Description

我々の人口が年を取るにつれて、骨粗鬆症骨折は、我々の人口の健康に対して高い影響を有することが予想される。今日、2千万人を超える米国人が骨粗鬆症に罹患していると推定され、米国では毎年150万件の骨粗鬆症骨折が起こっている(1)。確立された骨粗鬆症を有する患者において、現在利用可能な医療は、臨床的な非脊椎骨折のリスクを適度に低下させ得ることだけである(2、3)。現在、骨粗鬆症治療の主軸は、経口および静脈内のビスホスホネートの使用である。これらの薬物は、骨吸収を抑制して機能するが、骨形成も低下させる(4)。テリパラチド(TPTD、hPTH(1-34))は、現在利用可能な唯一の同化作用性薬剤であり、新規の骨形成の刺激(吸収を伴う)および内部の骨微小構造の再構築を含む機構により作用する(5〜7)。骨密度(BMD)に対するテリパラチドの効果は、脊椎では骨吸収抑制剤よりも優れているが、股関節部での効果はより緩やかであり、しばしば2年コースの治療の2年目まで遅延する(8、9)。股関節部骨折は、骨粗鬆症患者において特に一般的であり、骨粗鬆症患者におけるBMDの向上および股関節部骨折のリスクの低下のために、新規の治療を開発する必要がある。
PTHrPまたはそのアナログの治療有効量を、骨折の予防または低減を必要とする被験体に投与する工程を含む、該被験体において骨折を予防または低減するための方法が本発明において提供される。特定の態様において、PTHrPアナログは、配列番号:1に記載されるアミノ酸配列:

Ala Val Ser Glu His Gln Leu Leu His Asp Lys Gly Lys Ser Ile Gln Asp Leu Arg Arg Arg Phe Phe Leu His His Leu Ile Ala Glu Ile His Thr Ala (配列番号:2)である。PTHrPは、同じGタンパク質結合受容体に結合するアミノ末端で、PTHと相同性を有するタンパク質である。共通の受容体(PTHR)にかかわらず、PTHは主に、カルシウム恒常性の内分泌調節因子として働き、PTHrPは、軟骨内骨発生の媒介において重要なパラクラインの働きをする(11)。これらのタンパク質の異なる効果は、異なる組織発現だけでなく、異なる受容体結合特性にも関連し得る(12〜14)。過去数年にわたり、PTHrPは骨粗鬆症の有力な治療として研究されてきた。これらの研究の結果は組み合わされて、いくつかは、高用量のPTHrPの断続的な投与により、同時に骨吸収を刺激することなく骨形成が増加されることを示唆し、他のものは、骨吸収の適度な刺激および有意な高カルシウム血症を報告している(15〜17)。
実施例1. 骨粗鬆症を有する閉経後の女性における骨折の減少における使用のためのPTHrPアナログアバロパラチドの評価。
骨粗鬆症を有するかそうでなければ健常な閉経後の女性において、アバロパラチドのACTIVEフェーズ3骨折予防試験を行った。含まれる被験体を、80マイクログラム(μg)のアバロパラチド、適合するプラシーボまたは承認された日用量20μgのテリパラチドで18ヶ月間治療した。全ての患者群において、ACTIVE試験により、骨折の割合、骨折リスク、BMDおよび骨代謝回転バイオマーカー(例えばCTXおよびP1NP)を評価した。アバロパラチド治療群およびプラシーボ治療群における適格被験体は延長試験を続け(ACTIVExtend)、この試験で被験体は、骨粗鬆症管理について承認されたアレンドロネート治療を6ヶ月受け、骨折発生率が評価された。
PTHrPアナログアバロパラチドについてのACTIVE中枢フェーズ3骨折予防試験、試験BA058-05-003(ClinicalTrials.gov参照)は、80マイクログラム(μg)のアバロパラチド、適合したプラシーボまたは承認された日用量20μgのテリパラチドの1つの18ヶ月間の日用量を受けることについて無作為化された閉経後骨粗鬆症女性における無作為化した、二重盲検、プラシーボ対照試験であった。80μgの日用量でのアバロパラチドまたはプラシーボによる治療は、試験の間中全ての群(party)について盲検のままであった。使用したテリパラチドは、特許された予め充填された薬物であり、再度包装できないデバイスの組合せであった。そのため、その同一性は、治療する医師および患者に対して、いったん使用が開始されると盲検にはできなかった。試験医薬は、最大で18ヶ月間皮下注射により毎日自己投与された。全ての含まれる患者は、試験に含まれた時から治療期間の終了時点までカルシウムおよびビタミンDの補充も受けた。一月の追跡期間中もこれらの補充を続けることが患者に推奨された。
骨折リスク低減
プラシーボと比較した第2の終点において、アバロパラチドにより、決定された(adjudicated)患者の非脊椎骨折サブセット(プラシーボ群:n=33、骨折率4.0%;およびアバロパラチド群:n=18、骨折率2.2%)において43%の統計的に有意な骨折リスクの減少(p=0.0489、95% CI=0.32〜1.00)が達成され(図4A)、脊椎骨折および非脊椎骨折の両方を含む決定された臨床的骨折群(プラシーボ群:n=49、骨折率6.0%;およびアバロパラチド群:n=27、骨折率3.3%)において43%の統計的に有意な減少(p=0.0165、95% CI=0.35〜0.91)が達成され(図2A)、決定された非脊椎骨折(図4B)および臨床的骨折の患者サブセット(図2B)の両方で、最初の発生非脊椎骨折までの時間に統計的に有意な差が達成された。 オープンラベルテリパラチド[rDNA起点]注射治療群は、プラシーボと比較して、決定された患者の非脊椎骨折サブセット(図4A)において28%の骨折リスクの減少(p=0.2157、95% CI=0.42〜1.22)、および決定された臨床的骨折群(図2A)において29%の減少(95% CI=0.46〜1.09)が達成された。アバロパラチド治療群において観察される骨折リスクの減少は、オープンラベルテリパラチドと比較して、統計的に有意ではなかった(図2Aおよび4A、ならびに表1)。
図5Aには、全ての患者群:プラシーボ(ひし形)、アバロパラチドで治療した患者(正方形)、およびテリパラチドで治療した患者(三角形)における手首BMDの変化を示した。テリパラチドとの比較において、アバロパラチドは、予期されずに、遠位(ultra-distal)橈骨において18ヶ月目にBMD維持の有意な向上を示した。
図4Bには、治療対象(intent-to-treat)集団における治療群による第1の発生非脊椎骨折(指、趾、胸骨、膝蓋骨、頭蓋骨および顔面骨を除く)に対する時間のカプラン-マイヤー曲線を示す。図2Bには、治療対象集団における治療群による第1の発生臨床的骨折(指、趾、胸骨、膝蓋骨、頭蓋骨および顔面骨を除く)に対する時間のカプラン-マイヤー曲線を示す。カプラン-マイヤー曲線は、アバロパラチドで治療した群における非脊椎骨折および臨床的骨折のリスクの有意な減少を示す。
ACTIVE試験により、血液カルシウムレベル、起立性低血圧、悪心、めまいおよび注射部位反応を含むいくつかの潜在的な安全性測定も評価した。任意の治療群において≧5%で報告された有害事象(AE)を、プラシーボ、アバロパラチドおよびテリパラチドのそれぞれで治療した群について以下の表7にまとめた。
この実施例は、FRAXツールを使用したベースライン骨折リスクに対するPTHrPアナログアバロパラチドの効果を示す。
患者および方法
試験被験体
健常な閉経後の55〜85歳の女性(無月経およびFSHの血清レベルの上昇の5年歴に基づく)を、骨粗鬆症の以下の定義の1つを満たす場合に試験に登録した。
1) 腰椎または大腿骨頸部または総股関節部でのDXA由来BMD Tスコア≦-2.5。
2) 過去5年の以前の低い外傷の前腕、上腕骨、脊椎骨、頭蓋骨、骨盤、股関節部、大腿骨または脛骨の骨折の病歴を有するDXA由来BMD Tスコア≦-2.0。
3) さらなる骨粗鬆症リスク因子、例えば65歳以上の年齢または骨粗鬆症の強い母方の病歴(BMD基準により決定される骨粗鬆症に関する骨折または骨粗鬆症自体として定義される)を有するDXA由来BMD Tスコア≦-2.0。
この試験(clinicaltrial.gov #NCT00542425)は、米国、アルゼンチン、インドおよび英国の30の研究施設で行われた、無作為化、並行群、複数施設、用量所見、二重盲検プラシーボ対照化試験であった。全ての被験体は、何らかの試験手順を開始する前に書面でのインフォームドコンセントを提供された。適格性について被験体をスクリーニングし、以下の24週間の自己投与治療群:毎日のプラシーボ皮下注射、毎日のPTHrPアナログアバロパラチド(20μg、40μgまたは80μg)皮下注射または毎日の20μgのテリパラチド(Forteo(登録商標); Eli Lilly)の皮下注射の1つに対して無作為化した。全ての被験体は、1回の局所的な実施毎に補充カルシウム(500〜1000mg)およびビタミンD(400〜800IU)を受けた。患者および研究者は、試験を通じてアバロパラチドおよびプラシーボでの治療について盲検のままであったが、テリパラチドに無作為化された患者は、市販の薬品および送達デバイスを使用する必要があったために盲検ではなかった。ベースラインおよび治療開始の3および6ヶ月後にDXAによりBMDを評価した。骨代謝回転の生化学マーカー、血清アバロパラチドレベルおよび抗アバロパラチド抗体形成測定値を治療期間中に得た。薬物投与の4時間および24時間後に血液カルシウムレベルを評価した。各訪問での有害事象(AE)および注射部位での局所耐性について被験体をモニタリングした。臨床および実験室安全性パラメーター、心電図もそれぞれの試験訪問で測定した。
二重X線吸収測定法:それぞれの局所部位でDXAスキャンを得て、次いでスキャンが品質管理検討にかけられ、それぞれの製造業者のガイドラインに従って分析される中央画像化読み込み機(BioClinica Inc. Newton, PA)に送信した。ベースラインスキャンに使用されたものと同じ装置により治療期間中に行われるスキャンを取得した。それぞれの試験場所で経時的に装置品質管理(Instrument Quality Control)(装置標準化およびファントム(phantom)補正)を行い、中央読み取り機(central reader)により検討した。
試験薬物の少なくとも1つの用量を受けた全ての無作為化された患者を使用して有効性および安全性を評価した。記述統計を使用してベースライン特性および安全性を要約した。主な有効性終点は、ベースラインから24週までのBMDおよび骨代謝回転マーカーの変化であった。治療群、試験訪問および訪問による治療の相互作用を固定された効果として含んだそれぞれの訪問における変化の混合モデル反復措定分析を使用して有効性終点を分析した。訪問の間の分散-共分散マトリックスは組織化されていない(unstructured)ものとみなした。80mg群から開始して、次いで40mgおよび最後に20mgまでの連続的な様式でこのモデルを使用して、24週でのそれぞれのアバロパラチド用量 対 プラシーボについてのベースラインからの平均変化の比較を評価した。このモデルを同様に使用して、テリパラチド 対 プラシーボの比較を行った。骨マーカーの結果におけるベースラインからのパーセント変化の非対称性(skewedness)のために、中央値および四分位数間領域を報告する。治療の比較のために、混合モデル反復測定分析を行う前に骨マーカーの結果を対数変換した。テリパラチド群を除外する同じモデルを使用した3つのアバロパラチド用量群およびプラシーボ群の線形対比を試験して、アバロパラチドの増加用量と有効性応答の増加の用量応答関係を評価した。ポストホック(post-hoc)分析において、発明者らは、プラシーボ、テリパラチドおよびアバロパラチド80μg群のみにおける治療の24週後に脊椎、大腿骨頸部、総股関節部において>3% BMDを達成した患者数(%)も評価した。3%の95%信頼限界でのBMDにおける最小有意変化(LSC)に対応する約1%のDXAスキャナの正確性に基づいて3%閾値を選択し、以前の応答体分析(responder analysis)を確認した(22〜28)。応答体分析には、ベースラインおよび24週BMD測定の両方を有する患者のみを含んだ(有効性完了体(valid-completers))。治療群の間の応答体の数(%)の差をχ二乗検定で評価した。2面5%有意さレベルで全ての仮説を試験した。これはフェーズIIの用量-応答仮説生成試験であったので、複数の比較についてp値は調整しなかった。統計分析についてSASシステムバージョン8.2 (SAS Institute Inc.)を使用した。
試験を進行中にしたまま、補正としてプロトコルに24週の延長を追加した。延長に適格であるために、試験被験体は、最後の治療用量を受けてから2週間以内であることが必要であった。合計69名の患者が延長について適格であり、そのうち55名は治療を48週まで継続した(プラシーボ群n=11、アバロパラチド20μg n=13、アバロパラチド40μg n=10、アバロパラチド80μg n=7、テリパラチド20μg n=14)。48週の訪問でBMDを再度測定した。
図11には、試験被験体の傾向を示す。無作為化された222名の患者のうち、1名を除き全てが少なくとも1用量の試験薬を受け、191名(86%)の患者が12週でBMDの測定を受け、184名(83%)が24週の訪問を通じて試験を完了した。5治療群の被験体は、ベースラインBMD測定および骨代謝回転の生化学的マーカーのレベルを含む人口統計的かつ臨床的な特性に関して同様であった。
図12には、種々の治療群:プラシーボで治療された患者(正方形)、20μgのアバロパラチドで治療された患者(三角形)、40μgのアバロパラチドで治療された患者(逆三角形)、80μgのアバロパラチドで治療された患者(ひし形)およびテリパラチドで治療された患者(塗りつぶされた丸)における腰椎(図12A)、大腿骨頸部(図12B)および総股関節部(図12C)のBMDの24週での変化を示す。
応答体分析の結果を図13に示す。腰椎で>3%のBMD増加を有する被験体のパーセンテージは、アバロパラチド群(80μg用量、86%)でプラシーボ群(36%)よりも大きかったが(*p<0.001)、テリパラチド群(70%)より大きくはなかった(p=0.092)(図13A)。さらに、テリパラチド(16%、p<0.02)またはプラシーボ(15%、p<0.04)で治療した女性よりも多くのアバロパラチドで治療した女性が>3%の総股関節部BMD増加(37%)を有した(図13C)。3つの群のいずれかにおいて大腿骨頸部で>3% BMDの増加を経験する女性のパーセントに統計的に有意な差は見られなかった(図13B)。
図14には、種々の治療群:プラシーボで治療した患者(正方形)、20μgのアバロパラチドで治療した患者(三角形)、40μgのアバロパラチドで治療した患者(逆三角形)、80μgのアバロパラチドで治療した患者(ひし形)、およびテリパラチドで治療した患者(塗りつぶされた丸)における、骨形成(P1NP(図14B)、OC(図14C))および骨吸収(CTX、図14A)の血清生化学的マーカーの24週の変化を示す。a:24週でプラシーボに対してp<0.002。b:24週でテリパラチドに対してp<0.003。
24週の治療期間中、221名の患者中164名(74%)で治療突発的AE(TEAE)が報告された。TEAEを経験した患者の割合は、プラシーボ、アバロパラチド20、40および80μgならびにテリパラチド群のそれぞれにおいて71%、72%、74%、76%および78%で、治療群間で同様であった。試験治療に関係がある可能性があるまたはおそらく関係があると研究者に考えられるTEAEは、221名の患者中66名(30%)で、プラシーボ、アバロパラチド20、40および80μgならびにテリパラチド群のそれぞれにおいて27%、21%、35%、38%および29%であると報告された。頭痛の発生率は、プラシーボと比較して、アバロパラチド40μgおよび80μgで数的により高く、プラシーボ、アバロパラチド20、40および80μg群のそれぞれで患者の7%、5%、14%および13%であり、テリパラチドに対しては同様であった(13%)。眩暈も、アバロパラチド80μgで最も高く、プラシーボ、アバロパラチド20、40および80μgならびにテリパラチド群のそれぞれで4%、0%、9%、11%および4%であった。注射部位反応の大部分は、強度において緩やかまたは中程度であり、アバロパラチドおよびテリパラチド治療群で同様であった。TEAEの大部分は重症度において緩やか〜中程度であった。8名の患者(4%)は、24週間の試験中に強度において重症であった少なくとも1つの事象を経験し、重症な事象の発生率は治療群を通じて同様であった。重症な事象には、背部および胸部の疼痛(プラシーボ群)、インフルエンザ、腹水および卵巣上皮癌(アバロパラチド20μg群、治療の14日後に診断された)、頭痛(アバロパラチド40μg群)、消化不良、失神、下痢および上部腹痛(アバロパラチド80μg群)、ならびに関節痛および関節障害(テリパラチド群)が含まれた。重度の1つの事象、アバロパラチド80μg群の患者における失神を、試験治療と関連があり得るものとして評価し、該事象は、1日以内に解消され、治療を必要としないものであると報告された。重度の全ての他の事象は試験治療に関係のないものとして報告された。重大なTEAEは、プラシーボ治療患者において急性気管支炎、アバロパラチド20μgに割り当てられた患者において腹水を伴う卵巣癌およびアバロパラチド80μg群の患者において憩室炎が3名の患者(1%)で報告された。治療に関連があると分類されたものはなく、死亡は報告されなかった。アバロパラチド20μgおよび40μg群において各一人(2%)、アバロパラチド80μg群において3名の患者(7%)およびテリパラチド群において2名の患者(4%)を含む7名の患者(3%)は、AEのために中断された。ECGパラメーターについてのプラシーボおよび活性治療群の間で臨床的に意味のある差は見られなかった。
≧10.5mg/dLの血清カルシウムレベルが、プラシーボ群の1名の患者(2%)、アバロパラチド20μg群の3名の患者(7%)、アバロパラチド40μgの6名の患者(14%)、アバロパラチド80μg群の5名の患者(11%)およびテリパラチド群の18名の患者(40%)において投与の4時間後に観察された。4時間での高カルシウム血症の発生率は、それぞれのアバロパラチド群よりもテリパラチドにおいてより高かった(p<0.01)。最後の注射の24時間後に測定した場合、≧10.5mg/dLの血清カルシウムレベルが、プラシーボ群の1名の患者(2%)、アバロパラチド20μg群の2名の患者(5%)、アバロパラチド40μgの3名の患者(7%)、アバロパラチド80μg群の4名の患者(9%)およびテリパラチド群の7名の患者(16%)で観察された(群の間で有意な差はなかった)。投与の4時間後に全ての患者で得られた最高値は、プラシーボ、アバロパラチド20μg、アバロパラチド40μg、アバロパラチド80μgおよびテリパラチド群のそれぞれで10.5、11.0、11.2、11.6および12.6mg/dLであった。投与の24時間後の全ての患者で得られた最高値は、プラシーボ、アバロパラチド20μg、アバロパラチド40μg、アバロパラチド80μgおよびテリパラチド群のそれぞれで10.7、11.3、11.1、10.7および11.2mg/dLであった。
24週後、アバロパラチドを受けた16名(12%)の患者は、陽性の低い(≦1:20)抗アバロパラチド抗体力価を示した。この群のAEの数および種類は、AE全体と同様であった。抗体陽性患者において免疫関連事象は報告されなかった。アバロパラチド40μg群の1名の抗体陽性患者は、24週でインビトロアバロパラチド中和活性の証拠を有したが、この患者における有効性減衰(24週でのすべての分析可能な脊椎BMDにおいて9.3%増加)または関連のある安全性事象の明らかな証拠はなかった。
延長集団のベースライン人口統計およびベースライン特性は、全試験コホートのものと同様であり、治療群当たりの被検体数は7〜14名の女性であった。48週で、腰椎BMDは、プラシーボ、アバロパラチド20、40および80μg群ならびにテリパラチド群のそれぞれで0.7%、5.1%、9.8%、12.9%および8.6%増加した。総股関節部BMDは、プラシーボ、アバロパラチド20、40および80μg群ならびにテリパラチド群のそれぞれで0.7%、1.9%、2.1%、2.7%および1.3%増加した。大腿骨頸部BMDは、プラシーボ、アバロパラチド20、40および80μg群ならびにテリパラチド群のそれぞれで1.0%、3.9%、1.8%、4.1%および2.2%増加した。延長試験に小さな数が与えられる場合、プラシーボと比較してアバロパラチド40μg、アバロパラチド80μgおよびテリパラチド群においてより大きく増加した脊椎BMD以外では、群の間で有意差は見られなかった。
この試験において、24週間のアバロパラチドにより腰椎、大腿骨頸部および総股関節部においてBMDが増加した。これらの増加の規模は、現在利用可能な療法と比較すると大きなものであった。腰椎において、試験用量でのアバロパラチドとBMDの増加の用量応答関係が示された。さらに、股関節部において、40μgおよび80μgの日用量のアバロパラチドにより、現在市販される20μgの日用量のテリパラチドよりも大きくBMDが増加した。さらに、80μg/日のアバロパラチドを受けている女性において、テリパラチド20μgを毎日受けている女性よりも大腿骨頸部および股関節部においてBMD消失は小さかった。最終的に、延長試験に登録される限定された集団中で観察されたBMD変化により、アバロパラチドにより増加したBMDは、治療の最初の一年間は比較的直線的であることが示唆される。
TBSにより間接的に評価されるように海綿骨微小構造に対するPTHrPアナログアバロパラチドの効果を評価するために、プラシーボ、アバロパラチド20μg、アバロパラチド40μg、アバロパラチド80μgまたはテリパラチド(TPTD)20μgの24週間の毎日皮下注射を受けることについて無作為化された222名の閉経後骨粗鬆症女性(55〜85歳)において0、12および24週に盲検様式で、TBS(TBS Calculator v2.2, Medimaps group, Plan-les-Ouates, Geneva, Switzerland)を遡及的に計算した。平均パーセントTBS変化における群間の差を非対称(unpaired)t検定で評価した。
221名の治療した女性中、77名の女性は、DXAスキャナがTBSソフトウェアと適合しなかったために評価できなかった。5治療群中の被験体(N=145)は、ベースラインBMD測定値および骨代謝回転の生化学的マーカーのレベルを含む人口統計的および臨床的特性に関して同様であった。12週後、TBSは、アバロパラチド20μg、アバロパラチド40μg、アバロパラチド80μgおよびTPTD群のそれぞれにおいて+1.2%、+1.7%、+1.9%および+1.5%有意に増加し、プラシーボ群(PBO)で-0.2%低下した。アバロパラチド40μgおよびアバロパラチド80μg治療群のTBSの12週平均パーセント増加はプラシーボ群よりも有意に高かった(両方p=0.05)。24週後、TBSは、アバロパラチド20μg、アバロパラチド40μg、アバロパラチド80μgおよびTPTD群において+2.4%、+2.7%、+3.6%および+2.6%増加し、プラシーボ群(PBO)で-1.1%低下した。TBSの24週間の増加は、プラシーボ群の変化と比較して全ての治療群で有意に高かった(p<0.005)。
アバロパラチドによる24週間の治療により、TBSにより間接的に評価されるように海綿骨微小構造が有意に向上した。BMD対するアバロパラチドの効果と合わせると、これらの結果は閉経後骨粗鬆症における同化作用療法としてのアバロパラチドのさらなる研究を支持する。
成体卵巣摘出(OVX)骨粗鬆症ラットへのPTHrPアナログアバロパラチドの6週間毎日投与の骨同化効果を評価した。OVX骨粗鬆症ラットにおける骨質量はアバロパラチド処理に応答して顕著な増加を受けた。骨質量の増加は、腰椎および大腿骨における海綿骨画分だけでなく、大腿骨の皮質骨(大腿骨骨幹)においても観察された。これらの用量依存的な骨質量の増加は、骨微小構造の向上および骨生化学的性質の増加と関連した。
動物
全ての手順、プロトコルおよび試験設計は、Radius Healthの施設内ケアおよび使用委員会(Institutional Care and Use Committee)(IACUC)で検討され、承認され、監督された。10週齢の雌Sprague-Dawleyラット(Charles River Laboratories)を、食餌および水に自由に接触できる通気したポリカーボネートケージ中で個別に飼育した。その環境は、18〜26℃、30〜70%相対湿度および12時間の明/暗周期で維持された。
12週齢でSprague-Dawleyラットに、偽手術(Sham)または卵巣摘出手術(OVX)のいずれかを施し、未処理のまま8週間放置した(骨消耗期間(bone depletion period))。骨粗鬆症OVXラット(n=20〜24/群)を、ビヒクル(0.9% NaCl)、アバロパラチド5μg/kgまたはアバロパラチド20μg/kgで6週間皮下注射(SC)により1日1回処理した。Shamラットはビヒクルで処理した(n=24)。試験設計の概略を表9に示す。
ラットをイソフルランで麻酔し、DXA (PIXImus, GE-Lunar Corporation, Fitchburg, WI)を使用して、第4腰椎(L4)および全大腿骨のインビボ骨密度(BMD)(1平方センチメートル当たりのグラム)を測定した。ベースラインおよび6週投与期間の終了時点でBMDを測定した。
定量的微細コンピューター連続断層撮影(mCT40μCTスキャナ、Scanco Medical AG, Basserdorf, Switzerland)をエクソビボで使用して、第4腰椎骨および遠位大腿骨骨幹端中の海綿骨形態、および中間大腿骨骨幹での皮質骨幾何学を評価した。
脊椎骨(L4)を圧縮試験により機械的にアッセイした。新鮮な凍結脊椎を室温まで融解し、次いで後部椎弓根、骨棘、ならびに頭蓋骨および仙骨末端を取り出し、2枚の平行な表面を有し高さが約4mmの脊椎体検体を得た。断面積の計算のために頭蓋骨および仙骨末端の中央-側方および前-後方向の幅を測定した。脊椎を2つの板(platens)の間に配置し、Instron Mechanical Testing Instrument (Instron 4465改装5500)における破壊まで6mm/分の一定変形(displacement)速度で荷重をかけた。装置ソフトウェア(Bluehill v2.5, Instron)により荷重変形曲線(load and displacement curve)を記録した。破壊時の最大荷重の位置、剛性および吸収されるエネルギーを荷重変形曲線から手動で選択し、装置ソフトウェア(Bluehill v2.5, Instron)により計算した。固有の性質、最終的な強度、弾性率および靱性を、最大荷重(N)、剛性(N/mm)、吸収されるエネルギー(mJ)、断面積および高さ(mm)から計算した。
結果は平均および標準偏差で表す。ANOVA、その後テューキー多重比較検定(Tukey’s multiple comparison test) (Graphpad Instat, Cary, NC; release 9.1)を使用して統計分析を行った。文脈中で行った全ての比較は、そうではないと記載されなければ統計的に有意である(p<0.05)。
骨密度
骨消耗期間の終了時点で、全大腿骨BMDは、Shamラットと比較してOVXにおいて有意に低下した(11%、Shamに対してp<0.001、データは示さず)。OVX処理対照ラットにおけるBMD値は、処理の6週後にインタクトなshamラットと比較して低下したままであった(14%低下、Shamに対してp<0.001)。
BMD測定値と一致して、OVXは、特に海綿骨画分において重大な骨の劣化と関連した(図16、表7および8)。Sham対照ラットと比較して、OVX-Vehラットは、脊椎海綿骨において36%低いBV/TVを有した(図16A、表8、Shamに対してp<0.001)。さらに、Tb.N、Tb.ThおよびBDは、Sham対照ラットと比較して、OVX-Vehラットの脊椎骨におけるより高いTb.Spと一緒になって、より低かった(表10)。
L4最大荷重および最終的な強度は、Sham対照ラットと比較して、OVX-Vehラットにおいて約28%低かった(p<0.01、表12)。L4の比較試験により、アバロパラチド5μg/kgおよびアバロパラチド20μg/kgで処理されたラットは、OVX-Veh処理ラット対照と比較して、最大荷重(170%および180%、OVX-Vehに対してそれぞれp<0.05および0.01、表12)、吸収されるエネルギー(280%および290%、p<0.001)、最終的な強度(170%および180%、p<0.001)および靱性(270%、両方の群、p<0.001)の有意に高い機械的試験値を有することが示された。さらに、Sham対照ラットに対してアバロパラチド20μg/kgで処理したOVXラットにおいてL4脊椎の最大荷重(126%、p<0.05)および靱性(170%、p<0.01)の有意な増加が見られた。
合成PTHrPアナログの例であるアバロパラチドの6週間の毎日投与の骨同化作用効果を、成体卵巣摘出骨粗鬆症ラットにおいて評価した。結果は、アバロパラチド処理は、骨質量の増加を促進し、骨微小構造を修復して、骨消失およびOVX誘導骨減少症に関連する骨機械的特性の悪化を逆転させたことを示した。アバロパラチドによる治療は、OVX-Vehラットと比較して、処理の6週間後に、アバロパラチドで処理したOVXラットにおけるBMD、海綿骨および皮質微小構造、ならびに大腿骨頸部および大腿骨骨幹強度値の増加により示されるように、骨質量を逆転させ、骨の質を修復した。さらに、アバロパラチドでの処理により、Sham-ビヒクル対照群以上の値が生じた。ラットOVX誘導骨粗鬆症モデルにおけるアバロパラチドでの処理後の著しい骨同化活性のこれらの観察は、骨粗鬆症を有する閉経後の女性におけるアバロパラチド治療の効果(例えば実施例1)において見られるBMD増加と一致する。

Claims (14)
- 骨折のリスクの低減を必要とする被験体においての該低減のための医薬の製造における配列番号:1のポリペプチドの使用であって、該医薬が、18ヶ月の第1の期間の間に該被験体に80μgの該ポリペプチドを投与し、その後第2の期間の間にアレンドロネートを投与するためのものである、使用。
- 該被験体が高い皮質骨多孔率を有する、請求項1記載の使用。
- 該被験体が、骨粗鬆症を有する、請求項1記載の使用。
- アレンドロネートが、経口投与される、請求項1記載の使用。
- アレンドロネートが、5もしくは10mg/日の用量または70mg/週の用量で投与される、請求項4記載の使用。
- 第2の期間が少なくとも6ヶ月である、請求項1記載の使用。
