Description
(a)本発明の2つの組織切除装置の鞘を、切除ゾーンの少なくとも一部を鞘同士の間に位置させて、(例えば、放射線学的処置で一般的に使用され、以下に詳細に説明するニードル-ワイヤ-拡張器-鞘処置で)患者内に経皮的に位置させる。
(b)電極が第1の構成で展開するように、鞘に対して組織切除装置のプローブを配向する。
(c)電極を第1の構成で配備し、そのように配備された電極間の組織を切除して、第1の切除部分を形成する。
(d)電極をそれぞれのプローブに後退させる。
(e)電極が第2の構成で展開するように、鞘に対してプローブを再配向する。
(f)電極を第2の構成で配備し、そのように配備された電極間の組織を切除して、第2の切除部分を形成する。
(g)組み合わされた切除部分が切除ゾーンを画定するまで、ステップ(d)から(f)を繰り返す。そして、
(h)患者から鞘を引き出す。
(a)本発明の組織切除装置の鞘を、患者における切除ゾーンに(例えば、放射線学的処置で一般的に使用され、以下に詳述するニードル-ワイヤ-拡張器-鞘処置を介して)経皮的に位置させる。
(b)電極が第1の構成で展開するように、鞘に対して組織切除装置のプローブを配向する。
(c)電極を第1の構成で配備し、組織を切除して第1の切除部分を形成する。
(d)電極をプローブに後退させる。
(e)電極が第2の構成で展開するように、鞘に対してプローブを再配向する。
(f)電極を第2の構成で配備し、組織を切除して第2の切除部分を形成する。
(g)組み合わされた切除部分が切除ゾーンを画定するまで、ステップ(d)から(f)を繰り返す。そして、
(h)患者から鞘を引き出す。
(a)本発明の複数の(例えば2つ以上の)組織切除装置の鞘を、切除ゾーンの少なくとも一部を鞘の間に位置させて、患者内に(例えば経皮的に)位置決めする。
(b)電極が第1の構成で展開するように、鞘に対して組織切除装置のプローブを配向する。
(c)第1の構成で電極を配備し、そのように配備された電極間の組織を切除して、第1の切除部分を形成する。
(d)電極をそれぞれのプローブに後退させる。
(e)電極が第2の構成で展開するように、鞘に対してプローブを再配向する。
(f)第2の構成で電極を配備し、そのように配備された電極間の組織を切除して、第2の切除部分を形成する。
(g)組み合わされた切除部分が切除ゾーンを画定するまで、ステップ(d)から(f)を繰り返す。そして、
(h)患者から鞘を引き出す。
本発明の実施形態は、添付図面を参照して、以下でさらに詳細に説明する。
(a)本発明の2つの組織切除装置の鞘を、切除ゾーンの少なくとも一部が実質的に鞘間に位置するように、(経皮的に)患者内(例えば、インターベンショナル・ラジオロジストが使用する従来のニードル-ワイヤ-拡張器アプローチを使用して事前に配置された拡張器上)に位置決めする。
(b)電極が第1の構成で展開するように、鞘に対して装置のプローブを配向する。
(c)電極をそれらの第1の構成で配備し、そのように配備された電極間の組織を切除して、第1の切除部分を形成する。
(d)電極をそれぞれのプローブに後退させる。
(e)電極が第2の構成で配備するように、鞘に関してプローブを再配向する。
(f)電極を第2の構成で配備し、そのように配備された電極間で組織を切除して、第2の切除部分を形成する。
(g)組み合わされた切除部分が切除ゾーンを画定するまで、ステップ(d)から(f)を繰り返す。そして
(h)患者から鞘を引き出す。

(g)組み合わされた切除部分が切除ゾーンを画定するまで、ステップ(d)から(f)を繰り返す。そして
(h)患者から鞘を引き出す。
(a)2つの組織切除装置の鞘を、切除ゾーンの少なくとも一部を鞘同士の間に位置させて、(例えば経皮的に)患者内に位置させる。
(b)電極が第1の構成で展開するように、鞘に対して装置のプローブを配向する。
(c)電極を第1の構成で配備し、そう配備された電極間の組織を切除して、第1の切除部分を形成する。
(d)電極をそれぞれのプローブに後退させる。
(e)電極が第2の構成で展開するように、鞘に対してプローブを再配向する。
(f)第2の構成で電極を配備し、そう配備された電極間の組織を切除して、第2の切除部分を形成する。
(g)結合された切除部分が切除ゾーンを画定するまで、ステップ(d)から(f)を繰り返す。そして、
(h)患者から鞘を引き出す。
(a)組織切除装置の鞘を、患者における切除ゾーンに(例えば経皮的に)位置させる。
(b)電極が第1の構成で展開するように、鞘に対して装置のプローブを配向する。
(c)電極を第1構成で配備し、組織を切除して第1の切除部分を形成する。
(d)電極をプローブに後退させる。
(e)電極が第2の構成で展開するように、鞘に対してプローブを再配向する。
(f)第2の構成で電極を配備し、組織を切除して第2の切除部分を形成する。
(g)組み合わされた切除部分が切除ゾーンを画定するまでステップ(d)から(f)を繰り返す。そして、
(h)患者から鞘を引き出す。
A.電極は最初に0°/0°で配備させる
B.電極を後退させて、プローブをその場で回転させ、そして電極を180°/180°で再び配備させる
C.電極を後退させて、プローブをその場で回転させ、そして電極を90°/270°で再び配備させる
D1.電極を後退させて、プローブをその場で回転させ、そして電極を135°/225°で再び配備させる
D2.電極を後退させて、プローブをその場で回転させ、そして電極を45°/315°で再び配備させる
・現在市場に出ているものよりも小さな切除装置を使用して、現在市場に出ている大型の装置によって生成可能なものに匹敵するか、又はより大きいボリュームを有する切除を生成することができる。
・鞘の小さなゲージは、経皮的な処置で装置の使用を可能にし、処置の複雑さを軽減し、可能な合併症を軽減する。
・電極のサイズ、形状及び構成の選択、ならびに配備の角度は、処置が開始された後でさえ、切除ボリュームに対する制御の前例のないレベルをオペレータに提供する、及び、
・処置の開始時に鞘のわずかな誤配置は、配備及び/又は配備された電極のサイズ又は構成の角度に対応する調整によって、処置をやり直すことなく、修正することができる。
Claims (14)
- 組織切除装置であって、
体組織内の位置決めのための鞘で、遠位端、近位端及びそれらの間に延びる内腔を含む鞘と、
前記内腔にスライド自在に受けられるように構成された伸長部を含むプローブで、前記伸長部は、前記伸長部の前記遠位端が前記鞘の遠位端に又はそれを超えて位置する場合に、前記伸長部の遠位端から実質的に平面的に配備された構成に配備可能な電極を収容するプローブとを備え、
前記プローブの遠位端からの前記電極の体内組織に対する配備角度は、前記鞘に対して前記プローブを配向させることによって選択可能であり、
前記プローブは、前記伸長部の遠位端が前記鞘の遠位端又はそれを超えて位置する場合に前記鞘の近位端で受けられるように構成された鞘隣接部を更に含み、
前記鞘隣接部及び前記鞘の近位端が、前記鞘隣接部及び前記鞘の近位端の間の相対的な配向を示す視覚又は触覚的な手段を含む組織切除装置。 - 前記鞘隣接部及び前記鞘の近位端が、使用時に互いに隣接する表面を含み、前記鞘隣接部及び前記鞘の近位端のそれぞれの表面がそれらの間の相対的な配向を視覚的に示す印を備える請求項1に記載の組織切除装置。
- 前記鞘隣接部及び前記鞘の近位端が、使用時に互いに隣接する表面を含み、前記鞘隣接部及び前記鞘の近位端が互いに所定の角度を向いた場合に合致するように構成された相互補完的な突起及び凹部を前記鞘隣接部及び前記鞘の近位端のそれぞれの表面に備えた請求項1又は2に記載の組織切除装置。
- 前記所定の角度が、約0°、90°、180°及び270°である請求項3に記載の組織切除装置。
- 前記電極の配備構成への配備時に、前記電極がコイル状に曲がる請求項1から4のいずれか一項に記載の組織切除装置。
- 前記電極の前記配備構成が略円形である請求1から5のいずれか一項に記載の組織切除装置。
- 前記電極が平線電極、丸線電極、平管電極又は丸管電極である請求1から6のいずれか一項に記載の組織切除装置。
- 前記電極が複数の電極を備え、各電極は配備時に体内組織に対して異なる配備構成をとる請求項1から7のいずれか一項に記載の組織切除装置。
- 前記複数の電極が、前記プローブの遠位端における前記伸長部の端部及び/又は側面に沿って設けられた複数の開口のそれぞれを通してそれぞれ独立して配備可能である請求項8に記載の組織切除装置。
- 前記装置で使用する前記プローブが複数の利用可能なプローブから選択可能であり、前
記利用可能なプローブ内の前記電極は、選択可能な配備構成をなすよう構成されている請求項1から9のいずれか一項に記載の組織切除装置。 - 前記プローブの遠位端から前記電極を配備するよう操作可能な配備アクチュエータを更に含む請求項1から10のいずれか一項に記載の組織切除装置。
- 前記配備アクチュエータが、前記配備構成と格納構成との間で前記電極を前進及び後退させる操作が可能である請求項11に記載の組織切除装置。
- 前記鞘の直径が約2.0mm未満である請求項1~12のいずれか一項に記載の組織切除装置。
- 接合部材が、接合された組織切除装置間の可変間隔を画定するように構成され得る請求項1から13のいずれか一項に記載の組織切除装置。
