塩酸、pH1.2緩衝液を調製した。II-38(2.5mg)を0.125mLのDMSO(5%)に溶解した。DMSOに溶解したII-38を、37℃に予熱したフランツセル拡散チャンバー内の、全体積2.375mLのHCl緩衝液、pH1.2(表1)に、ゆっくりと次第に添加することにより、反応を開始した。溶液を、30分毎に30秒

異なる非酵素的水性緩衝液中におけるII-38の安定性研究からのデータは、II-38が、酸性および塩基性緩衝媒質中において有意な加水分解を示さなかったことを明らかに指示している。II-38は、経口的に投与された場合、GI管において単一の化学実体として吸収されるために十分安定であろうと結論付けることができる。しかしながら、80%ラットおよびヒト血漿中におけるII-38の加水分解により、II-38は、全身循環中で加水分解して、その親薬物、2,4-DNPを産生するであろうと予測された。II-38におけるカルバメート結合は、血漿中で開裂されて2,4-DNPを緩やかな速度で産生し、これは、II-38を、2,4-DNPの化学的レザバとして長期間にわたって作用させる。
II-38プロドラッグおよびII-38プロドラッグから放出された2,4-DNPの経口バイオアベイラビリティおよび血漿薬物動態(PK)特性を、スプラーグドーリー(S-D)雄ラットにおいて、1.6mg/kgのII-38(1mg/kgの2,4-DNPに相当)の単回i.v.注射ならびに5、25および50mg/kgの2,4-DNPに相当する経口用量後に調査した。血液試料を最大24時間採取した。
ラットの使用を伴うすべての手順は、National Institutes of
HealthのGuide for the Care and Use of Laboratory Animals(1996年)によって設置したUniversity of Arkansas for Medical Sciences(UAMS)Institutional Animal Care and Use Committeeによって明記されたガイドラインに従って行った。重量250~300グラムの雄スプラーグドーリーラットを、UAMS研究用動物資源部門において、食物および水に自由にアクセスさせて、1ケージ当たり3匹ずつ収容した。ラットにイソフルラン(isofluorane)(2~5%)/酸素(1.5~2.0L/分)で麻酔をかけ、カニューレを頚(i.v.投薬)および大腿(血液採取)静脈に外科的に埋め込んだ。手術の3~4日後、ラットを、手術部位における局所感染の徴候について、毛の黄変について、鼻または目の周りの血液の存在について、食欲減退の兆候について、および糞便活動の減退または消失の徴候について観察した。手術からの成功裏の回復の適切な証拠の後、IVまたは経口投薬を実施し、血液試料(0.15mL)を、0、5、15、30、45、60、120、240および480分で収集した。血漿試料を調製し、LC/MS/MS分光分析を使用して分析を実行した。
2,4-DNPを、5%DMSOおよび20%PEG-400を含有するリン酸生理食塩水緩衝溶液、pH7.4に溶解し、0.2μmのフィルターに通して濾過した。経口経路では、ラットに、0.625/mLの2,4-DNPを含有するストック溶液から、全体積8mL/kgにおいて5mg/kgを胃管栄養した。i.v.注射では、全体積0.8mL/kgを、1.25mg/mLを含有するストック溶液から1mg/kgの用量で使用した。
i.v.注射では、全体積0.8mL/kgを、1.6mg/kg(1mg/kgの2,4-DNPに相当)の用量で使用した。経口経路では、1、5および25mg/mLの2,4-DNPに対する当量を含有するII-38のストック溶液を調製し、それぞれ1、5および25mg/kgの2,4-DNPに相当する最終用量でラットに投与した。8mg/kgのII-38の経口用量(5mg/kgに相当)を、5%DMSOおよび20%PEG-400を含有するリン酸生理食塩水緩衝溶液、pH7.4に溶解し、一方、40mg/kgの用量(25mg/kgに相当)を、1%DMSOを含有する0.5%メチルセルロース(メトセル)を用いて製剤化した。高用量のII-38(50mg/kgに相当)を、1%DMSOおよび40%PEG-400を含有する水を用いて製剤化した。II-38の製剤において使用される異なるビヒクルのため、DMSO、PEG-400および水(1%:40%:59%)からなるビヒクルを使用して、25mg/kgの2,4-DNPに相当する用量を繰り返した。後者の用量は、製剤における差異について、II-38から放出された2,4-DNPのPKデータの比較に利用した。
頚および大腿静脈にカテーテルを挿入したスプラーグドーリー雄ラット(重量250~300g)を、5mg/kgの単回経口用量(n=4)または1mg/kgの単回i.v.用量(n=3)の2,4-DNPで処置した。各i.v.および経口実験では、血液試料(0.15mL)を、0、5、15、30、45、60、120、240および480分で収集した。吸引した血液をヘパリン化生理食塩水(0.15mL)で置きかえた。血液試料を、10,000rpmにて室温で10分間にわたって遠心分離し、血漿試料を調製し、高感度LC/MS/MS分光分析方法を使用して分析を実行した(以下を参照)。
頚および大腿静脈にカテーテルを挿入したスプラーグドーリー雄ラット(重量250~300g、n=4)を、8、40および80mg/kgのII-38(それぞれ、5、25および50mg/kgの2,4-DNPに相当)の単回経口用量で処置するか、または1.6mg/kgのII-38(1mg/kgの2,4-DNPに相当、n=2)の単回i.v.用量を注射した。血液試料(0.15mL)を、0、5、15、30、45、60、120、240および480分で収集した。血漿試料を調製し、2,4-DNP、II-38、およびII-38濃縮物から放出された2,4-DNPを、後述するLC/MS/MS分光分析アッセイ方法を使用して定量化した。
ラット血漿から検体を単離するために、50μLの対照または処置血漿を、内部標準として10μLの2,4-DNP-d3(10μg/mL)で強化し、これに、0.3mLのメタノール、続いて、0.3mLのアセトニトリルを添加した。混合物を30秒間にわたってボルテックス混合し、10,000rpmにて室温で10分間にわたって遠心分離した。上清を5mLのガラス管に移し、窒素ガス下、37℃で蒸発乾固させた。ペレットを50μLのアセトニトリルで再構成し、30秒間にわたってボルテックス混合し、続いて、1分間にわたって音波処理した。1000rpmにて室温で10分間にわたる遠心分離の後、次いで、5μLの上清をカラム上に注入し、LC/MS/MS分光分析によって

1- 1mg/kgの2,4-DNPの単回i.v.注射または5mg/kgの経口投与の後の2,4-DNPの血漿中濃度;2- 1mg/kgの2,4-DNPに相当する1.6mg/kgのII-38のi.v.注射またはそれぞれ5、25および50mg/kgの2,4-DNPに相当する8、40および80mg/kgのII-38の経口用量の後の、II-38およびII-38から放出された2,4-DNPの血漿中濃度。質量分析計は、Agilent四重極型質量分析計であり、多重反応モニタリング(MRM)モードで操作した。2,4-DNP-d3を内部標準として使用した。2,4-DNP、II-38および2,4-DNP-d3は、ガードカラム:Alltima C18、5μm、4.6×7.5mm(Grace Discovery Sciences、IL、USA)を装備したAlltima C18カラム、5μm、3.2×150mm(Grace Discovery Sciences、IL、USA)を使用して分離した。移動相は、溶媒Aとしての0.005%ギ酸を加えた水および溶媒Bとしての0.005%ギ酸を加えたアセトニトリルからなる。2,4-DNP、II-38および2,4-DNP-d3化合物の分析のために、3.5分で10から90%溶媒B、これを次の3.30分間にわたって90%Bに維持し、次いで、3.20分間かけて平衡化し初期条件に戻る勾配を使用して、分離を達成した。流量は0.8mL/分であり、カラム温度は30℃であった。試料注入体積は5μLであった。質量分析計は、15Vの衝突エネルギーおよび75Vのフラグメンター(fragmentor)で、2,4-DNP、II-38および2,4-DNP-d3の標準を使用して決定された最適なイオン源の設定を用いて、負のエレクトロスプレーイオン化モードで操作した。モニターしたMRM遷移は、次の通りであった:2,4-DNPについては、m/z183.0/123.0、m/z183.0/153.0、2,4-DNP-d3については、m/z186.0/126.0、m/z186.0/156.0、II-38については、m/z297.2/183.2。ラット血漿中における2,4-DNPおよびII-38の2つの別個の検量線を生成し、それぞれ2,4-DNPおよびII-38を定量化するために使用した。
2,4-DNPのストック溶液を、アセトニトリル中で調製した。8つの異なる濃度を持つ検量線を調製した。較正曲線は、曲線下面積(AUC)対2,4-DNP濃度の二次最小二乗回帰を使用して取得した。曲線は、0.01~1μg/mlの間で線形であり、R2の相関係数=0.9874であった(図15)。
2,4-DNPおよびII-38ならびに内部標準(2,4-DNP-d3)のそれぞれのストック溶液を、アセトニトリル中で調製した。2,4-DNPおよびII-38溶液の連続希釈物を調製し、2,4-DNPおよびII-38較正基準のLC/MS/MSクロマトグラムを、各検体について血漿(n=5)中1~2000ng/mLの濃度範囲にわたって生成した。較正曲線およびクロマトグラムは、2,4-DNPおよびII-38についてそれぞれ20~2000ng/mLおよび200~1000ng/mLの間の優れた直線性を示し、2,4-DNPおよびII-38についてそれぞれR2の相関係数=0.9988および0.9739であった(図16および17)。
i.v.ボーラス投与後の個々の2,4-DNPおよびII-38プロドラッグ血漿中
濃度-時間プロファイルを、1コンパートメントオープンモデルおよび一次排泄(Phoenix WinNonlin、Professional、バージョン6.2、Pharsight、Mountain View、CA)を使用して算出した。
経口投与後、非コンパートメントオープンモデルおよび一次吸収によってデータを分析して、ピーク濃度(Cmax)、Tmax、および0から無限大までの曲線下面積(AUC0~inf)を決定した。2,4-DNP、II-38プロドラッグ、およびII-38から放出された2,4-DNPの実際の見かけのバイオアベイラビリティを、方程式1:
vitro安定性研究(第1章)は、pH1.2および7.4に調整された化学緩衝液中におけるII-38からの2,4-DNP形成の証拠を24時間の時間経過にわたって指示しなかった。同様に、II-38プロドラッグは、USPの方法によって調製された擬似胃液および擬似腸液中において、24時間(II-38および2,4-DNPのいずれも24時間後はグラフに提示されていない)の時間経過にわたって安定であった。故に、経口的に送達されたII-38は、GI管を介して全身循環に安定な実体として吸収され、全身循環中、酵素的条件下で加水分解されて、血漿中において2,4-DNPを放出するであろうと予測することができる。
g/kgのII-38の単回経口用量(それぞれ、5、25および50mg/kgの2,4-DNPに相当)または1.6mg/kgのi.v.ボーラス用量(1mg/kgの2,4-DNPに相当)後の、II-38から放出された2,4-DNPの血漿中レベルの時間経過を決定し、データを、ラットにおいて5mg/kgの単回用量で経口的に投与された、または1mg/kgでi.v.注射された2,4-DNPと比較した。
1コンパートメントモデルは、2,4-DNPおよびII-38のi.v.投薬について記述するものである。放出された2,4-DNPのピーク血漿中レベル(Cmax:2.2±1.1μg/mL)は5分以内に検出され、II-38の急速な開裂を指示した。非コンパートメントモデルは、経口II-38プロドラッグおよびその放出された2,4-DNPのPKについて適切に記述するものである。任意の時点で、II-38プロドラッグが、2,4-DNPよりも低い濃度で存在することを示すことができる(図19、20、21および22、ならびに表8および9)。故に、II-38プロドラッグの吸収は非常に急速であり、2,4-DNPの高速開裂および持続放出が続くように思われる。なぜなら、後者が、経口投薬後5分以内に血漿中で検出され、120~180分のtmax以内で3.2~15.2μg/mLのCmaxに達したからである。排泄の半減期(t1/2)は、2,4-DNPの経口およびi.v.投薬について、それぞれ136.3±10.9および90.0±9.9分である。これらの結果は、25(メトセル)、25(PEG-400)および50mg/kgに相当する用量のII-38から放出された2,4-DNPについての、248.2±45.3、185.9±18.1および334.1±63.0の長い平均終末半減期と比べて劣る(表2.2)ものであり、II-38の経口投与が、2,4-DNP単独の投与と比較して、2,4-DNPの送達における有意な改善をもたらしたことを示唆している。
血漿中において、II-38プロドラッグおよびその放出された2,4-DNPは、他の代謝部位に自由に分配するのに対し、in vitro血漿研究は血漿酵素のみに依拠していた。しかしながら、141.7~334.1分の長い半減期にわたるそのプロドラッグからの2,4-DNPの放出およびその緩徐放出に起因する120~240分の遅延したtmaxは、放出された2,4-DNPからの長期曝露および予想外というほどではない高ピークを意味する。換言すれば、経口II-38の後に放出された2,4-DNPは、急速な吸収および高い血漿Cmaxに起因する2,4-DNPの急性の有害作用を抑制する際に好適である。
この研究において、II-38プロドラッグの経口投与は、時間をわたってII-38から有意に放出され利用可能な2,4-DNPをもたらし、2,4-DNPの長期曝露(AUC)および有意なバイオアベイラビリティにつながった。見かけ上、II-38プロドラッグは、製剤目的のために安定であり、ラット血漿中で急速に加水分解される。
