Description
本明細書の文脈における用語「MR1」は、MR1遺伝子(Entrez 3140)又はMR1遺伝子産物(Uniprot Q95460)のいずれかを指す。
広い意味において、本発明は癌の治療法に関するものであって、MR1発現癌細胞に反応するT細胞(MR1T細胞)から単離されたTCR配列は、患者のT細胞の集団中に遺伝子導入した後に発現される。これらの外来の、遺伝子導入で発現されたTCR配列は、患者の腫瘍に対する治療法としてT細胞へのMR1発現癌細胞の特異的認識を与えるために使用される。
本発明の第1の態様は、非多型MHCI関連MR1抗原提示分子と関連する癌細胞によって提示された癌抗原と特異的に結合することができるT細胞受容体を発現するT細胞を同定及び/又は単離する方法に関する。この方法は以下のステップを含む
a.患者又は健康なドナーから単離されたT細胞の調合液を提供するステップ、そして、
b.接触するステップにおいて外因性の微生物由来抗原がない状態でMR1タンパク質を発現する癌細胞と、単離されたT細胞のこの調合液を接触するステップ、特に共培養するステップ、そして、
c.単離するステップにおいてMR1依存的様式で前記癌細胞に特に反応するT細胞を単離するステップ。
a.患者から得られた腫瘍サンプルを提供するステップ
b.MR1と反応する、下記のいずれかの複数のMR1T細胞受容体分子を用いて前記腫瘍サンプルと接触するステップ

細胞。次のヒト細胞株がアメリカンタイプカルチャーコレクション(American Type Culture Collection)から得られた:A375(メラノーマ)、THP-1(骨髄単球性白血病)、J.RT3-T3.5(TCRβ欠損T細胞白血病)、LS 174T(結腸腺癌)、HCT116(結腸癌)、Huh7(肝細胞癌)、HEK 293(ヒト胚腎臓)及びCCRF-SB(急性B細胞リンパ芽球性白血病)。SKW-3細胞(TCRα、β、γ及びδ遺伝子が欠損しているヒトT細胞白血病)は、ライプニッツ研究所DSMZ・ドイツ微生物細胞培養コレクション(Leibniz-Institute DSMZ-German Collection of Microorganisms and Cell Cultures)から得られた。2つの代表的なMAITクローン(MRC25及びSMC3)及び1つのTCRγδクローン(G2B9)(Gober et al.,The Journal of experimental medicine 197,163-168(2003))は対照細胞としてこの研究の中で使用され、2人の健康なドナーの血液から生成され、以前に記述される(Lepore et al.,NatCommun 5 3866(2014))ような培養液中で維持された。MR1T細胞は、「Ethikkommision Nordwest und Zentralschweiz/EKNZ(139/13)」の承認のもと、血液採取の時にインフォームドコンセントがドナーから得られた後に健常人の末梢血から単離された。簡潔に、ネガティブセレクション(EasySepTMHuman T Cell Enrichment Kit、StemCell)によって精製されたT細胞は、3週間放射線を照射された(80グレイ)A375-MR1細胞(比率2:1)で週に一度刺激された。ヒトrIL-2(5U/ml;Hoffmann-La Roche)、rIL-7及びrIL-15(両方とも5ng/ml、Peprotech)は、各刺激の後に+2日及び+5日で加えられた。最後の刺激の12日後、細胞は洗浄され、そしてA375-MR1細胞(比率2:1)と一晩共培養された。その後、CD3+CD69+CD37+細胞は、PHA(1μg/ml、Wellcome Research Laboratorie)、ヒトrIL-2(100U/ml、Hoffmann-La Roche)及び照射されたPBMC(5x105細胞/ml)がある状態で限界希釈法によって分類されクローニングされた。他の実験では、MR1T細胞クローンは、A375-MR1細胞(比率2:1)を用いた単一の一晩の刺激を受けてソートされたCD3+CD69+CD137+からの同じプロトコルを使用して生成された。T細胞クローンは同じプロトコルに従い周期的に再度刺激された(Lepore et al.,同書中)。単球及びB細胞は、製造者の取扱説明書によってEasySep Human CD14 and CD19 positive selection kits(Stemcell Technologies)を使用して、健康なドナーのPBMCから精製された(>90% 純度)。Mo-DCは、以前に記述されるとおり(Lepore et al.,同書中)GM-CSF及びIL-4がある状態で培養によって精製されたCD14+モノサイトから分化された。ヒト正常腸上皮細胞(GEC)は、公表されたプロトコルによる腫瘍のない個体の腸生検から単離された(Graves et al.,Journal of immunological methods 414,20-31(2014))。

発明者は、ヒトMAIT細胞のレパートリーに関する初期の研究の間に微生物のリガンドに反応しなかった異型のMR1制限T細胞クローンを検出した。このT細胞クローン(DGB129)は、表面のMR1(CCRF-SBリンパ芽球性白血病細胞又はTHP-1単球性白血病細胞;図1A)を本質的に表示する細胞株を認識し又は任意の外来性の追加の抗原がない状態でMR1遺伝子(A375メラノーマ細胞;A375-MR1;図1A)を用いてトランスフェクトされた(図1B)。MR1+標的細胞の無菌の(sterile)認識は、抗MR1単クローン抗体(mAbs)(図1B)でブロックすることにより完全に阻害され、したがって同時にアッセイされた大腸菌由来抗原へのMAIT細胞応答に似ていた(図1C)。重要なことには、DGB129 T細胞は、合成MAIT細胞アゴニスト6,7-ジメチル-8-D-リビチルルマジン(RL-6、7-diMe;図1D)を認識せず、対照MAIT細胞クローンとは異なり、その代りに、この化合物によってMR1依存的様式で刺激された(図1E)。DGB129細胞は、MAIT細胞に典型的な古典的な半不変のTCRを発現しなかった(表1)。
発明者は、次に腫瘍細胞へのMR1T細胞応答性がTCRによってもたらされるかどうか調査した。TCR欠損SKW-3細胞の異なるMR1T細胞クローンからクローニングされた対になったTCRα及びβ遺伝子の発現は、腫瘍細胞のMR1認識を与え、オリジナルのMR1T細胞によって表示されたそれに匹敵し、それは完全に抗MR1-mAbsによってブロックされた(図4A-C)。対照実験では、代表的なMAIT細胞クローンのTCRα及びβ遺伝子の導入は、大腸菌抗原がある状態でのみMR1依存的様式でA375-MR1細胞を認識する能力を与えた(図4D)。これらのデータは、腫瘍細胞のMR1T細胞認識を媒介する際のTCRの重大な役割を強調し、MR1T細胞TCR遺伝子導入がMR1発現腫瘍細胞への選択されたT細胞の反応性を有効に再指示することができることを示唆した。
MR1を発現するA375メラノーマ細胞に反応するMR1T細胞クローンの大きなパネルを生成したことで、発明者は次に、THP-1骨髄単核細胞、Huh7ヘパトーマ細胞、HCT116結腸癌細胞及びLSの174Tのゴブレット状の結腸腺癌細胞を含む表面のMR1を本質的に発現する他のタイプの腫瘍細胞を認識することができたかどうかを調査した。すべてのこれらの細胞型は、微生物抗原がある状態及びMR1依存の方法(図5A)でMAIT細胞活性化を支援した。同じ細胞は、様々な程度まで選択したMR1T細胞クローンの無菌の活性化を引き起こすことができた。THP-1細胞は、大多数のテストされたMR1T細胞クローンによって、続いてHuh7ヘパトーマ細胞、LSの174Tのゴブレット状の細胞及びHCT116結腸癌細胞(図5B)によって認識された。重要なことには、反応はすべて抗MR1 mAbsによってブロックされた。
発明者は、次に腫瘍細胞へのMR1T細胞応答性の基準を検討した。まず、MAIT細胞との相似で、MR1T細胞クローンが微生物抗原を認識することができた可能性を決定的に除外しようと努力した。対照MAIT細胞クローンは大腸菌溶解物がある状態でのみA375-MR1細胞と反応したが、異なるMR1T細胞クローンの活性化は大腸菌溶解物によって増強されなかった(図6A)。これらのデータと一致して、MR1ネガティブのA375-WT細胞は大腸菌溶解物が添加されたかどうかに関係なく、一方のタイプのT細胞を刺激しなかった(図6A)。また、重要なことには、抗MR1 mAbsは効率的にMR1T及びMAIT細胞応答の両方をブロックした(図6A)。これらの発見は、大腸菌及び刺激性MAIT細胞の中にある微生物のリガンドがテストされるMR1T細胞を刺激しないことを確認した。
MR1T細胞の抗腫瘍活性を評価するために、発明者は、生体外で腫瘍細胞を直接殺す能力をテストした。2つの代表的なMR1T細胞クローン(DGB129及びDGB70)が、効率的に様々なエフェクタ:ターゲット比率でMR1発現THP-1及びA375細胞の両方を殺した(図9A、B)。ターゲットが大腸菌感染した場合、それは完全に殺すことができたが(図示しない)、対照MAIT細胞クローンはこれらの2つの細胞タイプを殺さなかった。これらの結果は、MR1T細胞がMR1を発現する腫瘍細胞に対する特異的な細胞毒性活性を表示することを示した。
発明者は、最後にA375-MR1腫瘍細胞によって刺激時の代表的なMR1T細胞クローンのサイトカイン分泌プロフィールを分析した。テストされたクローンはすべてIFN-γを放出した(図10A)。しかしながら、発明者は、さらにTh1(IL-α及びTNF-α及び、TNF-β)、Th2(IL-3、IL-4、IL-5、IL-6、IL-10、IL-13)及びTh17サイトカイン(IL-17A、G-CSF、GM-CSF)の種々の発現プロフィール及び他の可溶性因子(MIP-1β及び可溶のCD40L PDGF-AA及びVEGF;図10B)を観察した。可変組合せ及びMR1T細胞によって発現された多量のサイトカインは、この集団内の相当な機能的可塑性を示唆した。例えば、クローンDGA4は、IL-17A、IL-6、TNF-α及びGM-CSFを大量分泌したが、プロトタイプのTh2サイトカインIL-4、IL-5、IL-10又はIL-13を分泌せず、したがって「異型の」Th17のような発現型を表示した。対照的に、クローンTC5A87は相当な量のVEGF及びPGDF-AAを放出したが、Th1又はTh2サイトカインをほとんど放出せず、IL-17Aは放出しなかった。顕著に、研究された7つのクローンのうちの4つは(DGB129、CH9A3、DGB70、JMA)、サイトカイン放出のTh2に優位に傾いたプロフィールを表示し、それは最近保護抗腫瘍免疫に関係している機能的な発現型である。
癌患者の新鮮な組織又は新鮮な凍結組織の生検は、ヒトMR1特異的なAbs及びMR1 mRNAのPCR増幅を使用して、MR1発現について分析される。
- 生体外で単離された初代MR1+癌細胞は以前に特徴付けられたMR1T細胞クローンのライブラリを刺激するために使用される。各クローンは異なるTCR遺伝子を発現し、異なるタイプの癌細胞を認識する。
- 患者の癌細胞に最も良く応答するMR1T細胞クローンが選択され、それらのTCR遺伝子はTCR遺伝子療法に使用される。応答は、サイトカイン放出機能及び/又は表面マーカ発現としてアッセイされる。細胞は、CD3、CD69、CD137、CD150及び/又はICOS(表面マーカ)及びINF-γ及びGM-CSF(サイトカイン)に例証されるが制限されずに、分析された活性化マーカに反応的な抗体で染色される内部(サイトカイン)又は表面マーカによってアッセイされる。
- 利用可能な場合、可溶性MR1T TCRは多量体化され、腫瘍生検から単離された腫瘍細胞を染色するのに使用されるだろう。腫瘍細胞と結合するMR1T TCR多量体は、その患者の中の遺伝子療法にふさわしいMR1T TCRの迅速な選択ができるだろう。
- いくつかの患者の循環T細胞個体群は、受容体T細胞(ナイーブ、セントラルメモリ、エフェクタメモリ、CD4+、CD8+、又はCD4、CD8ダブルネガティブT細胞)として使用され得る。ナイーブT細胞は、MR1腫瘍抗原を認識するTCR遺伝子で形質導入される場合に、刺激されていないTリンパ球が、腫瘍細胞がある状態で成熟することを可能にするために選択されている。MR1を発現する腫瘍細胞の認識で即時の増殖及びエフェクタ機能(腫瘍致死)を提供するので、セントラル及びエフェクタメモリ細胞が使用される。CD4細胞は動員を促進するTヘルパー細胞の十分な数及び抗腫瘍機能を備えた他の細胞の増殖を提供するために選択される。CD8 T細胞は腫瘍細胞の死滅を促進するために選択される。CD4-CD8ダブルネガティブT細胞は、大量のキラー・エフェクタ分子(TNFα、グランザイム及びグラニュリシン)の即時放出のようなそれらの自然様の機能に選択される。
- 形質導入されたTCR遺伝子を発現し選択されたエフェクタ機能を有するT細胞は、養子関係の細胞治療(ACT)に使用される。
- 自己由来のTILは、我々の以前に確立しているプロトコル(De Libero、同書中に)による癌組織生検から準備されている。
- T細胞は、IL-2、IL-7及びIL-15を補充された培地を使用して、2~3週間生体外で増殖する。
- 増殖したT細胞は、自己由来のMR1+癌細胞に対する反応に関してテストされる。
- 活性化マーカ(CD137、CD150、CD69、ICOS)の表面発現を増加させるT細胞は、癌に特有であると考えられる。また、抗MR1単クローン抗体のある状態によって阻害される場合、それらはMR1依存的であると考えられる。
- 上に概説されるように、癌反応的なT細胞は上記の活性化マーカのうちの1つの発現によってソートされ、増殖し、ACTのために使用される。
Claims (5)
- MR1分子に関連する癌細胞によって提示される抗原と特異的に結合することができるT細胞受容体を発現するT細胞を単離する方法であって、
a.患者又は健康なドナーから単離されたT細胞の調製液を提供するステップ、そして
b.接触工程においてMR1を発現する癌細胞とT細胞の前記調製液を接触させるステップ、そして
c.単離工程において前記癌細胞と特異的に反応するT細胞を単離するステップであって、該単離工程はCD3 + CD69 + CD137 + T細胞を選択することを含む前記ステップ
を含むことを特徴とする、前記方法。 - 前記接触させるステップは増殖させるステップを含み、単離されたT細胞の前記調製液はMR1を発現する癌細胞が存在する状態で増殖することを特徴とする、請求項1に記載の方法。
- 単離するステップにおいて単離されたT細胞のT細胞受容体をコードする核酸配列を決定することをさらに含むことを特徴とする、請求項1又は2に記載の方法。
- MR1分子に関連する癌細胞によって提示される抗原と結合することができるT細胞受容体を発現するMR1T細胞の調製液を調製する方法であって、
a.患者から獲得された腫瘍サンプルを提供するステップ;
b.前記腫瘍サンプルを
i.各T細胞クローンがMR1分子と関連する癌細胞によって提示される抗原と特異的に結合することができるMR1T細胞受容体分子によって特徴付けられる、請求項1に記載の方法を実施し、それによって得られるT細胞の複数のクローン;又は、
ii.MR1T細胞受容体分子から単離された標識化及び多量体化された複数の可溶性TCR
と接触させるステップ、
c.前記腫瘍サンプルと特異的に反応するMR1T細胞受容体を同定するステップ;
d.T細胞調製液を提供するステップ;
e.ステップcにおいて前記腫瘍サンプルと特異的に反応するとして同定されたT細胞クローン上で発現されたMR1反応性T細胞受容体分子をコードする核酸発現コンストラクトを前記T細胞調製液へ導入し、導入遺伝子T細胞調製液を産出するステップ
を含むことを特徴とする、前記方法。 - 癌の治療法又は予防方法で用いる、請求項4に記載の方法によって獲得されたMR1特異的T細胞の調製液。
