Description
胃への送気の後、構造支持部104の近位端を体外に残した状態で固定部品10を経皮的に胃の内部に挿入する。拡張用バルーン130を配置するが、このとき、固定用バルーン100の凹面内に収容されることを意図された拡張用バルーン130の端部132が、経皮的に胃内に挿入され、他方の端部134は構造支持部104の近位端を完全に覆わない形で体外に残されるように拡張用バルーン130を配置する。固定用バルーン100を膨張させると、牽引力(引っ張り力)が構造支持部104の近位端に加わり、膨張された固定用バルーン100の近位端105が胃に対して引っ張られる。拡張用バルーン130を膨張させ、挿入管を拡大して瘻孔を形成し、次いで収縮させる。リテーナ202が挿入状態(即ち、たたまれた、つぶれた、または収縮した状態)にある経腸栄養チューブ200を、構造支持部104、収縮した拡張用バルーン130上で挿入し、瘻孔を通して膨張された固定用バルーン100の凹面内に入れる。リテーナ202を展開して、凹面内で保持状態(広げられた、拡張された、または膨張された状態)とし、リテーナ202が胃壁に付くようにチューブ200を配置する。固定用バルーン100を収縮させて、牽引力を解放する。構造支持部104、固定部品10、及び他の配置用デバイス(例えば拡張用バルーン130)を、チューブ200を通して取り除く。
固定部品10を、従来型のガイドワイヤの助けによって経皮切開部を通して胃の内側から配置することができる。そのようなインサイド・アウト配置のためには、経皮切開部、胃、及び食道を通って口の外に出されたガイドワイヤを、構造支持部104の近位端に取り付ける。内視鏡のスネアを用いてガイドワイヤを引っ張ることもできる。ガイドワイヤを切開部を通して外向きに引っ張り、それで固定部品10を胃内に引き入れ、切開部を通して上述したように固定部品を配置する。装着手順の残りの部分は、上述の場合と同様である。
Claims (9)
- カテーテルを瘻孔を通して体腔に挿入するとき、該カテーテルを案内するための構造支持部を有する固定部品であって、
前記カテーテルは、使用時に前記体腔内で展開される固定用のリテーナを有し、
前記固定部品は、
遠位端及び近位端を有する膨張可能な固定用のバルーンを前記構造支持部上に備え、
固定用の前記バルーンは、少なくとも1つのカラーを前記遠位端または前記近位端において有し、
前記バルーンは、前記バルーンの前記近位端に凹部が形成される膨張時形状を有し、
前記バルーンは、体腔内で膨張したとき、体腔内で展開された前記カテーテルの前記固定用のリテーナを、前記凹部が受容するように構成されることを特徴とする固定部品。 - 前記バルーンは、円錐形及び半球形からなる群から選択された形状を有することを特徴とする請求項1に記載の固定部品。
- 前記カテーテルは、栄養チューブの部品であり、
前記バルーンは、頂部と基部とを備えた形状を有し、前記基部は患者の内側腹壁に接触し、前記基部は、前記リテーナより大きい幅を有することを特徴とする請求項1に記載の固定部品。 - 前記バルーンは、非柔軟または半柔軟であるポリマーから作られることを特徴とする請求項1に記載の固定部品。
- 前記バルーンに対して形状保持を与えるための、前記基部の一点から他の一点にわたる補強結合部を更に備えることを特徴とする請求項3に記載の固定部品。
- 固定用の前記バルーンに対してより大きな膨張時形状保持を与えるための、固定用の前記バルーンの周囲のスカート部を更に備えることを特徴とする請求項1に記載の固定部品。
- 前記バルーンが、前記頂部と前記基部との間に壁を有し、前記壁は前記頂部及び前記基部より硬いことを特徴とする請求項3に記載の固定部品。
- 前記カテーテルは、経腸栄養チューブの部品であり、
前記固定部品は、瘻孔を拡張するための別体の拡張用バルーンを更に含むことを特徴とする請求項1に記載の固定部品。 - 固定用の前記バルーンと前記拡張用バルーンとは、互いに一体に結合され、かつ別々の膨張用ルーメンを前記構造支持部内に有することを特徴とする請求項8に記載の固定部品。
