2. Description of the Related Art It has long been attempted to observe the inside of the human body under an endoscope, and various instruments have been developed. Not only observation but also treatment and surgery can be performed under the endoscope. Has been done in recent years. Like cholecystectomy,
In 1987, the operation method of performing the operation by the conventional laparotomy under the endoscope.
The first surgery was performed in France in 2010, and the number of surgeries using this technique has increased in Europe, the United States, and Japan, and surgery other than cholecystectomy has been studied and attempted under laparoscopic surgery. The advantages of this operation are as follows: (1) Because it does not depend on laparotomy, it does not leave a cosmetic deformity. (2) Post-operative pain is light. (3) The hospital stay is short. Drawbacks include (1) Inadequate surgery because the field of view is narrow and the skill of the operator is required. (2) Bleeding or biliary tract injury may occur. This bleeding and organ damage are caused by accidentally damaging blood vessels and organs with instruments such as a scalpel, forceps, and a laser, and often lead to serious consequences.

EXAMPLE Since the present invention is intended to open the spatula attached to the tip of the spatula in the shape of a fan, two or more are used, but an example in which three are used will be described. FIG. 1 is a side view of a pressure-fixing hook for endoscopic surgery according to the present invention, FIG. 2 is a plan view of a state where the spatula portion at the tip of the pressure-fixing hook is closed, and FIG. 3 is a fan-like shape for the spatula portion. 4 is a plan view in an open state, and FIGS. 4 to 6 are structural explanatory views of the tip of the pressure-fixing hook.
Is a side view, B is a plan view, C is a view of the tip part of A view from the left, and D is a view of the right end face of A view. Show. FIG. 5 shows a shaft rod tip structure that is inserted into the axis center of the pressure-fixing hook main body and is moved back and forth in the axial direction by a grip operation at the body side. A is a plan view, B is a side view,
C is a bottom view, D is a view of the B view from the left, and E is a view of the B view from the right. Figure 6 is attached to the tip of the pressure-fixing hook 3
FIG. 3 shows a plan view of a sheet of spatula, where A is an upper spatula out of the three sheets, B is an intermediate spatula, and C is a lower spatula. Like the surgical forceps, the pressure-fixing hook of the present invention has elastic leaf springs 4 and 5 inside each of the open / close handles 2 and 3 pivotally supported by the upper end 1, and one of the handles 2 is provided. An outer tube 6 for insertion into the body is attached integrally with the upper part of the outer tube 6, and an upper tube of the other handle 3 is fitted into the shaft center of the outer tube 6 and the outer tube 6 is gripped by the outer tube 6 and the outer tube. The base portion of the shaft rod 7 that moves back and forth in the axial direction of 6 is connected. As shown in FIG. 1 and FIG. 4, the tip of the outer tube 6 is cut off with the upper half 9 leaving a part 8 of the tip, and the cut cavity has the same shape as the cut upper half of the outer tube. A member 10 that is integrally attached to the tip of the shaft rod 7 and that moves in the axial direction is installed. Further, as shown in FIG. 4, a vertical split groove 11 is provided in the axial direction from the tip portion 8 of the outer tube 6 to a part of the cut portion 9, and the tip portion 8 is provided in the lateral direction at right angles to the split groove 11. 6, there are provided lateral grooves 12 and 13 into which the open / close base sides of the upper spatula A and the lower spatula C are fitted. A narrow protrusion 14 is formed integrally with the member 10 at the center of the axial direction at the tip and the lower surface of the member 10 attached to the tip of the shaft rod 7. It is fitted in the split groove 11 to accurately guide the axial movement of the shaft rod tip member 10. In addition, the base side of the upper, middle, and lower three spatula A, B, C is inserted into the tip of this protruding piece, and the center part of each spatula base is shared by the shaft pin 20 with the protruding piece 14 of the shaft rod tip member 10. A cut groove 15 is provided so that the groove 15 can be connected to. To attach the base side a, b, c of the upper, middle, lower three spatula A, B, C as shown in FIG. 6 to the body tip assembled with the outer tube 6 and the shaft rod 7 having the above-mentioned structure, Vertical grooves 11 and lateral grooves 12, 1 provided on the outer tube tip portion 8 and the projecting piece 14 of the shaft rod tip member.
The base sides a, b and c of the three spatulaes A, B and C are fitted into the gap groove formed by 3, 15 as shown in FIG. The outer pipe tip C is pinned to one side 16 of the outer pipe tip 8 with a pin 17, and the outer portion of the base portion c of the lower spatula C is axially fixed to the other vertical division 18 of the outer pipe tip with a pin 19. Next, the central portions of the base portions a, b, c of the upper, lower, middle three spatula A, B, C are fixed to the projecting piece 14 of the shaft rod tip member by the common pin 20. As described above, since the three spatula A, B, and C are attached to the tip of the main body, the intermediate spatula B advances axially when the shaft bar 7 is advanced by operating the base handle, but the upper and lower spatula A and C rotate outward with the base outer shaft pins 17 and 19 as supporting shafts, respectively, and open like a fan as shown in FIG. Also, when the shaft rod 7 is retracted, the upper and lower spatula A and C are rotated inward, that is, toward the central axis with the spatula base outer shaft pins 17 and 19 as the support shaft, as shown in FIG. As described above, the three spatulaes A, B, and C are overlapped with each other, and the outer shape is reduced from the open fan shape to one spatula width.

EFFECTS OF THE INVENTION Since the pressure-fixing hook for endoscopic surgery of the present invention is configured as described above, in the case of endoscopic surgery, the pressure-fixing hook of the present invention is operated using a puncture tube. Insert into the body near the location, and open the spatula at the tip of the pressure-fixing hook by operating the handle at the hand of the pressure-fixing hook while observing the blood vessels and organs near the surgical location with the endoscope inserted separately. This wide open spatula can push away blood vessels, organs, etc. near the surgery site, prevent damage to blood vessels, organs, etc. near the surgery site during surgery, and allow the operator to keep an eye on the surrounding tissue. You don't have to use the so you can focus on your surgery. Also, because the spatula with multiple open ends overlaps with the spatula of one width due to the retreat of the shaft rod, the outer shape becomes smaller, so it is possible to use a pressure hook outer tube with a small diameter and use it when inserting it into the body. The puncture tube puncture also reduces the size of the wound left on the skin surface of the body.
