグロビンのリガンドに共有結合型に付着された溶血性不活性の
ポリマーからなる表面改修されたポリマー材料であって、複数の前記第1連鎖の
半分に付着された表面改修されたポリマー材料と、
を含む中空繊維または平らなシート状膜に関するものである。
第1の実施例によれば、表面改修されたポリマー材料は、第2連鎖の半分を介
して複数のヘモグロビンのリガンドに供有結合型に付着されたヒドロキシアルキ
ルセルロースである。第2の実施例によれば、表面改修されたポリマー材料は、
第2連鎖の半分を介して複数のヘモグロビンのリガンドに共有結合型に付着され
たポリエチレンイミンである。リガンドの好例は、前述のとおりである。好まし
い実施例によれば、リガンドは、ジヒドラジド第2連鎖の半分を介してポリエチ
レンイミンに、または、ジヒドラジドまたは、ジアミン第2連鎖の半分を介して
ヒドロキシアルキルセルロースに付着され、第2連鎖の半分がアジピック・ジヒ
ドラジドまたはエチレンジアミンが好ましい。
さらなる一面によれば、本発明は、血液と前述の平らなシート状または中空繊
維の膜との間に流動接触を作るステップを含む、全血液または血液分画からヘモ
グロビンを取り除くための方法に関するものである。

好適な実施例を含む詳細な説明
本発明は、1実施例において自由ヘモグロビン、白血球及び断面で約8μmよ
り大きいその他の断片又は構成要素を同時に除去するためのフィルターを、他の
1実施例において膜と装置とを適切に設計してあれば同様な粒子サイズを対象と
する機械的炉過に使用され得るであろう主として自由ヘモグロビン除去用の中空
繊維又は平面シート状膜を提供する。膜は、又適切な調整を行えれば、ヘモグロ
ビン及び自由ヘモグロビンの存在水溶液の構成部分の両者を除去するのに使用可
能であろう。後に明らかになるように、フィルターと膜とは、又、リン酸エステ
ル緩衝剤処理サリン(PBS)又は他のリン酸エステル含有希釈液でフィルター
又は膜に捕縛されたヘモグロビンを移動させてヘモグロビンを生き返えらせるの
にも使用可能であろう。
このフィルターは米国特許第5,190,657号広報の中に記載のフィルタ
ーの調整品であり、この特許の開示をこゝに引用することによって本発明の記述
にむすび付けるものとする。この特許の図面が明確化の目的で以下の記述の中で
使用される。短的に云えば、フィルターは形状を維持するように配された織物の
フィルター材料から成る。図1に示すように、織物はフィルターを形成するよう
円形状に切断され、円筒形フィルター担体の中に負荷されるのに適したものとさ
れている。
織物の厚みは、少なくとも1mmであり、最も望ましいのは2mmで、約8mmにも
達し得る。置かれた織物の密度は約0.05から0.4g/cm3の間にある。
図1のフィルターの断面の一部を極端に線図的に図解している図2見られるよ
うに、フィルター材料は複数の基質織物繊維5から成り、これらの織物繊維は平
均して約0.05から0.75デニールの太さを有する。繊維の少なくとも60
%、好ましくは少なくとも70%、もっと望ましいのは少なくとも80乃至85%
は上記の範囲内の太さと12,000乃至180,000m/gの長さとを有し
て欲しい。
図2に見られるように、織物繊維は、織物繊維の基質を形成するべく織物を通
して実質的に均一に分布される。基質はかみ合わされた繊維の隣接するすき間6
の間にある空間7を有する。これらの空間の中には、非常に大きな表面積を有す
る複数の原繊維からなる粒子10が存在する。この原繊維からなる粒子10は、
基質織物繊維5の中及びそれ沿いに配置されるのと同様に空間7の中にも配置さ
れる。
基質織物繊維5の一部は上記と同じ太さ及び長さのさやと芯とを持ち得る。さ
やは低融点温度のポリマーのものであり、芯はより高融点温度のポリマーのもの
である。フィルター材料の基質織物繊維の少なくとも一部がさや/芯繊維の場合
には、フィルター材料の織物は、ポリマーのさやを軟化して織物繊維同志が幾分
くっつき合うように、熱を受けることになろう。一般には、基質織物繊維の5乃
至35%がさや/芯構成である。
基質織物繊維は、普通、ポリオレフィンの又はポリオレフィンのさやの付いた
繊維、ポリアミドやポリスルホンやポリエステルやポリビニール アルコールや
ポリ(エチレン ビニール アルコール)共重合体の繊維のような、合成ポリマー
繊維である。ポリオレフィン繊維は、アルカリ性加水分解に対して極めて強い抵
抗力を有し、それ故に極めて好ましいものである。織物繊維と原繊維との両者の
限界特性は血液に適合し得ることであり、特に、重大な溶血反応の原因とならな
いことである。臨床状態においては、自由ヘモグロビンに10%より大きな増加
をもたらす溶血反応は重大であると考えられよう。
使用されるとすると、さや繊維は上記の織物繊維材料の芯を有し、さやはいづ
れかの低融点のポリマーである。ポリエチレン又はポリプロピレンのようなポリ
オレフィン ポリマーが望ましい。その理由は、ポリオレフィン ポリマーが比較
的低融点を提供し、所望の接着を提供するために容易にさやが軟化されることに
ある。加えて、ポリオレフィン ポリマーは、アルカリ性加水分解に対して抵抗
力を有する。さやは、普通、芯直径の5〜30%とされるであろう。
原繊維からなる粒子は、ポリエステル繊維材料、アクリル繊維材料、ナイロン
繊維材料、ポリオレフィン繊維材料又はセルロース繊維材料である。酢酸セルロ
ースが通常使用されるが、その理由は、非常に数多くの原繊維がこの材料で生産
され、この材料は天然の親水性を有するからである。
以下の実施例に使用されるフィルターは、リダル インコーポレイテッド(L
ydall Inc.)から市販されているもの(マンチェスター CT)を利用
でき、プリプロピレン繊維と酢酸セルロース“ファイブレッツ(fibrets
)”とから構成される。一般的に、酢酸セルロース成分と塩基加水分解に抵抗力
を有する形状維持織物とから成るどのフィルターでも本発明の方法の中で機能す
る。
リダルのポリプロピレン/酢酸セルロースのフィルター パッド(厚さ1.9m
m、型式825B)は時間長を変更するために直径90mmの円板をアルカリ水溶
液の中に浸すことによって調整される。フィルター パッドは洗浄が中性に近づ
く迄フィルター上で広範に洗浄され、それから30°乃至40℃で空中乾燥され
る。この加水分解が酢酸セルロースを自由ヒドロキシル基包含セルロースに変換
する。加水分解から充分なときには、続く反応で非常に少数のヒドロキシル基を
利用できるに過ぎない結果に終り、それ故に結果としてのヘモグロビン炉過能力
を低下させる。一方、広範な加水分解はセルロース基質の減成をもたらす結果と
なる。1実施例においては織物が水酸化ナトリウム水溶液で加水分解される。温
度と水酸化ナトリウム濃度と処理時間との変化が加水分解の範囲及びセルロース
基質の減成に影響を及ぼすことは、当業者なら認識するであろう。一般に、処理
条件には、60℃より近い温度での0.1N超5N未満の水酸化ナトリウム濃度
を含み、約8時間乃至13日間の処理時間が白血球除去効率及び赤血球(RBC
)通過効率で示される過剰なセルロース基質の減成のない適切な加水分解を提供
する。この特殊なパッドに対しては、室温で8時間超312時間迄の1Nの水酸
化ナトリウム溶液処理が適切な加水分解と受容可能なセルロース基質の減成とを
提供する。水酸化カリウムや水酸化リチウム等のような他の塩基が同様に機能す
ることが期待される。
“加水分解されたパッド”、即ち、セルロースの自由OH機能性の保持は、普
通に知られている方法のいづれかによってでも活性化され得る。グレッグ T ハ
ーマンソン、A.クリスナ マリア及びポール K.スミスの固定された親和力の リガンド技術
、アカデミック プレス インコーポレイテッド、カルフォルニア州
サン ジェゴ発行、(1992年)の第51〜132頁と、P.D.G.ディー
ン、W.S.ジョンソン及びF.A.ミドルにより編集された親和力クロマトグ ラフィ
、実践的アプローチ、第31〜59頁、IRL出版社、エンシャム、オク
スフォード0X81JJ,イギリス(1987年)と、その開示を引用すること
によって本発明に関連付ける米国特許第3,389,142号広報とを参照され
度い。2つの好適な方法としては、臭化シアン活性化法と過沃素酸塩活性化法と
が含まれる。
臭化シアンは、セルロースのビシナル ジオールと反応して炭酸イミド及び/
又はシアン酸塩中間生成物を提供する。この中間生成物は、求核性試薬攻撃に向
けて大いに活性化され、続いて第1アミンを包含するリンカー或いはリガンドと
反応し得る。この反応の結果、リガンド又はリンカーがカルバミン酸エステルを
介してセルロースに共有結合の形で添加される。活性化反応はアクセン他による
〔自然214、第1302〜1304頁(1967年)〕及びカトレカサス他に
よる〔米国自然アカデミー協会誌61,第636〜643頁(1968年)〕に
記載の方法或いは僅かに調整された方法によって実行される。
過沃素酸塩活性化法は過沃素酸塩誘導のシビナル ジオールのアルデヒドへの
酸化的開裂を包含し、アルデヒドはリンカー又はリガンド内の第1アミンに向け
て同様に反応的である。硼水素化シアン ナトリウム又は同様な還元剤による還
元ステップが、やや加水分解的に不安定なシフ塩基をアルキシルアミンに変換す
るために通常は採用される。これらの反応は当業者にはよく知られている。
リガンドがヘモグロビンとの相互作用に対して必要ではない第1アミン基を包
含する場合には、原則としてリガンドは上記のように活性化セルロースに直接的
に添加され得る。しかしながら、多くの場合、アミン包含リンカーがセルロース
とリガンドとの間の架橋を提供するのに採用されるであろう。リンカーを使用す
るいくつかの理由が存在する。即ち、(1)リガンドは使用可能な第1アミン基
を有し得ないし、(2)安定した共有結合を提供する化学作用はリンカー上でよ
り容易に実行され得るし、(3)標的分子(ヘモグロビン)上の結合部位により
良くアクセスし得るような或る程度の移動性を伴ったリガンドの提供が望まれ得
る。この観点では、リンカーはリガンドと比較的固いポリマー(セルロース)の
中枢との間の一種のつなぎ綱として機能する。以下に記述する多くの実施例にお
いては、アジピン酸ジヒドラジド(adipic dihydrazide)が
リンカーとして採用されて来たが、いづれかの2重機能的アミン(例えばヘキサ
メチレンジアミン)又はジヒドラジンが使用可能であろう。
ヘモグロビンが小さな多価陰イオン分子、特に2,3−ジホスホグリセリン酸
(DPG)分子及びアデノシン三隣酸(ATP)分子と特別に結合することは知
られている。これらの分子はフィルターのセルロースに添加するためのリガンド
として特に魅力的であるけれども、本発明はDPGやATPに限定されるもので
はない。現在知られている或いは後で発見されるその他のリガンドは同様に機能
するものと期待される。ATPの場合、リボース残分はセルロースでの場合と同
様に臭化シアン又は過沃素酸塩で活性化され得る。好適な方法は、レイムド他に
よる〔生化学と生物物理学、論文a 304、第231〜235頁(1973年
)〕に記述されている過沃素酸塩酸化であり、この論文を引用することによって
本発明と結合させることにする。活性化されたリガンドは、それから、活性化セ
ルロースのリンカーとの反応に関して上述したのと同様に、リンカーのアミン機
能性と反応させられる。DPGの場合、カルボキシル酸残分はカルボジイミドで
活性化され得る。
人間のヘモグロビンに対する既知のリガンドの中には、ATPとDPGとが好
ましく、その理由はその妥当な結合定数と低毒性とにある。その他の可能性のあ
るリガンドには、イノシットヘキサリン酸、ピリドキサル隣酸、ADP及び隣酸
塩がある。ピリドキサル隣酸とイノシットヘキサリン酸とは、フィルター炉過の
必要がある場合にはその毒性が問題を提起するので魅力が薄れる。ADPとアデ
ノシン単隣酸とは、人間のヘモグロビンに対する親和性の低さがリガンドとして
使用しているフィルターの効率低下に反映されるが故に魅力が乏しい。
ヘモグロビン除去フィルターの生成に適用された前述の化学も又、ヘモグロビ
ン除去用の中空繊維又は平面シート状膜の生成に適用可能である。この場合、中
空繊維膜が米国出願の特許第07/956,432の方法、特に以下に記述する
第75〜77頁の方法に従って準備される。即ち、
ドープ準備手順は混合状態にされた2つのポリマーの計量と前処理とを包含す
る。(ICIアメリカからビクトレックス(Victrex)グレード5200
Pとして市販に供されている)ポリエーテルスルホン(PES)は、150℃の
炉の中で3時間乾燥されてから更に数時間60℃に迄冷却され得る。炉中の加熱
時間の合計は約24時間よりは少なくない。
酸化ポリエチレン(PEO)(即ち、ユニオン カーバイド コーポレーション
からのポリオックス(Polyox)301,MW4000KD)は真空炉内で
室温約24時間の前処理を受ける。混合前の経過期間中、開放空気中に前処理剤
のポリマーを残さないよう注意が肝要である。
N−メチルピロリドン(NMP)(即ち、GAF化学会社からのNピロール(N
−Pyrol)カタログ番号1−3−72755/000及び5−72)と(バ
ックスター サイエンティフック プロダクト グループ、マリンクロッドのカタ
ログ番号#5092−4の分析試薬の)グリセリンとの両者は受け取られた儘使
用されるが、それぞれの包装から取り出したら直ちにミキサーに入れることによ
って大気中の濕気の吸収を最小限に抑えるよう予防策が講ぜられる。
NMP(2812g)とグリセリン(1128g)とは、室温でロス(Ros
s)ミキサー(モデルPVM2)の中に入れる前に4リットルの容器内で予備混
合する。ロス ミキサーは窒素清浄化のために窒素源に接続される。PEOが加
えられる迄全ての液体の上が不活性雰囲気で覆われる。予備混合されたNMP/
グリセリンをロス ミキサーに供給すると同時に2枚の混合ブレードがスタート
する。即ちアンカー ブレードは135rpmで、散乱ブレードは3,500r
pmで回転する。PEO(360g)が約1分間に亘って室温混合中に加えられ
る。NMPの500gの量がそれから加えられてドープ中のNMPの全体が33
12gに達する。この時点で散乱ブレードはスイッチ オフされ、モーコン(M
okon)熱交換器ユニットが120℃にセットされる。3時間混合後PES(
1200g)を2〜3分間に亘って加え、アンカー ブレードを135rpmに
維持した儘温度を記録する。更に18時間経ってから、モーコンを60℃にセッ
トしてしっかりした温度降下を開始させる。この温度変更を行って約1時間半以
内にドープは通常約75±5℃の温度に達し、その時点で混合剤のガス抜きのた
め除々に真空が加えられる。最終的な真空が通常15分以内に達成され、更に5
分間真空が保たれる。ミキサーはそれからスイッチ オフされるが、ガス抜きは
尚続けられる。混合容器の中に60℃の雰囲気圧力を再確立するための窒素導入
を行う前に尚1〜2分間長く真空が保持される。
この準備手順は典型的にはドープ粘度が60℃で約100,000(±20,
000)cpsになる結果をもたらす。しかしながら、この標準からたまたま外
れたとしても膜特性には如何なる悪影響も現れない。このようなドープは約78
℃(LCST)と約57℃(UCST)とに相境界を有する。
上述の如くに準備され60℃で123,000cps粘度を有するドープは、
同軸押出し紡績突起を介して押し出される。紡績突起の温度は実験期間を通して
80℃に維持される。他の固定パラメータには、望ましくは、次のものがある。
即ち、
・ドープ ポンプ速度----------約70rpm
・クエンチ浴温度-------------約90℃
・クエンチ浴組成-------------脱イオン(DI)水
・環内流体組成---------------約70%NMP:30%DI水(V/V)
・環外流体組成---------------約70%NMP:30%DI水(V/V)
・環内流体流量---------------約30.2(±1.2)ml/分
・第1及び第2ゴデット浴温---約42.5(±2.5)℃
他の可変パラメータには、クエンチ浴上の空隙乃至は紡績突起の高さ(これは
直線フィート/分単位の繊維取り出し速度乃至は繊維生産速度の変化をもたらす
。)と環外流体流量とがある。後者は、紡績突起高さ範囲7.5乃至18cmに対
してゼロから66ml/分迄変化し得る。環外流体流がゼロで生産される繊維は
、従来型のチューブ内オリフィス式紡績突起で生産される繊維に相当する。環外
流体量が10ml/分では2乃至3×10-7cm3/dyne−secの低点を有
する中空繊維が生産される。結果的にヒドラジド鑑定誘導体に変換された中空繊
維は、それからリガンド添加のため上記のように処理される。
以下の実施例は、ヘモグロビンに対するリガンドが、ゲル独特の信頼性、即ち
機械的安定性の欠如を克服する助けとなっている(いわゆるハイパーDTHベッド
と云われる)シリカ基質の中に支持されているアミン調整メタクリルアミドゲル
に添加される実験を含んでいる。これらの実験は、先行技術を説明するものでは
なく、本発明を定義付ける方向に向けられた実験である。下記のテーブルIVに見
られる如く、自由ヘモグロビンのレベルを減成させるよりもむしろ増大させてし
まっていて、アミン調整メタクリルアミドが溶血反応を誘発してしまったよう
に見えるので、結果は不満足なものであった。
実施例
調整基質の準備
A.ATPを共有結合形態に添加したフィルターの準備
ポリプロピレン/酢酸セルロースのフィルター パッド(厚さ1.9mm、型式
825B,リダル ウエステックス、POボックス109、ハンプトンビル、N
C27020)は、直径90mmの円板を1Nの水酸化ナトリウム溶液中に浸して
室温で44時間の調整を受ける。フィルター パッドは洗浄が中性に近づく迄フ
ィルター上で広範に洗浄され、それから30℃で空中乾燥された。
1.臭化シアン(CNBr)活性化法
非加水分解パッドと1Nの水酸化ナトリウムの中で8時間加水分解されたパッ
ドとが開始素材として使用された。1gのフィルター パッド当たりの2gのC
NBrの比率(4gのCNBr/直径90mmの円板)を保つようにして標準手順
が使用された。フィルター パッドは500mlのナルゲン(Nalgene)
プラスチック フィルター容器の中に置かれ、フィルター容器の底即ち多孔質膜
は流量改善のために取り除かれた。フィルター パッドは炉過を繰返してCNB
r処理がなされた。CNBrで活性化されたパッドは、それから0.1Mの炭酸
ナトリウム緩衝剤(pH9.5)の中の(約90g/lの)アジピン酸ジヒドラ
ジド(adipic dihydrazide)の飽和溶液と反応させられるが
、この反応は、レイムド他によってアガロース ヘッドに対して記述されている
様に、4℃で一晩中続行することが可能であった。ヒドラジド処理パッドは更に
以下に述べるように鑑定誘導体に変換され得る。
2.酸化法
10個の90mmパッドが底部膜を除去されたナルゲン プラスチック フィルタ
ーの上で室温で3時間、1lの0.5Mのメタ過沃素酸ナトリウムと反応させら
れた。パッドは、水で広範に洗浄され、それから(アルドリッチ ケミカル社、
ミルウォーキー、WIの)2%アジピン酸ジヒドラジドとpH7.4の下で4時
間反応させられた。各1lのヒドラジド溶液に対して、ほぼ10個のパッドが処
理された。
4時間経った時に固体の(0.1モルの)硼水素化シアン ナトリウムが溶液に
加えられて一晩中反応させられ、最後にパッドはナルゲン フィルター上で広範
囲に水洗された。
(0.01Mの)ATP5.5gが室温で1lの脱イオン水に溶解され、10
Nの水酸化ナトリウム溶液を使ってpHを4.5に調整された。3.2gの(0
.015Mの)メタ過沃素酸ナトリウムが別のビーカーの中の500mlの水に
溶解され、pHを4.5に調整された。露光を最小限に抑えるため過沃素酸塩溶
液を覆うよう注意が払われた。2つのビーカーの内容物がよく混合され、3時間
暗室内に保持された。結果的に酸化されたATP溶液が12個のアジピン酸ジヒ
ドラジド処理フィルター パッドに加えられ、パッドは1晩中室温で立たされた
。それから固体の3.14gの(0.05M、シグマ)硼水素化シアン ナトリ
ウムが1リットルのATP溶液に加えられて更に4時間パッドと反応した。最後
にパッドは、余分のATPと硼水素化シアン ナトリウムとを除去するために広
範に水洗された。
3.酸化法−8時間加水分解
実施例A1及びA2の中で44時間の加水分解に対して記述されたように、1
Nの水酸化ナトリウムの中で8時間フィルター パッドが加水分解された。それ
からパッドはメタ過沃素酸ナトリウムを用いて酸化され、アジピン酸ジヒドラジ
ドで調整された。ATPは上述のように移動を止められた。
B.共有結合したATPを備えた中空ファイバーの準備
共出願07/956432の第87頁に記載されたようなポリエーテルスルホ
ンの中空ファイバー(外径1500ミクロン、内径1000ミクロンの90中空
ファイバーを含む36束)を個々の束のスロットを備えた容器内に置き、20リ
ットルのアセトニトリルで16時間洗浄した。容器は排出口とポンプを備え、液
体を容器の上部に循環させると共に、必要に応じて溶液を加熱するオンラインヒ
ーターと、混合用のタンクを備えている。次にアセトニトリルを容器から排出し
、ファバーを脱イオン化した20リットルの水で10分間2度洗いする。次にフ
ァイバーを27リットルの脱イオン化水と、3リットルのエチレングリコルジグ
リサイドルエーテル(EGDGE、ウイシコンシン州、ミルウォーキー、アルド
リッチ ケミカル カンパニー)と、240グラム50%ソジウム水酸化物溶液の
溶液と反応させた。溶液は容器内を3時間ファイバー中を循環し、容器から排出
し、ファイバーは20リットルの脱イオン化水で10分間2度洗いした。ファイ
バーは27リットルの脱イオン化水と、2kgの30%のポリエチレンイミン溶
液(PEI)(エポミンP−1000、ニューヨーク州、レイクサクセス、アセ
ト コーポレーション)と、1440gの50%ソジウム水酸化物溶液の溶液と反
応した。溶液は5分間容器内のファイバーを通して5分間循環し、オンラインヒ
ーターにより溶液循環時、容器内の溶液温度を75℃の温度に設定する。溶液は
3時間循環し、容器から排出させ、ファイバーは20リットルの脱イオン化水で
10分間2度洗いした。水を排出し、30リットルの新しい脱イオン化した水を
容器に加え、16時間循環するようにした。水を排出し、27リットルの脱イオ
ン化した水に代えた。240グラムの予備混合された固体リン酸塩緩衝剤塩(ミ
ズーリ州、セントルイス、シグマ ケミカル カンパニー)をタンク内に加え、5
分間混合してPH7.4を達成した。3リットルのグルタルアルデヒド溶液(2
5%、アルドリッチ ケミカルカンパニー)をタンクに加え、5分間混合した。溶
液を容器内のファバーを通して4時間循環させ、排出させ、前記したように脱イ
オン化した水で4回すすぎ洗いをした。二回目のPEIコーティングを27リ
ットルの脱イオン化した水と2kgのPEI溶液で作った溶液を用い、5分間混
合してファイバーに施した。内容物を室温で2時間循環し、排出させた。次にフ
ァイバーを20リットルの脱イオン化した水で4回洗浄し、16時間循環させる
最終洗浄をなした。水を排出し、27リットルの脱イオン化した水に代えた。2
40グラムの予備混合された固体リン酸塩緩衝剤塩(ミズーリ州,セントルイス
,シグマ ケミカル カンパニー)をタンク内に加え、5分間混合してPH7.4
を達成させた。3リットルのグルタルアルデヒド溶液(25%,アルドリッチケ
ミカル カンパニー)をタンクに加え、5分間混合した。溶液を容器内のファイ
バーを通して4時間循環させ、排出させ、前記したように脱イオン化した水で4
回すすぎ洗いをした。
水を排出し、30リットルの脱イオン化した水をタンクに加えた。アジピンジ
ヒドラザイド、600g(アセト コーポレーション)と、ソジウムシアノ水酸
化ホウ素、1000g(アルドリッチ ケミカル カンパニー)をタンクに加え、
5分間混合した。溶液のPHは5%ソジウム水酸化物と1:10希釈塩酸溶液を
用いて7.0に調整される。溶液は室温で5時間ファイバーを通して循環させる
。内容物を排出し、前記したように脱イオン化した水で4回洗浄し、37℃で1
6時間乾燥した。ファイバーは使用まで室温で蓄積される。
90のヒドラジド処理した中空ファイバー(長さ40cm、外径1.5mm、内径
1mm、ほぼ10gドライウェート)が10cm長さに切断され、1リットルのナル
ジン(Nalgene)プラスチックのフラスコに挿入された。1リットルのP
H4.5の酸化ATPをフラスコに導入し、一晩混合した。酸化ATPのPHは
7.4に調整され、固体ソジウムシアノ水酸化ホウ素、3.14g(0.05M
)をフラスコに加え、更に4時間混合した。溶液をファイバーから排出し、ファ
イバーを水で充分洗浄し、空気中で乾燥させた。
C.共有結合したATPを備えたアミノ化コロマトグラフィービードの準備
アミノ化したHyper DTMビード(マサチューセッツ州,マールボロ,セプ
レーカー インコーポレイテッド((Sepracoh,Inc.))から手に
入る)のMグレード(平均サイズ、85μm,36μmeg/mLアミノグルー
プ)、100gを1L Nalgeneのプラスチックのフラスコに配置した。
Hyper DTMビードは1992年10月5日出願の米国出願第07/956
,404号に記載されているように架橋連結したアミノ化メタクリルアミド誘導
体からなるゲルを小孔に有する多孔性シリカビードである。1リットルの上記の
ように用意された酸化ATP5.5g(0.01M)をフラスコに加え、回転ミ
キサ内で3時間混合する。3時間の終りで、浮遊物のPHを10Nソジウム水酸
化物を用いて7.4に調整し、一晩中混合した。固体ソジウムシアノ水酸化ホウ
素、3.14g(0.05M)をフラスコに加え、更に4時間混合した。ビード
を1リットルの焼結ガラスのじょうご(25−50μmフリット)に移し、水で
充分洗って、空気中で乾かした。二つの他のグレードのアミノ化したHyper
DビードをATPで同様に派生させた。三つのグレードは下記の表にCa、Cb
,Ccと指定されている。
D.2−3−ジホスホ−Dグリセリン酸(DPG)フィルターパッドの準備
ポリプロピレン/セルロース酢酸塩のフィルターパッドを1N NaOHで4
4時間加水分解し、前記のごとく、ソジウムメタ過ヨウ素酸塩で酸化させた。エ
チレンジアミンの1M溶液(アルドリッチ ケミカル)、PH7.5、1.5リ
ットルを濃縮したHCLでPHを調整することによって作成し、アジピンジヒド
ラザイドのために前記したように16時間酸化したパッド(13)と反応させた
。固体ソジウムシアノ水酸化ホウ素(Sigma)をエチレンジアミン溶液に加
え、濃度をソジウムシアノ水酸化ホウ素中0.1Mにし、更に4時間反応させた
。フィルターを脱イオン化した水で十分洗浄し、余分のシアノ水酸化ホウ素とエ
チレンジアミンを取り除き、30℃で一晩乾燥させた。
2,3−ジホスホ−D−グリセリン酸,ペンタソジウム塩,(DPG),20
0g(Sigma)を1.92gの2−(N−モルフォリノ)エタンスルホン酸
(シグマ ケミカル)を100mLの脱イオン化した水に解かすことによって且
つPHを1Nソジウム水酸化物溶液で4.5に調整することにより準備された5
0mlの0.1M MESバッファ中で溶解した。1−エチル−3−(3−ジメ
チルアミノプロピル)カルボジイミド(EDAC)、200g、(Sigma)
を10mLの0.1M MESバッファに溶解し、PHを1Nリジウム水酸化物
で4.5に調整した。EDAC溶液2.5mLを50mLのDPG溶液と一分間
混合し、エチレンジアミンで変更した二つの乾いた90mmフィルターパッドに緩
やかに注ぐ。パッドはボトムメンブランのない1L Nalgeneプラスチッ
クフィルタホルダで支持される。15分後、DPG溶液を吸引によりフィルター
パッドから除去し、別の2.5mLの溶液をフィルター上のフィルタパッドに緩
やかに加える。DPG/EDAC溶液は重力でフィルタパッドを通して排出でき
、30分毎にパッドは吸引により乾かされる。ろ過液はパッドの上部に緩やかに
注がれ、この処理を7回くり返す。フィルタパッドは2リットルの1Mソジウム
塩化物で洗浄され、脱イオン化した水で充分洗浄され、本反応のDPGとEDA
Cを除去する。フィルターを30℃の空気中で乾かし、人のヘモグロビンのテス
トに用いるまで30℃で蓄積される。
ATP変更マトリックスのテスト
E.ボビンヘモグロビン溶液を用いるテスト
0.9%NaCl中0.5mg/mLのボビンヘモグロビン溶液を0.9%Nac
l250mL中にボビンヘモグロビン(BHG)(シグマ ケミカル,H−26
25)125mgを1時間室温で溶解することにより形成し、0.22μmのセル
ロースアセテートメンブランのフィルタを通してろ過した。ATP誘導パッド、
ビード又はメンブランの測定された制御量を15mLポリプロピレンの遠心管に
配置し、ヘモグロビン溶液の算定料を加え、一時間混合した。管に遠心力を作用
し、405nmの吸収度が各サンプル浮遊物とオリジナルのヘモグロビン溶液に測
定された。ヘモグロビン摂取量が除去により測定され、単位マトリックス量あた
りの摂取量として表わされた。表1は制御パッドに対するBHGの非特定吸収値
及び例A2に従って準備されたATP不能パッドに対するBHGの特定の結合性
の値である。
表IIに示した別の実験例では、BHGの特定の結合がATP不能フィルタパッ
ド、中空ファイバー及び三等級のクロマトグラフィービードについて試験された
。比較のためアガロースゲルに結合した商業的に手に入るATP(ミズーリ州,
セントルイス,シグマ ケミカル カンパニー)についても試験された。その結果
、ATP改変パッドが1.4mg/mLのBHGに最も高い産出量を与え、1.3
8mg/mLのヒドラジド処理中空ファイバーと1.28mg/mLがそれに続い
た。
BHGの結果は、牛と人間のヘモグロビンの間に組織的且つ構造的差異がある
ので、人的ヘモグロビン(HHG)の質的指示をなすものである。
F.人的ヘモグロビン(HHG)溶液を用いたテスト
15mLの遠心管中のATP、DPG又は制御不能マトリツクスに計算された
量の人的ヘモグロビン溶液を加え、一時間混合した。ヘモグロビン摂取量は除去
により計算され、単位マトリツクス量あたりの摂取量として表示された。制御体
として、アルカリ加水分解していないフィルタパッドを買ったまま(1)テスト
し、次にソジウムメタ過ヨウ素酸塩で処理した後(2)、Aに記載したようにア
ジピンジヒドラジッドとATPで処理した。ヘモグロビン溶液は150mg%HH
Gを含んでいた。結果は表IIIに示す。制御体はどれも有用なヘモグロビン摂取
を示さなかった。
他の実験セットをATP(例A−C)にHHGのない800mg%及びDPG
(例D)に793mg%を用いて行なった。表IVはパッド、中空ファイバー及び
ビードに対するHHGの結合を示している。その結果、ATPを有するパッドは
9.5HHG/mLのパッドを定め、ヒドラジドメンブランは3.1mgHHG
/mLのメンブランを定め、最良のHyperDTMビードは2.1mgHHG/
mLの設定されたビードを定める。アガロース上の制御体ATPは11.6mg
HHG/mL周辺を定める。DTPを有するパッドは1.7mg/mLのHHG
を定める。
例A(3)からの一つの乾かした90mm(12mL)パッドをペトリ皿に置
き、185mg%HHGを含むHHG溶液25mLをペトリ皿に加えた。溶液を
かきまぜ、パッドと一時間混合させるようにした。浮遊溶液中のHHG濃度を測
定し、185mg%であることがわかった。これは1N NaOH中8時間又は2
3度以下の室温では効果的技能を得るのに十分な加水分解が備えられないことを
示すものである。
G.パックした人の赤血細胞を使用したテスト
ヘモグロビン溶液で説明したものと同様に少量(<5mL)の試料を、パック
した人の赤血細胞(ヘモグロビンを含む)を加え、一時間混合し、浮遊物に残れ
たヘモグロビンを測定することによりテストした。ヘモグロビン産出量を除去に
より測定し、産出量/単位マトリックス容積で表わした。実験はHHGのない6
22mg%と36%のヘマトクリットを含むパックされた赤血細胞を使用して行わ
れた。表Vはパックされた赤血細胞の存在するマトリックスの容量測定の結果を
示している。その結果、テストされた状況下で、1時間テストチューブ中で回転
させることにより、強度の溶解が生じ、制御テスト用パックされた赤血細胞溶液
中にはじめに存在した以上に、HHGが浮遊物中により生成されることが示され
た。例外はフィルターパッドの場合で、これは細胞が若干破壊するにもかかわら
ず、2.2mgHHG/mLの産出量を示した。
H.装置中にパッドを用いることの試験
ATPにより改変されたフィルタパッド保持装置中で試験した。実験は、フィ
ルタパッド、ビード及び赤血球を充填した膜を用いて実施された。ビード29m
L)又は中空膜(20mL)は、単独では試験せず、2個の90mmATP改変
パッド(12mL)中に挟んで試験した。200mLの充填された赤血球(ビー
ドの場合には20%ヘマトクリット、中空繊維膜の場合には36%ヘマトクリット
)が10分間に装置中を通過した。ヘモグロビンの取り込み量を、瀉出から測定
し、単位基質体積当たりの取り込みとして表示した。
パッド間にハイパーDビードを有するフィルタパッド装置の結果を表VIに示す
。この結果は、上澄HHG濃度が開始時の622mg%から濾過後に829mg%
に増加したことを示している。明らかに、試験管の測定の場合と同様に、ビード
が赤血球を溶解しHHGを遊離させたように思われる。これらの結果は、ビード
を用いることが実行可能ではないことを示している。パッド間にヒドラジド膜A
TPを有するフィルタパッド装置の結果も表VIに示す。この結果は、上澄HHG
濃度が開始時の622mg%から濾過後に393mg%に減少したことを示してい
る。存在した全体の自由HHGは796mgであり、濾過の後503mgが濾過
液中に存在し、このことは293mgが取り除かれたことに対応する。このこと
は、自由HHGの37%が取り除かれたことに対応する。
ATP固定パッド及び膜を有する装置に対するHHG除去容量は、HHG溶液
の実験で得たデータから計算することができる。ATPパッドとATP膜に対す
る最大のHHG容量は、それぞれ9.46mg HHG/mLと3.06mg H
HG/mLであった。これらのデータに基づき、パッドと膜に対して期待する容
量を以下のごとく計算できる。
ATPパッド及び膜装置
この結果は、溶液中にHHGを用いたHHG容量の評価を装置中の期待される
容量の評価に用いることができることを示している。
本発明は、全血液、血漿、又は静脈、動脈若しくは毛細血管の血液源からの血
清を必要とする診断上の試験にも適用できる。診断上の試験において最も一般的
な障害は溶血である。溶血は試料の雑な取扱又は静脈穿刺部位での過度の探索に
より惹き起される。溶血は、それがどんな量であっても、診断結果を変えてしま
う試料中の自由ヘモグロビンの発生を帰結する。第2の一般的な障害要素は、患
者の循環システム中に合成又は代替ヘモグロビンが存在することである。(ヘモ
グロビン又は改変ヘモグロビンは、手術又は障害の結果として失われたヘモグロ
ビンに取って代わる酸素担体としてしばしば与えられる。)
型にはまった分光光度計による血液解析に先立って血液試料からヘモグロビン
と改変ヘモグロビンとを除去する本発明のフィルタの有用性を証明するために、
