Description
本発明は薬剤含有滅菌包帯、さらに詳しくは、
外科用包帯として薬剤を経皮的に浸透させる滅菌
包帯に関する。この新規な包帯に関しては、その
化学組成及び物理的特性が非常に重要である。
これまで、粘着性及び吸収性がある無菌の包帯
を開発しようと努力がなされてきた。たとえば、
米国特許第3339564号は、口腔の湿つた粘膜のよ
うな湿気を含んだ表面に粘着するように作られた
粘着性の包帯を開示している。しかしながら、こ
の粘着性包帯の主要材料の1つは粘着性ゴム、好
ましくはポリイソブチレンであつて、これは疎水
性である。同様に、米国特許第3598122号及び第
3598123号は、粘着層から連続的に遊離される薬
剤を含む包帯を開示している。これらの包帯は、
粘着層中に薬物を包封した層状材料から形成され
る。これらの先行特許に開示された包帯は、粘着
性であると述べられており、また薬剤に対して満
足な賦形剤であるが、例えそうであつても無菌で
ない限りは外科用包帯として有効に機能できると
は考えられない。この点に関しては、外科用包帯
は接着部位に薬剤を放出し且つ液体を吸着できる
ものでなければならないだけでなく、無菌である
ことが必須要件であることが指摘される。実用に
即した唯一の滅菌法は放射線照射(特にγ線照
射)によるものであり、先行技術の包帯はその粘
着特性や寸法安定性に影響を与えずに照射できる
とは考えられない。
従つて、本発明の一般的目的は、汗等の液体滲
出物を容易に吸収する、無菌で且つ粘着性のある
新規な薬剤含有包帯を提供することにある。この
包帯は、柔軟な裏打ち材と、汗又は傷口からの液
体滲出物を連続的に吸収するのに適した高度の吸
収性並びに薬剤の当接部位への放出性を有する
が、このような吸収の間には寸法安定性が保たれ
且つ水分の存在下において徐々に粘着性になる粘
着性支持材とから成る。滅菌は、支持材の寸法安
定性に最小の影響を与えるのみでその粘着性に影
響を与えることなく、放射線照射によつて実施で
きる。
本発明によれば次の二成分(a)及び(b):
(a) 綿織物、紙、化繊織物及びプラスチツクから
成る群から選ばれた柔軟性のある裏打ち材、及
び
(b) 皮膚への粘着に適した粘着性を有し且つ体の
外形に従つて変形するのに充分に柔軟な、均質
で安定な親水性マトリツクスを含んでなる、該
裏打ち材に結合せしめられた支持材
を含んでなる柔軟性且つ液体吸収性の接着ガーゼ
包帯であつて、該マトリツクスが(i)ポリアクリル
酸、ポリアクリルアミド及びそれらの類似体から
成る群から選ばれた合成樹脂を含む該マトリツク
ス全重量の約30乃至50%の固相ならびに炭水化物
及び/又は蛋白質の溶液または乳濁液から成る該
マトリツクスの約30乃至70重量%の液相を含み且
つ所要の薬剤を含有し、(ii)放射線照射によつて滅
菌したものであり、そして(iii)該接着ガーゼ包帯を
適用される傷口からの液体滲出物を効果的に吸収
でき且つかかる吸収の間において寸法安定性及び
粘着性を保持できる接着包帯が提供される。
本明細書中において、接着包帯(adhesive
bandage)、すなわち、絆創膏とは、薬剤を含有
する圧力によつて接着する物質をフイルムまたは
布等に被覆したもので滅菌されていて個々に使え
る被覆材一般を意味する。
本発明のこれら及びその他の目的及び利点は、
添付図面に関連して述べる以下の記載からより詳
細に理解されよう。図面中、同一の参照記号は全
図において同一または同様の部分を指す。
添付図面において、
第1図は患者の腕に適用された、本発明に係る
新規な包帯を示す斜視図であり、
第2図は第1図に示した包帯の斜視図であり、
第3図は外科用包帯として使用される本発明に
係る包帯の斜視図であり、そして
第4図は第2図の線分4−4に概ね沿つて切断
された、矢印の方向からの断面図である。
本発明の包帯は、皮膚との接触を保持するのに
適した粘着性を有すると同時に皮膚の動きに対し
てある程度の弾性を有する。この包帯は扱い易い
ように作られており、また患者に対してすべての
点で非刺激性である。
ここで、図面に言及すると、図面には本発明の
包帯が示されている。参照番号10で一般に示さ
れる包帯は裏打ち材11と該裏打ち材の一面に固
定された粘着性支持材12とを含む。裏打ち材1
1及び支持材12はともに、厚さの均一な材料か
らなる長方形のシートとして示されている。包帯
10は厚さが均一となるように作られており、ま
た、長方形の形状が好ましいが任意の形状である
こともできることが指適される。使用する際に
は、包帯は非外科的創傷、外科的創傷またはやけ
どした組織を覆うように支持材12を皮膚と直接
接触させて適用する。第3図に示される実施態様
は外科用包帯であり、外科的創傷を覆うように患
者に適用する。
この包帯の独特の構造にとつて最も重要な性質
は、皮膚への粘着を増大させる、支持体の親水粘
着性である。支持体は水分を吸収するにつれてま
すます粘着性になる。
支持材12はカラヤガム、グワーガム、アカシ
アガム、ロウカスト・ビーンガム(locust bean
gum)、照射カラヤ(2.5Mradsのγ線に暴露)の
ような天然材料ならびにソルビトール及び他の多
糖類から形成することができる。支持材はまた、
カルボキシメチルセルロース、カルボキシプロピ
ルセルロース、ポリアクリル酸、ポリアクリルア
ミド及びそれらの類似体のような合成ポリマーか
ら形成することもできる。合成ポリマーの例とし
ては、分子量250000、450000、1000000及び
4000000のポリアクリル酸ならびにハーキユルズ
社(Hercules Co.)から商標名レテン(Reten)
で販売されているポリアクリルアミドを挙げるこ
とができる。アクリル酸またはアクリルアミドの
ようなモノマーを重合させる場合には、活性剤の
使用が必要である。重合過程において使用する活
性剤としては硫酸第一鉄、ピロ亜硫酸ナトリウム
及び過硫酸カリウムを挙げることができる。又、
マトリツクス固相には出願人の米国特願58684に
示す接着性ポリマ、天然ガム、液相にはグリセリ
ン、プロピレングリコール等のヒドリツクアルコ
ール(多価アルコール)を使用してもよい。
マトリツクスの形成には多糖類または蛋白質の
溶液または乳濁液を用いることができる。あるい
は、多糖類、糖類または蛋白質の溶液または乳濁
液の組合わせをマトリツクスの液相に使用するこ
とができる。
安定なマトリツクスである支持材11として
は、合成ポリマーマトリツクス、カラヤマトリツ
クスまたはカラヤ及び合成ポリマーのマトリツク
スを含んでなる固相を挙げられる。マトリツクス
の固体はマトリツクス11の30乃至50重量%を占
める。
マトリツクス連続相は、次いで例えば2.5Mrad
のγ線照射に付されるものであるが、もしマトリ
ツクスがカラヤと多価アルコールのみより成る場
合、照射により軽微な加圧のみによつても変形す
る程その寸法安定性は損なわれ且つ吸水性も大き
な悪影響を受け操作不能となる。しかるに、前出
Reten等の高分子量ポリアクリルアミドの添加に
より、これらの損傷は効果的に抑制される。
尚、支持材(基材)に含有させる薬剤として
は、ポリビニルピロリドン溶解ヨウ素(ポピド
ン・イオデイン)等の各種殺菌抗菌剤、サリチル
酸メチル、カンフオール等の消炎・鎮痛剤、ハイ
ドロコルチドン等のホルモン製剤、ビタミンA,
B1,B2、等の栄養剤、抗ヒスタミン剤等々、所
望のものを適宜使用し得るものであるが、本発明
に於いて特に有用なものとしては、マトリツクス
に対し0.1−15重量%含有せしめられるニトログ
リセリンをあげ得る。通常、ニトログリセリンは
血管拡張剤として軟膏形態で使用されているが、
急性心不全等に対する予防的使用のためには安定
した徐放性、皮膚固着性が要求されるためこれら
軟膏では目的を達し得なかつたものである。
これに対し、自己粘着性、吸湿性及び保形性を

れるものである。
特に、マトリツクスにカラヤガムを包含する場
合、ニトログリセリンとの好適な相溶性が認めら
れ、画期的な心筋梗塞乃至心不全予防用プラスタ
ー剤が提供され得る。
尚、上記各種薬剤添加に際し、ビヒークルとし
てシナモン油、レモン油、オリーブ油、ヤシ油等
を併用してもよい。
カラヤガムまたは他のガムをマトリツクスを形
成するのに用いる場合には、放射線照射の間にお
いてカラヤを保護するためにポリアクリル酸及
び/またはポリアクリルアミドを用いる必要があ
る。しかしながら、ポリアクリルアミド及び/ま
たはポリアクリル酸の代りに、あらかじめ決めた
濃度の塩、たとえば、硫酸アルミニウムまたは塩
化ナトリウムをカラヤと共にマトリツクス中に使
用できる場合もある。たとえば、マトリツクスの
固相を形成する場合には、約6%の濃度の塩化ナ
トリウムまたは硫酸アルミニウムを用いることが
できる。
また、酢酸ビニル−マレイン酸ジオクチルコポ
リマーもマトリツクスの固相を形成するのに有利
に使用できることが判明した。酢酸ビニル−マレ
イン酸ジオクチルコポリマー〔エア・プロダク
ツ・アンド・ケミカルズ、Inc.(Air Products
and Chemicals,Inc.)から商標名「フレツクス
ボンド(Flexbond)150」で及びザ・ボスチツク
(the Bostik)Co.,Inc.から商標名「ボスチツク
(Bostik)8761」で販売されている〕は包帯の粘
着力を強化する。
尚、裏打ち材としては柔軟性を有する布材、紙
材、プラスチツクシート乃至フイルム等、所望の
ものを使用し得る。
ここで、マトリツクス形成材として有用な他の
ガム質材の1例として、スターチ・グラフト共重
合体をあげ得る。即ち、コーン・スターチとアク
リロニトリルとの共重合体であつてケツヘル社よ
り“FVP502Sアブプロテツト”(商品名)として
市販されているスターチ・グラフト共重合体又
は、“スターチ・ダツシユ・コ−ポリマ”(商品
名)として市販のアクリルアミド−アクリル酸ナ
トリウム−スターチのグラフト・ター・ポリマ
等々の各種澱粉誘導体もマトリツクス形成材とし
て有用である。
又、本発明包帯の構造に於いて、裏打ち部材と
して接着テープを使用し、その中心部分のみに薬
剤含有支持部材を粘着したものも有用である。
以下、支持部材の組成を示す実施例により本発
明をより詳細に説明する。

The bandages disclosed in these prior patents are said to be adhesive and are satisfactory excipients for drugs, but even so, they are not effective as surgical bandages unless they are sterile. It is inconceivable that it would be possible to function. In this regard, it is pointed out that surgical dressings must not only be able to release drugs and absorb liquids at the site of attachment, but also be sterile. The only practical method of sterilization is by irradiation (particularly gamma irradiation), and it is unlikely that prior art dressings can be irradiated without affecting their adhesive properties and dimensional stability. Accordingly, it is a general object of the present invention to provide a new sterile and adhesive drug-containing dressing that readily absorbs liquid exudates such as sweat. The bandage has a flexible backing and a high degree of absorbency suitable for continuous absorption of sweat or liquid exudate from the wound as well as release of the drug to the site of application; In between is a tacky support material which maintains dimensional stability and gradually becomes tacky in the presence of moisture. Sterilization can be carried out by irradiation, with minimal impact on the dimensional stability of the support material and without affecting its adhesion. According to the invention, the following two components (a) and (b): (a) a flexible backing material selected from the group consisting of cotton fabrics, paper, synthetic fabrics and plastics; and (b) skin adhesion. a support material bonded to the backing material comprising a homogeneous, stable hydrophilic matrix having suitable tackiness and sufficient flexibility to deform to conform to the contours of the body. A flexible, liquid-absorbent adhesive gauze bandage, wherein the matrix comprises (i) a synthetic resin selected from the group consisting of polyacrylic acid, polyacrylamide, and analogs thereof; 50% solid phase and about 30 to 70% liquid phase by weight of said matrix consisting of a solution or emulsion of carbohydrates and/or proteins and containing the required drug; (ii) sterilized by irradiation; and (iii) an adhesive bandage is provided that is capable of effectively absorbing liquid exudate from a wound to which the adhesive gauze bandage is applied and retains dimensional stability and cohesion during such absorption. . The adhesive bandage of claim 1, wherein the liquid phase comprises a polysaccharide solution. 3. The adhesive bandage of claim 1, wherein said solid phase of the matrix comprises a natural gum selected from the group consisting of karaya gum, acacia gum, locust bean gum, and guwar gum. 4. The adhesive bandage of claim 1, wherein said liquid phase comprises glycerol. 5. The adhesive bandage of claim 1, wherein said matrix comprises 5 to 45% by weight of Karaya, 2 to 40% by weight of polyacrylamide, and 30 to 70% by weight of glycerol. 6 The matrix is polyacrylamide 10 to
50% by weight of glycerol and 30-70% by weight of glycerol. 7 The matrix is polyacrylamide 2 or
40% by weight of polyacrylic acid, 2 to 40% by weight of polyacrylic acid, and 30 to 70% by weight of glycerol. 8. The adhesive bandage of claim 1, wherein said matrix is formed from 2 to 50% by weight polyacrylic acid and 30 to 70% by weight glycerol. 9. Adhesive bandage according to claim 1, characterized in that said matrix contains 0.1-15% by weight of povidone-iodeine. 10. The adhesive bandage of claim 1, wherein said matrix contains 0.1-15% by weight camphor. 11. The adhesive bandage of claim 1, wherein said matrix contains 0.1-15% by weight of methyl salicylate. 12. The adhesive bandage of claim 1, wherein said matrix contains 0.1-15% by weight nitroglycerin.
