Description
本発明は、低鉄血症の治療のための二価鉄の新たな投与方法および新たな投与計画に関し、かつ、鉄を口腔内吸収、好ましくは舌下吸収のために放出するのに適した二価鉄化合物を含有する新たな製剤に関する。
二価鉄(すなわちFe(II)、Fe2+またはFe++)の塩または錯体を用いた経口による抗低鉄血症治療(すなわち胃腸レベルでの吸収のための治療)は、通常は有効であるにもかかわらず、その使用を厳しく制限する可能性があるいくつかの副作用を特徴とする。硫酸第一鉄、アスコビル酸第一鉄、またはグルコン酸第一鉄もしくはクエン酸第一鉄等のその他の塩/錯体の経口投与には、胃のレベルでの不耐性という第1の負の効果があり、これは、吐き気、嘔吐、上腹部の痛みを引起し、そのため、患者が治療を拒否することになる場合が多い。また、二価鉄は、十二指腸‐空腸の腸部分において酸化し易い(中性‐塩基性環境において、鉄の最も安定した酸化状態は三価状態である)ので、吸収が大幅に減少し、それは投与量を増すことで埋め合わされる。そうすると、胃のレベルでの負の効果が大きくなる。それだけでなく、実際、より深刻な障害につながる、すなわち全身レベルでの有毒性である。鉄欠乏性貧血、特に慢性鉄欠乏性貧血の治療において、特に投与間隔が長期、たとえば2週間から6ヶ月の場合に、経口投与できる二価鉄の最大量は、この疾患の重症度および使用する薬剤の種類に応じて、1日当たり150mgと450mgの間で変動する。吸収された鉄は通常、タンパク質によって運搬され鉄が欠乏している細胞内に沈着させられる(トランスフェリンは主な鉄キャリアの1つである)。しかしながら、吸収された鉄の量が、トランスフェリンを飽和させるような量であり(または吸収がトランスフェリンの錯体形成速度よりも速い場合)、かつ、三価鉄のようにイオン化した形態で血液内で循環するような量である場合、毒性状態が発生する可能性がある。この非トランスフェリン結合過剰(いわゆる遊離鉄または「NTBI」‐非トランスフェリン結合鉄)は、活性酸素ベースの化合物の形成を生じさせることでその毒性を示す。そのような化合物は、たとえば、スーパーオキシドアニオン(O2−−)、過酸化水素(H2O2)またはヒドロキシルラジカル(OH・)であり、これらはすべて、タンパク質、脂質、およびDNA自体を酸化させることによって細胞に損傷を与えることができる(Gutteridge J.M. 三価鉄(すなわちFe(III)、Fe3+またはFe+++)は、還元鉄と異なり、2.0よりも明らかに低い、極端な酸性pHレベルでしか吸収されることができない。なぜなら、このような条件でのみ、カチオンとして安定であり沈殿しないからである。これが理由で、三価鉄塩の経口投与(すなわち胃腸レベルでの吸収用)は適用できない。なぜなら、鉄を吸収する役割が与えられた部分である十二指腸‐空腸の腸管の、中性に近いpHが、鉄の沈殿を生じさせて吸収を不可能にするからである。代わりに、三価鉄の経口投与は、クエン酸アニオン、EDTAその他‐または単糖類および二糖類といった、錯化剤を用いることで可能になっている。これらはまた、錯体形成によって、オキシ水酸化物の形態の三価鉄(FeOOH)でさえ可溶にすることができる。したがって、このような状態で、上記錯体に含まれる三価鉄は、十二指腸‐空腸の腸管レベルで沈殿せずに、胃腸吸収のために生物学的に利用できる状態を保ち、よって、経口投与に使用できる。
しかしながら、これら錯体の開発において観察された欠点は、健康なボランティアの人々における薬物動態の研究が困難であることに関連がある。実際、これら化合物の吸収の度合いは、個人がどれだけ鉄レベルを高めることを必要としているかによって決まると思われる(W. Forth and S.G. Schaefer, Arzneim.-Forsch./Drug. 本願の発明者らによって現在行なわれている一連の研究は、上記目的に適した投与方法が、二価鉄塩の口腔内、より好ましくは舌下吸収に関連したものであることを示した。本明細書に記載の目的のための口腔内吸収は、今までまだ考察されておらず研究されていなかった。このことは以下の概要から明らかである。
上記概要で示したように、本願の発明者らは、驚くべきことに、本発明の上述の目標および目的が、二価鉄化合物の口腔内吸収、好ましくは舌下吸収用の新たな製剤を提供することにより達成されることを見出した。
実施例1
二価鉄化合物の舌下吸収を、図1に例示されるフランツセルを用いたインビトロ浸透実験によって評価した。ブタの食道上皮をメンブレンとして用いた。この実験モデルは、薬剤の舌下吸収の評価において有効で信頼性があるものと認められる[I. Diaz Del Consuelo, G.P. Pizzolato, F. Falsom, R.H. Guy, and Y. Res. 22: 1525-9 (2005)]。本明細書に記載の方法に従いメンブレンを調製した。食道は動物から屠殺後1時間以内に採取した。粘膜を筋肉組織からランセットを用いて分離した。上皮を分離するために、粘膜を60℃の蒸留水に2分浸した後、上皮を下にある組織からスパチュラを用いて取除いた。このようにして調製した組織は、使用時まで−20℃で保管し、使用時になると、図1に概略図が示されるフランツタイプの縦型拡散セル上に置く前に、室温まで解凍した。食道上皮は、内腔側を上にして再生セルロースフィルタ(孔直径0.45μm)上に置き、フランツセルのドナーコンパートメントとレセプタコンパートメントの間に配置した。このフィルタは支持体として機能するだけであり活性成分の浸透には干渉しない。露出した面は0.6cm2であった。レセプタコンパートメントを、約4mlの生理溶液(0.9%NaClw/v)で充填し、実験の期間全体に亘って一定の磁気撹拌を続けた。システム全体はサーモスタット制御で37℃にした。レセプタコンパートメントのサンプリングは、200μlのレセプタ溶液を入れて行ない、これを直ちに同じ体積の新鮮な生理溶液に置換えた。サンプルは予め設定された時間間隔、好ましくは0、1、2、3、4、5、および6時間の間隔で採取した。
濃度が異なるアスコルビン酸第一鉄の溶液を、アスコルビン酸を加えておよび加えずに調製し、実施例1に従うフランツセルにおいて測定した。
4gの粉末を、10mlの生理溶液に懸濁させ、15分間撹拌下で放置した後、この溶液を、再生セルロースフィルタ(孔直径0.45μm)で濾過した。アスコルビン酸第一鉄の濃度は、397.92±25.78mg/mlであることがわかった。pHは4.4に等しかった。
6gの粉末を、10mlの生理溶液に懸濁させた。懸濁液を、25分間磁気撹拌下で放置した後、再生セルロースフィルタ(孔直径0.45μm)で濾過した。アスコルビン酸第一鉄の濃度は、638.84mg/mlであることがわかった。pHは4.4に等しかった。
7gの粉末を、6mlの生理溶液に懸濁させた。懸濁液を、1時間磁気撹拌下で放置し、その最後に再生セルロースフィルタ(孔直径0.45μm)で濾過した。アスコルビン酸第一鉄の濃度は、984.72±74.47mg/mlであることがわかった。pHは4.4に等しかった。
250mgのアスコルビン酸第一鉄を5mlの生理溶液に溶かした。過剰量のアスコルビン酸(2g)をこの溶液に加えた。懸濁液を、30分間磁気撹拌下で放置し、再生セルロースフィルタ(孔直径0.45μm)による濾過後そのまま使用した。溶液のpHは2.6であった。
1.05gのアスコルビン酸第一鉄を3mlの生理溶液に溶かした。溶液には、過剰量のアスコルビン酸(525mg)を加え、溶液のpHは、NaOHを5Nを用いて4.4という初期値に戻した。溶液のタイター(titer)は350.89mg/mlであることがわかった。
実施例1のフランツセルで測定された、FeOOHとマルトデキストリンとの錯体(Fe+++‐ポリマルトース、分子量300,000Da)をFe+++として用いた。4gの粉末を10mlの生理溶液に懸濁させた。懸濁液を、15時間磁気撹拌下で放置し、その最後に、再生セルロースフィルタ(孔直径0.45μm)で濾過した。溶解度に相当するFe+++濃度は、270.23±14.47mg/mlであることがわかった。実施例1に従う実験設計を用いたところ、ポリマルトース第一鉄は浸透レベルを示さず、その溶解度にもかかわらず、舌下経路を通して投与した場合生物学的に利用不能であることを示した。
実施例1と同様の吸収試験で使用したFe+++のもう1つの化合物はクエン酸第二鉄であった。過剰量の粉末を10mlの生理溶液に懸濁させた。懸濁液を15時間磁気撹拌下で放置し、その最後に再生セルロースフィルタ(孔直径0.45μm)で濾過した。溶解度に対応するクエン酸第二鉄の濃度は280mg/mlであることがわかった。溶液のpHは1.2であった。吸収チャートは図3に示される。
‐舌下錠剤
36.35kgのアスコルビン酸鉄(Fe++5kgに等しい)、60.00kgのマルトデキストリン、20.00kgのアスコルビン酸を、円錐形のミキサに加える。15分混合した後、1.50kgのステアリン酸マグネシウム、25.00kgのポリビニルピロリドン、および3.00kgのレモン香料を順に加える。さらに15分混合した後、粉末を直径7.0mmのキャンバーパンチを用いて圧縮し、平均重量145.85mgの錠剤(一錠あたりFe++5mgに等しい)を得た。
アスコルビン酸鉄36.35kg(Fe++5kgに等しい)、60.00kgのマルトデキストリン、20.00kgのアスコルビン酸を、円錐形のミキサに加える。15分の混合後、47.39kgのフルクトースおよび3.00kgのレモン香料を順に加える。さらに15分混合した後、粉末を、45.75mg含む小袋(1袋当たりFe++5mgに等しい)に分けた。

アスコルビン酸第一鉄40mg(5mgのFe++に等しい)、フルクトース60mg、アスコルビン酸10mg、レモン香料5mg。
Fe2+アスコルビン酸としてFe2+30mgを舌下投与
本発明に従う新たな組成物のインビボバイオアベイラビリティを調べるために、ランダム化臨床研究を健康なボランティアの患者に対して実施し(n=2)、患者には、毎日30mgのFe2+を本明細書に記載の組成物の形態(上記実施例6に従う40mgのFe2+アスコルビン酸を含む6つの小袋)で投与した。
血液サンプルを、投与の24時間前と22時間前、投与時(0.0時間)、投与後0.033(2分)、0.10(6分)、0.20(12分)、0.33(20分)、0.50(30分)、0.75(45分)、1.00(60分)、1.25(1時間15分)、1.50(1時間30分)、1.75(1時間45分)、および2.00(2時間)の時点で、採取した。
マイクログラム/dlで測定した鉄総量(ICP−MS)
マイクロモル/リットルで測定した限外濾過による遊離鉄(=NTBI)(ICP−MS)
マイクログラム/dlで測定した総鉄結合能(カラム分離、ビトロス(Vitros)解析器)
%で測定したトランスフェリン飽和度(PTS)
マイクログラム/dlで測定した血清鉄(SI)、比色法(ビトロス解析器)
結果:
以下、これまでの章の教示に鑑み、本発明のいくつかの好ましいが排他的ではない実施形態について説明する。
低鉄血症の治療において患者に口腔内吸収を通して、好ましくは舌下吸収を通して投与するために使用する二価鉄化合物であり、この使用は、ヒトの患者に対する1日当たりの二価鉄の用量を、1〜90mg、好ましくは1〜60mg、より好ましくは4〜40mg、さらに好ましくは5〜30mgの範囲に含まれる用量として、二価鉄を投与することを含み、それにより、胃腸投与と比較して、トランスフェリン飽和度が同等であるのに対し、遊離の非トランスフェリン結合鉄の発生を減じる。
好ましくは第1の実施形態に従う、セリアック病または慢性炎症性腸疾患を原因とする鉄の吸収不良の治療において、患者に口腔内吸収を通して、好ましくは舌下吸収を通して投与するために使用する二価鉄化合物であり、この使用は、ヒトの患者に対する1日当たりの二価鉄の用量を、10〜90mg、好ましくは30〜90mg、より好ましくは40〜90mg、さらに好ましくは45〜90mgの範囲に含まれる用量として、二価鉄を投与することを含む。
第2の実施形態に従う、セリアック病または慢性炎症性腸疾患を原因とする鉄の吸収不良の治療において、患者に口腔内吸収を通して、好ましくは舌下吸収を通して投与するために使用する二価鉄化合物であり、この使用は、ヒトの患者に対する1日当たりの二価鉄の用量を45〜65mgの範囲に含まれる用量として、二価鉄を投与することを含む。
好ましくは第1の実施形態に従う、失血を原因とする慢性鉄欠乏性貧血の治療において、患者に口腔内吸収を通して、好ましくは舌下吸収を通して投与するために使用する二価鉄化合物であり、この使用は、ヒトの患者に対する1日当たりの二価鉄の用量を、10〜90mg、好ましくは30〜90mg、より好ましくは40〜90mg、さらに好ましくは45〜90mgの範囲に含まれる用量として、二価鉄を投与することを含む。
第4の実施形態に従う、失血を原因とする慢性鉄欠乏性貧血の治療において、患者に口腔内吸収を通して、好ましくは舌下吸収を通して投与するために使用する二価鉄化合物であり、この使用は、ヒトの患者に対する1日当たりの二価鉄の用量を、45〜65mgの範囲に含まれる用量として、二価鉄を投与することを含む。
以前は二価もしくは三価鉄の化合物を用いた胃腸療法によって治療されていた、または、以前は三価鉄化合物を用いた非経口療法によって治療されていた再帰性貧血状態の治療において使用する二価鉄化合物であり、この使用は、胃腸投与と比較して、トランスフェリン飽和度の増加量が同等であるのに対し、遊離の非トランスフェリン結合鉄の発生を減じるために、患者に二価鉄化合物を口腔内吸収を通して、好ましくは舌下吸収を通して投与することを含む。
第6の実施形態に従う、以前は二価もしくは三価鉄の化合物を用いた胃腸療法によって治療されていた、または、以前は三価鉄化合物を用いた非経口療法によって治療されていた再帰性貧血状態の治療において使用する二価鉄化合物であり、この使用は、二価鉄を、患者に、口腔内吸収を通して、好ましくは舌下吸収を通して、ヒトの患者に対する1日当たりの二価鉄の用量を、1〜90mg、好ましくは1〜60mg、より好ましくは4〜40mg、さらに好ましくは5〜30mgの範囲に含まれる用量として、投与することを含む。
好ましくは第7の実施形態に従う、以前は二価もしくは三価鉄の化合物を用いた胃腸療法によって治療されていた、または、以前は三価鉄化合物を用いた非経口療法によって治療されていた、失血による慢性鉄欠乏性貧血を原因とする再帰性貧血状態の治療、または、セリアック病もしくは慢性炎症性腸疾患による鉄の吸収不良を原因とする再帰性貧血状態の治療において使用する二価鉄化合物であり、この使用は、二価鉄を、患者に、口腔内吸収を通して、好ましくは舌下吸収を通して、ヒトの患者に対する1日当たりの二価鉄の用量を、10〜90mg、好ましくは30〜90mg、より好ましくは40〜90mg、さらに好ましくは45〜90mgの範囲に含まれる用量として、投与することを含む。
第8の実施形態に従う、以前は二価もしくは三価鉄の化合物を用いた胃腸療法によって治療されていた、または、以前は三価鉄化合物を用いた非経口療法によって治療されていた、失血による慢性鉄欠乏性貧血を原因とする再帰性貧血状態の治療、または、セリアック病もしくは慢性炎症性腸疾患による鉄の吸収不良を原因とする再帰性貧血状態の治療において使用する二価鉄化合物であり、この使用は、二価鉄を、患者に、口腔内吸収を通して、好ましくは舌下吸収を通して、ヒトの患者に対する1日当たりの二価鉄の用量を45〜65mgの範囲に含まれる用量として、投与することを含む。
第1〜第9の実施形態のうちの1つ以上に従う二価鉄化合物であって、その使用は、1日当たり1〜6回、好ましくは1〜3回の投与により、第1〜第9の実施形態に従う1日の用量を満たすようにする治療計画を含む。
第1〜第10の実施形態のうちの1つ以上に従う使用のための、第1〜第10の実施形態のうちの1つ以上に従う二価鉄化合物であって、その投与は舌下吸収を通して行なわれる。
第1〜第11の実施形態のうちの1つ以上に従う使用のための、第11の実施形態に従う二価鉄化合物であって、投与の際は、二価鉄化合物を含有する製剤および/または唾液を、この二価鉄化合物を含有する製剤を口腔、好ましくは舌の下に置いてから、少なくとも30秒、好ましくは少なくとも1分、より好ましくは少なくとも2分、さらに好ましくは少なくとも4分の期間、飲み込まないよう、患者に対する指示がなされる。
胃腸投与との比較で、トランスフェリン飽和度の同等の増加量に対し、遊離の非トランスフェリン結合鉄の発生を減じるために、患者における低鉄血症の治療において使用する、一回当たりの二価鉄の用量が、0.5〜30mg、好ましくは3〜20mg、より好ましくは3〜10mgの範囲に含まれるように調製された、口腔内吸収、好ましくは舌下吸収用の二価鉄製剤。
以前は二価もしくは三価鉄の化合物を用いた胃腸療法によって治療されていた、または、以前は三価鉄化合物を用いた非経口療法によって治療されていた、セリアック病もしくは慢性炎症性腸疾患を原因とする鉄の吸収不良の治療、失血を原因とする慢性鉄欠乏性貧血の治療、または、再帰性貧血状態の治療に使用する、第13の実施形態に従う二価鉄製剤。
口腔内錠剤、舌下錠剤、経口薬ストリップ(すなわち口腔内または舌下フィルム)、ウェハース(すなわち口腔内または舌下平坦要素)、粒状または粉末状材料、飛沫、泡、ジェル、および口腔内ペーストからなる群より選択される投与形態の、第13または第14の実施形態に従う製剤。
第15の実施形態に従う製剤であって、口腔内錠剤、舌下錠剤、経口薬ストリップ(すなわち口腔内または舌下フィルム)、またはウェハース(すなわち口腔内または舌下平坦要素)は、フィルム状の形状を有し、特に、総厚さが0.05〜3mm、好ましくは0.1〜1mm、さらに好ましくは0.1〜0.5mmであり、面積が好ましくは0.5〜20cm2、好ましくは1〜10cm2である。
舌下吸収用の第13〜第16の実施形態に従う製剤。
低鉄血症の治療において患者に口腔内吸収を通して、好ましくは舌下吸収を通して投与するために使用する、第13〜第17の実施形態のうちの1つ以上に従う製剤であって、この使用は、ヒトの患者に対する1日当たりの二価鉄の用量を、1〜90mg、好ましくは1〜60mg、より好ましくは4〜40mg、さらに好ましくは5〜30mgの範囲に含まれる用量として、二価鉄を投与することを含む。
第13〜第18の実施形態のうちの1つ以上に従う二価鉄製剤、または、第1〜第12の実施形態のうちの1つ以上に従う二価鉄化合物であって、二価鉄化合物は、口腔内吸収を通した投与のために薬学的に許容可能なpH値で可溶である二価鉄の塩または錯体で構成される。
第19の実施形態に従う製剤または化合物であって、二価鉄化合物は、硫酸第一鉄、クエン酸第一鉄、アスコルビン酸第一鉄、フマル酸第一鉄、グリシン酸第一鉄、およびグルコン酸第一鉄からなる群より選択され、上記化合物は好ましくはアスコルビン酸第一鉄である。
第13〜第20の実施形態のうちの1つ以上に従う製剤であって、緩衝および/または還元賦形剤、好ましくはアスコルビン酸/アスコルビン酸塩を、患者の口腔内におけるpHとして2.0と6.5の間、好ましくは2.5と4.5の間のpHを得るようにされた量だけ、含む。
第13〜第21の実施形態のうちの1つ以上に従う製剤であって、製剤全体のうち、10〜90%(w/w)、好ましくは20〜70%(w/w)、より好ましくは20〜50%(w/w)を構成する少なくとも1つの高分子相を含む。
第13〜第22の実施形態のうちの1つ以上に従う製剤であって、粘膜付着性であることまたは少なくとも1つの粘膜付着面を示すことを特徴とする。
第13〜第23の実施形態のうちの1つ以上に従う固形製剤であって、水性環境において、好ましくは唾液の中で、好ましくは10秒〜5分以内、より好ましくは30秒〜3分以内、さらに好ましくは1分から2分以内に、素早く崩壊して二価鉄化合物全体を放出することを特徴とする。
第13〜第24の実施形態のうちの1つ以上に従う製剤であって、この製剤がたとえば葉酸等の追加の活性成分を含むことを特徴とする。
Claims (14)
- 低鉄血症の治療において患者に舌下吸収を通して投与することにより使用するための二価鉄化合物を含有する製剤であって、前記投与の際は、前記二価鉄化合物を含有する製剤および/または唾液を、前記二価鉄化合物を含有する製剤を舌下腔内に置いてから少なくとも30秒の期間飲み込まないよう患者に対する指示がなされ、前記使用は、ヒトの患者に対する1日当たりの二価鉄の用量を、1〜90mgの範囲に含まれる用量として、二価鉄を投与することを含む、二価鉄化合物を含有する製剤。
- 前記二価鉄化合物を含有する製剤は、(i)セリアック病もしくは慢性炎症性腸疾患を原因とする鉄の吸収不良の治療において、または(ii)失血により生じる慢性鉄欠乏性貧血の治療において、または(iii)失血により生じる慢性鉄欠乏性貧血を原因とする再帰性の貧血状態の治療またはセリアック病もしくは慢性炎症性腸疾患により生じる鉄の吸収不良を原因とする再帰性の貧血状態の治療において、患者における請求項1に記載の舌下吸収を通した投与により使用するためのものであり、前記再帰性の貧血状態は、以前は二価もしくは三価鉄の化合物を用いた胃腸療法によって治療されていた、または、以前は三価鉄化合物を用いた非経口療法によって治療されていたものであり、前記使用は、ヒトの患者に対する1日当たりの二価鉄の用量を、30〜90mgの範囲に含まれる用量として、二価鉄を投与することを含む、請求項1に記載の二価鉄化合物を含有する製剤。
- 前記使用は、ヒトの患者に対する1日当たりの二価鉄の用量を、45〜65mgの範囲に含まれる用量として、二価鉄を投与することを含む、請求項2に記載の使用のための二価鉄化合物を含有する製剤。
- 前記使用は、1日当たり1〜6回の投与により1日の用量を満たすようにする治療計画を含む、請求項1〜3のうちのいずれか一項に記載の使用のための二価鉄化合物を含有する製剤。
- 前記治療計画は1日当たり1〜3回の投与の計画である、請求項4に記載の二価鉄化合物を含有する製剤。
- 舌下吸収用の二価鉄製剤であって、二価鉄化合物が、舌下吸収を通した投与のために薬学的に許容可能なpH値で可溶である二価鉄の塩または錯体で構成され、前記製剤は、患者における低鉄血症の治療において使用する、一回当たりの二価鉄の用量が、0.5〜30mgの範囲に含まれるように調製され、治療における投与の際は、前記二価鉄化合物を含有する製剤および/または唾液を、前記二価鉄化合物を含有する製剤を舌下腔内に置いてから少なくとも30秒の期間飲み込まないよう患者に対する指示がなされる、二価鉄製剤。
- 前記二価鉄化合物は、硫酸第一鉄、クエン酸第一鉄、アスコルビン酸第一鉄、フマル酸第一鉄、グリシン酸第一鉄、およびグルコン酸第一鉄からなる群より選択される、請求項6に記載の使用のための製剤。
- 前記化合物はアスコルビン酸第一鉄である、請求項7に記載の使用のための製剤。
- 舌下錠剤、経口薬ストリップ(すなわち舌下フィルム)、ウェハース(すなわち舌下平坦要素)、粒状または粉末状材料、飛沫、泡、ジェル、および舌下ペーストからなる群より選択される投与形態の、請求項6〜8に記載の使用のための製剤。
- 緩衝および/または還元賦形剤を、患者の口腔内におけるpHとして2.0と6.5の間のpHを得るようにされた量だけ含む、請求項6〜9のうちの一項に記載の使用のための製剤。
- 緩衝および/または還元賦形剤を、患者の口腔内におけるpHとして2.5と4.5の間のpHを得るようにされた量だけ含む、請求項10に記載の使用のための製剤。
- 前記製剤が追加の活性成分を含むことを特徴とする、請求項6〜11のうちの一項に記載の使用のための製剤。
- 前記追加の活性成分は葉酸である、請求項12に記載の使用のための製剤。
- 前記二価鉄化合物を含有する製剤は再帰性の貧血状態の治療において使用するためのものであり、前記再発性貧血状態は、以前は二価もしくは三価鉄の化合物を用いた胃腸療法によって治療されていた、または、以前は三価鉄化合物を用いた非経口療法によって治療されていたものである、請求項1に記載の使用のための二価鉄化合物を含有する製剤。
