本発明は、心臓生理学に関する。より具体的には、本発明は、心臓における電気伝播の評価に関する。
「アノテーション」又は「アノテーション点」は、目的とするイベントを示すと見なされる電位図上の点又は候補を指す。本開示において、イベントは、通常は、電極により感知される電波の伝播の開始(局所興奮到達時間)である。
これから図面に目を向けると、開示される本発明の一実施形態に基づいて構築され動作可能である、生体被験者の心臓12に対してアブレーション処置を行うためのシステム10の描図である図1を最初に参照する。このシステムは、患者の脈管系を通って心臓12の腔又は脈管構造内に操作者16によって経皮挿入されるカテーテル14を備えている。通常は医師である操作者16が、カテーテルの遠位先端18を、心臓壁、例えば、アブレーション標的部位と接触させる。その開示が参照により本明細書に援用される、米国特許第6,226,542号及び同第6,301,496号、並びに本願と同一譲受人に譲渡された米国特許第6,892,091号に開示される方法に従って、電気的活動マップが作成され得る。

前記電位図のそれぞれのアノテーションを確立するステップと、
ある時間ウィンドウ内で、前記アノテーションから、1組の前記電位図とその前記電極の前記アノテーションにおける単調増加する局所興奮到達時間シーケンスパターンを含む速度パターンを検出するステップであって、前記組の各電極が前記組の少なくとも1つの他の電極から所定距離内に配置された、ステップと、
前記組の前記電位図を表示するステップと、を含む、方法。
(2) 前記電位図が、並行して記録される、実施態様1に記載の方法。
(3) 前記組をその前記電極の前記プローブ上の位置によってソートするステップを更に含む、実施態様1に記載の方法。
(4) 前記パターンが、リエントラント活性化回路である、実施態様1に記載の方法。
(5) 前記パターンが、マイクロリエントリパターンである、実施態様1に記載の方法。
(7) 前記パターンが、マクロリエントリパターンである、実施態様1に記載の方法。
(8) 前記組の前記電位図を表示することが、前記組をその局所興奮到達時間に従ってソートすることを含む、実施態様1に記載の方法。
(9) 新しい時間ウィンドウ内で前記パターンの変化を検出するステップと、
前記変化に応じて、前記組をソートするステップと前記電位図を表示するステップとを繰り返すステップと、を更に含む、実施態様8に記載の方法。

(12) 装置であって、
複数の電極を遠位部分に有するプローブと、
前記プローブが生体被験者の心臓内のある位置にあるときに、前記電極からの対応する時間的に変化する電位図を記録するための電気回路と、
前記電位図を記憶するためのメモリと、
表示装置と、
前記メモリに接続されたプロセッサであって、
前記対応する心内電位図から局所興奮到達時間のアノテーションを確立するステップであって、前記アノテーションが、前記心臓内の電気伝播速度を含むパターンを定める、ステップと、
ある時間ウィンドウ内で、1組の前記電位図とその前記電極の前記アノテーションの単調増加する局所興奮到達時間シーケンスパターンを含む速度パターンを検出するステップであって、前記組の各電極が前記組の少なくとも1つの他の電極から所定距離内に配置された、ステップと、
前記組の前記電位図を前記表示装置に表示するステップと、を実行するように動作可能なプロセッサと、を備える、装置。
(13) 前記電位図が、並行して記録される、実施態様12に記載の装置。
(14) 前記組をその前記電極の前記プローブ上の位置に従ってソートするステップを更に含む、実施態様12に記載の装置。
(15) 前記パターンが、リエントラント活性化回路、マイクロリエントリパターン、フォーカルソースパターン、及びマクロリエントリパターンからなる群から選択される、実施態様12に記載の装置。
(17) 新しい時間ウィンドウ内で前記パターンの変化を検出するステップと、
前記変化に応じて、前記組をソートするステップと前記電位図を表示するステップとを繰り返すステップと、を更に含む、実施態様16に記載の装置。
(18) 前記変化が、電気伝播方向の変化を含む、実施態様17に記載の装置。
(19) 前記変化が、電気伝播速度の変化を含む、実施態様17に記載の装置。
Claims (7)
- 装置であって、
複数の電極を遠位部分に有するプローブと、
前記プローブが生体被験者の心臓内のある位置にあるときに、前記電極からの対応する時間的に変化する電位図を記録するための電気回路と、
前記電位図を記憶するためのメモリと、
表示装置と、
前記メモリに接続されたプロセッサであって、
前記電位図から局所興奮到達時間のアノテーションを確立するステップであって、前記アノテーションが、前記心臓内の電気伝播速度を含むパターンを定める、ステップと、
前記複数の電極に対応する複数の前記電位図を、前記心臓内の前記電極の位置に従い複数のグループにグループ化するステップであって、前記グループ内の各電極は少なくとも1つの前記グループ内の他の電極から所定距離内に配置されている、グループ化するステップと、
ある時間ウィンドウ内で、前記グループ内の前記アノテーションの単調増加する局所興奮到達時間シーケンスパターンを含む速度パターンを検出するステップと、
前記電位図を前記表示装置に表示するステップであって、前記複数のグループを近接性に従いソートし、前記速度パターンが検出された場合、前記グループ内の前記電位図を前記局所興奮到達時間の順序でソートし、表示するステップと、を実行するように動作可能なプロセッサと、を備える、装置。 - 前記電位図が、並行して記録される、請求項1に記載の装置。
- 前記表示するステップは、前記複数のグループを各電極の前記プローブ上の位置に従ってソートするステップを更に含む、請求項1に記載の装置。
- 前記パターンが、リエントラント活性化回路、マイクロリエントリパターン、フォーカルソースパターン、及びマクロリエントリパターンからなる群から選択される、請求項1に記載の装置。
