Description
本発明は、国立衛生研究所によって授与された1R43CA176892-01A1、3R43CA176892-01A1S1、2R44CA176892、1R43HL126285-01、2R44HL126285-02A1、およびHHSN261201700051Cに基づく、政府の支援を受けてなされた。政府は、本発明において一定の権利を有する。
リン脂質シェルを有するペルフルオロヘキサンガスマイクロバブル(MB)を調製するためのBUBLES(またはBACS)技術による乳化法としては、例えば、国際特許出願公開WO2015/175344 A1に記載されているように、DSPE-PEG 3400-マレイミドが膜に含まれることにより、抗体あたり1~2個のチオール基を有するFcフラグメント特異的IgGと抗体とを共役させ、マイケル付加により、マレイミド活性化MBに共役させた。抗EpCAMのような標的抗体を2番目の工程で結合させた。大部分のMBは、3~10μmの直径を有する(ほとんどの哺乳動物細胞のサイズと類似する)。各MBは、平均367,000の抗FcIgG分子を有する。最近、出願者らと他の人々は、同じアプローチを使用して、Her2、EGFR、CD3、CD4、CD8、CD34、PD1などを含む他の細胞表面タンパク質を標的とするマイクロバブルの製造に成功した。出願者の以前の刊行物に記載されているように、MBと磁気ビーズの結合効率を並べて比較すると、両者が同等の効率で標的細胞とロゼットを形成することを示した。しかしながら、MBはより速い結合を示し、1分間の混合後に、85%を超える細胞がMBに付着した。
現在の臨床試験における細胞療法の大部分は、血液または組織中の稀な集団である幹細胞または免疫T細胞であり、例としては図12および図13に記載されている。腫瘍リダイレクトの低分化度のT細胞サブセットによる利点が圧倒的な前臨床データにより示されるにもかかわらず、臨床試験では、単一選択を用いたMACSによる末梢血単核球(PBMC)由来のCAR工学的T細胞(CAR-engineered T cell)が主に使用されている。この選択戦略は、図14および図15に記載されるように、他の望ましくない細胞混合物と混合された低い標的細胞からなる一貫性のない細胞製品を生成する。標的細胞注入の治療用量が不十分であると、患者に次善の臨床的利益をもたらす可能性が高い。連続選択は、図16に記載されるように、標的細胞を高純度で生成してその量を濃縮することができる。最近、細胞と親和性リガンドに共役した磁気ビーズとの可逆的結合を可能にする戦略が報告されている。例えば、ストレプタマー技術は、ストレプタグと融合するように設計されたストレプタクチン多量体化低親和性一本鎖抗体に共役することによって、磁気微粒子またはナノ粒子の標的細胞との可逆的結合を可能にする。他の技術では、細胞または磁気ビーズから解離することができる修飾抗体を使用する。これらの方法では、通常、標的抗体に対する大規模な修飾を必要とし、操作手順と複雑さを大幅に増大させる。一方、BUBLES技術は、特別な研究および臨床的必要に基づいて、十分に確立された抗体、容易に商業的な入手が可能な抗体または特定の抗体などの任意の望まれる抗体に共役できる方法を使用している。装置内に高い周囲圧力(例えば、哺乳動物細胞によく耐えられる)を加えることによってマイクロバブルを破壊できるというBUBLES技術の有する利点は、多重の細胞表面マーカーの連続ポジティブまたはネガティブ選択を可能とし、細胞混合物から標的細胞を単離し精製できる。さらに、細胞への物理的な悪影響を及ぼさないこの方法は、細胞の活性と特徴を保存するための付加的な利益をもたらし、これについては、以下でさらに説明する。
GMP製造細胞療法の最も重要な概念の1つは、収集および製造プロセスを通じて、製品の無菌性を維持するために、外部環境に閉鎖的なシステムに細胞を維持することである。BUBLESの明確な4つの利点は、細胞療法生産のために、簡単な操作と費用対効果的な方法で、1つの単一容器において、多数のインビトロ操作を可能にする。ここでは、CAR-T-CD19細胞療法を例として使用して、図17に記載されるような製造工程を説明する。図17において、Aで表示される第1構成が示されており、血液バンクまたは臨床センターから得られた出発物質であるアフェレーシスPBMC(8~9mlにおいては~約109個細胞)が、BUBLES注射器状の容器3に移される。マイクロバブル2を加え、当該マイクロバブル2は、試料としてPBMCにおける細胞混合物のスペースサブセットである標的細胞の第1の細胞表面マーカーを標的とする。注射器状の容器3内の内容積の約3分の1の空間は、空間0として残される。この構成Aでは、PBMCとマイクロバブル2との混合物は、例えば、10~60分間インキュベートすることができる。場合により、希釈のために緩衝液または培地を添加してもよい。この構成Aでは、PBMCサンプル、マイクロバブル2の第1セットおよび任意に希釈剤および/または媒体を容器3の中に入れた後、図5に示されるように、装置の頂部は、キャップ7またはルアーロック8/アダプタ9の組み合わせによって閉鎖される。構成Bに示されるように、次いで容器3が反転され、プランジャ5を押し下げることによって廃棄物12が廃棄される。図3による構成に示されるように、プランジャ5を押す距離によって容器3における内容物のどれくらいの量を廃棄するかを決定するので、マイクロバブル2に結合しなかった非標的細胞を含む廃棄物12が廃棄され、マイクロバブル2およびマイクロバブル2に結合した細胞が容器3に残るようにプランジャを押す。有用であれば、細胞選択工程の次の工程に進む前に、細胞洗浄工程を適用することができる。次の工程は、構成Cによって示され、すなわち、注射器状の容器3の向きが再び変えられ、プランジャ5が引き抜かれて、容器3内のAL容積を増大させる。次の工程として、キャップ7またはルアーロック8/アダプタ9の組み合わせが再び取り付けられて、容器3を密封するので、構成Dにより示される次の工程においては、プランジャ5を押し上げることによって、容器内の空気圧が上昇し、容器3内の空気量を減少させる。これにより、マイクロバブル2が破壊され、構成Eにより示されるように、これまでに選択された細胞混合物がマイクロバブル2から分離される。必要に応じて希釈剤または媒体を再度添加した後、図17のシステム構成A~Eにより示される工程は、標的細胞の第2細胞表面マーカーを標的とするマイクロバブルの第2セットについて繰り返すことができる。
幹細胞の古典的な定義は、自己複製性および多能性の特性を有するということである。体性幹細胞は、未分化状態のままで細胞分裂を行う能力を有する稀な細胞サブセットであり、受けた分化シグナルに応じて様々な成熟細胞型に発達する多能性がある。結果として、細胞治療の最終的な成功は、注入細胞の幹細胞の特徴の維持(sternness、すなわち、幹細胞性)程度に左右される。BUBLESは、インビトロ/エクスビボ操作過程において、標的細胞への物理的損傷を最小限に抑えるだけでなく、さらに重要なことには、生物学的意義のメカニズムを通して細胞効力を維持および刺激するという明確な利点も提供する。ここで、CAR-T細胞療法を例としてこの課題を具体的に説明しているが、患者の再生機能を達成するために注入された細胞の幹細胞活性の維持が重要であるため、類似の原理は、HSC、MSC、神経幹細胞などを含む他の幹細胞療法にも適用されることに留意されたい。
第一に、BUBLESシステムは、インビトロ操作工程を最小限に抑え、前記のように、単一容器内において、標的細胞の単離および濃縮、細胞の培養および増殖、遺伝子形質導入、ならびに他の操作を含み、さらに、工程間で細胞を洗浄、遠心分離、再懸濁する必要がない。また、最も圧縮性/柔軟性の高い脂質コートマイクロバブルは、ガスコアと共役して、外力(例えば、超音波および結合細胞)に対して自己成形性があり、細胞にとって非常に優しい材料である。簡単な例として、風船(マイクロバブル)で緩和された卵(細胞)である。BUBLESシステムのこれらの有利な利点は、操作上および物理的な便益を提供して、細胞の損傷を大幅に最小限に抑える。
Tメモリー幹細胞(TSCM)は、メモリーリンパ球の稀なサブセットであり、自己再生する幹細胞様能力、およびメモリーとエフェクターT細胞サブセットの全スペクトルを再構成する多能性能力を有する。T細胞は、増殖能力の連続的な喪失を受けるが、抗原未経験のナイーブ細胞(TN)からCD62L+セントラルメモリー(TCM)、CD62L-エフェクターメモリー(TEM)およびエフェクター(TE)T細胞サブセットへの分化の過程において、エフェクター機能の獲得が起こる。ヒトT細胞分化の階層モデルを示す表は、Gattinoni L、Nat Rev Cancer、2012;12(10):671-684から以下の表3として提供される。
T細胞のインビトロ細胞培養は、免疫療法に十分な用量を達成するための受容体工学および細胞増殖に不可欠である。しかしながら、ナイーブで低分化度のT細胞は、細胞培養および増殖期間中に急速な喪失および成熟期を経験し、異なる分化プロトコールによって各T細胞サブセットの異なる結果を提示する。培地および血清に加えて、培養物には、様々なサイトカインおよび抗体/刺激物質が含まれる必要がある。典型的に、T細胞増殖を促進するためにはIL-2の添加が必要であるが、ある程度のT細胞成熟をも誘導するので、メモリーT細胞表現型を維持するためにIL-7、IL-15およびIL-21を補足する。T細胞受容体(TCR)の刺激により標的T細胞の機能と活性を確保するために、インビトロ培養の期間においてT細胞受容体活性を維持することが重要であり、抗CD3(またはプラス抗CD28)は、T細胞と抗原提示細胞(APC)との間の免疫学的シナプス形成を模倣するために、図18に示されるように、人工合成材料(例えば、磁性微粒子)またはフィーダー細胞(例えば、PBMCまたはK562細胞)のいずれかの表面に提示する必要がある。非専門的細胞(フィーダー細胞)または固体微粒子に由来する人工APC(aAPC)は様々な程度の成功が報告されている。堅牢なスケールアップ製造プロトコルのためのより制御されたシステムを得るために、インビトロアッセイおよび大規模生産を開発するための合成aAPCを使用することが好ましい。実際に、生体細胞上でのHLAの発現は経時的に変動することが報告されている。一方、固体粒子に基づくaAPCは、リガンドの不動性という別の問題を抱えており、これは免疫学的シナプス形成の動的過程を妨げることになる。研究では、インビトロT細胞増殖へのダイナビーズ-CD3/CD28の添加が、ナイーブT細胞およびメモリーT細胞の急速な喪失を引き起こすことも示された。
物理的なT細胞-APC接触は、このモードのジャクスタクリン(juxtacrine)シグナル伝達に固有するTCR、接着分子、キナーゼ、細胞骨格要素および細胞小器官の分極軸を設定する。さらに、TCRと接着分子を放射状対称コンパートメントに横方向に組織化し、免疫学的シナプスは、T細胞-APC相互作用の持続時間を制御するために、交差する対称面と規制された対称性の破れの可能性を明らかにした。免疫学的シナプスは、シグナル伝達機構の組織化に加えて、サイトカインおよび細胞溶解剤の偏極輸送および分泌を支配してシナプス間隙を横切り、また、受容APCおよび環境中の他の細胞に機能的に影響を与える生物活性微小胞(bioactive microvesicle)の生成およびエキソサイトーシス放出のための部位であり、図18および19を参照する。
細胞外環境の特定のアスペクトを捉える人工または工学的表面は、細胞-細胞接触(ジャクスタクリン)シグナル伝達の研究において中心的な役割を果たす。上記の背景の概要で簡単に要約したように、CD3およびCD28抗体はリガンド-受容体相互作用を模倣するためのコアTCR複合体として使用できるが、人工システムは少なくとも、(1)原型IS構造の動的複雑性、(2)細胞内の下流シグナル伝達をよりよく誘発するための細胞膜上の分子の移動性および空間的相互作用、ならびに(3)数μm~数十nmの範囲の複数の距離スケールで膜タンパク質と相互作用する基礎細胞骨格ネットワーク、を要約する必要もある。
T細胞活性化に対する効果は、ダイナビーズ対マイクロバブルを用いて、試験した。抗CD3/CD28に共役したマイクロバブルは、特に2回目の刺激後に、T細胞増殖において、抗CD3/CD28ダイナビーズより優れた。図21に示されるように、PBMC由来のT細胞は、最初の2週間以内に、抗CD3/CD28に共役したダイナビーズまたはマイクロバブルで最初に刺激した後に強く増殖した。注目すべきことに、マイクロバブルに基づくT細胞増殖は、その後の2回の刺激でも同じ傾向を続けたが、ダイナビーズベースに基づく増殖は、2回目の刺激後に劇的に減少した(図21の(a))。T細胞増殖は、2回目の刺激が行われた際に、刺激試薬が添加されなかったときに停止したので、抗CD3/CD28に依存する(図21、模擬グループまたは対照グループ)。結果は、同じドナー(図21の(a)および(b))または異なるドナー(図21の(b)、(c)および(d))からのPBMCを使用することで再現可能である。Liらの研究では、Fc受容体担持フィーダー細胞の存在下で、抗CD3/CD28ダイナビーズおよび可溶性抗CD3で刺激されたT細胞応答の比較において、「急速拡張プロトコル(Rapid Expansion Protocol、REP)」が指定された類似の観察が報告されており、フィーダー細胞は、抗CD3(そのFc受容体と結合した)およびB7(抗CD28に相当するCD80およびCD86)を提示するためのAPCとして機能する。マイクロバブルおよびフィーダー細胞は、刺激後のT細胞生存、増殖、および表現型の応答がREPおよびマイクロバブルと類似するように、液体脂質表面によって囲まれている。一方、フィーダー細胞は培養物から除去される必要があるが、MBに基づくT細胞活性化においては除去は必要とされない。したがって、マイクロバブルに基づくaAPCには、次の表4に示されるように、ダイナビーズおよびREPより多くの利点がある。
ISシグナリ伝達における空間的考慮の重要性は、あらゆる種類の細胞-細胞の通信および細胞-ECMの通信においても見られる。細胞間の情報伝達は、シグナル伝達分子自体だけでなく、シグナルが分配され受容体に提示される方法から生じる多くの文脈的および位置的合図によっても行われる。膜貫通型および分泌型増殖因子は、細胞外マトリックス(ECM)成分およびインテグリンと接触依存的に関連して、それらの受容体にシグナル伝達する。シグナル伝達分子は、シナプス様構造を介して標的細胞と接触し、活性化するサイトネームを介して離れた位置にある細胞に到達することもできる。上記の免疫学的シナプスシステム以外、他の多くの細胞-細胞および細胞-マトリックスの接触システムは、さまざまな生物学的機能を調節するための重要な役割を果たす。

b)気体を包囲する内部気泡壁と、サンプルに含まれる細胞の第1サブセットの第1細胞表面マーカーに結合することができる第1抗体に共役した外部気泡壁とを有する柔軟なシェルを備える第1の柔軟なシェルマイクロバブルを加える工程、
c)第1抗体と第1細胞表面マーカーとの間の相互作用を可能にして、第1抗体を第1細胞表面マーカーに結合させるように、十分な時間スパンにわたってインキュベートする工程、
d)サンプルの残り部分から、浮揚により、第1の柔軟なシェルマイクロバブルおよび第1の柔軟なシェルマイクロバブルに結合した細胞の第1サブセットを分離することを可能にする工程、
e)サンプルにおける細胞の第1サブセット以外の細胞を含む、廃棄物を容器から除去する工程、
f)単離された細胞の第1セットが浮揚しなくなるように、第1の柔軟なシェルマイクロバブルを破壊する工程、
g)気体を包囲する内部気泡壁と、細胞の第1サブセットに含まれる細胞の第2サブセットの第2細胞表面マーカーに結合することができる第2抗体に共役した外部気泡壁とを有する柔軟なシェルを備える第2の柔軟なシェルマイクロバブルを加える工程、
h)第2抗体と第2細胞表面マーカーとの間の相互作用を可能にして、第2抗体を少なくとも一つの第2細胞表面マーカーに結合させるように、十分な時間スパンにわたってインキュベートする工程、
i)サンプルの残り部分から、浮揚により、第2の柔軟なシェルマイクロバブルおよび第2の柔軟なシェルマイクロバブルに結合した細胞の第2サブセットを分離することを可能にする工程、および
j)細胞の第1サブセットにおける細胞の第2サブセット以外の細胞を含む、廃棄物を容器から除去する工程
を含む、浮力による分離方法。
l)細胞の第3サブセットのために、標的されていなかった標的細胞表面マーカーに結合することができる使用されていなかった抗体に共役した追加の柔軟なシェルマイクロバブルを利用することにより、工程g)から工程k)までを繰り返す工程
をさらに含む、実施形態1による方法。
m)気体を包囲する内部気泡壁と、T細胞と免疫学的シナプスを形成することができるリガンドに共役した外部気泡壁とを有する柔軟なシェルを備えるリガンド提示柔軟なシェルマイクロバブルを、細胞のスパースサブセットに加える工程、
o)T細胞のスパースサブセットを活性化および増殖するために、十分な時間スパンにわたってT細胞およびリガンド提示柔軟なシェルマイクロバブルをインキュベートする工程、
p)組換えMHCに結合したユニークなペプチド、および抗CD28または組換えCD80やCD86のような補助刺激分子と組合せることで、特異的T細胞の活性化を達成する工程、および
q)抗CD3、および抗CD28または組み換えCD80やCD86のような補助刺激分子と組合せることで、非特異的T細胞活性化を達成する工程
を含む、前記の実施形態のいずれかの方法。
内部容器空間を画定する内壁を有する容器(3)、
内壁と密封的に接続され、内壁に対して可動なプランジャ(5)、
内部空間を周囲空間に接続させ、気体、液体および固体材料を内部空間に送り込んだり内部空間から送り出したりすることを可能にする、開閉可能な弁(8、9)を備えたポート、および
内部容器空間における柔軟なマイクロバブル(2)
を含み、
前記マイクロバブル(2)は、気体を包囲する内部気泡壁と、液体サンプルに含まれる細胞の第1サブセットの第1細胞表面マーカーに結合することができる第1抗体に共役した外部気泡壁とを有する柔軟なシェルを備える、システム。
b)気体を包囲する内部気泡壁と、T細胞と免疫学的シナプスを形成することができるリガンドに共役した外部気泡壁とを有する柔軟な脂質シェルを備えるリガンド提示柔軟なシェルマイクロバブルを、単離されたT細胞に加える工程
を含む、単離されたT細胞を活性化および増殖するための方法。
単離されたT細胞の活性化および増殖は、キメラ抗原受容体(CAR-T)細胞療法のような養子細胞移入のためのインビトロT細胞調製のために用いられる、実施形態21の方法。

Claims (14)
- 様々な異なる細胞を含むサンプルから、複数の異なる細胞表面マーカーによって区別された細胞のスパースサブセットを単離するための、浮力による分離方法であって、
a)サンプルを、分離方法全体にわたって使用される単一容器に入れる工程、
b)気体を包囲する内部気泡壁と、サンプルに含まれる細胞の第1サブセットの第1細胞表面マーカーに結合することができる第1抗体に共役した外部気泡壁とを有する柔軟なシェルを備える第1の柔軟なシェルマイクロバブルを加える工程、
c)第1抗体と第1細胞表面マーカーとの間の相互作用を可能にして、第1抗体を第1細胞表面マーカーに結合させるように、十分な時間スパンにわたってインキュベートする工程、
d)サンプルの残り部分から、浮揚により、第1の柔軟なシェルマイクロバブルおよび第1の柔軟なシェルマイクロバブルに結合した細胞の第1サブセットを分離することを可能にする工程、
e)サンプルにおける細胞の第1サブセット以外の細胞を含む、廃棄物を容器から除去する工程、
f)単離された細胞の第1セットが浮揚しなくなるように、第1の柔軟なシェルマイクロバブルを破壊する工程、
g)気体を包囲する内部気泡壁と、細胞の第1サブセットに含まれる細胞の第2サブセットの第2細胞表面マーカーに結合することができる第2抗体に共役した外部気泡壁とを有する柔軟なシェルを備える第2の柔軟なシェルマイクロバブルを加える工程、
h)第2抗体と第2細胞表面マーカーとの間の相互作用を可能にして、第2抗体を少なくとも一つの第2細胞表面マーカーに結合させるように、十分な時間スパンにわたってインキュベートする工程、
i)サンプルの残り部分から、浮揚により、第2の柔軟なシェルマイクロバブルおよび第2の柔軟なシェルマイクロバブルに結合した細胞の第2サブセットを分離することを可能にする工程、および
j)細胞の第1サブセットにおける細胞の第2サブセット以外の細胞を含む、廃棄物を容器から除去する工程
を含む、浮力による分離方法。 - 第1および第2の細胞表面マーカーは、CD8、CD62L、CD45RA、CD3およびCD28のうちの1つである、請求項1に記載の方法。
- k)単離された細胞の第2サブセットが浮揚しなくなるように、第2の柔軟なシェルマイクロバブルを破壊する工程、および
l)細胞の第3サブセットのために、標的されていなかった標的細胞表面マーカーに結合することができる使用されていなかった抗体に共役した追加の柔軟なシェルマイクロバブルを利用することにより、工程g)から工程k)までを繰り返す工程
をさらに含む、請求項1に記載の方法。 - 細胞表面マーカーは、CD8+、CD62L+およびCD45RA+であり、細胞のスパースサブセットは、ナイーブT細胞である、請求項3に記載の方法。
- 細胞のn番目のサブセットのために、標的されていなかった標的細胞表面マーカーに結合することができる使用されていなかった抗体に共役した追加の柔軟なシェルマイクロバブルを利用することにより、工程I)を繰り返すことをさらに含む、請求項4に記載の方法。
- 容器の底部から廃棄物を除去することを含む、容器から廃棄物を除去する工程をさらに含む、請求項1に記載の方法。
- 望ましくない細胞をそれぞれのマイクロバブルと結合させ、容器の上部に浮揚させ、容器の上部から、これらのそれぞれのマイクロバブルに結合した望ましくない細胞を除去することによって、望ましくない細胞としての廃棄物を容器から除去することにより、ネガティブ選択で容器から廃棄物を除去する工程をさらに含み、請求項1に記載の方法。
- 容器内でプランジャを移動させることを含む、容器から廃棄物を除去する工程をさらに含む、請求項6に記載の方法。
- 容器内でプランジャを移動させることを含む、容器から廃棄物を除去する工程をさらに含む、請求項7に記載の方法。
- 柔軟なシェルマイクロバブルが受ける圧力を増加させることを含む、第1、第2およびその後の柔軟なシェルマイクロバブルを破壊する工程をさらに含む、請求項1に記載の方法。
- 容器内でプランジャを移動させることを含む、マイクロバブルが受ける圧力を増加させることをさらに含む、請求項10に記載の方法。
- 細胞のスパースサブセットは、T細胞を含んでおり、
m)気体を包囲する内部気泡壁と、T細胞と免疫学的シナプスを形成することができるリガンドに共役した外部気泡壁とを有する柔軟なシェルを備えるリガンド提示柔軟なシェルマイクロバブルを、細胞のスパースサブセットに加える工程、
o)T細胞のスパースサブセットを活性化および増殖するために、十分な時間スパンにわたってT細胞およびリガンド提示柔軟なシェルマイクロバブルをインキュベートする工程、
p)組換えMHCに結合したユニークなペプチド、および抗CD28または組換えCD80やCD86のような補助刺激分子と組み合わせることで、特異的T細胞の活性化を達成する工程、および
q)抗CD3、および抗CD28または組み換えCD80やCD86のような補助刺激分子と組み合わせることで、非特異的T細胞活性化を達成する工程
を含む、請求項1に記載の方法。 - サンプルは、ヒトもしくは動物の血液、他のヒトもしくは動物の体液、または人工緩衝液からのT細胞、B細胞、腫瘍浸潤リンパ球、樹状細胞、ナチュラルキラー細胞、内皮細胞、幹細胞、および癌細胞のうちの1つまたはそれらの組合せのような様々な異なる細胞を含む、請求項1に記載の方法。
- 様々な異なる細胞を含むサンプルから細胞のスパースサブセットを単離することは、ポジティブ選択、ネガティブ選択、またはその両方の逐次的組合せによって行われる、請求項1に記載の方法。
