Description

(c)ステップ(b)で測定された少なくとも3種の各マーカーのレベルを、少なくとも3種の各マーカーの基準レベルと比較すること;
を含む、推定的がん療法を同定または確認する方法であって、
ステップ(b)で測定されたがん幹細胞マーカー、間葉マーカー、幹細胞性マーカー、mTORC1マーカーおよびケモカインマーカーのレベルの減少、ならびにステップ(b)で測定された上皮マーカーの増加が、推定的がん療法ががんの治療に有効であることを示す方法。

本開示は、がん幹細胞の存在を検出する方法およびがんの予後におけるその使用、がん転移のリスクを評価する方法、薬物候補を同定または確認する方法、および治療効果を決定する方法を提供する。本開示はまた、がん幹細胞の存在を検出するのに有用なキットならびにCAIXインヒビターを用いるがんを治療する方法を提供する。
本明細書で用いる「バイオマーカー」または「マーカー」は、その存在、不在または量が、特定の細胞型もしくは細胞プロセスに特有であるか、または特定の細胞型もしくは細胞プロセスによって要求される分子(たとえば、遺伝子、mRNAsまたはタンパク質)を指す。本明細書で用いる「バイオマーカー」はまた、特定のシグナル経路の構成要素、または類似した機能を有するタンパク質のグループまたは該タンパク質のグループの遺伝子もしくはmRNAの構成要素を指す。
一つの態様において、本開示は、異なるタイプのマーカーの組合せを用いる、細胞の集団中のがん幹細胞の存在を検出する方法を提供する。該方法は、(a)異なるタイプのマーカーのレベルを測定すること、および(b)このようなレベルを基準レベルと比較し、それによって、細胞の集団中のがん幹細胞の存在または不在を決定することを含む。がん幹細胞は、マーカーの対応する基準レベルと比較して、がん幹細胞マーカー、間葉マーカー、幹細胞性マーカー、mTORC1マーカーおよび/またはケモカインマーカーのレベルが統計的に有意に増加する場合、あるいは上皮マーカーのレベルが統計的に有意に減少する場合に検出される。
もう一つの態様において、本開示は、推定的がん療法を同定または確認する方法を提供する。該方法は、(a)がん細胞を推定的がん療法に曝露して、処置されたがん細胞を得ること、(b)異なるタイプのマーカーのレベルを測定すること、および(c)このようなレベルを基準レベルと比較すること、それによって推定的がん療法を同定または確認することを含む。推定的がん療法は、もしがん細胞を推定的がん療法に曝露したにも、がん幹細胞マーカー、間葉マーカー、幹細胞性マーカー、mTORC1マーカーおよび/またはケモカインマーカーのレベルにおいて、これらのマーカーの対応する基準レベルと比較して、統計的に有意な減少、および/または上皮マーカーのレベルにおいて、このマーカーの対応する基準レベルと比較して、統計的に有意な増加があるならば、同定または確認される。
もう一つの態様において、本開示は、(a)本明細書に開示のバイオマーカーおよび方法を用いて、患者からのサンプル中のCSCの存在を検出すること;および(b)もし、がんサンプル中にCSCが検出されるならば、患者に有効量のCAIXインヒビターを投与すること;を含むがんを治療する方法を提供する。
最適用量は、一般に、実験モデルおよび/または臨床試験を用いて決定することができる。CAIXインヒビターの最適用量は、被験者の体質量(body mass)、体重または血液量に応じて変わる。たとえば、0.01 mg/kg〜1000 mg/kg(たとえば、約0.1〜1 mg/kg、約1〜10 mg/kg、約10-50 mg/kg、約50-100 mg/kg、約100-500 mg/kgまたは約500-1000 mg/kg)体重の量のCAIXインヒビターを投与することができる(単回用量として、1日1回、週1回、月1回または任意の適切な間隔で投与することができる)。
一つの態様において、本開示は、本明細書に開示するバイオマーカーに対して特異的結合親和性を有するタンパク質結合分子を含むキットを提供する。

1.細胞培養
マウス乳がん細胞株4T1を発現しているルシフェラーゼの収集、生成および培養は、記載されている(Lou、Preobrazhenskaら、2008)。以前の記載にしたがって(Ebos、Leeら、2009)、MDA-MB-231 LM2-4Luc+細胞株を、Dr. Robert Kerbel(University of Toronto、Canada)から豊富に提供を受け、培養した。加湿インキュベーター中、5% CO2下、37℃にてすべての細胞をインキュベートした(正常酸素状態)。
5%ウシ胎児血清(FBS)(Sigma)を含んでいるDMEM(Gibco)中、週2回の継代培養によって、培養した4T1細胞を単層として増殖させた。続いて、製造業者の使用説明書にしたがって、Mammocult(商標)培地(StemCell Technologies、Vancouver、B.C.、カナダ)中、親のshNSおよびshCAIX 4T1を腫瘍スフェア(TS)として培養した(Lock、McDonaldら、2012)。別法として、攪拌培養によりTSを増殖させるために、以前の記載にしたがって(Oloumi、MacPhailら、2000)、5% CO2に予備平衡させたDMEM(5% FBS)中、37℃、150 RPMにて定速回転下、密封したガラススピナーフラスコ培養フラスコ(Bellco、バインランド、NJ)中、 2X105〜4X105細胞/mlにて、単層細胞をトリプシン処理し、単細胞懸濁液をインキュベートした。培養物を3週間増殖させた。培地は、3日毎に補充した。低酸素状態のために、スピナーフラスコに、窒素でバランスさせた5% O2および5% CO2を3日間連続的に通気した。

4T1 TSをトリプシンとともにインキュベートし、HF緩衝液(2%ウシ胎児血清含有HBSS)で1回洗浄し、次いで、100μl HF中、0.3 μlの抗体/106細胞を用いて抗-CD24-APCおよび抗-CD44-PECy7で染色し、氷上にて10分間インキュベートした。インキュベーション後、HF緩衝液にて細胞を洗浄し、4’,6-ジアミノ-2-フェニルインドール(DAPI;最終濃度、1 μg/ml)を含んでいるHF緩衝液300μl中に再懸濁させた。Aria(商標)細胞選別機BD(Biosciences、サンノゼ、CA、USA)にて細胞を分離した。前方および側方スキャッターに基づいて生細胞をゲートし、前方スキャッターおよびパルス幅に基づいて単細胞をゲートした。ゲートは、無染色細胞、アイソタイプ特異的コントロールおよび単一染色の分析によって決定した。得られた細胞の数が少なかったので、CD44+CD24-/lowまたはCD44+CD24+細胞の純度は評価しなかった。フローサイトメーターの細胞カウンターを用いて、細胞数を決定した。細胞をDMEM培地またはHF緩衝液に採取した。
以前の記載にしたがって(Turner、Broadら、2006)、ガラスカラースリップ上で培養した4T1細胞を固定し、透過性にし、免疫染色した。親4T1の攪拌培養TSを採取し、ドライアイス上のOCT凍結培地に懸濁した。凍結したら、クライオスター(商標)HM560クライオスタット(Microm International, GmbH、ウォルドルフ、ドイツ)を用いて5〜7 μmの切片を調製する。凍結切片を30秒間風乾し、固定のために4%パラホルムアルデヒドに移して20分間置き、PBS(5分間)で3回洗浄し、0.2% トリトン-X-100(10分間)で透過性にした。続いての免疫染色ステップは、以前に記載されている(Lock and Hotchin 2009)。
FACS選別細胞を72時間培養し、低酸素条件下で細胞溶解した。製造業者の使用説明書にしたがって、QIAGEN RNAeasy(商標)マイクロキット(トロント、オンタリオ、カナダ)を用いて、mRNA抽出を行った。Applied Biosystems(フォスターシティ、CA、USA)7900HTアナライザーにおいてPreAMP RT2マウス上皮間葉移行(EMT)PCRアレイ(PAMM-090、Qiagen)を用い、5〜10 ngのRNAを出発物質として、定量的PCRを行った。標準2-ΔΔct法(Livak and Schmittgen 2001)を用いることによって、遺伝子発現の相対的倍率変化を計算した。
インビボ研究のために、以前の記載にしたがって(Lou、McDonaldら、2011、Supuran 2008)、CAIX特異的インヒビターCAI017(Supuran 2008)

すべての動物手順および研究は、BC Cancer Research Centreの実験動物委員会およびブリティッシュコロンビア大学(バンクーバー、BC、カナダ)によって認可されたプロトコルにしたがって行った。以前の記載にしたがって(Lou、McDonaldら、2011)、原発性乳房腫瘍異種移植のために、2x106個のMDA-MB-231 LM2-4Luc+変異細胞を、50%マトリゲル(商標)/PBS溶液に懸濁し、6〜8週齢の雌性NOD.CB17-prkdcscid/Jマウスに同所移植した。腫瘍が200 mm3に達したときに、療法を開始した。修正楕円体の式(LxW2)/2を用い、キャリパー測定から原発性腫瘍増殖速度を計算した。以前の記載にしたがって(Lou、McDonaldら、2011)、腫瘍生成および転移の進行を、生物発光の画像化を使用して、モニターし、定量した。
以前の記載にしたがって(Lou、McDonaldら、2011)、腫瘍を採取し、パラフィン包埋し、CAIX(Santa Cruz Biotechnology)について腫瘍切片を染色した。
GraphPad(商標)ソフトウェアを用い、両側T-検定および記述統計を行った。他に特記しない限り、誤差バーは、3つの独立した実験の最小に対するSEMを表す。
翌日80%の集密度に達するように4T1-shNSおよび4T1-shCAIX細胞を播種した。PBSで細胞を2回洗浄し、次いで、無血清培地を細胞に加えた。21%または1% O2中で24時間細胞をインキュベートした後、培地を除去し、それぞれのO2張力において細胞溶解した。RNeasyミニキット(QIAGEN、トロント、オンタリオ、カナダ)を用い、製造業者の使用説明書にしたがって、細胞溶解物からmRNAを抽出した。ナノドロップによって、RNA量を決定し、2%アガロースゲル上の電気泳動によってインテグリティを決定した。
Superscript III First-Strand合成キット(Life Technologies、カールズバッド、カリフォルニア、USA)を用いるcDNAの製造において、各サンプルからの全RNA500 ngを用いた。Applied Biosystems 7900HT(フォスターシティ、カリフォルニア、USA)においてFAST SYBR Green MasterミックスリアルタイムqPCRを用いてDNAの増幅を行った。式:q=E^-CT(式中、Eはプライマー効率を表し、CTは閾値サイクルを表す)を用い、個々の遺伝子のレベルの定量を行った(Rebollo、Miceli-Royerら、2012)。
上述のとおり、4T1-shNSおよび4T1-shCAIX細胞を増殖させた。24時間後、それぞれのO2張力において培地を採取し、4℃にて1500 rpmで5分間の遠心分離によって清澄にした。0.2 μmシリンジフィルターを通して培地をろ過し、-80℃にて保管した。
製造業者の使用説明書にしたがって、マウス特異的CXCL10およびG-CSF Quantikine(登録商標)ELISAキット(R and D Systems、ミネアポリス、ミネソタ、USA)を用い、ならし培地から分泌されたケモカインレベルを評価した。総タンパク質レベルに基づいて、ならし培地を標準化した。
非接着性腫瘍スフェアとしての、無血清条件下での懸濁液中のインビトロ増殖が、乳がん細胞の特徴であることが実証されている(Ponti、Costaら、2005)。
これらのデータを確認するために、十分に特徴付けられたCAIX小分子インヒビターであるCAI017を用いて、親4T1細胞株におけるCAIX活性を阻害した(Lou、McDonaldら、2011、Supuran 2008)。実施例1と同じ実験方法を用いて、親4T1の単細胞懸濁液を、規定の無血清培地に2倍希釈にて播種し、CAI17またはビヒクルを加え、正常酸素状態または低酸素状態において、非接着性TSとして7日間培養した。今回も、TSを形成するための親4T1細胞の数は、正常酸素状態コントロールと比較して、低酸素状態において、有意に減少し、CAIX活性の阻害は、低酸素状態においてTSを形成するのに必要な播種細胞の数を有意に増加させたが、このことは、CAIX活性単独で、この細胞株においてがん幹細胞様集団を調節しうることを示唆する(図6)。
親4T1細胞を、正常酸素状態および低酸素状態において、腫瘍スフェア(TS)形成条件下で培養した。7日後、TSを採取し、解離させ、CD44およびCD24について細胞染色した。CD44+CD24-/low細胞またはコントロールCD44+CD24+集団を、正常酸素または低酸素状態にて、ガラスカバースリップ上で3日間培養し、次いで、固定し、CAIX(図9)またはEMTマーカー(図8、9および10)について免疫染色した(Lock、McDonaldら、2012)。CD44+CD24-/lowおよびCD44+CD24+コントロール集団の両方において、低酸素培養物におけるCAIXの発現は、上方調節された。正常酸素培養物においては有意なCAIX染色は観察されなかった(図9)。正常酸素状態および低酸素状態において、CD44+CD24+コントロール集団の細胞境界では、有意なE-カドヘリン発現が観察されたが、正常酸素状態および低酸素状態において、CD44+CD24-/low細胞細胞においては、上皮マーカーであるE-カドヘリンの発現の有意な喪失が観察された(図10)。CD44+CD24-/low間葉表現型のさらなる証拠は、CD44-CD24+コントロール集団におけるごくわずかな染色と比較して、CD44+CD24-/low 4T1細胞において観察された間葉マーカーである平滑筋アクチンが上方調節されることによって確認された(図11)。Snailについての免疫蛍光染色もまた、コントロールCD44+CD24+集団において染色が少ないことと比較して、正常酸素状態および低酸素状態のCD44+CD24-/low集団においてこのEMT調節因子が上方調節されることを実証した(図12)。
がん細胞のCSC「幹細胞性」を維持することにおけるCAIX発現のための要件を、EMT表現型を評価することによってさらに分析した。shNSおよびshCAIX 4T1細胞を、低酸素状態において7日間培養し、解離させ、CD44+CD24-/lowおよびコントロール集団をFACSによって採取した。細胞を、低酸素状態において、ガラスカバースリップ上で3日間培養し、固定し、IFによって評価した(Lock、McDonaldら、2012)。上皮マーカーであるE-カドヘリンの発現は、コントロールと比較して、shNS CD44+CD24-/low細胞において有意に減少し(図16)、これは、shNS CD44+CD24-/low細胞、が4T1 および/またはやCD44+CD24-/low集団と同様に、EMTを受けることを示唆する。対照的に、shCAIX CD44+CD24-/low細胞は、shCAIX CD44+CD24+ control細胞に匹敵するE-カドヘリンを発現し続ける(図16)。
インビボでの腫瘍増殖およびがん幹細胞拡張におけるCAIX阻害の効果を評価するために、MDA-MB-231細胞株のの変異体であるMDA-MB-231 LM2-4Luc+を用いて、原発性乳房腫瘍異種移植片を作った。これらの細胞は、インビボにおいて転移性が高く、低酸素状態においてCAIX発現を強く誘発する。細胞を同所的に移植し、腫瘍形成が確認されたら、2つの以前に述べたCAIX特異的インヒビターであるグリコシルクマリンMST-205およびウレイド-スルホンアミドMST-104でマウスを処置した(Lou、McDonaldら、2011、Lock、McDonaldら、2012、Pacchiano、Cartaら、2011)。キャリパー測定およびIVIS画像化によって腫瘍増殖を評価した。これまでの報告と同様に、腫瘍体積測定は、ビヒクルコントロールと比較して、いずれかのインヒビターで処置されたマウスにおいて原発性腫瘍増殖の有意な阻害を示した(図21)(Lou、McDonaldら、2011)。
選択されたEpCAM+ MDA-MB-231 LM2-4細胞が腫瘍を形成するかどうかを試験するために、我々は、それぞれ50、500および5000個のEpCAM+またはEpCAM- MDA-MB-231 LM2-4細胞をNOD/SCIDマウスの乳房脂肪体に注入した(50,000個のEpCAM-ve細胞もまた注入した)。腫瘍形成が、2、3日以内に、非常に少数のEpCAM+細胞の出発集団から観察され得たので、EpCAM+細胞は、腫瘍原性が高かった。EpCAM-細胞もまた、より多くの細胞が注射された場合に腫瘍を形成することができたが、EpCAM誘導腫瘍の体積は、常に、EpCAM+誘導腫瘍の大きさよりもはるかに小さかった(Lock、McDonaldら、2012)。腫瘍進行速度は、EpCAM-誘導腫瘍において、EpCAM+誘導腫瘍よりも遅かったが、これは、EpCAM+細胞が、より侵攻性の腫瘍を形成することを示している。
パクリタキセルは、迅速に分割する腫瘍細胞を標的とする強力な有糸分裂インヒビターであり、標準的乳がん療法のため現在の第一選択化学療法薬である。腫瘍を有するマウスにおけるパクリタキセル処置の有無によるCAIX特異的インヒビター処置の効力が研究された(Lock、McDonaldら、2012)。サンプル結果の記載:NOD-SCIDマウス乳房脂肪体をLM24乳がん細胞とともにインキュベートした。10日後、マウスにパクリタキセルまたはMST-104を、あるいは両方の薬物を併用で、あるいはビヒクルを投与した。キャリパー測定によって腫瘍体積を評価し(図26)、肺転移のIVIS画像化を図27に示す。パクリタキセルと併用でのCAIXインヒビター処置は、NOD-SCIDマウスにおいて、LM 2-4腫瘍増殖および転移を抑制した(図26−28)。人道的な終点において、腫瘍を切除し、FACSによって生存しているEpCAMポジティブ集団を評価した(図29A−29E)。図30に示すように、パクリタキセル単独による処置は、ビヒクルコントロールと比較して、EpCAM+ CSCの平均パーセンテージの増加をもたらした。MST-104単独による処置は、EpCAM+集団において有意な現象をもたらした。重要なことには、パクリタキセルと併用でのMST-104処置は、ビヒクル処置マウスに見られるのと同様のレベルまで、EpCAM+集団を救助した(図30)。これらのデータは、腫瘍サイズおよびCSC集団の調節における、CAIX特異的阻害の臨床的可能性を実証する。
正常酸素状態および低酸素状態において培養した4T1 TS細胞溶解液にてmTORC1シグナル伝達を評価した(Lock、McDonaldら、2012)(図31)。低酸素状態におけるCAIXの枯渇は、mTORC1の構成要素であるmTORおよびRaptorの発現を減少させ、翻訳調節因子4EBP1への下流シグナル伝達を阻害し(図31)、4EBP1リン酸化の減少および移動度の増加をもたらした(矢印)(図31)。これらのデータは、mTORおよびその下流エフェクターが、低酸素状態においてCAIXに依存していることを実証する。低酸素状態においてCAIXインヒビターとともに培養された親4T1 TSにおけるCAIX阻害後に、同様の結果が観察され(図32)、このことは、mTOR経路の構成要素が、がん細胞によるCAIX機能の阻害への反応のバイオマーカーとして使用されうることを実証する。
十分に確立された乳がんの4T1モデルを用いて、CAIXの枯渇が転移をブロックし、最終的に、腫瘍体積の縮小をもたらすことが明らかにされている(Lou、McDonaldら、2011)。正常酸素状態および低酸素状態において増殖させた4T1 shNSおよびshCAIX細胞からのmRNAレベルの分析は、CAIXを標的とするXshRNA配列が、CAIXメッセージを減少させることにおいて非常に有効であることを明らかにしたが(図34)、これによってタンパク質レベルにおけるこれまでの分析が実証される(Lou、McDonaldら、2011)。CAIXの枯渇は、腫瘍細胞によって生成され、転移に関与する、「活性,正常T細胞の発現および分泌を調節するケモカイン(RANTES)」、「顆粒球コロニー刺激因子(G-CSF)」、「CXCケモカイン受容体3(CXCR3)」および「CXCL10」(図34)などのいくつかのケモカインのmRNAレベルの有意な減少をもたらした(Yu、Kortylewskiら、2007)。CAIXのサイレンシングもまた、胎盤増殖因子(PlGF)のmRNAレベルの減少をもたらした。さらに、CAIXレベルの減少は、IL-1αメッセージの減少をもたらさなかったが、これは上述のケモカインにおける効果が特異的であることを実証する。G-CSFおよびCXCL10 mRNAレベルの減少は、ELISAによって、タンパク質レベルにおいてさらに確認された(図35および36)。低酸素状態において増殖させた4T1 shCAIX細胞から単離されたならし培地中のCXCL10(図35)およびG-CSF(図36)の両方のタンパク質レベルは、同じ条件で培養されたノンサイレンシングコントロール細胞よりも有意に低かった。これらのデータは、低酸素状態におけるがん細胞によるCAIX発現が、腫瘍の微小環境に役立つ分泌された因子のプールに影響を及ぼすことを示唆し、これらの因子は、がん細胞によるCAIX発現および/または活性の阻害に対する応答のバイオマーカーである。

- がん幹細胞マーカー、間葉マーカー、上皮マーカー、幹細胞性マーカー、mTORC1マーカーおよびケモカインマーカーから選ばれる少なくとも5種のマーカーに対して特異的なオリゴヌクレオチドプライマーまたはプローブを用いてマーカーのレベルを測定することを含む、請求項1または2に記載の方法。
