Alternatively, autologous blood transfusion in which blood is returned by performing filtration or the like is becoming popular.

The autologous blood collection and transfusion methods are generally classified into two types, a non-washing method and a washing method. The non-washing method returns blood simply by passing collected blood through a blood filter or the like. From the viewpoint of safety, it is limited to chest related surgery with a low degree of contamination. On the other hand, the cleaning method is a blood inflow part, a blood collecting reservoir, a suction pump,
It is composed of a blood concentrating and washing mechanism (centrifugation method is widely used) and a blood return bag, but it has a suction mechanism that can be managed individually, and it is required to have a safe and simple blood collection. There is.

Although a non-washing type device is commercially available for use in thoracic surgery, a tube for chest drainage is connected to a blood bag, and the blood bag is separated after the blood is stored. It uses a method of returning blood to itself. However, even in thoracic surgery, there is a risk of liver and renal damage due to free hemoglobin in the stored blood. Therefore, in most cases, blood is returned after washing.

In order to cope with such a current situation, the present inventors previously combined the case where autologous blood is collected, filtered and directly transfused into a patient, and combined with a device for concentrating, washing and returning blood. Invented a blood collection and transfusion device that can be used in both cases and is convenient for fine-grained management after surgery.
As disclosed in Japanese Patent No. 236960, however, in any of the devices, the blood storage part is attached to the side of the main body part including the liquid collection, water seal, and suction pressure setting part. After the completion, the main body is used as a drainage device, but the operation of attaching and detaching the connecting tube and the opening and closing of the opening and closing mechanism accompanying it is very complicated, and there is a possibility of being contaminated by outside air.

The liquid collecting part (1) may have either a cylindrical shape or a rectangular tube shape, but in view of forming the container,
A rectangular tube shape is desirable from the viewpoint of compactness for easy handling, and it is made of a transparent or semitransparent material so that the amount of collected body fluid can be easily measured and the state of the body fluid can be observed. Is good. The liquid collecting part (1) is preferably designed with a scale on the side so that the amount of bleeding immediately after the operation can be accurately measured, particularly up to about 200 ml. As shown in FIG. 1, by providing a partition wall (15) in the liquid collecting part (1) and dividing it into two tanks, the accuracy of measurement can be improved, and after returning the collected blood to the patient, When continuously used as a drainage for low pressure continuous suction, when the amount of body fluid increases, it can overflow from the upper part of the partition wall and flow into the second tank. The material of the liquid collecting part (1) is not particularly limited as long as it is a lightweight material, but it is suitable to use, for example, a hard vinyl chloride resin.

On the other hand, on the lower side wall of the liquid collecting part (1), the opening end is located in the vicinity of the bottom of the liquid collecting part (1) and the connection port (14) of the body fluid extracting pipe (12) for taking out blood or the like. However, an opening / closing mechanism B (13) is attached as an attachment. The shape of the open end of the bodily fluid extraction tube (12) is cut diagonally or a V-shaped or U-shaped notch is formed so that the bottom wall of the liquid collecting section (1) is not blocked. It is desirable to provide it.

On the other hand, when the partition wall (15) is provided in the liquid collecting part (1) as described above to divide the tank into two tanks, the check valve (16) is provided at the bottom of the partition wall (15) as shown in FIG. Attaching the two tanks so that the two tanks communicate with each other at the bottom of the liquid collecting section so that the blood flows from the second tank to the first tank only in one direction, the collected blood will cross the partition wall (15). Even when the blood is collected in the second tank, the first
When the blood in the tank is reduced, the blood in the second tank naturally flows into the first tank, so that the blood in the liquid collecting section (1) can be smoothly taken out.

The filtration mechanism (7) is for removing blood coagulation and other debris, and its installation position is in front of the blood inlet (8) of the liquid collecting part (1) as shown in the example of FIG. However, as shown in FIG. 3, it may be provided near the lower part of the body fluid inlet (8) in the container of the liquid collecting part (1), and when replacement due to clogging is considered. The former is suitable, but the latter allows the entire device to be more compact. As described above, there is an advantage in adopting either one. In addition, it is also possible to change the roughness of the filter or the like to be used to form a two-stage structure. A relatively coarse filter 100-250μ is suitable for the filtration mechanism (7). When the two-stage structure is used as described above, the first stage has a play filter shape of 0.2-0.5 mm, The second stage is a filter 100 to 250 with a certain degree of fineness.
It is appropriate to set μ.

Further, the suction pressure setting section (3) is in gas communication with the water sealing section (2) and the liquid collecting section (1) through a communication port provided on one side on the upper side of the partition wall. One side is connected to a negative pressure source such as a suction pump.
The suction pressure setting unit (3) may be of a water column type as shown in FIG. 1 or may have a mechanism in which the suction pump itself can finely adjust the pressure.

Next, a method of using the blood collecting and transfusing apparatus according to the present invention will be described. First, the CT connected to the body fluid inlet (8) above the liquid collecting part (1) via the filtration mechanism (7).
The other end of the tube (4) is inserted into the surgical wound and left in place, the opening / closing mechanisms B, C (13, 11) are closed, the opening / closing mechanism A (5) is opened, and suction is performed from the suction pressure setting section (3) side. Aspirate with a pump. Initially, the discharged blood mainly comes in, but the discharged blood gradually decreases and finally stops. Therefore, the recovered and stored waste blood is regenerated and returned to the patient.

INDUSTRIAL APPLICABILITY The autologous blood transfusion device of the present invention can collect and store blood discharged from a patient at an appropriate suction pressure, and can accurately control the amount of collected blood. The blood volume can be measured finely. Further, the autologous blood transfusion device of the present invention can effectively administer an anticoagulant for blood so as not to generate a thrombus, and even if a blood coagulation product or the like is generated, it can be effectively filtered and removed. Moreover, the autologous blood recovery transfusion device of the present invention can be used in a closed state without being contaminated by outside air because the liquid collecting part, the water sealing part, and the suction pressure setting part are integrated, and the blood is collected to the patient. It is a simple and compact blood recovery transfusion device with a suction mechanism that can be used as a drainage for low pressure continuous suction after the blood return, and is extremely useful in the medical industry.
