Claims (14)
- インプラント型医療装置であって、
心臓内血流動態信号を検出するように構成された少なくとも1つのインプラント型血流動態センサを備え、
検出された心臓内血流動態信号および/またはティーガーエネルギーオペレータ(TEO)の信号を適用することにより検出された心臓内血流動態信号から導出された信号を処理および分析するように構成されたコントローラを備え、
TEOは{x(n)}=Ψ(n)=x2(n―1)―x(n―2).x(n)として定義され、
ここで、
「x(n)」は心臓内血流動態信号であり、
「Ψ(n)」は出力血流動態信号であり、
「n」は所定のサンプルを意味し、さらに
前記コントローラは少なくとも1つの出力血流動態信号を考慮することによって、除細動操作の必要性を判断するように構成された少なくとも1つのアルゴリズムをさらに備えるインプラント型医療装置。 - 少なくとも1つの血流動態センサは、インプラント型加速度計、マイクロホン、圧電センサ、または心臓内血流動態信号を検出することができる圧力センサである請求項1に記載のインプラント型医療装置。
- 前記少なくとも1つの血流動態センサは、心音を検出できる請求項2に記載のインプラント型医療装置。
- コントローラの少なくとも1つのアルゴリズムは、出力血流動態信号の少なくとも一つの特性が心室細動現象に関連する持続時間よりも長い時間経過中に所定の閾値を超えない場合に、除細動操作の必要性を判断するよう構成される請求項1、2または3に記載のインプラント型医療装置。
- 出力血流動態信号の少なくとも1つの特性は、信号の振幅、エネルギー、平均値、または中央値、または二乗平均平方根値である請求項4に記載のインプラント型医療装置。
- 出力血流動態信号の振幅および/または出力血流動態信号のそれぞれが、所定の時間経過の間に所定の閾値を超える各信号の所定の閾値および/またはピークの数を昇順または降順で交差する回数が、心室細動の有無を判定するために考慮される請求項4または5に記載のインプラント型医療装置。
- 電気除細動信号を生成するように構成された、インプラント型除細動器と、除細動操作を開始する必要性が医療装置のコントローラによって決定されたときに、電気除細動信号を患者に送るように構成されたリードによって除細動器に接続された少なくとも1つの電極とをさらに備える請求項1〜6のいずれか1項に記載のインプラント型医療装置。
- コントローラは出力血流動態信号を少なくとも1つの電極によって心臓内血流動態信号から独立して検出された少なくとも1つの電気生理学的信号と比較することによって、除細動操作を開始する必要性を判断するように構成される請求項7に記載のインプラント型医療装置。
- コントローラは心臓内血流動態信号のみを考慮することによって、除細動操作の必要性を判断するように構成される請求項1〜7のいずれか一項に記載のインプラント型医療装置。
- コントローラは、検出された心臓内血流動態信号および/または1〜100Hzの範囲の検出された心臓内血流動態信号から導出された信号をバンドパスフィルタリングするように構成される請求項1〜9のいずれか1項に記載のインプラント型医療装置。
- 前記範囲は、7.5〜49Hzである請求項10に記載のインプラント型医療装置。
- コントローラはウィンドウ機能を適用することによって、出力信号をフィルタリングするようにさらに構成される請求項1〜11のいずれか1項に記載のインプラント型医療装置。
- 前記ウィンドウ機能がハミングウィンドウである請求項12に記載のインプラント型医療装置。
- コントローラは定期的に再計算され心臓内血流動態信号の1つまたはそれ以上の特性からまたは前記ティーガーエネルギーオペレータによるそれらの表現から導出される予め定義された静的閾値または動的閾値に従って除細動操作の必要性を決定するために、および/または除細動操作を開始するために構成される請求項4〜13のいずれか一項に記載のインプラント型医療装置。
