R1=芳香族もしくはヘテロ芳香族の5員環もしくは6員環(例えば、任意選択で置換されているベンゼン、ナフタリン、チオフェン、ピロール、イミダゾール、ピラゾール、オキサゾール、チアゾール);または直鎖もしくは分岐鎖の脂肪族残基(例えば、CH(C2H5)CH3、CH2−C(CH3)3)、シクロプロパン、好ましくは6個以上のC原子、より好ましくは6個のC原子もしくは11個以上のC原子を含む脂肪族残基(例えば、脂肪酸残基)である]
であって、前記処置が、治療有効量の前記物質の鼻腔内投与、舌下投与または頬投与から選択される経粘膜投与を含む化学物質を提供することである。

本明細書で検討される薬物のうちの1つであるガランタミンについて、鼻腔内処方物が、高溶解性塩の水溶液に基づき以前に開発された(国際公開第2005/102275A1号パンフレット;Leonhard AK et al.(2005)Development of a novel high−concentration galantamine formulation suitable for intranasal delivery.J Pharmaceut Sciences 94:1736−1746;Leonard AK et al.(2007)in vitro formulation optimization of intranasal galantamine leading to enhanced bioavailability and reduced emetic response in vivo.Int J Pharmaceutics 335:138−146)。
Memogainの溶液(2mlの96%エタノール中502mg、1.28mmol)に、酢酸(463mg、7.71mmol)を添加し、結果として生じた溶液を、いくらかの時間の間撹拌し、塩形成のために一晩放置しておいて、酢酸塩の沈殿を結果としてもたらした。収率をジエチルエーテルの添加により改善し、沈殿物を濾過し、96%エタノールで洗浄した。沈殿物を、室温にて40mbarで20時間にわたりデシケーター中で乾燥させた。結果:無色の固体(吸湿性)。収率:62%、融点:89.3〜91.2℃、HPLC>95%。元素分析:C24H25NO4 *1.5CH3COOHについての計算値 C:71.24、H:6.46、N:3.32 実測値 C:71.36、H:6.17、N:3.43。
メタノール(4ml)中の2.5gのMemogain(6.4mmol)の溶液に、メタノール(2ml)中95%のラセミ乳酸(7.85mmol)の溶液を40〜50℃で添加し、20分間撹拌した。溶媒を蒸発させ、結果として生じた残渣をまず回転蒸発器を用いて9mbarおよび50〜60℃で2時間乾燥させ、続いて、40mbarで室温にて一晩乾燥させて、吸湿性の高い固体淡黄色泡沫を結果としてもたらした。収率:98.92%、融点:62.9〜64.1℃、元素分析:C24H25NO4 *1.1C3H6O3についての計算値 C:66.84、H:6.49、N:2.86。実測値:C:66.69、H:6.45、N:2.80 HPLC純度>97%。
溶媒として乾燥エタノールを用いる以外は一般手順Bを用いて、クエン酸塩を、粘着性の固体として91.0%の収率で得た。これは、乾燥ジエチルエーテルを用いた粉砕、それに続く高真空蒸発後に融点:117.5〜119℃を有する無色の固体に変わった。元素分析:計算値 C:73.64、H:6.44、N:3.58 実測値 C:59.61、H:5.93、N:2.26。HPLC>97%
96%エタノール(4ml)中のMemogain(1120mg)の溶液に、60°の96%エタノール(3ml)中のサッカロラクトン(200〜604mg)の溶液を添加した。この高温溶液を酢酸エチルで即座に希釈して、無色の沈殿物の形成を結果としてもたらし、これを2時間にわたり5°まで冷却した後に濾過し、酢酸エチルで洗浄し、40mbarで20時間にわたり室温にて乾燥させて、融点:132〜134℃および>97%のHPLC純度を有する無色の固体として83.7%収率のサッカリン酸塩を得た。元素分析:C24H25NO4 *C6H10O8についての計算値 C:59.89、H:5.86、N:2.33。実測値:C:60.10、H:5.61、N:2.37。サッカリン酸のラクトンは、これらの条件下に存在する水により加水分解されて、記載される塩を結果としてもたらす。
溶媒としてジオキサンを用い、水(13mg、0.76mmol)を含有するジオキサン中のD−グルコン酸δ−ラクトン(68.2mg、0.38mmol)の溶液を用いること以外はMemogain(150mg、0.38mmol)から出発する一般手順Cに従い、透明な溶液が得られるまで50〜60℃で30分間撹拌し、続いて乾燥ジエチルエーテル(10ml)を冷却した溶液に添加して、無色の沈殿物を結果としてもたらし、これを濾過し、ジエチルエーテルで洗浄し、乾燥させて、170mg(75.6%)の塩を無色の結晶質固体として得た。融点:159.3〜159.4℃ HPLC純度>98% 元素分析:C24H25NO4 *1.5C6H12O7についての計算値 C:57.80、H:6.32、N:2.04。実測値:C:58.22、H:5.98、N:2.28。この塩の粉末回折図を図1に示す。

10mgの対応する塩および100マイクロリットルの水を、室温で5分間超音波処理した。結果として生じた溶液または懸濁液を3分間遠心分離機にかけ、フィルターチップを用いて濾過した。1.0マイクロリットルの濾液を容量フラスコに移し、水で10.0mlに希釈して、試料溶液を得た。20マイクロリットルのこの試料溶液をHPLCのために注入し、Memogainの量をMerck Chromolith RP18カラムおよび5%〜60%のアセトニトリルおよび水(両方の溶媒が0.1%ギ酸を含有する)の勾配を用いて定量化した(注入体積:20マイクロリットル)。
エマルションおよびSMEDDsは、脳送達系の確立された手段である(Botner S,Sintov AC(2011)Intranasal delivery of two benzodiazepines,Midazolam and Diazepam,by a microemulsion system.Pharmacol Pharmacie 2:180−188)。本出願において、それらは、調査下のプロドラッグを窒素塩基としてもしくは水素塩として種々の有機溶媒と混合することにより、または透明な溶液が達成されるまで撹拌下および/もしくは超音波下において好適な界面活性剤、油および共界面活性剤(co−surfactant)(全て安全と認められる;GRAS)と混合することにより、作製された。特に、本発明者らは、鼻粘膜または口粘膜のあらゆる刺激を回避するようにアルコールまたは他の刺激性化学物質を処方物中で使用しないようにした。そのようなマイクロエマルションの典型的な成分は、Labrasol、N−エチル−2−ピロリドン(NEP)、オレイン酸グリセリル、PEG、プロピレングリコール、Transcutol、および好適な油(例えば、パルミチン酸エステル)であった。本発明者らは、およそ50%の最大水可溶化能力と共に10%(w/w)以上のオーダーの薬物溶解度を達成した(水含有量が少ないほどより高い油濃度が達成され得、かつより高い窒素塩基の溶解度が達成され得た)。プロドラッグ窒素塩基または塩の最も高い溶解度は、マイクロエマルション中20%程度の水濃度で得られた。

第1の工程として、100gのマレイン酸Memogainを適切な鋼タンクに量り入れる。以下のものにおいて可溶化剤および脂肪油を順々に添加する。

経粘膜送達のための他の好適な処方物は、プロドラッグナノ結晶およびプロドラッグが吸着されているポリマー微粒子である。両方の場合において、より親油性の高いプロドラッグ塩基を使用した。ポリマーおよびプロドラッグの共沈により、パールミリングおよび水中での均質化により、または脂質複合体であるプロドラッグのナノ懸濁液として、処方物を得た。このような方法は当業者に知られており、本発明の化学物質および投与方法と共に適用され得た。
実験1:1mlのHBSS−Puffer(pH7,4)中の少量(0,1mg)のマレイン酸Memogainを、37℃で2,5時間、種々のキャリアと共にインキュベートした。次いで、自由な(キャリア物質の粒子に結合していない)Memogainの量を、HPLCにより測定した。典型的なキャリア物質量を適用し、表5に示した。
プロドラッグ処方物の透過挙動を、0.64cm2の透過領域および両側に1mlの容積を呈するUssing型チャンバーに挿入された3〜4cm2の新しく摘出されたブタ鼻粘膜または口粘膜の組織試料を用いて試験した。組織の頂端側が、ドナーコンパートメントに面していた。1mlの予め温めた(37℃)透過媒体を、ドナーおよびアクセプターチャンバーに添加した。チャンバー内の温度を、全実験の間中、37℃で維持した。15分の前インキュベーション時間後、ドナーチャンバー中の透過媒体を、調査下のプロドラッグ処方物の1%溶液に取り換えた。180分の時間にわたって、30分おきに、100μlのアリコートをアクセプターコンパートメントから取り出し、即座に100μlの新しい予め温めた透過媒体に置き換えた。収集されたアリコート中の化合物の濃度を、HPLCによって決定した。先に取り出された試料について補正を行った。見かけの透過係数(Papp)を算出した。180分後にドナーコンパートメントからコントロール試料を取り出し、調査下の処方物中の化合物の安定性を調査するために分析した。
鼻腔または頬腔における経粘膜送達後のプロドラッグおよび親薬物の薬物動態を、Wistarラットにおいて試験した。これらのデータは、調査下のプロドラッグの血液および脳への迅速な(数分のうち)取り込み、静脈内注射によりもたらされるものと同様のプロドラッグの脳内バイオアベイラビリティ、ならびに関連親薬物の錠剤としての経口送達に比べてはるかに高いBBRCを裏付けた。
この実験計画は、種々の処方物中のマレイン酸Memogainおよび臭化水素酸ガランタミンの鼻腔内適用に続く雌wistarラットにおけるプロガランタミンであるマレイン酸Memogainおよびガランタミンの血液および脳薬物動態プロファイルについて記載する。
a.水中5%ガランタミン、1鼻腔当たり10μL、合計20μL(1mg5%含有)
b.水中10%NEP中のMemogain塩、1鼻腔当たり10μL、合計20μL(1mg含有)
c.エマルション中5%Memogain塩、1鼻腔当たり10μL、合計20μL(1mg含有)
d.エマルション中20%Memogain塩、1鼻腔当たり10μL、合計20μL(4mg含有)
e.5mg\kgの用量割合でのMemogain塩の静脈内投与(先にコントロールとして実施された)
この実験計画は、種々の処方物中のマレイン酸Memogainおよび臭化水素酸ガランタミンの舌下適用に続く雌wistarラットにおけるプロガランタミンであるマレイン酸Memogainおよびガランタミンの血液および脳薬物動態プロファイルについて記載する。
a.水中5%ガランタミン、舌下20μL(1mg含有)
b.水中10%NEP中5%Memogain塩、舌下20μL(1mg含有)
c.エマルション中5%Memogain塩、舌下20μL(1mg含有)
d.エマルション中20%Memogain塩、舌下20μL(4mg含有)
e.コントロールとしての5mg\kgの用量割合でのMemogain塩の静脈内投与
さらなる試験を、グルコン酸Memogainについて行った。これは、ガランタミンよりもはるかに高いBBRCを有する(図6参照)。いくつかのガランタミン誘導体およびそれらの切断生成物であるガランタミンの薬物動態および脳対血液濃度比(BBRC)を、3mg/kgの用量でのSwiss albinoマウスにおける鼻腔内投与後に評価した。脳および血液からの抽出後に、薬物濃度をLC/MS/MSにより決定した。比較のために、親薬物であるガランタミンについてのBBRCも決定した。図に示されるように、調査されたプロガランタミンは全て、ガランタミンよりも大きなBBRCを示し、Gln−1062について特に大きなBBRCを示す。
データを図13に示す。マウスに、Memogainまたはガランタミンのいずれかを3mg/kg(i.v.)注射した。これらのデータは、ガランタミンは脳内にうまく分布しない(BBRC約1:1)のに対して、Memogainははるかに高いBBRC(8:1)を有するということを明示している。
鼻腔内Memogainがガランタミンよりもインビボで有効な認知増強剤であるか否かを試験するために、次の認知パラダイムを適用した。マウスをスコポラミンで処置して急性健忘症を誘導し、次いで、経口ガランタミンまたは鼻腔内Memogainの不在下または存在下において、T迷路における行動について試験した(図7)。明らかに、Memogainは、急性的に誘導された健忘症を逆転させることにおいてガランタミンよりも有効であった。マウスを、見当識障害/健忘症を誘導するためにT迷路アッセイにおいてスコポラミン(i.p.)で攻撃した(0%行動回復に設定した)。ガランタミン(i.p.)またはMemogain(登録商標)(i.n.)の共適用(co−application)は、用量依存的な様式でT迷路における見当識を救済する。
Gln−1062の初回通過効果を、Wistarラットにおいて3mg/kgの静脈内および門脈内投薬後に評価した(図8)。Gln−1062は、それがi.v.で投与されたかi.n.で投与されたかに関わらず、急速に低下する血液濃度レベルにより、初回通過効果を受けることが観察された。それに対して、酵素切断によりGln−1062から遊離されるガランタミンの濃度レベルは、同様に急速には低下しなかった。さらに、門脈内投与に比べてGln−1062のより高い最大濃度レベルが、i.v投与に続いて脳および血液中において観察された。これらのデータから、初回通過効果は35〜45%の間であると推定された。
この調査はイヌにおいて行われた。4mg/kgの鼻腔内Memogainの単回用量を投与し、Memogainおよび遊離ガランタミンの血漿レベルを投与後の時間の関数として決定した。Memogainは優先的に脳に分配されるので、ほんの少しのプロドラッグしか血液中に現れない。ガランタミンは迅速に代謝され排泄されるので、Memogainから遊離されたガランタミンのレベルははるかに低い(図9)。これは、i.n.投与後の血液中に存在する全身性ガランタミンが少量であることを考慮すると、副作用の可能性のかなりの減少に繋がる。
− Memogainの脳:血液比(投与後120分の時点)=9
− ガランタミンの脳:血液比(投与後120分の時点)=1〜1.5
− 血中Memogain t1/2=90分(意識のある動物)
− ガランタミン t1/2=6時間(意識のある動物)
− ガランタミンの低血中レベルは、より少ない副作用を示す
− Memogainの高い脳濃度は、主として脳内におけるMemogainからのガランタミンの放出を示す。
これらの調査は、フェレットにおいて行われ、これらのフェレットには、20mg/kgのガランタミン(最大耐量)またはそれぞれ20、40および80mg/kgのMemogainのいずれかが、i.p.投薬された。20mg/kgのMemogainにおいて、有害作用は何ら観察されなかった。有害作用の用量依存性から、この動物モデルにおいては少なくとも4倍低い毒性が、ガランタミンに比べてMemogainについて観察された(図10)。
この調査はイヌにおいて行われ、両方の薬物が静脈内ボーラスとして投与された。Memogainのより低い毒性は、プロドラッグの酵素切断の結果として生じるガランタミンのはるかに低い定常状態血漿レベルのためである(図11)。
重要な利益は次の通りである:
1.標的器官におけるより高いバイオアベイラビリティおよびより高い有効性
2.より低いレベルの抹消性副作用
3.薬物動態が医学的必要性に調整され得る(持続送達)
4.GI有害作用により投薬が制限されない
5.より速くかつより強力な医学的利益の開始
6.(コンプライアンスを高めるための)用量の上方滴定が必要とされない
7.有効用量の即時投与
8.改善された患者のコンプライアンス
Claims (16)
- 請求項1に記載の哺乳動物における認知障害に関連する脳疾患の処置のための薬学的組成物において、前記化合物GLN 1062が塩として存在することを特徴とする薬学的組成物。
- 請求項2に記載の哺乳動物における認知障害に関連する脳疾患の処置のための薬学的組成物において、前記化合物GLN 1062がマレイン酸塩として存在することを特徴とする薬学的組成物。
- 請求項1乃至3の何れか1項に記載の哺乳動物における認知障害に関連する脳疾患の処置のための薬学的組成物において、前記化合物GLN 1062またはその塩が、体積当たり少なくとも10重量%(w/v)の水への溶解度を有することを特徴とする薬学的組成物。
- 請求項1乃至4の何れか1項に記載の哺乳動物における認知障害に関連する脳疾患の処置のための薬学的組成物において、前記化合物GLN 1062またはその塩が、1日1〜3回、1〜100mgの投薬量で投与されることを特徴とする薬学的組成物。
- 請求項1乃至5の何れか1項に記載の哺乳動物における認知障害に関連する脳疾患の処置のための薬学的組成物において、前記化合物GLN 1062またはその塩が、1日2回、2〜40mgの投薬量で投与されることを特徴とする薬学的組成物。
- 請求項1乃至6の何れか1項に記載の哺乳動物における認知障害に関連する脳疾患の処置のための薬学的組成物において、前記舌下投与が:
(i)舌下錠によって;
(ii)1滴以上の溶液、または、凍結乾燥された粉末もしくはエマルションの形態のある量の微粒子を舌の下側に置くことによって;および/あるいは
(iii)予め選択された体積の液体組成物を舌の裏側に噴霧することによって、
舌下に治療有効量の前記化合物GLN 1062またはその塩を投与することで行われることを特徴とする薬学的組成物。 - 請求項1乃至7の何れか1項に記載の哺乳動物における認知障害に関連する脳疾患の処置のための薬学的組成物において、前記頬投与が、治療有効量の前記化合物GLN 1062またはその塩を、凍結乾燥された粉末もしくはエマルションとして、生体内で徐々に破壊される(bioeroding)高分子キャリアを含む頬投薬単位として、または、口腔内崩壊錠もしくは口腔内分散錠(ODT)として、口内の頬と歯茎との間の頬前庭に投与することにより行われることを特徴とする薬学的組成物。
- 請求項1乃至8の何れか1項に記載の哺乳動物における認知障害に関連する脳疾患の処置のための薬学的組成物において、処置されるべき前記脳疾患が、アルツハイマー病および/もしくはパーキンソン病、他の種類の認知症、統合失調症、癲癇、脳卒中、小児麻痺、神経炎、筋障害、低酸素症、無酸素症、窒息、心拍停止後の脳における酸素および栄養素欠乏、慢性疲労症候群、様々な種類の中毒、知覚麻痺、特に神経遮断性知覚麻痺、脊髄障害、炎症、特に中枢性炎症性障害、術後せん妄および/もしくは亜症候群性術後せん妄、神経障害疼痛、アルコールおよび薬物の乱用、中毒性のアルコールおよびニコチン渇望、ならびに/または放射線治療の影響から選択されることを特徴とする薬学的組成物。
- 請求項1乃至9の何れか1項に記載の哺乳動物における認知障害に関連する脳疾患の処置のための薬学的組成物において、前記薬学的組成物が、N−エチルピロリドンを含むことを特徴とする薬学的組成物。
- 請求項10に記載の哺乳動物における認知障害に関連する脳疾患の処置のための薬学的組成物において、前記薬学的組成物が、カプリル酸グリセリル、ポリエチレングリコール、プロピレングリコールおよび/またはジエチレングリコールモノエチルエーテルを含むことを特徴とする薬学的組成物。
- 請求項1乃至9の何れか1項に記載の哺乳動物における認知障害に関連する脳疾患の処置のための薬学的組成物において、前記薬学的組成物が、キトサンを含む徐放性処方物を含むことを特徴とする薬学的組成物。
- 請求項1乃至9の何れか1項に記載の哺乳動物における認知障害に関連する脳疾患の処置のための薬学的組成物において、前記薬学的組成物が、ラクトース一水和物、トウモロコシデンプン、ポリビニルピロリドン(PVP)および/またはステアリン酸マグネシウムと、任意選択で着香料とを含む、舌下錠を含むことを特徴とする薬学的組成物。
- 請求項1乃至9の何れか1項に記載の哺乳動物における認知障害に関連する脳疾患の処置のための薬学的組成物において、前記薬学的組成物が、マンニトール、デンプングリコール酸ナトリウム、クロスカルメロース、アスコルビン酸および/またはステアリン酸マグネシウムと、任意選択で着香料とを含む、舌下錠を含むことを特徴とする薬学的組成物。
- 請求項1乃至9の何れか1項に記載の哺乳動物における認知障害に関連する脳疾患の処置のための薬学的組成物において、前記薬学的組成物が、消化酸耐性被覆を有する多層錠剤を含むことを特徴とする薬学的組成物。
- 請求項1乃至15の何れか1項に記載の哺乳動物における認知障害に関連する脳疾患の処置のための薬学的組成物において、前記薬学的組成物が、舌下錠またはバッカル錠の形態で重量当たり2〜40重量%(w/w)で投与される前記化合物GLN 1062またはその塩を含むことを特徴とする薬学的組成物。
