Description
Claims (10)
- 患者に左心室心臓ペーシング療法を送達するための植え込み可能な医療デバイスシステムであって、
植え込み可能な医療デバイスハウジングと、
前記ハウジングに電気的に結合されることが可能な単一パス(single-pass)冠状静脈洞(coronary sinus)リードと、
前記患者の遠方場(far-field)心臓信号を検知するため、および心臓ペーシング療法を送達するために、前記単一パス冠状静脈洞リードに沿って位置付けられる複数の電極と、
前記ハウジング内に位置付けられ、前記左心室心臓ペーシング療法の送達を一時停止し、複数の電極の間に形成される1つまたは複数の遠方場検知ベクトルを介して遠方場心臓信号を検知し、検知された前記遠方場心臓信号に応答して遠方場心臓特徴を決定し、決定された前記遠方場心臓特徴に応答して第1のオフセット間隔および第2のオフセット間隔を決定し、決定された前記第1のオフセット間隔および前記第2のオフセット間隔に応答して前記左心室心臓ペーシング療法のAV遅延を調整し、調整された前記AV遅延を有する前記左心室心臓ペーシング療法の送達を再開するように構成されるプロセッサと、
を備える、植え込み可能な医療デバイスシステム。 - 前記プロセッサが、
検知された前記遠方場心臓信号に応答して、P波の始まり、前記P波の終わり、およびQRS群の始まりを決定し、
前記P波の始まりから前記P波の終わりまでに及ぶP波持続時間、および前記P波の始まりから前記QRS群の始まりまでに及ぶPQ持続時間を決定し、
前記P波持続時間に応答して前記第1のオフセット間隔を決定し、前記PQ持続時間に応答して前記第2のオフセット間隔を決定するように構成される、
請求項1に記載の医療デバイスシステム。 - 前記プロセッサが、
前記1つまたは複数の遠方場検知ベクトルの各々によって検知される前記遠方場心臓信号の各々に沿った第1の偏向(deflection)を決定して、第1に発生する偏向を生成し、
前記P波の始まりを前記第1に発生する偏向のうちの最早のものであるとして決定し、
前記第1の偏向の直後に発生する、前記1つまたは複数の遠方場ベクトルの各々によって検知される遠方場心臓信号の各々に沿った第2の偏向を決定して、第2に発生する偏向を生成し、
前記P波の終わりを前記第2に発生する偏向のうちの最後のものであるとして決定し、
前記第2の偏向の直後に発生する、前記1つまたは複数の遠方場ベクトルの各々によって検知される遠方場心臓信号の各々に沿った第3の偏向を決定して、第3に発生する偏向を生成し、
前記QRS群の始まりを前記第3に発生する偏向のうちの最早のものであるとして決定するように構成される、
請求項2に記載の医療デバイスシステム。 - 前記プロセッサが、前記P波持続時間および第1のオフセット間隔の合計を決定し、決定された前記合計に応答して前記第1のオフセット間隔を決定するように構成される、請求項2に記載の医療デバイスシステム。
- 前記プロセッサが、前記PQ持続時間を第2のオフセット間隔だけ低減し、低減された前記PQ持続時間に応答して前記第2のオフセット間隔を決定するように構成される、請求項4に記載の医療デバイスシステム。
- 前記第1のオフセット間隔が、30ミリ秒と設定され、前記第2のオフセット間隔が、50msと設定される、請求項5に記載の医療デバイスシステム。
- 前記プロセッサが、前記第1のオフセット間隔および第2のオフセット間隔のうちの最早のものを決定するように構成され、前記AV遅延を調整することが、前記第1のオフセット間隔および前記第2のオフセット間隔のうちの決定された前記最早のものに等しいAV遅延を設定することを含む、請求項5に記載の医療デバイスシステム。
- 前記プロセッサが、前記1つまたは複数の遠方場検知ベクトルの各々によって検知される前記遠方場心臓信号の各々について、固有伝導が検知されているかどうかを決定し、固有伝導が検知されている遠方場検知ベクトルについてのみ前記遠方場信号特徴を決定するように構成される、請求項1に記載の医療デバイスシステム。
- 前記プロセッサが、
前記1つまたは複数の遠方場検知ベクトルの各々によって検知される前記遠方場心臓信号の各々に沿った第1の偏向を決定して、第1に発生する偏向を生成し、
P波の始まりを前記第1に発生する偏向のうちの最早のものであるとして決定し、
前記第1の偏向の直後に発生する、前記1つまたは複数の遠方場ベクトルの各々によって検知される遠方場心臓信号の各々に沿った第2の偏向を決定して、第2に発生する偏向を生成し、
前記P波の終わりを前記第2に発生する偏向のうちの最後のものであるとして決定し、
前記第2の偏向の直後に発生する、前記1つまたは複数の遠方場ベクトルの各々によって検知される遠方場心臓信号の各々に沿った第3の偏向を決定して、第3に発生する偏向を生成し、
QRS群の始まりを前記第3に発生する偏向のうちの最早のものであるとして決定し、
前記P波の始まりから前記P波の終わりまでに及ぶP波持続時間、および前記P波の始まりから前記QRS群の始まりまでに及ぶPQ持続時間を決定し、
前記P波持続時間および前記第1のオフセット間隔の合計を決定し、前記第1のオフセット間隔を生成し、前記PQ持続時間を前記第2のオフセット間隔だけ低減し、前記第2のオフセット間隔を生成し、
前記第1のオフセット間隔および前記第2のオフセット間隔のうちの最早のものを決定するように構成され、前記AV遅延を調整することが、前記第1のオフセット間隔および前記第2のオフセット間隔のうちの決定された前記最早のものに等しいAV遅延を設定することを含む、
請求項1に記載の医療デバイスシステム。 - 命令が記憶されている非一時的なコンピュータ可読媒体であって、前記命令が、請求項1~9のいずれか一項に規定されるような動作を植え込み可能な医療デバイスに実施させる、非一時的なコンピュータ可読媒体。
