Description
インターフェロンは、多くの臨床的利益を有する。例えば、インターフェロンは、免疫系をアップレギュレートすることが公知である。したがって、それは、がん細胞を識別および攻撃するために患者の自然免疫系をリクルートするのに有用である可能性がある。しかしながら、抗がん剤としてのインターフェロン有効性は現在までに、十分ではないことが証明されている。これは不可解なことであった。
本発明者は、インターフェロン発現を刺激することに加えて、(外因的に投与されたか、または内因的発現をアップレギュレートするワクチンもしくは他の剤に応答して発現したいずれかの)インターフェロンはまた、CD279とも呼ばれる、プログラム細胞死タンパク質1の発現を刺激することを発見した。したがって、本発明者は、以前に認識されていないインターフェロン療法の副作用を識別した。インターフェロンは、患者の免疫系のある特定の局面を有利に刺激するが、プログラム細胞死タンパク質1の発現もまたアップレギュレートする。結果としてのプログラム細胞死タンパク質1の増加は、今度は、保護的T細胞機能をダウンレギュレートする。これは、がん細胞表面抗原を有する細胞を識別および攻撃することにおけるT細胞の有効性を損なう。したがって、インターフェロンは、2つの相反する作用を生成する。それは免疫系活性を増大させるが、がん細胞表面抗原を識別する免疫系の能力を阻害する。

インターフェロン療法
インターフェロンは、一群のシグナル伝達タンパク質である。それらは、いくつかの抗原性病原体、例えばウイルス、細菌および寄生生物ならびにまた腫瘍細胞の存在に応答してヒト細胞により発現および分泌される。典型的に、ウイルス感染細胞は、インターフェロンを放出し、近傍のバイスタンダー細胞にそれらの抗ウイルス防御を高めるようにシグナル伝達する。また、インターフェロンは、免疫細胞、例えばナチュラルキラー細胞およびマクロファージを活性化する。インターフェロンは、主要な組織適合性複合抗原の発現を増大させ、これは、今度は、免疫系への外来抗原の提示を増大させる。
プログラム細胞死タンパク質1(「PD-1」)は、CD279としても公知であり、ヒトにおいてPDCD1遺伝子によりコードされるタンパク質である。PD-1は、免疫グロブリンスーパーファミリーに属し、細胞表面受容体として機能し、2つの公知のリガンド、PD-L1およびPD-L2に結合する。
本発明者は、インターフェロンでのがん処置(インターフェロンポリペプチドを投与することによるか、または細胞がインターフェロンを発現することを誘導する剤を投与することによる)は、同時にPD-1発現を誘導することを発見した。
IFNαは、PD-L1およびTRAIL発現を誘導する。
IFNαは、用量依存的様式でPD-L1発現を誘導する。
マウスインターフェロンは、PD-L1発現を誘導する
材料および方法:BBN972およびMB49#1(MB49-luc)細胞を培養し、その後、0(零)~1×104国際単位のマウスインターフェロンを含有する培地に曝露した。続くPD-L1および(対照としての)アクチンの発現を測定した。
ポリイノシン酸:ポリシチジル酸(ポリI:C)は、PD-L1を誘導する
材料および方法:上記データは、インターフェロンがPD-L1発現を誘導し、用量依存的様式でそれを行い、迅速にそれを行い、かつ異なる種からのインターフェロンに応答して明らかにそれを行うことを示す。インターフェロンを採取した動物種にかかわらず、効果があるので、本発明者は、この効果はインターフェロンに限定されず、他の種類の免疫刺激剤により、より一般的に引き起こされ得ると仮定した。この概念を試験するために、本発明者は、ポリイノシン酸:ポリシチジル酸(多くの場合、「ポリI:C」と略される)を評価した。ポリI:Cは、免疫刺激剤である。それはウイルス感染をシミュレートするために、そのナトリウム塩形態で使用される。ポリI:Cは、構造的に二本鎖RNAに類似している。dsRNAは、一部のウイルスに存在する。本発明者は、ポリI:Cを、移植したplcまたはulc腫瘍を有する研究用マウスに腹腔内注射により投与した。
インターフェロンウイルス遺伝子療法は、ヒトにおいてPD-L1を誘導する
材料および方法:これらのデータは、BCG療法に非応答性であるか、またはBCG療法後に難治性である患者における、ヒトインターフェロンアルファ2b導入遺伝子を有するINSTILADRIN(商標)複製欠損アデノウイルス遺伝子療法ベクターについてのヒトフェーズIIヒト臨床試験から取られる。その研究計画は、公開されており、参照により本明細書に組み込む。
組合せ療法は、生存を増大させる
材料および方法:研究用雌ラットに腫瘍細胞を植え付け、細胞を測定可能な腫瘍に発達させた。その後、ラットを、生理食塩水(対照)、IgG(対照として)、抗PD1モノクローナル抗体(単独療法)、ポリI:C(インターフェロン発現を誘導する単独療法)およびポリI:Cと抗PD1モノクローナル抗体との組合せ(組合せ療法)で処置した。
表在拡大型黒色腫
材料および方法:表在拡大型黒色腫と診断されたヒト患者を、広範囲局所切除術およびセンチネルリンパ節生検により処置して、リンパ系または遠位器官への疾患の広がりがないことを確認する。その後、患者をINSTILADRIN(商標)とKEYTRUDA(商標)との組合せで処置する。処置は、外科的切除後実施可能な限りすぐに開始する。
表在拡大型黒色腫
材料および方法:上記の実施例のKEYTRUDA(商標)。
非小細胞肺がん
材料および方法:上記実施例7による医薬剤。ヒト試験対象は、非小細胞肺癌腫を有すると診断されている。患者をGreene, Frederick L.、Cancer Staging Manual(American Joint Committee on Cancer、出版、第6版)に従ってスクリーニングして、同等の試験対象が同等の疾患を有することを保証する。試験対象を、検証された試験により確認される発現である腫瘍PD-L1発現に基づいて、処置についてスクリーニングする。
上記実施例は、ある特定のがんの処置を論じている。しかしながら、本発明者らの発見はより一般的に、インターフェロンシグナル伝達から利益を得、かつCD279の過剰発現を患う任意の状態を処置するために使用することができる。

Claims (5)
- インターフェロンでヒトがん患者を処置する方法において使用するための組合せ物であって、前記組合せ物は、第1の剤と、モノクローナル抗体とを含み、前記方法は、
a.インターフェロンで処置され得るがんを有すると診断されたヒトがん患者に、治療有効量の前記第1の剤を投与するステップであって、前記第1の剤が、発現可能なインターフェロン導入遺伝子を含むウイルスベクターであり、前記インターフェロン導入遺伝子が、インターフェロンアルファを含むI型インターフェロンをコードし、前記第1の剤が、プログラム細胞死タンパク質1リガンドの発現をアップレギュレートするために十分な量で前記ヒト患者のインターフェロンレベルを増大させることができる、ステップと、
b.前記ヒトに、前記第1の剤により引き起こされるT細胞機能の減少を改善するのに有効な量の前記モノクローナル抗体を投与するステップであって、前記モノクローナル抗体が、プログラム細胞死タンパク質1リガンドに結合する、ステップと
を含み、これにより前記モノクローナル抗体が、前記第1の剤により引き起こされる免疫機能の減少を実質的に改善する、組合せ物。 - 前記患者が、前記第1の剤および前記モノクローナル抗体でほぼ同時に処置される、請求項1に記載の組合せ物。
- 前記第1の剤が、前記患者がインターフェロンを内因的に発現することを誘導する剤を含む、請求項1に記載の組合せ物。
- 前記第1の剤が、ヒトインターフェロンアルファ2b導入遺伝子を有する複製欠損アデノウイルス遺伝子療法ベクターを含む、請求項1に記載の組合せ物。
- 前記がんが筋層非浸潤性膀胱がんを含む、請求項1に記載の組合せ物。
