細胞の単離及びセルソーティング
ヒト抹消血単核球(PBMC)を、Ficoll-Paque Plus(GE Healthcare社)勾配遠心分離を使用して、健常な男性ドナーの新鮮バフィーコートから得た。総CD4+T細胞を、CD4マイクロビーズ(Miltenyi Biotec社)を用いた磁気分取を使用して単離し、次に、生存可能な細胞を、FACS Aria III(BD Biosciences社)でのCD25(43839、BD社)、CD127(351309、Biolegend社)及びCD45RA(560675、BD社)についてのフローサイトメトリーソーティングを使用して分けた。血液試料を、医薬倫理委員会により確立されたガイドラインに従い使用した文書のインフォームドコンセントのある匿名化したボランティアから得た。
細胞を、PBS 0.5%BSA中の蛍光色素でコンジュゲートされた抗体で30分間4℃にて標識した。細胞内及び核染色のため、細胞を固定し、製造元の指示に従い、Foxp3/転写因子固定化/透過バッファー(eBioscience社)を使用して透過処理した。細胞を、LSR Fortessa又はLSR IIサイトメーター(BD Biosciences社)にて解析した。次の分子に対する抗体を使用した:CD25、CD45RA、CD127、FOXP3(25-4777-42、eBioscience社)、ヘリオス(48-9883、eBioscience社)。A. Scott博士(Olivia Newton-John Cancer Research Institute、Heidelberg Australia)から頂いた、GPA33に対する抗体[Heath, J.K.ら、1997年]を、Lynx迅速APC抗体コンジュゲーションキット(LNK032APC、Biorad社)を使用してAPCで標識した。
FACSで精製したCD4+サブセットを、PBSにおいて洗浄し、直ちに、タンパク質LoBindチューブ(Eppendorf社)中の溶解バッファー(4%SDS、100mM DTT、100mM Tris HCl[pH8.0])にて溶解した。次に、試料を沸騰させ、超音波処理(Bioruptor)し、遠心分離(16,000g)後、上清を-80℃にて凍結維持した。次に、細胞溶解液を、試料調製を目的としたフィルター(FASP)を使用して処理した。簡単に言うと、溶解液を、還元、アルキル化及びシーケンスグレードのトリプシン(Promega社)での溶液内切断の対象にした。切断後、ペプチドを、StageTipsを使用して脱塩し、speedvacにて還元し、MSによる解析前に水中の0.1%TFA中の2%アセトニトリルにおいて復元した。ナノエレクトロスプレーイオン源(Nanospray Flex Ion Source、Thermo Scientific社)を介したOrbitrap Fusion Tribrid質量分析計(Thermo Scientific社)にオンラインで繋げたナノスケールのC18逆相クロマトグラフィーにより、MS実験をトリプリケートで行った。単離ウィンドウ1.6を有する四極子を使用した単離、高衝突解離(HCD)断片化、及びイオントラップにおける迅速スキャン質量分析解析により、タンデム質量分析を行った。
製造元のプロトコールに従い、TRIzol試薬(カタログ番号15596-018、Ambion Life Technologies社)を使用して1×106細胞から、総RNAを抽出した。総RNAペレットを、8分間風乾させ、適量のヌクレアーゼを含まない水(カタログ番号AM9937、Ambion life technologies社)に溶解し、続いて、ナノドロップUV-VIS分光光度計(Thermo Scientific社)を使用して総RNA定量を行った。製造元の指示に従い、MinElute Cleanupキット(カタログ番号74204、Qiagen社)を使用して、総RNAを更に精製した。総RNAの質及び量を、ナノチップ(Agilent社、Santa Clara、CA)を使用した2100 Bioanalyzerにより評価した。RNA強度数(RIN)>8を有する総RNA試料を、ライブラリー生成の対象にした。
製造元(Illumina社、パーツ番号15031047 Rev. E)の指示に従い、TruSeq鎖mRNA試料調製キット(Illumina Inc.社、San Diego、RS-122-2101/2)を使用して、鎖特異的ライブラリーを生成した。簡単に言うと、インタクトな総RNA由来のポリアデニル化RNAを、オリゴ-dTビーズを使用して精製した。精製後、RNAを、断片化し、アクチノマイシンDを添加して、SuperScript II逆転写酵素(Invitrogen社、パーツ番号18064-014)を使用してランダムプライム及び逆転写した。dTTPをdUTPに置換して、ポリメラーゼI及びRNaseHを使用して、第2の鎖合成を行った。生成したcDNAフラグメントを、3'末端でアデニル化し、イルミナ対末端シーケンスアダプターにライゲーションし、続いて、12サイクルのPCRにより増幅した。7500チップ(Agilent社、Santa Clara、CA)を使用して2100 Bioanalyzerにて、ライブラリーを解析し、希釈し、15-plex、10nMシーケンスプールに等モルでプールし、-20℃にて保存した。
RNA-Seq未処理読み取り値Fastqを、TopHat(バージョン2.0.12、Bowtieバージョン1.0.0、Samtoolsバージョン:0.1.19)を用いて、Ensembl参照ゲノム(Homo_sapiens.GRCh38.dna.primary_assembly)に整列させた。固有にマッピングした読み取り値を含むHTseq-カウントにより、読み取り値カウントを生成した。マッピングしてない読み取り値を取り除いた。配列読み取り値を、1つの試料当たり1000万の読み取り値に対して標準化し、式、log2(((発現遺伝子×÷ライブラリーサイズ)106)+1)(式中、ライブラリーサイズは、1つの試料当たりの全発現値の合計である)を用いて、log2変換した。差次的発現についてQlucore Omics Explorer(3.1)により、読み取り値カウントを更に解析した。
細胞を、様々な時間、抗CD3/CD28ビーズ(Miltenyi社:3つのビーズ:1個の細胞比)、300IU/ml IL-2と共に、IMDM(Lonza社)中の100nmラパマイシンあり又はなしで培養し、10%FCS、1%ペニシリン/ストレプトマイシン及び1%L-グルタミンを補充した。機能アッセイ(サイトカイン産生、抑制)のため、細胞から、ラパマイシンを取り除き(必要な場合)、IL-2を含む培地にて一晩休ませた。細胞内サイトカイン染色のため、細胞を、ゴルジ-プラグの存在下で4時間37℃にて、PMA(20ng/ml)及びイオノマイシン(1μM)で刺激し、続いて、細胞内サイトカインを使用して染色した。
異なるCD4 T細胞集団を、CD45RA及びCD25の発現により区別する
5つのCD4 T細胞集団を、CD25及びCD45RA表面マーカーの発現に基づき、健常ヒトドナー由来の末梢血単核球(PBMC)の間で同定することができる(図1)[Miyara, M.ら、2009年]。これらのうち2つの集団が、Tregを含有する。CD25+CD45RA+CD4 T細胞は、ナイーブTreg(図1において赤色で特徴付け、1を用いて示したnTreg)として知られている。これらの細胞は、決して活性化されず、定義により、恐らく、胸腺由来である。Tregの第2の集団を、CD25高CD45RA-発現特性に基づき同定する(eTreg、図1において橙色で特徴付けられ、2を用いて示す)。これらの細胞は、活性化されており、活性化tTregとiTregの混合物からなる。本発明者らの解析における他のCD4 T細胞集団は、CD45RA+CD25-ナイーブTconv細胞(図1において、濃い青色、及び4を用いて示す)及びCD45RA-CD25-エフェクター/メモリーTconv細胞(図1において、淡い青色、及び5を用いて示す)を含む。最後に、CD25+CD45RA-細胞の集団(図1において、P3-緑色、及び3を用いて示す)を解析した。エフェクターTreg(eTreg)からのこれらの後者の細胞の分離は、使用したマーカーに基づき、幾分恣意的である。CD127の発現も、追加のマーカーとして採用することはできるが、P3細胞の一部はCD127-である[Pesenacker, A.M.ら、2013年]ので、これは依然として、2つの集団の明白な分離を可能にしない。それらの表現型類似性にも関わらず、eTreg及びP3細胞は、劇的に異なる機能を有する。eTregは、免疫を抑制し、炎症性サイトカインを生じる能力を欠く一方、P3細胞は、炎症性サイトカインを生じ、抑制性でない[Miyara, M.ら、2009年;Pesenacker, A.M.ら、2013年;Ayyoub, M.ら、2009年]。事実、腫瘍における本物のTregの存在は、不良な予後と関連付けられる一方、これらのP3細胞の存在については、逆が真であり、これが、T細胞のこれらの表現型上類似する集団の間での機能の差の根底をなす[Saito, T.ら、2016年]。
特定のTreg集団の精製を可能にする新規の表面マーカーを同定するために、本発明者らは、CD25及びCD45RA発現により定義した5種のCD4 T細胞タイプでの全細胞定量質量分析(MS)を行った。サブセットを、FACSソーティングにより単離し、続いて、MSにより解析した。この解析において、本発明者らは、表面分子GPA33が、nTreg集団において、CD4、CD27及びCD28のような周知のT細胞表面分子の範囲内にあるレベルで優先的に発現されることを見出した(図2A)。他のCD4 T細胞集団もまた、この分子を発現するが、レベルは、nTregにおいて見出されるものの約10分の1の低さである。nTregサブセットにおけるGPA33の優先的発現をまた、mRNAレベルで表した(図2B)。MSは、溶解液のタンパク質濃度を測定するので、このより低い発現レベルは、1個の細胞当たりより少ない分子の存在又は細胞のより少ない集団でのこのマーカーの存在(若しくはその組合せ)を表す。
集団として、Tregは、IFNγ及びIL-17のような、エフェクターサイトカインをわずかに産生する。興味深いことに、これらのサイトカインを産生するnTreg及びeTreg集団におけるそれらの少数の細胞を、GPA33陰性及び中集団に主に限定されていた。この効果は、CD127-P3集団において最も著しく明らかであった。この集団におけるこれらのサイトカインを産生する能力は、GPA33の発現と逆に相関し、最高レベルのこのマーカーを発現する細胞は、ほとんど、検出可能なサイトカイン産生を全く示さない(図5)。したがって、CD127-CD4 T細胞(nTreg、eTreg及びP3)上でのGPA33の高発現は、エフェクターサイトカインを産生する最低の能力を有する集団を特徴付ける。
Tregを用いた養子細胞療法の課題の1つは、Treg調製物は、拡大を抑制し機能特性を変え得るラパマイシンを添加しない限り、Tconvの増加の方が過剰になってしまうことである[Hippen, K.L.ら、2011年]。この過剰増加は、TregのTconvへの変換、並びにTconvを含む開始Treg集団の混入の両方により説明し得る。本発明者らは、高発現のGPA33についての選択が、より純粋なTreg集団の単離をもたらし得ると推論した。実際、高発現のこのマーカー(nTregの75%がGPA33+であるレベルとして定義した)についての選択が、CD25+単独についての選択と比較して、FoxP3+及びヘリオス+細胞の割合を顕著に上昇した。純度は、現在の臨床上の調製物のため使用される[Trzonkowski, P.ら、2015年;Bluestone, J.A.ら、2015年]、CD25+CD127-特性に基づく選択と比較してさえ改善した。このより高い純度は、eTreg及びP3集団内で最も顕著であった(図6)。
養子細胞療法における使用のため、本質的な課題は、拡大したTregが、それらの抑制性機能を果たす能力を保持しているかどうかである。このためのゴールドスタンダードのインビトロ試験は、異なる数のTregの存在下でTconvを活性化することである。この試験を使用して、本発明者らは、ラパマイシンなしで2週間培養したCD25+CD127-GPA33高細胞が、TregのTconvに対する低い比でさえ、CD4+とCD8+Tconvの両方の増殖を強く抑制することを示す(図9)。
tTregは、iTregよりTreg系統により安定的にコミットされると考えられている[Sakaguchi, S.ら、2008年]。組織培養物におけるGPA33高細胞の徹底的な安定性を与えると、この表面分子が、ヒトtTreg集団を特徴付けることが想像できる。初代ヒト試料におけるiTregを明白に同定することは、現在可能でないが、かかる細胞を、TGFβの存在下でCD4 Tconvを培養することによりインビトロで生成することができる[Kanamori, M.ら、2016年]。これらの条件は、FoxP3の分化を誘導するが、Tconvへの抑制性能力を有するヘリオス発現細胞を誘導しない[Kanamori, M.ら、2016年]。しかしながら、Tregに分化するにもかかわらず、かかる細胞は、高発現のGPA33を獲得しない(図10)。他方、7日目のインビトロ培養物において、nTregは、高発現のGPA33を維持する(図10)。まとめると、これらの知見は、GPA33が、ヒトtTreg集団を安定的に特徴付けるという見解と一致する。
ヒトCD4+T細胞のサブセット(P3として言及する)は、Tregの特徴(FOXP3及びCD25)を発現するが、それにもかかわらず、Tconv機能を示す[Miyara, M.ら、2009年]。これらの細胞は、例えば、それらがCD127-であるときでさえ、高レベルのIL-17を容易に産生し、それ故、一般に使用されるTreg特性に対応する(図11)。他方、eTregは、IL-17を産生しない(図11)。重要なことに、CD127の発現にも関わらず、全てのIL-17産生細胞は、GPA33-亜集団において見出され、一方、GPA33+細胞は、常にこのサイトカインを産生する能力を欠いていた。
Aire転写因子は、自己反応性胸腺細胞のTreg系統への分化をもたらすのに必要な、胸腺の髄質上皮細胞における自己抗原の発現を調節する。Aire欠損は、かかるTregを生じることができないことに起因して、マウスにおいて強力な自己免疫疾患を引き起こす[Sakaguchi, S.ら、2008年]。同様に、Aireをコードする遺伝子における変異は、自己免疫性多腺性内分泌不全症・カンジダ症・外胚葉ジストロフィー(APECED)としても知られる自己免疫症候群の発症を引き起こす。一部のTregは、Aireが欠損していても依然として発生するが、明らかに、病理の発症を防ぐことができない。興味深いことに、APECED患者において依然として発生するnTregは、低減した発現のGPA33を有する(図12を参照)。
Claims (35)
- GPA33+調節性T細胞について細胞の集団を濃縮する方法であって、
a.細胞の集団を、GPA33に結合する能力がある剤と接触させる工程、
b.前記細胞のGPA33に結合する能力がある前記剤への結合を決定する工程、及び
c.細胞の前記集団におけるGPA33の発現の平均レベルより高い、GPA33の発現のレベルを有するCD4+T細胞を選択し、及び/又は単離する工程
を含む方法。 - 調節性T細胞を単離する方法であって、CD4+GPA33+T細胞を細胞試料から単離する工程を含む方法。
- CD4+CD25+CD127-GPA33+細胞を選択し、及び/又は単離する工程を含む、請求項1に記載の方法。
- CD4+CD25+CD127-GPA33+又はCD4+CD25+CD127低GPA33+細胞を選択し、及び/又は単離する工程を含む、請求項1に記載の方法。
- 細胞の前記集団又は前記試料が、哺乳類の血液、滑液若しくはリンパ液を含むか、又は哺乳類の血液、滑液若しくはリンパ液から得られるT細胞を含む、請求項1から4のいずれか一項に記載の方法。
- 前記血液が、臍帯血又は末梢血である、請求項5に記載の方法。
- 前記単離する工程又は接触させる工程が、前記試料又は細胞の集団を、GPA33に特異的に結合する抗体又はその抗原結合フラグメントと接触させる工程を含む、請求項1から6のいずれか一項に記載の方法。
- 前記細胞の増殖、活性化及び/又は増加を促進する1つ又は複数の因子の存在下で単離された細胞を培養する工程を更に含む、請求項1から7のいずれか一項に記載の方法。
- 調節性T細胞を同定する方法であって、細胞をCD4及びGPA33の発現について解析する工程を含み、CD4+GPA33+発現パターンが、調節性T細胞又は調節性T細胞の集団の指標である、方法。
- 前記細胞が、腫瘍又は腫瘍試料から得られる、請求項9に記載の方法。
- 調節性T細胞を検出する方法であって、試料における細胞をCD4及びGPA33の発現について解析する工程を含み、CD4+GPA33+発現パターンが、調節性T細胞又は調節性T細胞の集団の指標である、方法。
- 前記試料を、抗CD4抗体又はその抗原結合フラグメント及び抗GPA33抗体又はその抗原結合フラグメントと接触させる工程、及び前記試料における細胞と前記抗CD4抗体又はその抗原結合フラグメント及び前記抗GPA33抗体又はその抗原結合フラグメントの間での結合を検出する工程を含む、請求項11に記載の方法。
- 前記試料が、自己免疫疾患に罹患している個体由来の試料である、請求項11又は12に記載の方法。
- 前記試料が、未知の自己免疫疾患に罹患している個体由来の試料である、請求項13に記載の方法。
- 前記試料におけるCD4+GPA33+細胞のレベルを定量する工程を含む、請求項11から14のいずれか一項に記載の方法。
- CD4+CD25+GPA33+細胞のレベルを定量する工程を含む、請求項15に記載の方法。
- 自己免疫疾患を調節性T細胞不全により特徴付けられる自己免疫疾患として分類する方法であって、請求項13から16のいずれか一項に記載の方法に従い、自己免疫疾患に罹患している個体由来の試料におけるCD4+CD25+GPA33+細胞のレベルを定量する工程、及び前記レベルを参照試料におけるCD4+CD25+GPA33+細胞のレベルと比較する工程を含み、参照試料と比較したCD4+CD25+GPA33+細胞の低減が、調節性T細胞不全によって特徴づけられる免疫疾患の指標である、方法。
- 前記参照試料が健常個体由来の試料である、請求項17に記載の方法。
- 調節性T細胞についてのマーカーとしてのGPA33の使用。
- 細胞の少なくとも80%がCD4+GPA33+調節性T細胞である、細胞の単離された集団。
- 前記細胞が、CD4+CD25+CD127-GPA33+又はCD4+CD25+CD127低GPA33+細胞である、請求項20に記載の細胞の集団。
- 請求項20又は21に記載の細胞の集団、並びに医薬的に許容される担体、希釈剤、及び/又は賦形剤を含む医薬組成物。
- 療法における使用のための、CD4+GPA33+T細胞又は請求項20又は21に記載の細胞の集団。
- 免疫応答の抑制において使用するための、CD4+GPA33+T細胞又は請求項20又は21に記載の細胞の集団。
- 移植片対宿主疾患(GVHD)、移植片拒絶、慢性炎症状態又は自己免疫疾患の処置、緩和又は予防における使用のための、CD4+GPA33+T細胞又は請求項20又は21に記載の細胞の集団。
- それを必要とする対象において免疫応答を抑制するための、CD4+GPA33+T細胞、又は、請求項20又は21に記載の細胞の集団を含む医薬組成物。
- 移植片対宿主疾患(GVHD)、移植片拒絶、慢性炎症状態若しくは自己免疫疾患の処置、緩和又は予防するためのCD4+GPA33+T細胞又は請求項20又は21に記載の細胞の集団を含む医薬組成物。
- 前記状態又疾患が、1型糖尿病、多発性硬化症(MS)、全身性エリテマトーデス(SLE)、関節リウマチ、乾癬性関節炎、骨関節炎、強直性脊椎炎、再生不良性貧血、血小板減少性紫斑病、グレーブス病、アジソン病、乾癬、ぶどう膜炎、自己免疫性溶血性貧血、炎症性腸疾患、潰瘍性大腸炎、クローン病、アレルギー状態及び喘息状態からなる群から選択される、請求項25に規定の使用のためのCD4+GPA33+T細胞もしくは細胞の集団、又は、請求項27に記載の医薬組成物。
- 癌に罹患している個体を不良な予後又は良好な予後を有すると分類する方法であって、
a)腫瘍試料におけるCD4+CD25+GPA33+T細胞のレベルを決定する工程、
b)参照試料におけるCD4+CD25+GPA33+T細胞のレベルを決定する工程、及び
c)a)において決定された前記腫瘍試料におけるCD4+CD25+GPA33+T細胞のレベルをb)において決定された前記参照試料におけるCD4+CD25+GPA33+T細胞のレベルと比較する工程
を含み、
前記参照試料におけるCD4+CD25+GPA33+T細胞のレベルより高い、前記腫瘍試料におけるCD4+CD25+GPA33+T細胞のレベルが、不良な予後の指標である
方法。 - 癌に罹患している個体における療法に対する応答を予想する方法であって、
a)腫瘍試料におけるCD4+CD25+GPA33+T細胞のレベルを決定する工程、
b)参照試料におけるCD4+CD25+GPA33+T細胞のレベルを決定する工程、及び
c)前記腫瘍試料におけるCD4+CD25+GPA33+T細胞のレベルを前記参照試料におけるCD4+CD25+GPA33+T細胞のレベルと比較する工程
を含み、
前記参照試料におけるCD4+CD25+GPA33+T細胞のレベルより高い、前記腫瘍試料におけるCD4+CD25+GPA33+T細胞のレベルが、療法に対する不良な応答の指標である
方法。 - 腫瘍試料及び参照試料におけるCD4+T細胞の総集団におけるCD4+CD25+GPA33+T細胞のパーセンテージが、決定され、参照試料におけるCD4+CD25+GPA33+T細胞のパーセンテージより高い腫瘍試料におけるCD4+CD25+GPA33+のパーセンテージが、不良な予後又は療法に対する不良な応答の指標である、請求項29又は30に記載の方法。
- 前記参照試料における比又はパーセンテージの中央値+(2×標準偏差)より高い前記腫瘍試料において決定された比又はパーセンテージが、不良な予後の指標である、請求項31に記載の方法。
- 前記腫瘍試料及び前記参照試料が、単離された細胞、又は1つ若しくは複数の組織切片を含む、請求項29から32のいずれか一項に記載の方法。
- 前記参照試料が、前記個体由来の、前記腫瘍の周囲の健常組織の試料又は前記腫瘍を含有する組織と同じタイプの組織の健常組織の試料である、請求項29から33のいずれか一項に記載の方法。
- 癌が、結腸直腸癌であり、参照試料が、健常な結腸直腸組織の試料である、請求項29から33のいずれか一項に記載の方法。
