Abstract

PURPOSE:To provide the anastomosing device which can move freely a light emitting terminal and can perform easily an intended anastomosis. CONSTITUTION:Light outputted from a light source 1 is made incident on a fiber light guiding path 3 through a lens 2 and guided to a light emitting terminal 4. This light emitting terminal 4 is a contact type terminal being used while bringing a light emitting end face 4a into contact with an anastomosing object, and focuses the light from the fiber light guiding path 3 to an anastomosing position. Also, this light emitting terminal 4 consists of a transparency material, and its tip part is formed in a roughly conical shape in which the light emitting end face 4a is the apex. This light emitting end face 4a is covered with a thin film of a fluororesin, in which free energy of the surface is small, and which has such a quality as it is scarcely adsorbed to a living body tissue.

2. Description of the Related Art Conventionally, this type of anastomosis device has been used for vascular anastomosis, nerve anastomosis, tendon anastomosis, etc. For example, in the case of vascular anastomosis, it was used as follows. That is, the blood vessel to be anastomosed is sandwiched between the forceps and the blood flow is stopped.
Then, the ends of the blood vessels are brought into close contact with each other, and the laser emission end of the anastomosis device is brought close to this close contact portion for laser irradiation, whereby the close contact portion of the blood vessels is fused by the thermal energy of the laser irradiation. However, in this anastomosis device, it is difficult to maintain a constant distance between the laser emission end and the close contact portion of the blood vessel to be anastomosed, and the power density at the laser irradiation position becomes unstable. Further, it is difficult to accurately irradiate the blood vessel contact portion with laser light, and the intended anastomosis cannot be easily performed.

In order to solve such a problem, conventionally, an anastomosis device for converging and irradiating a laser beam while bringing a laser emitting end into contact with a blood vessel to be anastomosed has been realized. According to such an anastomosis device, the power density at the light irradiation position is kept constant, and moreover, the operation of the blood vessel anastomosis can be reliably and easily performed with the naked eye.

However, in the above-mentioned conventional anastomotic device having the contact type light emitting terminal,
In some cases, the blood vessel tissue is adsorbed to the light emitting end surface of the laser light emitting terminal, which makes it difficult to move the laser light emitting terminal smoothly. This adsorption is due to the fact that the anastomosis uses heat energy generated by laser irradiation to fuse biological tissues, and the light emitting end surface of the light emitting terminal and the vascular tissue in contact with this end surface are irradiated with the laser light during irradiation. It is caused by an increase in temperature. As a result, a new problem has arisen that this adsorption makes it impossible to perform the intended anastomosis.

The light source 1 is composed of one or a plurality of semiconductor lasers or light emitting diodes, and is set to an appropriate optical power depending on the difference in blood vessel diameter, the difference between arteries and veins, and the like. The light output from the light source 1 is incident on the fiber light guide 3 via the lens 2. The lens 2 is a coupling optical system that couples the light source 1 and the fiber light guide path 3. The fiber light guide 3 is made of an optical fiber and guides the incident light to the light emitting terminal 4. The light emitting terminal 4 is a contact type terminal that is used while the light emitting end surface 4a is in contact with the anastomosis target, and focuses the light from the fiber light guide path 3 to the anastomosis position. The light emitting terminal 4 is made of a transparent ceramic or crystal, and its tip is formed into a substantially conical shape with the light emitting end face 4a as the apex. This light emitting end face 4
The surface a is covered with a thin film of fluororesin, which has a small surface free energy and is hard to be adsorbed to living tissue.

First, sutures 7 and 8 are passed through two points on each end of the blood vessels 5 and 6 to be anastomosed, and these sutures 7 and 8 are pulled in a direction orthogonal to the connecting axis of the blood vessels 5 and 6. Be done. Therefore, the ends of the blood vessels 5 and 6 are brought into close contact with each other by this pulling force. Next, the light emitting end surface 4a of the light emitting terminal 4 described above is lightly contacted with this contact portion, and the light from the light source 1 is applied to this contact portion. The temperature of each end of the blood vessels 5 and 6 that have been brought into close contact with each other rises due to the heat energy due to light irradiation,
The fibrous proteins of each blood vessel wall coalesce. Therefore, by moving the light emitting end face 4a of the light emitting terminal 4 along the line where the blood vessels 5 and 6 are in close contact with each other and irradiating the contact portion with point-like light at each contact point. The close contact area will heal. As a result, the blood vessels 5 and 6 are anastomosed.

According to such an anastomosis device, since the light emitting end surface 4a of the light emitting terminal 4 is coated with the fluororesin, the blood vessel tissue is not adsorbed to the light emitting end surface 4a at each contact irradiation spot 9. Therefore, according to the present embodiment, the light emitting terminal 4 can be smoothly moved along the contact portions of the blood vessels 5 and 6, and the intended anastomosis can be easily performed.

In this anastomosis, a water-soluble polyvinyl alcohol tube 10 is inserted into each blood vessel 5 and 6, and the blood vessel ends are brought into close contact with each other while maintaining the lumen of each blood vessel 5 and 6.
Then, the light emitting end surface 4a of the light emitting terminal 4 is lightly brought into contact with this contact portion and is irradiated with light, whereby the anastomosis of the blood vessels 5 and 6 is performed in the same manner as the anastomosis described above.
Also in this anastomosis, since the light emitting end face 4a is covered with the fluororesin, the blood vessel tissue is not easily adsorbed to the light emitting end face 4a, and the light emitting terminal 4 can be moved smoothly.

As described above, according to the present invention, even when the temperature of the object contact surface of the light emitting terminal and the temperature of the anastomosis object rise, the object contact surface of the light emitting terminal is adsorbed to the living tissue. Since the light emitting terminal and the anastomotic object are not made to stick to each other because they are made of a material that is difficult to do. Therefore, the light emitting terminal can be freely moved, and the intended anastomosis can be easily performed. Therefore, no skill is required for the anastomosis operation, and even an operator with little experience of anastomosis can easily perform the anastomosis operation.
