[Background Art] In surgical operations, vascular anastomoses are often performed to repair abnormal or damaged blood vessels. Vascular anastomosis is generally performed using sutures, but vascular anastomoses are described in Japanese Patent Application Laid-Open No. 49-51785, Japanese Patent Application Laid-Open No. 50-38388, and Japanese Patent Application Laid-Open No. 56-3052, etc. A method has also been proposed in which the tube is inserted into a blood vessel to support the blood vessel and fixed using an adhesive.

[Problems to be solved by the invention] Microvascular anastomosis is often required in neurosurgery, plastic surgery, etc., but suturing requires a high level of skill and can only be done by a person who is proficient in the technique. The surgery was difficult. Furthermore, the continuous suturing method, which is quick and easy to operate, cannot be used because it tends to cause stenosis of blood vessels due to over-tightening of the thread, and interrupted sutures, which are complicated to operate, have been used. Furthermore, methods using adhesives have not yet been technically established.

An object of the present invention is to provide an instrument that can easily and reliably perform blood vessel anastomosis using sutures. Another object of the present invention is to provide an instrument that does not cause damage to blood vessels during suturing.

By the way, the most important point in vascular anastomosis is
This is to fix the intima (inner surfaces) of the blood vessels so that they are in close contact with each other over the entire circumference when anastomoses are performed. If the intima are not in close contact with each other, blood will come into contact with parts other than the intima, making it easier to form a blood clot. Conventionally, the ends of the blood vessels were inverted to expose the intima and the intima of the two blood vessels were brought into close contact with each other before anastomosis was performed. There was a risk of damaging the lining of blood vessels. Therefore, a further object of the present invention is to
A preferred embodiment is to provide an instrument that can bring the intima into close contact with each other for anastomosis without inverting the blood vessel.

[Means for Solving the Problems] The present invention is an instrument for effectively performing suturing anastomosis by supporting the blood vessels to be anastomosed from the inside during blood vessel anastomosis, and the instrument is formed entirely into a tube shape. A disk-shaped protrusion is provided near the center of the instrument, and the outer diameter rapidly increases from both ends toward the center, and the disk-shaped protrusion is provided with a suture that has passed through the blood vessel wall. This is an instrument for blood vessel anastomosis characterized by having a plurality of grooves formed in the axial direction of the instrument for passage.

That is, first, an instrument with an appropriate outer diameter is prepared depending on the thickness of the blood vessel, and one end of the instrument is inserted into one of the blood vessels to be anastomosed, and the other end is inserted into the other blood vessel. After the intima at the ends of the two blood vessels are brought into close contact with each other, they are sutured with suture thread.Since multiple grooves are provided near the center of the instrument, that is, at the junction of the two blood vessels, the suture needle is inserted into the groove. By inserting the needle into the blood vessel along the line, suturing can be easily performed without damaging the intima. If the groove is not provided, the instrument and the blood vessel will come into close contact with each other, and there is a risk that the suture needle may damage the intima of the blood vessel.

FIG. 3 is a perspective view showing the operating procedure when performing anastomosis of blood vessels using the instrument of the present invention shown in FIGS. 1 and 2. FIG. First, as shown in FIG. 3a, an instrument 1 of an appropriate size is prepared depending on the thickness of blood vessels 7 and 8 to be anastomosed. The ends of the device 1 are then inserted into the blood vessels 7 and 8 as shown in Figure b. As shown in Figure c, the end of the blood vessel has a disc-shaped protrusion 4.
When the instrument 1 is inserted deeper until it reaches , since the blood vessel is highly elastic, the end of the blood vessel is pushed wide by the disk-shaped protrusion 4, and the intima at the end of the blood vessel are brought into close contact with each other. It becomes like this. When this condition is reached, as shown in Figure d, insert the suture needle 9.
By passing it through the groove 3 provided in the instrument and suturing it, anastomosis can be performed with the intima in close contact with each other. Although the groove 3 is covered with the blood vessel wall during suturing, the groove 3 can be seen through the blood vessel wall, so the needle can be moved without any problem. The width and depth of the groove 3 are such that the suture needle 9 and suture thread 10 can pass therethrough and appropriate anastomosis can be performed.
There are no particular restrictions. However, as shown in FIG. 2, if the entrance angle θ of the groove with respect to the surface of the disk-shaped protrusion 4 is made close to 90°, the needle can be inserted almost perpendicularly into the blood vessel wall during suturing, so the needle This is preferable because it can minimize damage to the blood vessel wall.

Since the device of the present invention remains indwelled within the blood vessel, it is preferably made of a blood-soluble or blood-degradable material so that it disappears after use. Examples of such materials include polyvinyl alcohol polymers, polyacrylamide,
Examples include polyvinylpyrrolidone, dextran, soluble starch, and sucrose, but polyvinyl alcohol polymers are most preferred in terms of material hardness and dissolution rate. Furthermore, polyvinyl alcohol polymers are convenient because the dissolution rate can be adjusted by changing the degree of saponification and degree of polymerization. Anticoagulants such as heparin and urokinase can also be added to these materials to prevent thrombus formation immediately after anastomosis.

The instrument of the present invention can of course be used for anastomosis between natural blood vessels, but can also be used for anastomosis between natural blood vessels and artificial blood vessels.

[Effects of the Invention] Since the instrument of the present invention supports the blood vessel to be anastomosed from the inside, suturing can be easily performed. Further, since the groove is provided, a space is secured between the instrument and the vascular intima, and suturing can be performed without damaging the vascular intima with a needle.

Furthermore, since the intima of the blood vessel can be easily and reliably fixed in close contact with each other, the success rate of the surgery can be increased. In addition, it is easy to operate because it does not require sophisticated techniques such as inversion, and intimal damage does not occur due to inversion. Further, since the blood vessel is supported from the inside by the instrument during suturing, the blood vessel does not become narrowed even when suturing is performed continuously, and the suturing operation can be performed more quickly than before.

Furthermore, if the device is made of a blood-soluble or blood-degradable material, the device will disappear after the anastomosis, which is safe and preferable since it will not interfere with blood flow.
