Description
(膣壁に対する出産の影響)
出産後にまたは加齢とともに、女性は、自身の膣筋肉の衰えまたは弛緩を経験する場合がある。たとえば、出産は、膣弛緩症(VRS)としても知られる、塑性膣壁拡張を引き起こす可能性がある。査読済みの文献により、出産は、塑性膣拡張、尿失禁および便失禁、ならびに骨盤臓器脱を含むいくつかの骨盤底疾患の、4倍から7倍の増加(1)と関連付けられることを明らかにしている。文献は、出産が、骨盤底の横紋筋(肛門挙筋と名付けられている)の直接の筋肉外傷または除神経負傷を引き起こし、このことにより、骨盤臓器の筋肉支持の不具合につながることを提示している。これらの疾患は、米国の成人女性の3分の1に影響を与え、その生活の質に大きな影響を及ぼしている(2、3、4、5)。149,554人の成人女性のある研究は、80歳までに骨盤底疾患または失禁に関する1回の手術を経験するリスクが11パーセントであることを報告し、これらの女性の29パーセントが複数の外科手術を必要とすることを見出した(6)。
膣筋肉組織構造、膣粘膜壁、および膣筋肉組織は、人体における他の組織と同様に、タンパク質線維の細胞外マトリックス(ECM)およびECM内の線維芽細胞を含む。膣筋肉組織を引き締めるために、タンパク質線維のECMを、またはより具体的には、タンパク質線維自体を、引き締めることができる。ECMは、コラーゲン、及びエラスチンなどの主要な構造タンパク質を含む、複数のタンパク質の線維を含む。コラーゲンタンパク質は、個々の鎖が水素結合によって1つに保持された3重らせん構造を有し、ばねのような引っ張り特性を有する原線維を形成する1つに集約された個々の鎖に起因して、弾性で復元力のある特性を組織にもたらす。
研究によれば、振動が、ECMのリモデリングにおける関与物である線維芽細胞を含む、様々なヒト細胞型に対して効果を有することも示されている(13、14、15、16、17)。生体内には、機械的ひずみが高い場所において特異的に発現する2つのECM糖タンパク質、テネイシンおよびXII型コラーゲンが存在する。テネイシンは、治癒中の創傷の周囲に現れ、コラーゲン結合に関与する重要なタンパク質である、フィブロネクチンの関与する制御反応の一部である。ひずみのあるコラーゲンマトリックスに付着した線維芽細胞は、2つのECM糖タンパク質を、弛緩したコラーゲンマトリックスに付着した線維芽細胞よりも多く産生する(13)。したがって、振動の効果を利用することによって筋肉強度、平衡感覚、および柔軟性を改善するために、リハビリテーションおよびスポーツ活動において、全身振動訓練が広く用いられている(18)。
振動に加えて、研究は、低レベルレーザ光(LLLT)も、線維芽細胞増殖、転移、およびコラーゲン合成を刺激することによって、線維芽細胞に影響を及ぼすことを示している。いくつかの近赤外部分を含むスペクトルの濃い赤色部分におけるレーザ光も、アポトーシスおよび細胞死を防止し、炎症反応および抗酸化反応を調整し、血管形成および組織修復を刺激する。LLLT効果は少量の光に特有のものであり、今や運動選手の筋骨格の負傷の修復のための十分に受け入れられたモダリティである(21)。

- 前記発光機構は600〜1000nmの近赤外光範囲内でおよび可視光スペクトル内で
動作するように構成され、前記近赤外光範囲は前記膣組織において新コラーゲン形成、新線維形成、または新弾性線維形成を誘起するのに十分であることを特徴とする請求項1乃至4のいずれか一項に記載のデバイス。 - 前記発光機構は低レベルレーザ光療法のためにスペクトルの濃い赤色部分で動作するレーザを使用するように構成されることを特徴とする請求項1乃至5のいずれか一項に記載のデバイス。
- 冷却機構は受動伝導を使用するヒートパイプ組立体、低温流体を使用する同軸外部鞘体組立体、または冷却モジュールを備えることを特徴とする請求項2乃至6のいずれか一項に記載のデバイス。
- 前記振動機構は高効率共振デバイス機構を備えることを特徴とする請求項1乃至7のいずれか一項に記載のデバイス。
- 前記振動機構は5〜10Hzの範囲内でおよび1〜15kHzの範囲内で動作するように構成されることを特徴とする請求項1乃至8のいずれか一項に記載のデバイス。
- 前記制御装置は前記振動機構および前記ユーザインターフェース上で前記使用者によって選択される継続時間設定に基づいて前記安全温度機構を自動的に調節するように構成されることを特徴とする請求項1乃至9のいずれか一項に記載のデバイス。
- 前記ユーザインターフェースは複数の熱的負荷設定のための制御装置を備え、前記設定は前記複数の発光器のうちのどの発光器がオンまたはオフであるかの指定を制御することを特徴とする請求項1乃至10のいずれか一項に記載のデバイス。
- 前記ユーザインターフェースは、
複数の振動設定、
複数の継続時間設定、および
複数の発光設定
のための制御装置を備えることを特徴とする請求項1乃至11のいずれか一項に記載のデバイス。 - 前記デバイスは、前記膣組織の表面に対する前記発光器の表面の屈折率に調和するカスタマイズされた医療用潤滑剤とともに使用されるように構成されることを特徴とする請求項1乃至12のいずれか一項に記載のデバイス。
- 前記デバイスは家庭での使用向けに構成された片手持ちデバイスであることを特徴とする請求項1乃至13のいずれか一項に記載のデバイス。
- 再活性化およびマッサージデバイスであって、
垂直軸に沿った長さを持つ外殻と、
膣内に挿入可能な挿入可能なデバイス部分であって、前記外殻が、膣組織に接触するように適合された挿入可能な部分の外面を形成し、前記挿入可能な部分は、加熱して前記膣組織に熱を送達することができ、前記挿入可能な部分は、
前記外殻の周囲に形成された前記外殻の透明部分と、
前記透明部分の遠位にある前記挿入可能な部分の丸い端部を形成する前記外殻の第1の不透明部分と、
前記外殻の周囲に形成され、かつ、前記透明部分の近位に位置する前記外殻の第2の不透明部分と、
を含む、挿入可能な部分と、
前記挿入可能な部分内に組み込まれ、かつ、前記透明部分に接している前記膣組織の標的領域上に前記透明部分を介して通過する光を照射して前記膣組織において新コラーゲン形成、新線維形成、または新弾性線維形成を誘起するのに十分である温度で熱的負荷を前記膣組織の前記標的領域上に適用するように構成された、発光機構と、
前記挿入可能な部分内に組み込まれ前記熱的負荷の適用中に前記膣組織をマッサージするために前記膣組織に振動を提供するように構成された振動機構と、
前記挿入可能な部分内に組み込まれ前記挿入可能な部分と接触している前記膣組織
の温度を測定するように構成された安全温度機構と、
前記挿入可能な部分と関連付けられた電源と、
前記安全温度機構、前記振動機構、および前記電源に結合された前記挿入可能な部分内に組み込まれた制御装置と、
前記制御装置に結合されかつ使用者によってアクセス可能なユーザインターフェースであって、前記制御装置は前記ユーザインターフェースを使用する前記使用者からフィードバックを受信するように構成されたユーザインターフェースと
を備えることを特徴とする再活性化およびマッサージデバイス。 - 前記発光機構は前記膣組織内の5mmまでの深さにおいて60〜80℃の第2の温度範囲を結果的にもたらし得る35〜41℃の温度範囲で動作するように構成され、前記第2の温度範囲が前記膣組織において新コラーゲン形成、新線維形成、または新弾性線維形成を誘起するのに十分であることを特徴とする請求項15に記載のデバイス。
- 前記発光機構は600〜1000nmの近赤外光範囲内でおよび可視光スペクトル内で動作するように構成されることを特徴とする請求項15または16に記載のデバイス。
- 前記発光機構は、低レベルレーザ光療法のためにスペクトルの濃い赤色部分で動作するレーザを使用するように構成されることを特徴とする請求項17に記載のデバイス。
- 受動伝導を使用するヒートパイプ組立体、低温流体を使用する同軸外部鞘体組立体、または冷却モジュールを備える冷却機構をさらに備えることを特徴とする請求項15乃至18のいずれか一項に記載のデバイス。

ことを特徴とする請求項15乃至21のいずれか一項に記載のデバイス。 - 前記デバイスは家庭での使用向けに構成された片手持ちデバイスであることを特徴とする請求項15乃至22のいずれか一項に記載のデバイス。
- 前記発光機構は、少なくとも1つの効果を生じさせ、前記少なくとも1つの効果が、膣組織において、アポトーシスおよび細胞死の防止、線維芽細胞増殖、転移、およびコラーゲン合成の刺激、炎症反応および抗酸化反応の調整、ならびに血管形成および組織修復の再現から成る群から選択されることを特徴とする請求項15乃至23のいずれか一項に記載のデバイス。
- 前記振動機構は筋原線維生成および新コラーゲン形成を向上させるのに十分な範囲内で動作するように構成されることを特徴とする請求項15乃至24のいずれか一項に記載のデバイス。
