Description
グルコース率増加検出器の設計:概要
ヘルスモニタリングシステム(HMS)のモジュールであるグルコース率増加検出器(GRID)を、食事事象を検出し食事ボーラスを安全に発射するためにグルコースコントローラーと並列で作動するように設計した。
HMSのモジュールのそれぞれは、APの特定の成分または或るタイプの有害事象もしくは攪乱を、干渉なく、途切れなくモニタリングするように設計される。最も一般的でリスクを伴う事象は低血糖である。したがって、コントローラーに対して並列状態で重篤な低血糖を予測し、予防するために、低グルコース予測装置(Low Glucose Predictor)(LGP)を設計した。LGPは、ゾーン−モデル予測制御(zone−Model Predictive Control)(ゾーン−MPC)コントローラーと組合されて臨床において有効であることが示されている22−24。
GRIDアルゴリズムは、グルコースの変化率(ROC)を推定し食事関連グルコース変動を検出するためにCGMデータを使用する。GRIDは以下の3つの主要小区分からなる:1)分析のためのCGMデータを準備するための前処理部、2)グルコースのROCを概算するための推定部、および3)食事事象を論理的に特定するための検出部。
GRIDアルゴリズムを評価するための基準として、標準的なカルマンフィルター(KF)を用いた。KFは、5分のサンプリングでの使用のために修飾されPalermら28により使用された最適推定アルゴリズムの変法であった。該検出論理が実装されたのは、それがGRID内に存在し、状態の数(グルコース値およびグルコースの変化率を含む2つの状態ならびにグルコース変化の加速度をも含む3つの状態)ならびに特異性および検出速度に関するQ対Rの比と共に調整されて、GRIDとは若干異なる調整をもたらした場合であった。
食事事象の知見は攪乱拒絶のために役立ち、推論制御の形態として用いられうる。GRIDを使用して、食事事象に関する該システムの状態を推定する。個別の食事事象がGRIDモジュールにより検出されたら、攪乱を拒絶するための一連の事象が始動される。図1に示されているとおり、本明細書においては2つのモードを検討する。使用者入力(User−Input)モードにおいては、該検出が、食事情報を要求する警告を発し、ついでこれを用いて完全または部分食事ボーラスを運搬し、自動モードにおいては、中等度の量の食事ボーラス、または低い正常グルコースレベルへの補正を計算し、自動的に運搬する。どちらのモードも、推奨ボーラスを調節するために最近履歴補正(Recent History Correction)(RHC)機能を有効にして作動しうる。RHCは以下の2つの機能を有する:1)最後の60分間にわたるインスリン運搬を計算し、基礎量を超える量を推奨ボーラスから差し引くこと、および2)過去の60分間における最低グルコース値に関する140mg/dLへの補正を計算し、それを推奨ボーラスに加えること。140mg/dLへの補正がネガティブ(負)となることもあり、これは、最近のグルコース値が目標範囲の下端にある場合には、推奨ボーラスを低減する。この動作は過剰運搬に対する追加的な安全策となる。これらの計算の全ては、インスリン対炭水化物の比および補正係数を含む、被験者(患者)の臨床パラメータに基づく。
GRIDおよびKFアルゴリズムを、臨床試験からのトレーニングデータを使用して調整し、臨床データの検証セットおよびインシリコデータセットに関して試験し、これらの全ては無告知の食事で行った。前記のとおり、重要性の順序で、低い検出時間、低い偽陽性率(高い特異性)および正として特定された高い食事数に関して、該アルゴリズムを調整した。全ての臨床試験からの研究の詳細は表1に示されており、更に詳細な結果は幾つかの参考文献に詳細に記載されている29−32。
トレーニングデータは、人工膵臓システム(Artificial Pancreas System)(APS(著作権))33を使用してSansum Diabetes Research Instituteにおいて行われた、LGPを伴うHMS、および80〜140mg/dLの目標ゾーン(範囲)を有するゾーン−MPCを使用する、T1DMを有する被験者での12個の完全閉ループ24時間臨床試験から構成されるものであった。被験者は少量ないし中等度の量の食事(40〜50gのCHO)が与えられ、30分間の適度の運動を行い、数名の被験者は運動前に16gのCHOの軽食が与えられ、数名は16gの救援用のCHO(HMSに従う)が与えられた。全ての被験者はDexcom(登録商標)SEVEN(登録商標)PLUS,(Dexcom(登録商標)San Diego,CA)CGMを5分のサンプリング周期で使用し、皮下インスリン運搬を受けた。
調整パラメータのセットを更に比較するために、10名の成人被験者からなる米国食品医薬品局(Food and Drug Administration)(FDA)により承認されたUVA/パドバ(Padova)代謝シミュレーターを使用して、インシリコ試験を行った。シミュレーションを午前3:00に開始し、インスリン・オン・ボード(Insulin−on−board)(IOB)制限を伴うゾーン−MPCを使用する閉ループ制御を午前5:00に開始した。ゾーン−MPC目標グルコース範囲は午前7:00から午後10:00までは80〜140mg/dL、午前零時から午前5:00までは110〜170mg/dLであり、それらの間に円滑な移行期を設けた24。50、75および100gの食事をそれぞれ午前7:00、午後1:00および午後6:00に与え、制御を翌日の午前3:00まで継続した。データを、1分のサンプリング時間を用いて集め、5分へのダウンサンプリングの後、GRIDおよびKFアルゴリズムを使用して試験した。
食事攪乱の自動拒絶の成功は検出速度に大きく左右される。検出が遅すぎると、それは何の役にも立たないか、あるいはコントローラー補正を超えて過剰のインスリンが運搬されると、低血糖を起こすことさえある。該シミュレーターは1分のサンプリング周期を与え、したがって、検出速度、検出時の上昇、および検出される食事の比率に対する、より速いサンプリング速度の利点の分析を行った。
能動的(アクティブ)に作動し食事ボーラスを発射するGRIDを使用した場合の幾つかのインシリコシナリオを実施して、該アルゴリズムを試験した。全てのシナリオは5分のサンプリング周期を用いた。
標準的な基礎ボーラス療法を受けている被験者に関しては、食事ボーラスが特に青年期の又は忙しい成人によって取り損なわれることがある35。グルコースピークを鈍化させ、高血糖の時間を減少させるためにタイミングよく、取り損なわれたボーラスを該アルゴリズムがCGM使用者に知らせる能力を評価するために、標準的な基礎ボーラス療法中の取り損なわれた食事ボーラスをシミュレーションした。表2に示されているように、4.5時間の時点における50、75または100gのCHO食での18時間のシナリオを幾つかのプロトコルでシミュレーションした。使用者の応答を待つことによる遅延をシミュレーションするために、使用者入力ボーラスを検出後のサイクルで運搬する。
前記のとおり、食事攪乱を検出し、該攪乱を拒絶するためのインスリンボーラスを計算し、この情報をゾーン−MPCコントローラーに供給することによる推論制御の形態として、GRIDを制御スキーム内に組込んだ。HMSのLGPモジュールも、65mg/dLの予測閾値および100mg/dLの始動閾値で作動した22,23,36,37。50、75および100gのCHOの3食の24時間のシナリオを、HMSに従うCHOにおける前記のものと同様に実施した。
より速いサンプリングのコスト(費用)は高価なセンサーならびに該センサー、受信機およびコントローラーによるエネルギー消費の増大の形態で認識可能であり、これらはより短い寿命および貨幣原価の増大につながりうるであろう。グルコースサンプリング周期が増加するにつれて、食事の検出は悪化すると予想され、したがって、より速いサンプリング周期は、食事検出を用いる推論制御を有するコントローラーの性能を改善しうるであろう。図4に示されているとおり、このシステムの費用効果分析は、1〜30分のサンプリング時間を試験することにより行われた。50gを超えるCHOの食事に関しては、1分のサンプリングから5分のサンプリングへと増加させると、検出までの時間における5分の増加および検出時のグルコースにおける15mg/dLの増加が生じ、一方、全ての食事が尚も検出された。より少量の食事に関するメトリックスは、グルコース変動がそれほど顕著ではないため、より大きな影響を受けた。少量の食事は一般に、食事検出からの追加的なインスリンの使用を伴うことなく対処されうる。この結果は、5分のサンプリング周期が中等度ないし大量の食事の食事検出に十分であることを示しているが、信頼性のある1分のサンプリングが容易かつ安価に利用可能である場合には、食事検出が改善されうるであろう。
GRIDは食事の開始から約40〜45分でポジティブ(正)な食事検出を示し、無告知の食事と比較した場合、食事ピークおよび高血糖の持続時間の両方を低減した。GRID−アクティブ・プロトコル中のボーラスにおける遅延の結果は、欠如食事プロトコル(B)と比較して大きな改善である。
図8における範囲時間でゾーン−MPCプロトコルの詳細な結果が決定された。GRIDは食事の開始から約40〜45分のポジティブ(正)な食事検出を示し、前記のとおりの計算されたボーラスを運搬した。自動モードボーラスプロトコル(E)の場合には、食事ピークおよび80〜180の範囲時間は無告知の場合(B)より有意に良好であった。全ての食事に関して、80〜180の範囲の時間は、無告知プロトコル(B)と比較して、自動モードボーラスプロトコル(E)および使用者入力モードプロトコル(D)の両方により改善された。LGPを伴うHMSに従い5回までの低血糖治療が行われたが、10名中7名の被験者は低血糖(<70mg/dL)を示さず、治療回数および70mg/dL未満の時間は、告知の食事と比較された場合の該プロトコルのいずれに関しても、有意には高くなかった。閉ループ制御の場合、RHCを伴う75gのCHO食のための完全ボーラスが検出時に推奨され(E)、無告知プロトコル(B)より有意に良好な範囲時間および高血糖範囲における遥かに短い時間が達成される。詳細な結果を表4に示す。
食事グルコース変動を正確かつ迅速に特定し、食事攪乱の拒絶のためにインスリンボーラスを論理的に推奨するために、HMSのGRIDモジュールを設計した。シミュレーションにおいて使用された同じコントローラーならびに無告知の食事および幾つかの軽食でのノイズ含有臨床試験データを使用して、アルゴリズムを調整した。検出速度の調整が最優先事項であったが、1日当たり2.0個を超える偽陽性検出を示したアルゴリズムは除外されたことに注目すべきである。それらのアルゴリズムが含まれていたとしても、KFまたはGRIDに関する最速検出時間は35分であったであろう。したがって、制御されたデータおよび中等度の量の食事では、CGMデータに基づく食事検出に関する30+分の遅延が検出速度の限界である。

Claims (6)
- 人工膵臓(AP)又は他の連続的グルコースモニタリング(CGM)における使用のた
めのグルコース率増加検出器(GRID)であって、
グルコース率増加検出器が、食事に関連したグルコースの持続的増加をヒトにおいて検
出し、そして閉ループ制御中には食事ピークを安全に鈍化させるために食事ボーラスを発
射し、または開ループ制御中には食事のためのボーラスに関して患者に警告し、ここで、
該検出器は、(a)該検出が、食事情報を要求する警告を発し、ついでこれを用いて完全
または部分食事ボーラスを運搬する使用者入力モード、及び(b)中等度の量の食事ボー
ラス、または低い正常グルコースレベルへの補正を計算し、自動的に運搬する自動モード
の2つのモードで作動するように設計されており、該検出器はグルコースの変化率を推定
し、且つ、食事関連グルコース変動を検出するための連続的グルコースモニタリングデー
タを使用するグルコース率増加検出器アルゴリズムを含み、
該アルゴリズムが、
a)ノイズ−スパイクフィルターを使用して連続的グルコースモニタリングデータをフ
ィルターにかけ、グルコースの変化率を推定するための前処理部、
b)フィルタリングされた連続的グルコースモニタリングデータを用いてグルコースの
変化率を計算するための推定部、および
c)計算されたグルコースの変化率を使用して食事事象を論理的に特定するための検出
部
を含み、ここで、該検出器は、食事に関連した血中グルコース濃度の持続的増加を使用者
において検出するように設計されており、該検出器は閉ループ制御中には食事ピークを安
全に鈍化させるために食事ボーラスを発射するか又は手動制御へ移行し、あるいは開ルー
プ制御中には食事のためのボーラスに関して使用者に警告することを特徴とする、前記グ
ルコース率増加検出器(GRID)。 - 請求項1に記載の検出器であって、前処理部が、ノイズ−スパイクフィルターを使用し
て該データをフィルターにかける:
