Description
本発明は、吐き出された二酸化炭素(carbon dioxide:CO2)濃度を監視してカプノグラムを表示するための比色カプノグラフを組み込んだ、改良された手動蘇生器に関する。より特定的には、本発明は、小型で安価で使い勝手が良く、心肺蘇生(Cardio Pulmonary Resuscitation:CPR)の際に特に有利なアセンブリに関する。
赤外線(infrared:IR)吸収は吐き出された二酸化炭素(CO2)濃度を測定するための最先端の方法であり、この技術(IRカプノグラフ)に基づく器具は30年以上臨床使用されてきた。それらは、手術室内および術後ケアにおいて監視のために日常的に使用される。吐き出されたCO2濃度カーブ(いわゆるカプノグラム)も、人工呼吸器治療とともに、かつ、たとえば肺機能のより高度な診断に使用されてきた。
上述された目的は、独立請求項に係る本発明によって達成される。
まず、図1を参照すると、典型的な自動膨張式蘇生器の特徴および動作が示されている。装置の主要な構成要素は、自動膨張式のラバーバッグ1と、患者への接続を提供する呼吸回路ブロック2とである。バッグが握られたとき(3)(内側に向かう矢印で示される)、ガスが患者に送られて(吸気)、バッグ内で陽圧が生じる。これによって、いわゆるフィッシュマウス(fishmouth)一方弁4が開き、たとえばマッシュルーム(mushroom)弁5で呼気ポートが閉じられて、ガス6がバッグから患者7に向けられる。バッグが解放されて(3)、自己拡張(3)(外側に向かう矢印で示される)によってバッグ内で陰圧が生じたとき、呼気が始まる。ここで、フィッシュマウス弁4が閉じてマッシュルーム弁5が開き、呼気ガスが蘇生器のポート8から外に出るのを許容する。ポート8では、吸気時、一方向弁9がガス流入を妨げる。バッグは、バッグ底部の一方向弁10を通じて周囲(付加された酸素有りまたは無し)からガスを吸い込むことによって、最終的には再び満たされる。
Claims (10)
- 換気を施されている人の呼吸ガス(6)中の二酸化炭素(carbon dioxide:CO2)濃度の測定を可能にするように構成された手動蘇生器アセンブリであって、
− CO2に晒されたときに、前記CO2濃度に応じて迅速かつ可逆的に色を変化させるように適合された検出器表面を備えた選択的比色CO2検出器(15)と、
− 前記比色検出器を受入れて取付けるように適合された検出器保持部と、
− 自動膨張式のラバーバッグ(1)と、患者への接続を提供するための呼吸回路ブロック(2)と、二方向ガスコンジット(12)とをさらに含む手動蘇生器とを備え、前記ガスコンジット(12)は、前記ガスコンジット(12)内のシャント流を提供するための前記蘇生器の前記呼吸回路ブロック(2)の呼気部(33)と吸気部(34)との間にあり、前記蘇生器の前記呼気部と前記吸気部との間の前記ガスコンジットは、前記呼気部および前記吸気部の壁における開口と、前記開口を前記検出器保持部に接続する剛性のガスコンジットとによって実現され、前記ガスコンジットは、換気中、前記呼吸ガスのうち所定の割合が前記検出器保持部に流入して前記検出器表面に接触するように構成されており、前記アセンブリはさらに、

- 前記携帯型ユニットはスマートフォンである、請求項1〜4のいずれか1項に記載のアセンブリ。
- 前記画像取込手段はカメラユニットである、請求項1〜5のいずれか1項に記載のアセンブリ。
- 前記携帯型ユニットは照明手段を含み、前記照明手段のうち少なくとも1つは、測定中に前記検出器表面を照らすように適合されており、前記照明の色は、CO2が無い状態での前記検出器表面の光学的特徴と関連して選択される、請求項1〜6のいずれか1項に記載のアセンブリ。
- 前記処理手段は、カプノグラムを算出して前記ディスプレイに表示するように適合されており、前記処理手段は、さらに、呼気終末CO2値および呼吸数を算出して前記ディスプレイに表示するように適合されている、請求項1〜7のいずれか1項に記載のアセンブリ。
- 前記処理手段は、前記検出器表面の少なくとも1つの特徴的な光学的特性を記憶するように適合されており、前記処理手段は、前記検出器表面の前記特徴的な光学的特性と、対応するCO2濃度との間の関係を少なくとも1つ記憶するように適合されている、請求項1〜8のいずれか1項に記載のアセンブリ。
- 前記携帯型ユニットは、測定開始時に、前記検出器の色をCO2が無い状態での基準と比較してずれを補償する、請求項1〜9のいずれか1項に記載のアセンブリ。
