PAM-RTによる局所的な腫瘍制御の有効性を調査するために、触知可能な皮下3LL腫瘍を有するC57BL/6マウスを、パイロット研究のために5つの治療コホートに分けたが、これらは、未治療、治療1日目または5日目に24Gy、1Gy×4の後に20Gy、あるいは20Gyの後に1Gy×4であった。基本的には、20Gyの単一分割(fraction)の前または後のいずれかに原発性腫瘍に4回の1Gyの分割を与えることと、RTによる24Gyの単一分割を比較した。図7A~Dに示されているように、単一線量のRTと比較すると、低線量のRTを用いた前治療は、最小の腫瘍制御しか示さなかったが、1Gy×4回の分割のPAM-RTの後は大幅な増殖の遅延と、生存率の向上が見られた。
また、3LL細胞に対するPAM-RTの免疫調節の作用をインビトロで研究した。実験用スキームは、図9Aに示されている。治療の6時間後には、細胞死に差は見られなかったが、24時間後には、PAM-RTによって、焼灼のみよりはるかに多くの細胞死が引き起こされた(図9B)。未治療の細胞や低線量の放射線で治療した細胞と比較すると、PAM-RTを用いるまたは用いない焼灼放射線は、RT後6時間目に、細胞表面免疫調節マーカー(CD80)、ストレスマーカー(CRT、Hsp70、FasおよびMHCI)および免疫抑制マーカー(CD47およびPDL1)を増加させた。興味深いことに、図9Cで見られるように、24時間後に、PAM-RTで治療された細胞は、焼灼のみのものと比較して、4-1BBL、CRT、Fas、Hsp70およびPD-L1の細胞表面発現がより強かった。
腫瘍微小環境(TME)上のPAM-RTの免疫学的結果を調査するために、図10Aに示されているように、放射線を当てた3LL腫瘍を、RTの開始後6日目および10日目に採取した。焼灼RTのみと比較して、PAM-RTを用いて治療した腫瘍において白血球の浸潤の大幅な増加が見られた。浸潤した白血球を表現型分類すると、焼灼RTのみと比較して、PAM-RTで治療したマウスにおいて、6日目に、腫瘍内のTregの大幅な減少が見られ(図10B参照)、CD8/Treg割合の大幅な増加が見られた。図10Cで見られるように、腫瘍全体のRNAに対するRT-PCRは、6日目に、PAM-RTで治療された腫瘍において、FOXP3 mRNA発現の大幅な減少を示した。図10Dで見られるように、6日目および10日目に、グランザイムBを分泌する腫瘍内のCD8+T細胞の増加傾向に伴って、この再構築が生じ、これは、エフェクターCTL応答の増強を示していた。腫瘍内の骨髄性細胞集団を特性付けることによって、図10Eに示されているように、6日目に、IL-10を分泌するマクロファージの大幅な減少およびCD206発現のわずかな減少が明らかになり、ならびに、治療の開始後10日目に、IL-10の分泌の減少傾向およびCD206発現の大幅な減少傾向が明らかになった。要するに、PAM-RTは、免疫抑制性TregおよびM2マクロファージを減少させる一方で、CTL活性を増加させることによって、TMEの再構築を促進する。
二次リンパ器官の免疫細胞を検討するために、以上の研究(例えば図10A)のように、RTの開始後6日目および10日目に、腫瘍流入領域リンパ節および脾臓を採取した。図11Aおよび11Bで見られるように、6日目および10日目に、PAM-RTで治療したマウスにおいて、脾臓および流入領域リンパ節の白血球の大幅な増加が見られた。腫瘍流入領域リンパ節(TDLN)については、図12Aで見られるように、PAM-RTで治療したマウスにおいて、6日目および10日目の両方に、CD8+細胞の大幅な増加およびTregの減少傾向が見られた。脾臓の分析によって、単一焼灼線量と比較して、PAM-RTで治療したマウスにおいて、6日目には、CD8 T細胞の最小限の変化およびTregの大幅な減少が明らかになり、10日目にこれが逆転して、CD8+ T細胞の大幅な増加およびTregの最小限の変化が明らかになった(図12B)。ELISPOTを用いた、6日目および10日目の脾臓T細胞の機能状態に関する調査によって、PAM-RTで治療したマウスにおいて、6日目に、グランザイムBを分泌するエフェクター細胞の増加傾向(図12C)およびIFNγを分泌するエフェクター細胞の大幅な増加(図12D)が明らかになった。10日目の分析によって、グランザイムBを分泌するエフェクター細胞の大幅な増加およびIFNγを分泌するエフェクター細胞の増加傾向が明らかになった。これらのマウスにおける治療は、原発性腫瘍に局所化されていたが、腫瘍で生じた傾向と同様に全身的な調節が生じ、免疫抑制が減少し、T細胞応答が増強された。
転移を生じやすい器官に対するPAM-RTによって、低免疫原生、高転移性癌における原発性腫瘍に対する一連の寡分割照射の焼灼RTの後に進行を抑制することができるか否かを調査するために、Balb/cマウスの同所4T1乳癌に、一連の局所的なPAM-RT(22Gy+0.5Gy×4)を施した。原発性腫瘍に対するPAM-RTは、局所的な腫瘍の進行を遅延させる傾向があったが、生存率が上昇せず、治療したマウスのすべてが転移性疾患に屈した(図13A~C)。PAM-RTを用いた局所的な治療がTMEを再構築したことを考慮し、原発性腫瘍の焼灼後に転移性腫瘍の臓器を治療するために、PAM-RTの使用を移行した。局所的な腫瘍制御を十分にするために、触知可能な4T1腫瘍を有するBalB/Cマウスを、連続3日間にわたって原発性腫瘍に3回の20Gyの線量を与えて治療した。期待した通り、原発性腫瘍に対する焼灼のみでは、肺転移から動物を救うことはできなかった。原発性腫瘍に対するRT後に抗腫瘍免疫応答を誘発したにもかかわらず、RTによって誘発されたCTLは、転移部位では浸潤から除外されている可能性があった。これを試験するために、転移を生じやすい器官である肺全体を、原発性腫瘍に対するRT完了後に4日間、12日間にわたって、1日0.5Gyの線量を用いて治療した(図14A)。図14Bに示されているように、原発性腫瘍に対するRTのみと比較して、肺全体に対するPAM-RT後にこれらの動物において生存率が大幅に上昇した。治療したマウスの転移の負荷を調査したことから、図14Cで見られるように、また、図14Dの組織学的検体で見られるように、India ink注入後の肉眼検査によって、PAM-RTで治療した肺では転移巣が少なかったことが明らかになった。4T1マウスのPETスキャンは、肺からの採取物と同様の結果を示し、PAM-RTを受けた肺は転移の負荷が小さかった(図15AおよびB)。これらのデータは、局所的に制御するために原発性腫瘍にPAM-RTによる0.5Gy×4の線量を直接投与するか、あるいは、全身的な疾患を治療する場合に、転移を生じやすい器官にPAM-RTを遅らせて投与して、腫瘍の進行を遅延させ、生存率を上昇させるかのいずれでもよいことを示している。
PAM-RTで治療した肺の免疫細胞を特徴付けることによって、これらの細胞の免疫抑制性表現型が減少し、局所的なPAM-RT治療と同様の結果が示された。図16Aの図で見られるように、PAM-RTで治療した肺全体において、Tregが大幅に減少し、CD8/Treg割合が大幅に減少することにつながり(図26)、図16Bは、原発性腫瘍に対する焼灼の19日後のフローサイトメトリーによる肺全体の表現型を図示する(図面)。肺のT細胞を表現型分類によって、CD8+とCD4+ T細胞の両方でGzBの分泌が大幅に増加したことが明らかになった(図16C)。原発性腫瘍に対する焼灼のみを受けたマウスの病巣と比較して、PAM-RTで治療した肺のわずかな転移巣における微小転移巣の組織染色によって、CD8+ T細胞が大量に浸潤し、一方で、FoxP3+細胞が減少したことが示された(図16D)。未治療の動物では、脾臓の肥大およびCD45+白血球の減少を伴った脾臓の転移腫瘍が存在していた。原発性腫瘍に対するRTと、原発性腫瘍に対するRT+肺に対するPAM-RTによって、脾臓の大きさが減少し、CD45+白血球が大幅に増加し(図16E)、そして、CD3+ T細胞数が増加し(図16F)、RT+肺に対するPAM-RTによって、腫瘍を有さない野生型の動物において見られるような基準レベルに戻った。周辺の骨髄細胞の増大は、G-CSFを分泌する4T1腫瘍に関連付けられ、以上の報告に即するものであり、4T1腫瘍を有するマウスの脾臓においてCD45+の白血球中のマクロファージが大量に増加し、焼灼のみを用いて治療したマウス対焼灼+PAM-RTを用いて治療したマウスにおいてそれぞれ、これらの細胞が段階的に減少した。興味深いことに、未治療の腫瘍を有する動物において、脾臓のマクロファージは未成熟であり、MHCクラスII発現が減少した。原発性腫瘍に対する焼灼+/-肺に対するPAM-RTを用いて治療すると、マクロファージ中でクラスII MHC発現が大幅に増加し、PAM-RT治療によってマクロファージが活性化されたことを示した(図16G)。これらの実験によって、PAM-RT治療が、Tregを減少させ、マクロファージを活性化させて炎症性表現型にさせ、転移性腫瘍でCD8+ CTLを浸潤するのを促進することによって、原発性腫瘍ならびに転移部位におけるTMEの再構築を促進することができることが示されている。
細胞株およびマウス
マウス・ルイス肺癌細胞株である3LLは、ATCCから購入し、添加DMEM(10%FBS、5%ピルビン酸ナトリウム、2.5%NEAA、1%抗菌薬/抗有系分裂薬)で増殖させた。マウス乳癌細胞株である4T1は、ATCCから購入し、添加DMEM(10%FBS、1%抗菌薬/抗有系分裂薬)で増殖させた。細胞株は、4継代と8継代の間で使用し、マイコプラズマは、MycoAlert(Lonza LT07-705)を用いて4ヶ月毎に検査した。6~8週齢のC57BL/6マウスおよび10から12週齢のBalB/CマウスをNCIから注文し、胸腺欠損ヌードマウスをCharles Riverから注文した。実施された研究はすべて動物実験委員会(Institutional Animal Care and Use Committee )によって承認された。
C57BL/6マウスを、皮下にて1.5×105 3LL細胞で攻撃し、BalB/Cマウスを、乳房脂肪体に同所にて2×105 4T1細胞で攻撃した。治療前に、腫瘍を直径5mmになるまで増殖させた。腫瘍の大きさを週2回測定した。楕円体の式:V=(π/6×奥行×幅×高さ)を使用して腫瘍体積を計算した。
放射線を、Xstrahl LimitedのSmall Animal Radiation Research Platform(SARRP)を使用して与える。画像誘導放射線療法を、SARRPの内蔵のコーンビームCT(CBCT)を使用して実施する。CBCT取得後に、Muriplanを使用して治療計画を構築する。
腫瘍細胞を培養し、播種し、具体的な照射スキームを用いて治療した。5日目の最後の治療の6時間後および24時間後に細胞を採取した。細胞を採取し、フローサイトメトリーのために染色した。骨髄由来マクロファージは、M1(LPS 100ng/mL+IFNg 50ng/mL)、M2(IL-4 10ng/mL)に分極化し、24時間後に放射線を用いて治療した。T細胞集団は、CD3、CD8、CD4、CD25抗体を用いてナイーブC57BL/6マウスの脾臓から選別し、放射線治療前に一晩休ませた。免疫細胞は、最後の放射線線量の6時間後に採取した。
腫瘍または肺全体を、氷上で採取し、計量し、洗浄した。剃刀を用いた手動の摘出後に、腫瘍または肺全体を、磁気撹拌棒を備えた15mLのコニカルチューブ内にある1mLの消化緩衝液(10%FBS、コラゲナーゼIおよびIV(100u/mL)(Sigma)ならびに1xDNase1(Thermo Scientific))に移す。チューブを回しながら、15分間37°Cでインキュベートし、15分間手動で消化するために撹拌プレートに移す。単一細胞懸濁液を濾過する。フローサイトメトリーのために細胞を再懸濁する。
表面染色抗体を用いて、細胞を30分間4°Cで染色した。使用した抗体は、CD45、CD3、CD4、FOXP3、CD8、GzB、CD11b、MHCII、IL-10、TNFaおよびCD206を含んだ。洗浄後に、細胞を4%PFAを用いて固定するか、あるいは説明書に従ってBD Pharminogen Transcription Factorバッファーセットによる細胞内染色のために浸透化させる。細胞内染色は、FOXP3、TNFα、IL-10およびグランザイムBであり、続いて、4%PFAを用いて固定する。細胞をLSRIIフローサイトメーター(BD Biosciences)で収集し、Flow Joソフトウェア(Tree Star)によって分析する。
脾臓および流出領域リンパ節を、氷上で採取し、単一細胞懸濁液で処理した。脾臓において、赤血球はACK溶解緩衝液(Lonza)を用いて溶解した。完全RPMI(10%熱不活性化FBS、1%抗菌薬/抗有系分裂薬)での再懸濁前に、細胞を計数する。細胞内サイトカイン分析のために、GolgiStopおよびモネンシンを3時間37°Cで添加した。単一細胞懸濁液を、腫瘍を用いて行うときのように、フローサイトメトリー分析のためにさらに処理する。
免疫分析のために処理した脾臓は、1ウェル当たり1x10^6細胞で、コートELISPOTプレートに播種し、さらなる処理前に一晩インキュベートした。IFNγのELISPOT用試薬をBD biosciencesから注文し、メーカーのプロトコルに従った。グランザイムBのELISPOT用試薬をR&Dから注文し、メーカーのプロトコルに従った。
腫瘍および肺を採取し、4%PFAまたは10%ホルマリンに移し、4℃で保存した。その後、試料を70%に移し、パラフィンに包埋した。ブロックを切片にし、CD8およびFOXP3を用いて二重染色した。
統計分析は、PRISM 7(Graphpad Software)のソフトウェアを使用して実施した。分析は、student’s t検定またはANOVA分析を使用して多群比較によって実施した。データは、代表的な平均±標準偏差である。生存曲線は、ログランク(マンテル・コックス)検定およびゲーハン・ウィルコクソン・ブレスロー検定によって分析した。統計的有意性のために、P値は、*p<0.05、**p<0.005、***p<0.0005、****p<0.0001で表す。
治療開始の6日後に、腫瘍を氷上で採取して計量する。氷上で腫瘍を手動で摘出し、その後、RNAに移す。処理のために、試料を氷上で解凍し、Trizolを使用して、メーカーのプロトコルに従ってRNAを抽出する。260nm/280nmの比率が約2であるNanodropで、RNAを定量化する。キットのプロトコルに従ってVerso cDNA合成キット(Thermo Fisher)を使用してcDNA合成を達成する。ABI 7900HT上にて384ウェルプレートのRT-PCRのために、Powerup SYBRグリーンキット(Thermo Fisher)を使用した。使用したプライマーは、FOXP3 F-ACTCGCATGTTCGCCTACTTCAGおよびR-GGCGGATGGCATTCTTCCAGGT、ならびに、β 2ミクログロブリン F-TTCTGGTGCTTGTCTCACTGAおよびR-CAGTATGTTCGGCTTCCCATTCである。ΔCTは、SDS2.4を使用して計算し、未治療の対照群と比較した。
マウスを一晩絶食させ、撮像施設に運んだ。マウスに、0.1mlの生理食塩水中の[18F]フルオロ-2-デオキシグルコース(FDG)300~400uCi(12~15MBq)を尾静脈を介して注射し、撮像は、注射の1時間後に開始する。マウスをイソフロランを用いて麻酔し、Inveon Multimodalityスキャナ(Siemens)上で撮像した。ASIPROとIRW(両方ともSiemens)専用ソフトウェアのいずれかを使用して分析を実施する。
治療した4T1担癌マウスを屠殺した。肺を、10%India inkを用いて気管内注入によってふくらませた。肺を採取し、マクロ転移の脱色を可能にするために、Feket溶液(70%EtOH(300ml)、37%ホルムアルデヒド(30ml)、氷酢酸(5ml))で保存する前に1Lの水で洗浄した。転移巣は、計数することによって列挙した。
放射線治療スキームを試験するために、6週齢のC57BL/6マウスに、皮下にて3LLを足背に接種した。治療前に、腫瘍を50mm3になるまで増殖させた。試験期間中、腫瘍の大きさを週3回測定した。楕円体の式:V=(π/6×奥行×幅×高さ)を使用して腫瘍体積を計算した。
1.3回の20Gyの線量、
2.3回の20Gyの線量および抗PD1治療薬、
3.3回の20Gyの線量の12日後に4回の1日当たり0.5Gyの線量(肺全体に投与)、あるいは
4.3回の20Gyの線量および抗PD1治療薬の12日後に4回の1日当たり0.5Gyの線量(肺全体に投与)および抗PD1治療薬。
治療計画を図23に図示する。図24Aおよび24Bに示しているように(生存率を悪化させる可能性がある原発性腫瘍の局所不全マウスを除外する)、第4の群で治療されたマウスは、最も長い生存率を示した。4T1細胞を静脈内投与し、抗PD1治療薬のみ、4回の1日当たり0.5Gyの線量のみ、あるいは抗PD1治療薬および4回の1日当たり0.5Gyの線量のいずれかを用いて治療したマウスは、未治療のマウスと同様の生存率を有していた(図25参照)。
細胞株およびマウス
マウス・ルイス肺癌細胞株である3LLは、ATCCから購入し、添加DMEM(10%FBS、5%ピルビン酸ナトリウム、2.5%NEAA、1%Pen/Strep)で増殖させた。4T1マウス乳癌をATCCから購入し、添加DMEM(10%FBS、1%Pen/Strep)で増殖させた。
放射線を、Xstrahl LimitedのSmall Animal Radiation Research Platform(SARRP)を使用して与える。画像誘導放射線治療を、SARRPの内蔵のコーンビームCT(CBCT)を使用して実施する。CBCT取得後に、Muriplanを使用して治療計画を構築する。
免疫療法の治療群に無作為化した担癌マウスは、抗原ワクチンを用いるまたは用いないで、合計5回、3日毎に腹腔内(i.p.)にPD-1(200ug)、αCD40、4-1BBL、GM-CSFを受けた。
抗原に特異的なエフェクターCD8+ T細胞の活性化を誘発するために、腫瘍型に特異的な抗原にT細胞の共刺激ドメイン41BBLをコンジュゲートするSyntac構築物を使用する。Syntacに無作為化された担癌マウスは、300ugの腹腔内治療を1回受ける。抗原に特異的なSyntac治療を受けたマウスと、無関係なSyntac、または共刺激ドメイン41BBLのみを用いて治療したマウスを比較する。
腫瘍を氷上で採取し、計量した。剃刀の刃を用いて1mm×1mmの切片の手動の摘出後に、腫瘍を、磁気撹拌棒を備えた15mLのコニカルチューブ内にある1mLの消化緩衝液、5%FBS、コラゲナーゼIおよびIV(100u/mL)(Sigma)ならびに1xDNase1(Thermo Scientific)に移す。チューブを30分間37℃でインキュベートし、30分間手動で消化するために撹拌プレートに移す。単一細胞懸濁液を濾過し、分析のために再懸濁した。
表面染色抗体を用いて、単一細胞懸濁液の細胞を30分間4℃で染色した。洗浄後に、細胞を4%PFAを用いて固定するか、あるいは説明書に従ってBD Pharminogen Transcription Factorバッファーセットによる細胞内染色のために浸透化させた。細胞内染色は、FOXP3およびKi-67であり、続いて、4%PFAを用いて固定した。細胞をLSRIIフローサイトメーター(BD Biosciences)で収集し、Flow Joソフトウェア(Tree Star)によって分析した。
免疫蛍光抗体:使用した一次抗体は、CD3、CD8、CD4、CD11b、Gr1(BD Pharminogen)とした。二次抗体は、ヤギ抗ウサギFITCおよびヤギ抗ラットAPC(Abcam)とした。
治療の6日後に、腫瘍を氷上で採取して計量した。氷上に残したまま、腫瘍を、1.7mLのEppendorfチューブに移し、組織1ミリグラム当たり1Xプロテアーゼおよびホスファターゼ阻害剤(細胞シグナル伝達)を用いて、4.5uLのPBSで均質化した。チューブを、14,000gで15分間スピンダウンさせた。上清を汚れていないチューブに移し、-80℃で保存した。
マウスに、横腹の皮下より1×10^5 3LLを注射した。腫瘍を約6mmになるまで、約14日間増殖させた。マウスを、治療なし、1日目24Gy、22Gy+2日目開始の0.5Gy×4、あるいは2日目開始の0.5Gy×4に無作為化した。治療開始の6日後に、70kDaのFITCデキストラン(25mg/mL)を静脈内注入し、20分間循環させた。その後、マウスを屠殺し、血液および腫瘍を採取した。腫瘍を計量し、1.5mLのコラゲナーゼIVを用いて一晩37℃でインキュベートした。その後、腫瘍を均質化し、細胞片を14000rpmで5分間スピンダウンした。上清を回収し、蛍光のために485/535で実行した。その後、1mg当たりの蛍光を計算した。注射制御として、血液血清を使用した。

- 前記第1の領域は、骨、リンパ節、肺、肝臓、脳、副腎、乳房、眼、腎臓、筋肉、膵臓、唾液腺および脾臓からなる群から選択される器官の転移性腫瘍の領域を含み、前記第2の領域は、乳房、膀胱、脳、結腸、直腸、子宮内膜、腎臓、膵臓、前立腺、肝臓、肺、皮膚、甲状腺、子宮、リンパ節、扁桃、胸腺、脾臓および骨髄からなる群から選択される器官の原発性腫瘍を含む、請求項1に記載のコンピューター可読媒体。
- 前記第1の領域は、特定されている腫瘍を含み、前記第2の領域は、特定されている腫瘍を含まない、請求項1に記載のコンピューター可読媒体。
- 前記焼灼線量は、前記第1の領域において20Gyと100Gyの間を含む、請求項1~17のいずれか1項に記載のコンピューター可読媒体。
- 前記サブ焼灼線量は、0.1Gyと2Gyの間を含み、任意選択で、前記サブ焼灼線量は、複数のサブ焼灼線量を含み、前記複数のサブ焼灼線量はそれぞれ、前記第2の領域において0.1Gyと2Gyの間を含む、請求項1~17のいずれか1項に記載のコンピューター可読媒体。
- 前記サブ焼灼線量は、0.1Gyと0.5Gyの間を含み、任意選択で、前記サブ焼灼線量は、複数のサブ焼灼線量を含み、前記複数のサブ焼灼線量はそれぞれ、前記第2の領域において0.1Gyと5Gyの間を含む、請求項1~17のいずれか1項に記載のコンピューター可読媒体。
- 3回のサブ焼灼線量が投与される、請求項1~20のいずれか1項に記載のコンピューター可読媒体。
- 3回を超えるサブ焼灼線量が投与される、請求項1~20のいずれか1項に記載のコンピューター可読媒体。
- 1回目のサブ焼灼線量は、前記焼灼線量の投与後1時間から12日以内に投与される、請求項1~20のいずれか1項に記載のコンピューター可読媒体。
- 前記放射線療法は、コンピューター断層撮影法スキャン、磁気共鳴画像法、陽電子射出断層撮影法、コンピューター断層撮影法スキャンからなる群から選択される画像法によって測定された場合、治療される腫瘍の大きさまたは強度を低下させる、請求項1~23のいずれか1項に記載のコンピューター可読媒体。
- 前記放射線療法は、対象の生存率を上昇させるか、対象が発症する症状の数または重症度を低下させるか、腫瘍の微小環境にある免疫細胞の数を増加させるか、あるいは腫瘍微小環境にある活性化免疫細胞の数を増加させる、請求項1~23のいずれか1項に記載のコンピューター可読媒体。
- 放射線は、X線照射、γ線照射、α粒子照射、β粒子照射、中性子粒子照射、体外ビーム照射および小線源治療からなる群から選択される、請求項1~23のいずれか1項に記載のコンピューター可読媒体。
- 放射線療法システムであって、該放射線療法システムは、
焼灼線量およびサブ焼灼線量をもたらす放射線源と、
前記放射線源に結合されたプロセッサーであって、請求項1~26のいずれか1項に記載のコンピューター可読媒体の命令とともに構成される、プロセッサーと、を含む、放射線療法システム。
