Description
用語の「敗血症」又は「感染性合併症」は、本願において、同義語として使用され、全ての症状が感染を伴う炎症の全身症状に関係している、「敗血症」、「感染性合併症」、「重症敗血症」、「敗血症性ショック」、並びにそれらの早期段階を包含すると、理解される。
本発明は、敗血症を有する又は有すると疑われる対象者の死亡率及び/又は転帰についての診断、予測、又はリスク層別化の方法に関し、以下を含む:
・前記対象者から採取したサンプル中のアンチトリプシン(ATT)又はその断片の決定並びに/あるいはトランスサイレチン(TTR)又はその断片の存在及び/又はレベルの決定、
・ATT及び/又はTTRあるいはそれらの断片の存在及び/又はレベルが、死亡及び/又は不良転帰の増大したリスクと相関しているとの決定;並びに
・ATT(成熟断片;55kDa)の存在及び/又はレベルが特定のカットオフ値未満である及び/又はそれらの断片のレベルが特定のカットオフ値を超えている場合に、前記死亡及び/又は不良転帰の増大したリスクがもたらされるかの決定並びに/あるいは
・TTRの存在及び/又はレベルが特定のカットオフ値未満である及び/又はそれらの断片のレベルが特定のカットオフ値未満である場合に、前記死亡及び/又は不良転帰の増大したリスクがもたらされるかの決定。
・事前に決定されたサンプルの集合体におけるATT及び/又はその断片のレベルの中央値に関する関連付け、
・事前に決定されたサンプルの集合体におけるATT及び/又はその断片のレベルの変位値に関する関連付け、及び
・例えば、Cox回帰モデル等の数学的モデルによる相関付け。
・ATTについて、正常集団の中央値より低い、及び/又は
・ATTの断片について、正常集団の中央値より高い、及び/又は
・TTRについて、正常集団の中央値より低い。
・対象者からサンプルを採取するステップ、及び/又は
・ATT又はその断片のレベルを決定するステップ、及び/又は
・TTR又はその断片のレベルを決定するステップ、及び/又は
・ATT又はその断片並びに/あるいはTTR又はその断片のレベルと、死亡のリスク又は生存の変位値とを関連付けるステップ。
・ 以下の群から選択される様々な時点で前記患者から少なくとも二つのサンプルを採取するステップ、
i. 治療開始前、及び/又は
ii. 治療開始後、及び/又は
iii.一つ又は複数の他の時点、
・前記対象者から採取された前記サンプル中のATT又はその断片並びに/あるいはTTR又はその断片のレベルを決定するステップ、
・前記対象者から採取された前記サンプル中のATT又はその断片並びに/あるいはTTR又はその断片のレベルと、前記ある薬剤に対する陽性又は陰性反応とを関連付けるステップ。
・以下の群から選択される様々な可能な時点で前記患者から少なくとも一つのサンプルを採取するステップ、
i. 治療開始前、及び/又は
ii. 治療開始後、及び/又は
iii.一つ又は複数の他の時点、
・前記対象者から採取された前記サンプル中のATT及び/又はTTRあるいはそれらの断片のレベルを決定するステップ、
・前記対象者から採取された前記サンプル中のATT及び/又はTTRあるいはそれらの断片のレベルと、ある治療の必要性とを関連付けるステップ。
(1)前記対象者から採取されたサンプル中のATT及び/又はTTRあるいはそれらの断片のレベルを決定するステップ、及び
(2)ATT及び/又はTTRあるいはそれらの断片の存在及び/又は前記レベルと、(i)初期敗血症、(ii)重度敗血症、又は(iii)敗血症性ショックとを関連付けるステップ、
(3)但し、前記ATT及び/又はTTRあるいはそれらの断片の存在及び/又はレベルと、(i)初期敗血症、(ii)重度敗血症、又は(iii)敗血症性ショックとの関連性は、ATTの前記レベルがあるカットオフ値より低い場合及び/又はATTのタンパク質分解断片の前記レベルがあるカットオフ値より高い場合に認められ、並びに/あるいは
(4)前記ATT及び/又はTTRあるいはそれらの断片の存在及び/又はレベルと、(i)初期敗血症、(ii)重度敗血症、又は(iii)敗血症性ショックとの関連性は、TTRの前記レベルがあるカットオフ値より低い場合に認められる。

データは、平均値±SD(数値は、小数第1に四捨五入した)及びパーセントで示す。SIRS−PCT-は、<0.3ng/mLの血清PCTレベルを表わし、SIRS−PCT+は、>0.3ng/mLの血清PCTレベルを表わす。*APACHE−IIは、急性生理学及び慢性健康評価IIスコアである。この尺度のスコアは、0から71までであり、スコアが高いほど疾患の重症度が高いことを示す。**SOFAは、逐次臓器不全評価スコアである。SOFAの小計スコアは、6の臓器系のそれぞれについて0から4までであり、総計スコアは0から24までであって、スコアが高いほど臓器不全の重症度が高いことを示す。
2002年9月から2003年9月まで及び2006年1月から2006年3月まで麻酔科及び集中治療科の集中治療室(ICU)に収容された患者の内、ACCP/SCCM合意会議の敗血症基準又は急性臓器不全を伴い非感染性病原のSIRSの基準を満たす患者それぞれを本研究に組み入れた(ICD−10−GMコード)。
非感染性病原SIRS患者とsSEPSIS患者とを識別するSELDI−TOF−MSの能力を試験するため、血漿タンパク質プロファイルを、臨床ACCP/SCCM合意基準に従って分類された患者からのサンプルにおいて分析した。患者159名からの166のサンプル、重度敗血症又は敗血症性ショック(sSEPSIS)の患者からの81のサンプル、非感染性病原SIRS患者からの85のサンプルが組み込まれた。例えば、SIRSからsSEPSISへと分類が変更された7名の患者においては、二つずつサンプルを採取し、それぞれのサンプル群に割り付けた。非感染病原性SIRS患者は、組み入れ日の血清PCT濃度により、PCT陰性(PCT<0.3ng/mL)SIRS患者(SIRS−PCT-)及びPCT陽性(PCT>0.3ng/mL)SIRS患者(SIRS−PCT+)にさらに分類された。診断質量パターン同定のプロトコルには、以下のステップが含まれていた:(1)決定木を得るための、既知の診断を有するサンプルを含む訓練セットの分析;(2)同一コホートの追加の患者からの試験セットにおいての、所定の決定木の識別能の検証;(3)5か月後に同一の試験セットの分析を繰り返すことによる、この識別能の再現性の試験;及び(4)独立した盲検化試験セットにおけるこの分析の繰返し。訓練群、試験群、及び予後試験群に割り付けられた、SIRS−PCT-患者、SIRS−PCT+患者、及びsSEPSIS患者の臨床特性を表1に示す。
PCTは、SIRS患者とsSEPSIS患者とを識別できない
我々は、最初に、我々の研究コホートにおける感染病原性SIRS患者と非感染病原性SIRS患者とを識別するためのPCTの診断値を評価した。SIRSサンプルについてのPCTの中央値(n=75;PCT=6.79[中央値];3.38〜11.50[25〜75パーセンタイル])並びにsSEPSISサンプルについての中央値(n=72;PCT=4.02[中央値];1.84〜12.46[25〜75パーセンタイル])の間で有意な違いは認められなかった(Mann−Whitney U検定によるp=0.62)。ROC曲線による解析は、PCTがSIRS患者からのサンプルとsSEPSIS患者からのサンプルとを識別するには不十分であることを、明らかにした(AUC=0.525;95%CI=0.428〜0.621;オンライン補足データの図S1A)。訓練群、試験群、及び予後試験群において、SIRS−PCT+サンプルとsSEPSISサンプルとを識別するPCTの識別能に注目する場合にも、同じである(AUC=0.411〜0.69;オンライン補足データの図S1B〜D)。
次に、我々は、質量分析結果がSIRS−PCT+患者とsSEPSIS患者とをさらに識別できるか試みてみた。この目的のために、54名の患者(sSEPSIS患者、n=31、PCT=25.84±75.4;SIRS−PCT+患者、n=23、PCT=20.01±34.3:表1)からのスペクトルによるCART解析を実施した。得られた分類アルゴリズムは、sSEPSIS患者とSIRS−PCT+患者との識別に重要なのが6.5kDaのピークであることを、明らかにした。図1に示す、二つのアルゴリズムの例は、それぞれ96%及び100%のSEPSIS患者及びSIRS−PCT+患者の正確な同定を可能とした。これらの結果の検証には、同様な構成の別の患者77名(表1:SIRS−PCT+患者、n=36、PCT=16.98±26.1;sSEPSIS患者、n=41、PCT=15.82±33.1)を含む盲検化試験セットを用いた。図1A/Bに示すこれらの分類アルゴリズムは、再度、患者37名中36名のsSEPSIS及び患者25名中24名のSIRS−PCT+をそれぞれ正確に予測した(感度=90.2%/87.8%、特異度=66.6%/69.4%:試験セットの予測AUC、sSEPSISについて0.756/0.753)。
盲検化試験セットは、訓練セットと同一の条件を用いて5か月後に再分析された。同一の決定木を用いて、我々は、sSEPSIS患者(78%;29/37;AUC=0.841)及びSIRS−PCT+患者(92.8%;26/28;AUC=0.872)の正確な同定についての同様な結果を得た(図1)。更なる検証のため、この分析を、サンプリング時にPCT陽性であって、後に従来からの臨床基準に従ってsSEPSIS群(n=9)又はSIRS−PCT+群(n=10)のいずれかに後向きに割り付けられた、19名のICU患者の独立した盲検化試験セットにおいて繰り返した。図1に示すように、サンプルは、それぞれsSEPSIS患者及びSIRS−PCT+患者の77%/66%及び100%/100%において正確に同定された。
次に、我々は、最初に特異的に発現されるマーカーCM_6.15の同定を試みた。この研究用にサンプルが収集された2002年及び2003年において、冠動脈バイパス移植手術(CABG)を受ける患者は、術中失血及び輸血の必要性を低減させるために一般にアプロチニン(商品名、トラジロール(登録商標))を投与されたが、我々のSIRS−PCT+患者にほとんども心臓手術を受けた(表1)。心臓手術患者98名中70名でアプロチニン用量の後向き評価のための記録が利用可能であって、そのほとんど(94.29%)は、5億単位から最高80億単位までの総用量を投与された。アプロチニンは、6.512kDaの分子量を有する、天然のタンパク質分解酵素阻害剤である。CM_6.15がアプロチニンであるエビデンスは、CM10チップ上に(登録商標)を塗布した後のSELDI−TOFスペクトルと、トラジロール(登録商標)を投与されたSIRS−PCT+患者からのスペクトルとの比較によってもたらされた(図1)。6512.74DaのマーカーCM_6.15は、6512.78Daのアプロチニンのピークに対応し、さらに、CM_6.15は、SIRS−PCT+患者(患者番号162;トラジロール(登録商標)50億単位を投与)に存在するが、トラジロール(登録商標)を投与されていないsSEPSIS患者(患者番号140)においてはほぼ存在していない。CM_6.15のピーク強度は、トラジロール(登録商標)15億単位以上を投与されたSIRS患者に適用されたトラジロール(登録商標)の総用量と相関している(Pearsonの相関係数=0.848;p<0.001)。したがって、CM_6.15は、心肺バイパス法を受けるそれらの患者における予防的なトラジロール(登録商標)投与を反映するため、異なる患者群を真に識別するが、CM_6.15は、感染性のSIRS(重症敗血症/敗血症性ショック)/重度臓器不全を有する非感染性SIRSの過程における病態生理学的マーカーではない。
代替の識別決定木作成のため、我々は、異なる分析設定、例えば、上記のマーカーCM_6.15の除外を評価した。図2には、練習セットにおいてsSEPSIS患者とSIRS−PCT+患者とを識別する(感度、93%;特異度、95%)、二つ目の決定木が示されている。興味深いことに、この決定木は、最初の決定木において重要な予測因子であることが既に示されている、二つのクラスター(4.7kDa、13.8kDa)を含んでいた。追加される主な識別要素は、Q10表面上で検知され、約55.7kDaの分子量を有していた。得られた正規化ProteinChipアレイ分析結果の代表的な例を図3に示す。この決定木の試験セットにおける最初の検証で、85.3%の感度、86.1%の特異度が得られた(図2)。この検証は、5か月後に、86.4%の感度、92.8%の特異度として再現された(図2)。独立した盲検化繰返し試験セットにおいてこの決定木を用いると、sSEPSIS患者で88.8%、SIRS−PCT+患者で90%の正確な同定が再度もたらされた(全分類アルゴリズムのROC曲線下面積(ROC積分値)、即ち、AUC=0.911、図2)。SIRS−PCT+及びsSEPSISの予測についての最初の試験サンプル及び独立した盲検化試験サンプルにおける三つの関連するマーカーのROC面積は、それぞれ0.895〜0.933(QC_55.7)、0.656〜0.910(C1_13.8)、及び0.712〜0.922(C1_4.7)の範囲であった(オンライン補足データの図S2A〜C)。
特異的に発現するタンパク質の同定のため、対象のタンパク質ピークが多量に存在するサンプルを使用した。同定のため、サンプルp218のアリコートの分別後に陰イオン交換クロマトグラフィーにより得られた、13.8kDaのタンパク質フラクション4は、12% Mini−PAGEによって精製された。m/z55.7kDaのピークは、10% Maxi−PAGE上で精製され、対応するサイズのスポットは、摘出され、ゲル内でトリプシン消化された。得られた断片のSELDI−TOF/TOF分析及びそれに続くMASCOTサーチは、トランスサイレチン(TTR)としてマーカーC1_13.8の同定を可能にした。TTRの特異的に発現した13.8kDaのタンパク質への割当は、TTRが主にSIRS−PCT+患者において発現される一方、sSEPSIS患者においては検知されないことを示す、ウェスタンブロット分析(図4A)によって最初に確認された。次に、TTRは、69名のsSEPSIS患者及び59名のSIRS−PCT+患者のサンプルで測定され、平均濃度は、それぞれ7.15mg/dL(95%CI=6.35〜7.95)及び10.7mg/dL(95%CI=10.0〜11,5)であって(図4B)、C1_13.8のピーク強度が従来からのTTRの免疫分析結果(図S2B)と相関していることを示している。最後に、TTR特異的抗体を用いる免疫喪失は、対照抗体を用いた後に認められるピーク強度に比べ、13.8kDaのピーク強度の大幅な低下をもたらした(図4C)。
Claims (12)
- 敗血症を有する対象者又は敗血症を有すると疑われる対象者の死亡率又は転帰についての予測、又はリスク層別化のための方法であって、下記の(a)〜(c)のステップを含む及び下記の(d)〜(f)のステップを含む方法:
(a)前記対象者から採取したサンプル中のアンチトリプシン(ATT)又は、配列番号4,5,8及び10のその断片のレベルを決定するステップ、
(b)但し、ATT又はその断片の前記レベルが、死亡リスク増大又は予後不良と相関しており、かつ
(c)ATTの前記レベルが、あるカットオフ値未満である場合及び/又はATTの断片の前記レベルが、あるカットオフ値を超えている場合に、死亡リスク増大又は予後不良と関係づけるステップ;
(d)前記対象者から採取したサンプル中のトランスサイレチン(TTR)のレベルを決定するステップ、
(e)但し、TTRの前記レベルが、死亡リスク増大又は予後不良と相関しており、かつ
(f)TTRの前記レベルが、あるカットオフ値未満である場合に、死亡リスク増大又は予後不良と関係づけるステップ。 - 請求項1に記載の方法であって、前記ATT又はその断片の前記レベル、及びTTRの前記レベルが、死亡又は転帰についての予測又はリスク層別化と、以下の選択肢から選択される方法によって相関している,方法:
(a)所定のサンプルの集合体におけるレベルの中央値に関する相関付け、
(b)所定のサンプルの集合体におけるレベルの変位量に関する相関付け、及び
(c)例えば、Cox回帰モデル等の数学的モデルによる相関付け。 - 請求項1及び2のいずれか1項に記載の方法であって、
(a)ATTの前記レベルの前記カットオフ値が2g/Lであって、分析対象の患者に応じて20%だけ偏位可能であり;
(b)TTRの前記レベルの前記カットオフ値が10mg/dLであって、分析対象の患者に応じて20%だけ偏位可能である、方法。 - 請求項1〜3のいずれか1項に記載の方法であって、ATT又はその断片の前記レベル、及びTTRの前記レベルが、
(a)ATTについては、正常集団の中央値より低いため、
(b)ATTの断片については、正常集団の中央値を超えているため、及び
(c)TTR又はその断片については、正常集団の中央値より低いために、死亡についてのリスク増大と相関している、方法。 - 請求項1〜4のいずれか1項に記載の方法であって、下記の(a)〜(c)のステップを含む及び下記の(d)〜(f)のステップを含む方法:
(a)対象者からサンプルを採取するステップ、
(b)ATT又はその断片のレベルを決定するステップ、及び
(c)ATT又はその断片の前記レベルと、死亡又は生存の変位量毎のリスクを相関させるステップ;
(d)対象者からサンプルを採取するステップ、
(e)TTR又はその断片のレベルを決定するステップ、及び
(f)TTR又はその断片の前記レベルと、死亡又は生存の変位量毎のリスクを相関させるステップ。 - 請求項1〜5のいずれか1項に記載の方法であって、サンプルが下記の時点で1回又は複数回採取される、方法:対象者が医療施設又は救急車に初めて収容される時、対象者が緊急治療室に入る時、対象者が集中治療室に入る時、治療前、治療開始後、治療開始から24時間後、治療開始から48時間後及び/又は治療開始から72時間後。
- 請求項1〜6のいずれか1項に記載の方法であって、前記対象者が、敗血症様感染症の先行状態、敗血症様感染症の進行状態、重度敗血症、及び敗血症性ショックを含む群から選択される状態の下にある、方法。
- 請求項1〜7のいずれか1項に記載の方法であって、前記対象者の死亡又は転帰についての予測又はリスク層別化に加え、前記対象者から採取されたサンプル中でのATT又はその断片の前記レベル、及びTTRの前記レベルの決定が、前記対象者が全身性炎症(SIRS)、初期敗血症、重度敗血症、又は敗血症性ショックを有する可能性があるかについての鑑別診断に用いられる、方法。
- 請求項1〜8のいずれか1項に記載の方法であって、前記サンプルが、血漿サンプル、血清サンプル、全血サンプル、血液サンプル又はその画分、リンパ液サンプル、尿サンプル、並びに前記サンプルのいずれかからの抽出物を含む群から選択される、方法。
- 敗血症又は敗血症様疾患を有する対象者において、ある薬剤に対する反応をモニターリングすることにより、前記疾患の重症度に関する個別の患者の療法に関して、医学的決定をおこなうための方法であって、下記の(a)〜(d)のステップを含む及び下記の(e)〜(h)のステップを含む方法:
(a)以下の群から選択される様々な時点で前記患者から少なくとも二つのサンプルを採取するステップ、
i. 治療開始前、
ii. 治療開始後、及び/又は
iii.一つ又は複数の他の時点、
(b)前記サンプル中のATT又はその配列番号4,5,8及び10の断片のレベルを決定するステップ、
(c)前記サンプル中のATT又はその配列番号4,5,8及び10の断片の前記レベルと、前記ある薬剤への陽性又は陰性反応とを関連付けるステップ、及び
(d)薬剤治療中にATTの前記レベルが増大する場合及び/又は薬剤治療中にATTの断片の前記レベルが低減する場合に、陽性反応と認めるステップ;
(e)以下の群から選択される様々な時点で前記患者から少なくとも二つのサンプルを採取するステップ、
i. 治療開始前、
ii. 治療開始後、及び/又は
iii.一つ又は複数の時点、
(f)前記サンプル中のTTRのレベルを決定するステップ、
(g)前記サンプル中のTTRの前記レベルと、前記ある薬剤への陽性又は陰性反応とを関連付けるステップ、及び
(h)前記薬剤による治療中にTTRの前記レベルが増大する場合に、陽性反応と認めるステップ。 - 対象者における疾患を鑑別診断するための方法であって、前記疾患が、(i)全身性炎症(SIRS)、(ii)初期敗血症、(iii)重度敗血症、又は(iv)敗血症性ショックを含む群から選択され、下記の(a)〜(c)のステップを含む及び下記の(d)〜(f)のステップを含む方法:
(a)前記対象者から採取したサンプル中のATT又はその配列番号4,5,8及び10の断片のレベルを決定するステップ、
(b)ATT又はその配列番号4,5,8及び10の断片の前記レベルと、(i)初期敗血症、(ii)重度敗血症、又は(iii)敗血症性ショックとを関連付けるステップ、及び
(c)但し、ATTの前記レベルがあるカットオフ値未満である場合及び/又はATTのタンパク質分解断片の前記レベルがあるカットオフ値を超えている場合に、ATT又はその断片の前記レベルと、(i)初期敗血症、(ii)重度敗血症、又は(iii)敗血症性ショックとの関連性を認めるステップ;
(d)前記対象者から採取したサンプル中のTTRのレベルを決定するステップ、
(e)TTRの前記レベルと、(i)初期敗血症、(ii)重度敗血症、又は(iii)敗血症性ショックとを関連付けるステップ、及び
(f)但し、TTRの前記レベルがあるレベル未満である場合に、TTRの前記レベルと、(i)初期敗血症、(ii)重度敗血症、又は(iii)敗血症性ショックとの関連性を認めるステップ。 - 敗血症患者における敗血症診断用の又はその予後予測用のキットであって、以下を含むキット:
(a)ATT又はその配列番号4,5,8及び10の断片、及びTTR、に対して特異性の抗体、及び
(b)サンプルに含まれるATT又はその配列番号4,5,8及び10の断片のレベルと予後との相関関係を示す標準データ、及びTTRのレベルと予後との相関関係を示す標準データ、及び任意選択で
(c)マニュアル。
