Due to diseases such as esophageal cancer, laryngeal cancer, and head and neck cancer,
Airway management is necessary for patients who have undergone a tracheotomy and have the tracheal stump sutured to the skin and are breathing through a fistula created there. In other words, when a healthy person breathes, inhaled air enters the respiratory tract from the nose and mouth through the throat, but while passing through these routes, the dust in the inhaled air is adsorbed, and the temperature and humidity are significantly different from those inside the body. It is introduced into the lungs through the trachea in a state where there is no air. On the other hand, inhaled air directly entering through a tracheocutaneous fistula has a considerable temperature and humidity difference from that of the body, and if left as it is, the airway mucosa cannot maintain its normal function, causing various disorders. For this reason, it is necessary to heat, humidify, and purify the intake air. There is not much of a problem when receiving treatment at a hospital because the equipment is in place for that purpose.
For patients in the rehabilitation stage or those receiving treatment at home, expensive and large equipment cannot be used, and it is desirable to use something as simple as possible. For this reason, conventionally, an apron-like wet cloth was hung from the neck to cover the vicinity of the tracheocutaneous fistula, and the inhaled air was allowed to enter the trachea through the underside of the cloth. However, methods are used to prevent inhaled air, which has the same temperature and humidity as the outside world and contains dust and water, from directly passing through the trachea and entering the lungs.

This purpose, according to the present invention, includes a bag portion having a skin contact side opening and an outside air side opening located away from the skin contact side opening, and an adhesive layer formed around the outside of the skin contact side opening of the bag portion. This is achieved by forming a breathing air passage between the skin contact side opening and the outside air side opening.

In the present invention, the pouch is adhesively fixed around the tracheocutaneous fistula using an adhesive layer formed around the opening on the skin side of the pouch, and the inhaled air enters the pouch through the open air side opening of the pouch, The air passes through the internal air passage to the opening on the skin side and enters the bronchus from there, but since the inside of the pouch is maintained at an appropriate temperature and humidity by exhaled air, the inhaled air passes through the pouch. During this time, the temperature and humidity are controlled and the tube is guided into the bronchus.

This is an open air side opening formed at the lower end of the separate bag portion. A double-sided adhesive bond 4 having the same opening is fixed around the skin-side opening 2, a protective paper 5 is provided on the outer adhesive surface, and the bag portion 1 is sealed except for the openings 2 and 3. . The outside air side opening 3 is made of a pipe having a size sufficient to pass the volume of air necessary for breathing, and can be connected to the bag portion 1 with a rubber band or a sealant. Reference numeral 6 denotes a breathable structure inserted into the bag part 1, which has the necessary breaks for breathing to prevent the bag part 1, which is usually made of a flexible material, from deforming due to breathing action and blocking the air passage. This is to ensure an air passage with a certain area.

Flexible plastic films with a thickness of 30 to 100 microns that can be heat-sealed or welded such as nylon or vinyl acetate can be used. Note that the bag 1 may not be sealed all around, but may have a fastener seal at the top, for example, so that the humidity inside the bag can be adjusted by opening and closing it as necessary and inserting wet gauze or the like.

長がある。 The adhesive layer 4 has the function of reliably sealing and fixing the bag portion 1 around the tracheocutaneous fistula, and is made of a double-sided adhesive bond coated with surgical grade adhesive such as paper, nonwoven fabric, cloth, plastic film, etc. Examples of the adhesive include rubber-based adhesives, acrylic ester-based adhesives, and what is called a skin protective agent made by kneading pectin and Karaya into a polyisobutylene base. Various methods can be considered for attaching the adhesive to the bag, but in the example shown in FIG. A so-called solid bonding method is shown, and the manufacturing process is simple, but since the adhesive layer is integrated with the bag, it is somewhat difficult to operate. In FIG. 3, a plastic film 8 is provided on one side of the adhesive band 7, and the plastic film 8 is heat-welded to the skin-side opening 2 of the bag portion 1 at the innermost position 10, leaving the periphery 9 of the plastic film 8, to form a ring shape. This shows the so-called flare connection method, which is a seal connection. Although this method requires a slightly more complicated manufacturing process, it is easier to operate because only the adhesive band can be grasped with your fingers during operation, and it also has the advantage of being easier to fit into complex areas of the neck.

Reference numeral 2 denotes an opening on the skin contact side formed above the bag portion, and numeral 13 denotes an open air side opening formed on the outside and lower side of the bag portion apart from the skin contact side opening 12. An adhesive layer 14 having the structure explained in FIG. 3 is fixed around the skin contact side opening 12, and a protective paper 15 is provided on the outer adhesive surface of the adhesive layer 14.
The bag portion 11 is sealed except for the openings 12 and 13. 16 is inserted into the bag portion 1 and the opening 12,
Similar to the embodiment shown in FIG. 1, the bag portion 1, which is usually made of a flexible material, deforms due to breathing action and does not block the air passage. This is to ensure an air passage with the cross-sectional area necessary for breathing, but
Unlike the air permeable structure shown in Figure 1, it is located around the air passage rather than within the air passage itself.
Therefore, breathability is not necessarily required. The shape is not limited to a ring shape, but may also be a parallel shape, a horseshoe shape, etc.
