HDLの役割は複雑であり、HDLコレステロール(HDL−C)は比較的粗製のバイオマーカであると考えられている。一般に認められているように、従来の脂質パネルによって提供されるHDL−コレステロールおよび/またはLDL−コレステロールレベルは、CHDまたはCVDのリスクの上昇の有無にかかわらず、集団を十分に区別することができない。

より多くの数のリポタンパク質サブクラスの分析は、全体的により高い測定精度の不正確さを生じる可能性があるので、代替の実施形態は、この不正確さを低減するために、より少ない数のHDLサブクラスを利用する。例えば、デコンボリューションモデルは、図1に示すように、上記のように26種類のサブクラスではなく、粒子サイズによって8つの異なるサブクラスに分けられたHDL粒子を分析するように調整してもよい。サブクラスの数が減少すると、精度が向上する可能性があります。例えば、特定の実施形態では、H1は約7.0〜7.6nmの直径範囲を表し、H8は約1.5〜13.5nmの直径範囲を表すことができる。
女性は一般に男性よりCVDリスクが低い。この現象は、HDLコレステロールの違いによっては十分に説明されていない。非常に驚くべきことに、大小のHDLサブクラスであるH1およびH8とのGlycA相互作用を性別で分析したところ、CVDリスクとの関係は性別が非常に高いようである。すなわち、H1およびGlycAによって評価される炎症とのその相互作用は、男性におけるCVD事象の予測因子であり、H8およびそのGlycA相互作用は、女性のCVD事象に特異的である。したがって、H1(男性の場合)およびH8(女性の場合)に少なくとも部分的に基づくジェンダー特異的マルチマーカの開発が適切かもしれない。CVDリスクの性差を考慮したテストまたはテストシステムが特に望ましい。
本発明の実施形態は、少なくとも1つのHDLおよび炎症バイオマーカ相互作用パラメータを含む定義された多成分リスク予測モデルを使用して、インビトロ血漿または血清患者サンプルから収集された定義されたパラメータの測定値を使用して、ヒトのCVDリスクを評価する方法、回路、NMR分光計またはNMR分析器、非一時的コンピュータ可読媒体、オンライン半自動リスク計算器、およびプロセッサである。

前記測定値に基づいて、少なくとも1つの個々のHDL粒子サイズサブクラスおよびGlycAにおけるNMR由来の濃度を決定する決定ステップと、
HDL粒子の少なくとも1つのサブクラスおよびGlycAの前記濃度を少なくとも用いて、被験体のリスクパラメータ(HxI)CVDをプログラム的に計算する計算ステップと、
