Description
本願は、2012年4月20日に出願された米国特許仮出願第61/636,492号(発明の名称:NATURAL ORIFICE MICROSURGERY SYSTEM)の権益主張出願であると共に2011年9月30日に出願された米国特許出願第13/250,398号(発明の名称:NATURAL ORIFICE MICROSURGERY SYSTEM)の一部継続出願であり、この米国特許出願は、2010年10月1日に出願された米国特許仮出願第61/389,091号(発明の名称:TRANSANAL ENDOSCOPIC MICROSURGERY SYSTEM)及び2011年5月12日に出願された米国特許仮出願第61/485,321号(発明の名称:TRANSANAL ENDOSCOPIC MICROSURGERY SYSTEM)の権益主張出願である。上述の出願を参照により引用し、これらの記載内容全体を本明細書の一部とする。
幾つかの実施形態では、ゲルパッド10530を製造するために用いられるトリブロックコポリマー/溶剤混合物/スラリは、約90重量%の鉱物油及び約10重量%のKRATON(登録商標)G1651を含む。熱力学的観点から、この混合物は、鉱物油と同様に挙動する。鉱物油は、比較的高い熱容量を有しているので、0.45kg(1ポンド)のスラリを約130℃で均質のゲルに変換するには、約3時間〜約4時間を要する場合がある。いったん形成されると、ゲルに対する見かけの悪影響を及ぼさないでできるだけ迅速に冷却するのが良い。幾つかの実施形態では、冷水浸漬によってゲルを冷却させる。他の実施形態では、ゲルを空冷する。当業者であれば認識されるように、他の実施形態では他の冷却技術が用いられる。
Claims (18)
- 生まれつき備わった開口のところで腹腔鏡下外科的処置を実施するようになった外科用アクセスポートシステムであって、前記外科用アクセスポートシステムは、レトラクタを含み、前記レトラクタは、
患者の生まれつき備わった開口の近くに配置されて該開口を実質的に包囲するよう構成された外側リングを有し、
少なくとも1つの腹腔鏡器械を挿通状態で受け入れるに足るほど大径の全体として円筒形の通路を備えた管状本体を有し、
前記外側リングと前記管状本体との間に延びると共に前記外側リングと前記管状本体を結合する漏斗セグメントを有し、前記漏斗セグメントは、前記レトラクタを体腔内に前進させるために用いられる栓塞子のための支承面となることができる内面を有し、さらに、前記外側リングの比較的大きな直径と前記開口の最小限の拡開状態で生まれつき備わった開口内に嵌まり込むよう寸法設定された前記管状本体の比較的小さな直径との間で直径の減少をもたらし、
前記漏斗セグメント内に設けられたインフレーションポートを有し、
前記管状本体の遠位縁部に設けられたインフレート可能な部材を有し、前記インフレート可能な部材は、チャネルによって前記インフレーションポートに連結され、
栓塞子であって、漏斗セグメントの内面に当接するような寸法形状の近位支承面と、近位支承面の周囲に沿って配置され且つインフレーションポートのための隙間をもたらすための寸法形状の凹みと、生まれつき備わった開口を拡張するような寸法形状の遠位拡開面と、を有する前記栓塞子を有している、外科用アクセスポートシステム。 - 前記管状本体は、実質的に軟質の材料から成る、請求項1記載の外科用アクセスポートシステム。
- 前記軟質材料は、KRATON(登録商標)材料、PELLETHANE(登録商標)材料及びシリコーンゴム材料から成る群から選択される、請求項2記載の外科用アクセスポートシステム。
- 前記管状本体は、ポリカーボネートから成る、請求項1記載の外科用アクセスポートシステム。
- 前記全体として円筒形の通路は、該通路を挿通状態で位置決めされた2つ又は3つ以上の外科器械を互いに対して並進させ又は回動させることができるほど十分に大径である、請求項1記載の外科用アクセスポートシステム。
- 前記管状本体は、1つ又は2つ以上の被膜を有する、請求項1記載の外科用アクセスポートシステム。
- 前記被膜は、抗菌性被膜から成る、請求項6記載の外科用アクセスポートシステム。
- 前記管状本体は、2つ又は3つ以上のインターロック区分を有する、請求項1記載の外科用アクセスポートシステム。
- 前記管状本体は、少なくとも1つの窓を有する、請求項1記載の外科用アクセスポートシステム。
- 前記管状本体は、少なくとも1つの小穴を有する、請求項1記載の外科用アクセスポートシステム。
- 前記漏斗セグメントは、前記比較的大きな直径と前記比較的小さな直径との間に実質的に直線状のテーパを有する、請求項1記載の外科用アクセスポートシステム。
- 前記漏斗セグメントは、前記比較的大きな直径と前記比較的小さな直径との間に湾曲したプロフィールを有する、請求項1記載の外科用アクセスポートシステム。
- 前記管状本体は、比較的軟質の材料で作られ、前記漏斗セグメント及び前記外側リングは、比較的硬質の材料で作られている、請求項1記載の外科用アクセスポートシステム。
- 取り外し可能なキャップを更に含み、前記キャップは、前記外側リングに密封的に係合するようになっている、請求項1記載の外科用アクセスポートシステム。
- 前記キャップは、封止可能なアクセス面部を有する、請求項14記載の外科用アクセスポートシステム。
- 前記封止可能なアクセス面部は、ゲルパッドである、請求項15記載の外科用アクセスポートシステム。
- 前記キャップは、少なくとも1つのガスポートを有する、請求項14記載の外科用アクセスポートシステム。
- 少なくとも1つのトロカール型アクセス器具を更に含み、前記トロカール型アクセス器具は、前記封止可能なアクセス面部を挿通して位置決めされるようになっている、請求項15記載の外科用アクセスポートシステム。
