Description
本願は、米国特許法第119条(e)及び特許協力条約第8条の下、2013年7月3日に出願された同時係属の米国特許出願第61/842,789号の優先権を主張する。また、本願は、2008年6月20日に出願された国際出願PCT/US2008/067725「Method,System and Computer Simulation Environment for Testing of Monitoring and Control Strategies in Diabetes」、2009年12月14日に出願された米国特許出願第12/664,444号「Method,System and Computer Simulation Environment for Testing of Monitoring and Control Strategies in Diabetes」、及び2012年2月10日に出願された米国特許出願第13/380,839号「System,Method and Computer Simulation Environment for In Silico Trials in Pre−Diabetes and Type 2 Diabetes」に関し、それぞれの開示内容をすべて本明細書に援用する。
モデル
食事中のグルコース−インスリン抑制系を記述したT1DMシミュレータに組み込まれているモデルについては、[5〜7]に詳しく記載されている。簡単に説明すると、このモデルでは、グルコースフラックスすなわち内因性グルコース産生(EGP)、グルコース出現速度(Ra)、組織によるグルコース利用(U)、腎排泄(E)、並びにインスリンフラックスすなわち皮下組織(SC)からのインスリン出現及びインスリン分解(D)の速度に対して、血漿濃度すなわちグルコースG及びインスリンIを関連付けている。グルコース2次系は、グルコース動態の2画分モデルから成り、インスリン非依存性利用が第1の画分で生じて血漿及び急速に平衡化する組織を表す一方、インスリン依存性利用が第2の画分で生じて、ゆっくりと平衡化する抹消組織を表す。また、インスリン2次系も2画分で記述され、それぞれ肝臓及び血漿を表す。皮下インスリン動態は、皮下インスリン注入モジュールにより表される。内因性グルコース産生は、血漿グルコース濃度、門脈インスリン濃度、及び遅延インスリン信号に線形依存するもの考えられる。グルコース出現速度は、グルコースの胃腸通過モデルで記述され、胃が2画分で表される一方、消化管の記述には1画分が用いられる。胃内容排出の速度定数は、胃中のグルコース量の非線形関数である。食事中のグルコース利用は、2画分で構成されている。脳及び赤血球によるインスリン非依存性利用は、第1の画分で生じ、一定である。インスリン依存性利用は、遠隔の画分で生じ、組織中のグルコースに対して非線形的に依存している。
1型糖尿病シミュレータには、100人の仮想成人、100人の若年者、及び100人の小児を含む。これら1型糖尿病仮想被験者の母集団は、適当なパラメータ同時分布からパラメータベクトルの異なる具現化をランダムに抽出することによって生成されたものである。T1DMにおける初期(2007年)のパラメータ同時分布は、成人の健康状態における利用可能な集合から(適当な調整の後に)導出されたものである。特に、被験者間のばらつきは、同じ(同じ共分散行列)と考えられていたが、臨床的に関連する特定の修正が平均パラメータベクトルに導入されており、例えば、基礎的な内因性グルコース産生は、健常者よりも1型糖尿病被験者において高い。同様に、小児及び若年者等、異なる1型糖尿病の母集団におけるパラメータ分布は、臨床的に関連する特定の修正を平均パラメータベクトルに導入することによって、1型糖尿病の成人の分布から得られたものであり、例えば、インスリン感受性は、成人よりも小児が高く、若年者が低い[10]。
血漿へのグルカゴン皮下輸送と、
グルカゴン内因性分泌と、
グルコース血漿平衡に対するグルカゴン作用と、
低血糖におけるグルコース摂取と、
インシリコ患者の糖尿病の発症年齢と、
を提供する。
ただし、Gt(t)は組織中のグルコース量、X(t)はグルコース利用に対するインスリン作用、[Vm0、Vmx、Km0、p2U]はモデルパラメータである。モデルパラメータ及び方程式の全リストについては、付録A1及びA2に報告されている。
A)モデル
低血糖危険因子としての特定の閾値を下回ってグルコースが減る場合は、インスリン依存性利用Uid(t)が増大するものと仮定されている[12]。
ただし、Gt(t)、X(t)、及び[Vm0、Vmx、Km0]は、上記に同じであり、
であって、Gbが患者の基礎グルコース、Gthが低血糖閾値(60mg/dlに設定)であり、
であって、[r1、r2]がランダムに生成される付加的なモデルパラメータである。
このモデルは最初に、13人の健常者(女性8人、男性5人、19〜47歳の年齢幅、それぞれの理想体重の10%以内の体重)のデータに対して評価済みである。それぞれにおいては、同位体的に決定されたグルコース出現速度(内因性グルコース産生EGP)及び血中グルカゴン濃度と併せて、インスリン誘発性低血糖(静脈注射によるインスリン0.04U/kgの投与により発生)からの血漿グルコース回復の決定がなされた[13、14]。データに対するモデル予測を図2に示す。
ただし、G(t)は血漿グルコース濃度、I(t)は血漿インスリン濃度、GIR(t)は外因性グルコース注入速度、SG、SI、p2、V、r1、及びr2はモデルパラメータである。

では、グルコースデータが低く見積もられるものと考えられる。低血糖の急速な下降を含めて、モデルはデータによく適合しており(図3)、パラメータの正確な推定値を与えている[15]。
A)モデル
グルカゴン分泌及び動態のモデルは、以下の方程式により表される。
ただし、Gn(t)は血漿グルカゴン濃度、GSR(t)はグルカゴン分泌、GSRbは基礎値、k0Gnはグルカゴン除去速度、G(t)は血漿グルコース濃度、I(t)は血漿インスリン濃度、Gthはグルコース閾値、Ithは閾値であり、その閾値の超過により血漿インスリンがグルカゴン分泌を阻止する閾値(通例、基礎レベルに近い)、dG(t)/dtはグルコース変化速度、kGSRsはグルコースレベルに対するアルファ細胞応答性、1/αgは静的グルカゴン分泌と血漿グルコースとの間の遅延、kGSRdはグルコース変化速度に対するアルファ細胞応答性である。
このモデルは、インスリン誘発性低血糖となっている13人の健常者のデータベースを用いたデータに対して評価済みである[13]。データに対するモデル予測を図4に示す。
A)モデル
内因性グルコース産生に対するグルカゴン作用のモデルは、以下の方程式により表される。
ただし、EGP(t)は内因性グルコース産生、Gp(t)は血漿画分中のグルコース量、XL(t)は肝臓中の遅延インスリン作用、XGn(t)はEGPに対する遅延グルカゴン作用、Gn(t)は血漿グルカゴン濃度、ki、kp1、kp2、kp3、kcounter、及びkXGnはモデルパラメータである。
このモデルは、上記と同じデータ集合(13人の健常者[14])を用いたデータに対して評価済みである。データに対するモデル予測を図5に示す。
危険関数をインスリン依存性利用に包含することによって、基礎グルコース(Gb)の選定が潜在的に重要となる。実際、インスリン感受性は、グルコースが基礎値を下回り次第、危険関数に追従して増大し始める。従来公表されているシミュレータにおいては、平均140mg/dlにて同時分布からランダムにGbが生成されていた。ただし、従来の実施態様のGbは、空腹時グルコースの既存概念に対応する健常、前糖尿病、及びT2DMにおいて導出されたものである。したがって、T1DMにおいては、Gbが患者のグルコース目標に対応していなければならない。これは、食事及び身体活動等の外部摂動なく、最適な基礎インスリン注入で達成されるグルコース値だからである。本発明者らの最近の1型糖尿病患者の臨床治験において[1〜3]、グルコース目標は、平均120mg/dl前後であった。したがって、パラメータ同時分布も相応に変化していた。
閉ループ系におけるグルカゴンの使用に対する関心が高まるにつれ[例えば、24]、前臨床段階の抑制設計及び試験の両者に関して、シミュレーションプラットフォームの使用が重要となっている。このような機能には、血漿中での出現を再現し、上述のモデルによって作用が抑制される外因性グルカゴンの薬物動態モデルを確立する必要がある。
ただし、x3は血漿グルカゴン濃度、x1及びx2はともに皮下画分であり、k5及びk6は皮下組織中のグルカゴンの分解に対応し、k1〜k4は皮下組織中の非線形輸送を表し(投与量が少ないほど時定数が小さい)、k7は血漿除去に対応する。
この評価に用いられる臨床データは、Dr.El−Khatibらが開発した2ホルモン閉ループ正常血糖抑制系に由来する[24]。彼らの実験設計においては、インスリンを産生できていない合計11人のT1DM成人が存在していた。構成層データは、年齢が40±16歳、体重が83±13kg、BMIが28±3kg/m2、糖尿病罹患期間が23±13年、HbA1cが7.3±0.8%であった。これらの被験者について、27時間にわたり、糖質に富んだ食事を規則的に3回提供して、詳しく調査が行われた。一部の被験者は、調査に2回参加したが、少なくとも5ヶ月は空いていた。13個のデータ集合を分析に利用可能である(一部の被験者は反復)。血漿インスリン及びグルカゴンが頻繁に測定されるとともに、各ホルモンの注射が記録された。血漿グルカゴンにおいては、モデル予測と収集データとが最小の逸脱であった。
新たなパラメータr1、r3は、[15]により導出された同時分布からランダムに生成されたものである(r2は、r1及びr3により導出されたものである)。一方、対数正規分布を用いて生成されたSQグルカゴン輸送モデルのパラメータは、以下のようなパラメータであった。
グルカゴン分泌パラメータの生成は、グルカゴン分泌が糖尿病罹患期間によって決まることが示されていることから、より複雑であった[25〜27]。[25]に提示されているデータの再現については、図8Aを参照可能である。
実際の患者において、CR(糖質比)及びCF(補正係数)は、患者の病歴、習慣、及び医師の経験によって経験的に決まる。最初に公表されたT1DMシミュレータでは、計算上最適なCR及びCFの何らかの規定が使用されていたが、これとは異なり、本発明者らは、実際の患者における経験的な決定に用いられた基準を可能な限り模倣している以下の規定を使用した[17〜20]。
CFは、いわゆる1700ルールにより決定された[20]。すなわち、
ただし、TDIは、1日の総インスリンであり、最適なCR及び基礎注入速度を用いて仮想患者ごとに決定され、平均的な食事では180gのCHOを仮定している。

上記基準を用いることにより、100人のインシリコ成人が生成された。300人のインシリコ被験者のモデルパラメータについては、付録A2に報告されている。実CR及びインシリコCRの分布を図11に示す一方、インシリコCFの分布を図12に示す。
試験1
データベース
T1DMシミュレータの臨床的妥当性の評価に用いられるデータベースは、24人のT1DM成人被験者から成り[1〜3]、バージニア大学(N=11)、Padua、Italy(N=7)、及びMontpelier、France(N=6)で採用された。各患者は、それぞれ開ループ及び閉ループの2回、22時間(午後3時から翌日の午後1時まで)にわたって入院した。簡単に説明すると、いずれの入院においても、被験者は、午後6時〜7時に夕食(70.7±3.3gのCHO)を摂るとともに午前7時〜8時に朝食(52.9±0.1gのCHO)を摂り、それぞれの血漿グルコースが頻繁に測定された(詳細については、[3]を参照)。各グルコーストレースは、夕食後(夕食の摂取から5時間後まで)、夜間(夕食後5時間から朝食の開始まで)、及び朝食後(朝食の摂取から5時間後まで)に細分化された。モデル評価のため、夕食後及び朝食後トレースを使用しており、合計96個の食後トレースであった。
臨床的観点からシミュレータが妥当であることを証明するには、実際の各1型糖尿病被験者について、同じ実験シナリオを経験した場合に挙動の臨床的観点から実際の被験者と同様に挙動する仮想被験者が存在している必要がある。すなわち、実際のグルコースプロファイルに対して必ずしも完全には適合しないものの、類似のパターンを示して、臨床的に関連する同じ領域(低血糖、正常血糖、高血糖)に存在している必要がある。シミュレーショントレースにおける最も重要な効果尺度の分布は、実際の実験で観察された分布を再現している。
図13は、2つの例示的な場合における上記低血糖修正がない場合(左枠)及び低血糖修正がある場合(右枠)の両者のFDA承認モデルの血漿グルコース測定データとシミュレーションプロファイルとの比較を示している。注目すべきは、元のFDAシミュレータで得られたシミュレーションプロファイルが正常血糖及び高血糖の実際のグルコースパターンを忠実に再現しているのに、グルコースの低下が十分に急速ではないため、低血糖の出現が十分に予測されないことである。一方、低血糖におけるグルコース利用の新たな表現及び本発明の逆調節を導入することによって、グルコースの下降がより急速に起こっており、シミュレータにより予測された低血糖の出現には、臨床治験中の観察結果が反映されている。
表1は、FDA承認シミュレータ及び新たなシミュレータで得られた実際の実験及びシミュレーション実験のCG−EGAを示している。
表2は、実際の実験及びシミュレーション実験の効果尺度の比較を示している。値は、正常分布の変数の場合は平均±SD(一対のT検定によるp値)、非正常分布の変数の場合は中央値[四分位範囲](ウィルコクソンの符号順位検定によるp値)である。
データベース
T1DMシミュレータの臨床的妥当性の評価に用いられるデータベースは、9人のT1DM成人被験者から成り[33]、バージニア大学で採用された。各患者は、それぞれ開ループ及び閉ループの2回、22時間(午前11時から翌日の午前9時まで)にわたって入院した。簡単に説明すると、いずれの入院においても、被験者は、午前11時に昼食、午後6時〜7時に夕食、午前7時〜8時に朝食を摂り、午後4時に運動した。また、血漿グルコースは、頻繁に測定された(詳細については、[33]を参照)。この分析では、運動の効果がシミュレーションプラットフォームに未実装であることから、昼食のみが用いられた(午前11時〜午後4時)。
臨床的観点からシミュレータが妥当であることを証明するには、実際の各1型糖尿病被験者について、同じ実験シナリオを経験した場合に挙動の臨床的観点から実際の被験者と同様に挙動する仮想被験者が存在している必要がある。すなわち、実際のグルコースプロファイルに対して必ずしも完全には適合しないものの、類似のパターンを示して、臨床的に関連する同じ領域(低血糖、正常血糖、高血糖)に存在している必要がある。
表3に示すように、18回の入院のうち、シミュレーションと生体内データとの全体的な一致が90%未満のものは、2つだけであった。いずれの場合も、低血糖事象がなかったためであるが、それ以外の一致は、低血糖で93%、正常血糖で99.6%、高血糖で100%であり、新たなシミュレーションプラットフォームの非常に堅牢な性能が実証されている。

Claims (16)
- T1DM被験者のグルコース−インスリン代謝系をシミュレートする電子システムであって、
(1)T1DM被験者の動的なグルコース濃度のモデルを実装するサブシステムであり、
(a)内因性グルコース産生(EGP(t))に対するグルカゴン作用のモデルを実装する電子モジュールと、
(b)食事のグルコース出現速度(Ra(t))をモデル化する電子モジュールと、
(c)方程式
に従ってインスリン依存性グルコース利用(Uid(t))をモデル化する電子モジュールで、Gt(t)が組織中のグルコース量、X(t)がグルコース利用に対するインスリン作用、[Vm0、Vmx、Km0、p2U]がモデルパラメータであり、
であって、Gbが患者の基礎グルコース、Gthが低血糖閾値(60mg/dlに設定)であり、
であって、[r1、r2]がランダムに生成された付加的なモデルパラメータである、電子モジュールと、
(d)グルコースの腎排泄(E(t))をモデル化する電子モジュールと、
を具備した、サブシステムと、
(2)前記T1DM被験者の動的なインスリン濃度をモデル化するサブシステムであり、
(a)インスリン分泌(S(t))をモデル化する電子モジュールと、
(b)グルカゴン皮下輸送のモデルを実装する電子モジュールと、
を具備した、サブシステムと、
(3)T1DM仮想被験者の母集団を含む電子データベースであり、各仮想被験者が、生体内T1DM被験者から導出された値の範囲内の値を有する複数の代謝パラメータを有する、電子データベースと、
(4)少なくとも1つの変化のある代謝パラメータ値を含む前記複数の代謝パラメータ値を前記グルコース濃度サブシステム及び前記インスリン濃度サブシステムに入力することによって、仮想被験者の前記グルコース−インスリン代謝系に対する前記少なくとも1つの代謝パラメータ値の変化の影響を計算する処理モジュールと、
を備えた、電子システム。 - 以下の方程式
に従ってグルカゴン分泌及び動態をモデル化する電子モジュールであり、Gn(t)が血漿グルカゴン濃度、GSR(t)がグルカゴン分泌、GSRbが基礎値、k0Gnがグルカゴン除去速度、G(t)が血漿グルコース濃度、I(t)が血漿インスリン濃度、Gthがグルコース閾値、Ithが閾値であり、該閾値の超過により血漿インスリンがグルカゴン分泌を阻止する閾値(通例、基礎レベルに近い)、dG(t)/dtがグルコース変化速度、kGSRsがグルコースレベルに対するアルファ細胞応答性、1/αgが静的グルカゴン分泌と血漿グルコースとの間の遅延、kGSRdがグルコース変化速度に対するアルファ細胞応答性である、電子モジュールをさらに備えた、請求項2に記載の電子システム。 - 前記電子データベースにおける仮想被験者の前記母集団が、
CR≦30g/U、
マハラノビス距離が95%パーセンタイル対応値よりも小さい、
kmax>kmin、
kmin>0.008、
b<1、
という基準を満足する、請求項1に記載の電子システム。 - 前記各サブシステム及び前記各モジュールが、コンピュータ可読記憶媒体に格納され、電子プログラマブルコンピュータにロードされるコンピュータ実行可能なソフトウェアとして実装された、請求項1に記載の電子システム。
- 前記各サブシステム及び前記各モジュールが、特定用途向け集積回路モジュールとして実装された、請求項1に記載の電子システム。
- 非一時的コンピュータ可読記憶媒体においてコンピュータ実行可能なコードとして具現化され、T1DM被験者のグルコース−インスリン代謝系をシミュレートするコンピュータ実行可能なプログラム製品であって、前記コンピュータ実行可能なコードが、
(1)T1DM被験者の動的なグルコース濃度のモデルを実装するサブシステムコードであり、
(a)内因性グルコース産生(EGP(t))に対するグルカゴン作用のモデルを実装する電子コードモジュールと、
(b)食事のグルコース出現速度(Ra(t))をモデル化する電子コードモジュールと、
(c)方程式
に従ってインスリン依存性グルコース利用(Uid(t))をモデル化する電子コードモジュールで、Gt(t)が組織中のグルコース量、X(t)がグルコース利用に対するインスリン作用、[Vm0、Vmx、Km0、p2U]がモデルパラメータであり、
であって、Gbが患者の基礎グルコース、Gthが低血糖閾値(60mg/dlに設定)であり、

を具備した、サブシステムコードと、
(3)T1DM仮想被験者の母集団を含む電子データベースであり、各仮想被験者が、生体内T1DM被験者から導出された値の範囲内の値を有する複数の代謝パラメータを有する、電子データベースと、
(4)少なくとも1つの変化のある代謝パラメータ値を含む前記複数の代謝パラメータ値を前記動的なグルコース濃度のモデルを実装する前記サブシステムコード及び前記動的なインスリン濃度をモデル化するサブシステムコードに入力することによって、仮想被験者の前記グルコース−インスリン代謝系に対する前記少なくとも1つの代謝パラメータ値の変化の影響を計算するコンピュータ実行可能なコードと、
を備えた、コンピュータ実行可能なプログラム製品。 - 以下の方程式
