Description
本発明者らは、数ある中でも特に、解決されるべき問題は完全皮下型除細動器の植込みに多数の切開場所が含まれることである、ということを認識してきた。侵襲性が最小限である技法及びツールセットが本明細書中に開示される。
上記に説明されたように、図1は例示的な完全皮下型の植込み式心臓刺激システムを植込まれた状態で示す。図2は、図1のようなデバイスを植込むための手順のある態様を例示する。この例示において、植込みのための手順は、位置30、32及び34における3つの切開部を使用することを必要とする。感染のリスクを低減するために、おおよそエリア36を含む滅菌野が準備されるかもしれない。これは主に、2012年の市販承認の時点でキャメロン・ヘルス(Cameron Health)及びボストン・サイエンティフィック(Boston Scientific)のS‐ICD(登録商標)システムについて米国で承認されたラベルに記載された植込みの方法をたどっている。
これらの非限定的な例はそれぞれ、単独のものであってもよいし、1つ以上の他の例と様々な順列又は組み合わせで組み合わされてもよい。
本文書において、用語「1つの」は、特許文献において一般的であるように、何らかの他の事例又は「少なくとも1つ」若しくは「1つ以上」の使用法とは無関係に、1つよりもむしろ1つ以上を含むように使用される。さらに、以降の特許請求の範囲において、用語「第1の」、「第2の」、及び「第3の」などは単に標識として使用されており、該用語の対象に数字上の要件を課するようには意図されていない。
Claims (15)
- 心臓治療装置で使用される皮下リード線の植込みのための導入器ツールキットであって、
第1の線に沿った第1の皮下トンネルの作出に使用されるポリマーチューブを備える第1の挿入ツールであって、患者の体内に挿入するための遠位端を有し、前記遠位端又はその近傍に開口部を備えた第1の挿入ツールと、
第1の皮下トンネルの作出の間、第1の挿入ツールを通して配置するための第1の支持部材と、
第1の挿入ツール内を通過して前記開口部から出ることにより第1の皮下トンネルに対して角度をなした第2の皮下トンネルの作出に使用される第2の挿入ツールであって、第1の挿入ツールの前記開口部の位置で湾曲するように構成されている第2の挿入ツールとを含む、導入器ツールキット。 - 第1の挿入ツール内の通過及び第2の皮下トンネルの作出の際に第2の挿入ツール上に配置するためのシースをさらに含む、請求項1に記載の導入器ツールキット。
- シースは、皮下リード線がシースを通って第2の皮下トンネルの中に導入されうるように、第2の挿入ツールの除去時及び除去後にその場に留まるようになされた分割式シースである、請求項2に記載の導入器ツールキット。
- 第2の挿入ツールは第2の皮下トンネルを設けるための駆動式要素を備えた遠位端を有する、請求項1〜3のいずれか1項に記載の導入器ツールキット。
- 第2の挿入ツールは第2の皮下トンネルの作出を支援するために超音波エネルギーを使用するための超音波振動子を備えた遠位端を有する、請求項1〜3のいずれか1項に記載の導入器ツールキット。
- 心臓治療装置で使用される皮下リード線の植込みのための導入器ツールセットであって、
内側トンネル作製ロッドであって、近位端、第1の位置における湾曲、及び皮下トンネルを鈍的に切り開くように適合された遠位端を有し、第1の位置は近位端よりも遠位端に近い、内側トンネル作製ロッドと、
導入器シースであって、近位端、遠位端、及び少なくとも第1の相対配置と第2の相対配置とを有する摺動可能な方式で内側トンネル作製ロッドを受け入れる大きさの内側ルーメンを有する導入器シースと
を含み、
第1の相対配置にあるとき、導入器シースは概ね直線状の組立体を維持するように内側トンネル作製ロッドの湾曲を収容し、
第2の相対配置にあるとき、内側トンネル作製ロッドが導入器シースの遠位端よりも遠位側に、導入器シースの軸に対して角度をなして伸びる、導入器ツールセット。 - 内側トンネル作製ロッドは、第1の線に沿った第1の部分と、第1の線に対して少なくとも60度の角度をなす第2の線に沿った第2の部分とを有する皮下トンネルの作出を容易にするために、少なくとも60度の湾曲を有する、請求項6に記載の導入器ツールセット。
- 第2の相対配置は、内側トンネル作製ロッドが導入器シースの遠位端を越えて5〜15cm(2〜6インチ)の範囲で伸びるような配置である、請求項6又は7に記載の導入器ツールセット。
- 導入器シースは超音波振動子を有する動力付きシースである、請求項6〜8のいずれか1項に記載の導入器ツールセット。
- 内側トンネル作製ロッドは、皮下組織を通って切り開くのを支援するための展開可能な切断要素を備えている、請求項6〜9のいずれか1項に記載の導入器ツールセット。
- 内側トンネル作製ロッドは超音波振動子を有する動力付きツールである、請求項6〜10のいずれか1項に記載の導入器ツールセット。
- 内側トンネル作製ロッド又は導入器シースのうち少なくとも一方を通り抜けるためのガイドワイヤであって、導入器シース及び内側トンネル作製ロッドの除去の際に皮下トンネルの中に残されるように構成されたガイドワイヤと、
導入器シース及び内側トンネル作製ロッドで作られた皮下トンネルを拡張するためにガイドワイヤ上を通過するための拡張器と
をさらに含む、請求項6〜11のいずれか1項に記載の導入器ツールセット。 - ガイドワイヤ上を通過する際に拡張器上に配置するために構成された分割式シースであって、それを通した皮下リード線の導入を容易にするためにガイドワイヤ及び拡張器の除去の際にその場に留まるように構成された分割式シースをさらに含み、分割式シースは、皮下リード線が配置されると分割式シースが分割して皮下リード線上から除去可能であるように構成されている、請求項12に記載の導入器ツールセット。
- 内側トンネル作製ロッドは選択的に湾曲を与える操舵機構を有する可動型ロッドである、請求項6〜13のいずれか1項に記載の導入器ツールセット。
- 内側トンネル作製ロッド又は導入器シースのうち少なくとも一方はその挿入の際にかかる力を同定するように構成された力覚センサを含む、請求項6〜14のいずれか1項に記載の導入器ツールセット。
