Description

本発明のいくつかの実施形態で、トリプタミンの存在、及び場合によってはその濃度は決定される。トリプタミンは、当技術分野で既知の方法を使用して分析され得る。例えば、トリプタミンは、サーモTSQクアンタムマックストリプル四重極型質量分析計を使用して、タンデム質量分析(LC-MS/MS)に連結した液体クロマトグラフィを用いて分析され得る。他の有用な方法は、多重反応モニタリング及び情報依存型モニタリングを含む(Sridharan et al.,Nature Communications5:5492,2014)。
いくつかの実施形態で、組成物は、トリプトファンデカルボキシラーゼ遺伝子をコードする、少なくとも1つの細菌種を含む。このような遺伝子を同定する方法は、当技術分野で既知である(例えば、Williamsら(上述)を参照)。実施例にて説明するように、予測したタンパク質構造の配列分析、及び周知の微生物トリプトファンデカルボキシラーゼ配列との比較は、細菌ゲノムの遺伝子を同定するために使用できる。場合によっては、トリプトファンデカルボキシラーゼ遺伝子の全配列は分析で使用される、及び/またはトリプトファンの予測結合部位の配列が使用される。
本発明の実施形態で、マイクロバイオーム組成物は、治療を必要とする対象の疾患の徴候または症状を寛解させることを含む、疾患または状態を治療する、または予防するために使用する。いくつかの実施形態で、in silico、in vitro及び/またはin vivoの方法は、対象のトリプタミン濃度を変えることで治療の有効性を決定するために使用できる。
いくつかの実施形態では、TA組成物による治療に適した対象は、例えば糞便、血液、血漿、血清、尿、脳脊髄液(CSF)または他の体液中の、異常なまたは望ましくない低濃度のトリプタミンまたは5-ヒドロキシトリプタミンを有するとして識別される。TA組成物による効果的な治療は、選択した体液、組織または糞便中のトリプタミンまたは5-ヒドロキシトリプタミン濃度を増大させる。通常、トリプタミンまたは5-ヒドロキシトリプタミンの増大した濃度は、治療した対象の消化管中のTA組成物からの少なくとも1つの種の検出可能な存在と関連している。いくつかの実施形態で、TA組成物からの細菌の少なくとも1つの種または菌株の存在は、治療した対象の大便で検出される。いくつかの実施形態で、TA組成物の少なくとも1つの種または菌株は、TA組成物での治療の後、少なくとも1日、2日、3日、4日、5日、6日、1週間、2週間、3週間、4週間、2か月、3か月、4か月、5か月、6か月、8か月または1年後に検出されることができる。投与された種のこのような持続性(例えば、治療前、その種の不存在または低濃度で)は、「生着」と称される。いくつかの実施形態で、投与された種は、限られた期間、持続または生着することができず、そのような種はTA組成物の反復投与で投与され得る。
「相乗作用」とは、組み合わせ要素の予想される追加の効果より大きい、例えば異なる微生物(例えば、異なる菌株、異なる種または異なるクレード)の組み合わせによって生成される効果を指す。本明細書で使用する場合「相乗作用」または「相乗的相互作用」とは、それらの別個の効果の総量より大きい複合効果を生成する、2つ以上の微生物の相互作用または協調を意味する。前記効果は、in vitroまたはin vivoでの観測に基づき得る。
本明細書に記載のTA組成物は、当技術分野において周知の方法を用いて調製されて、投与されることができる。一般に、組成物は、任意の好適な方法を使用できるが、経口、結腸鏡または経鼻投与用に調製される。
すべての技術的特徴は、そのような特徴のすべての可能な組み合わせに個別に組み込まれることができる。
実施例1:ヒト糞便のゲノム及びメタボロミック分析
マイクロバイオーム組成物の第II相試験に参加している対象から入手したヒト糞便試料は、マイクロバイオーム組成物の安全性及び有効性を評価するように設計された、カプセル封止された精製真正細菌胞子(clinicaltrials.gov、NCT02437487)から構成される。試料は、治療を受けた対象及びプラセボを受けた対象の両方からの試料を含む、NCT02437487試験の対象から入手した。トリプタミンの存在に関する試料のメタボロミック分析は、Metabolon(Morrisville、NC)で行われた。メタボロミックデータは、大便の質量当たりのトリプタミン濃度を含んだ(濃度に比例するピークAUC、及びアッセイの線形動力学を仮定)。試料は更に、MetaPhlAn2データベースに基づく種特異的ゲノムマーカーに基づいて示される、全メタゲノミクスショットガンシーケンシング及び種名を使用して配列決定された(Truong et al.,Nat.Methods12:902-903,2015)。
対の分類及びメタボロミックプロファイルの標準分析は多くの場合、存在量が代謝物質の存在量と相関している種を同定するための、種と代謝物質の存在量の間のペアワイズ相関(例えば、スピアマンまたはピアソン相関)を含む。このような相関分析は、コホート及びインターベンション試験の両方で示したように、通常、代謝物質の大きな群と相関性がある種の大きな群を示す。これは、標準的な相関分析が、どの種が選択した代謝機能に本当に機構的に関与しているかを適切に識別しないことを意味する。
実施例1に記載されているトリプタミンに関連する解析は、その存在が糞便試料中のトリプタミンの存在に関連した少数の種を同定した。表7は、トリプタミンの存在に最も強く関連した3つの種のランク分けリスト(1とランク付けされる、トリプタミンの存在に最も強く関連したものから順に記載)を提供し、最高濃度のトリプタミンと関連した3つの種は、Ruminococcus gnavus、Lachnospiraceae Bacterium9_1_43BFAA及びRuminococcus Torquesであった。
初期の研究で、Ruminococcus gnavus(uniprot id、A7B1V0_RUMGN)及びClostridium sporogenes(uniprot id、J7SZ64_CLOSG)分離株(Williamsら、上述)からのすでに同定されたトリプトファンデカルボキシラーゼ遺伝子を、他の配列決定した細菌ゲノムの相同体を検索するための問い合わせ配列として使用した。1155のヒト腸関連の細菌ゲノムのデータベース(WGS情報を使用してアセンブル)は、BLASTPによる相同物検索ためにアセンブルされた。配置した残基の対は、BLOSUM62アラインメントスコアマトリックスを使用してスコア化された。24の種は、いずれかの問い合わせ配列に対して1e~100未満と報告されているアラインメントE-valueを有するタンパク質相同体を含むと同定された(完全残基一致%、アラインメント長さ及びe-valueと共に。表8を参照)。同定した24の相同体の中で、同一残基の割合は38~99.4%の範囲であり、アラインメント長さは407~477の範囲だった。これらの解析で、驚くような発見があった。

方法
出願人は、それらの上澄み中のトリプトファン代謝物質の存在について、種々の細菌種を試験した。トリプトファン代謝物質の存在は、インドール化合物(インドール試薬、Anaerobe Systems)の検出のための比色アッセイを使用して判定した。比色インドールアッセイによるTrp代謝物質の産生株として同定された選択された株の上澄みは、GC-MSによって更に解析されて、産生される特定の代謝物質を識別した。
いくつかの種は、トリプタミンまたは5-ヒドロキシトリプタミン(5HT)を生成することを示した。特に、トリプタミンを生成する種は、Clostridium sporogenes及びRuminococcus gnavusを含み、3つの種のうちの2つは、ヒト糞便試料中のトリプタミンの存在と相関していることがわかった(表14)。5HTを生成する種は、Bacteroides ovatus及びBacteroides stercorisを含む。
