Description
本発明は、過敏性腸症候群(IBS)および炎症性腸疾患(IBD)と関連する細胞バリア機能不全を検出するための方法および組成物に関する。
過敏性腸症候群(IBS)は、便通が頻繁であること(bowel frequency)、軟らかさおよび腹部不快感の変化によって特徴付けられる一般的な臨床状態である。Rome II基準を使用した疫学研究から、IBSの有病率は、北米で5%〜12%、アジアで1%〜22%、および欧州で1〜8%まで変動することが示されている。女性に多いことが、大部分の研究(特に、西洋諸国の)において認められる。IBSの主要な要因(driver)のうちの1つは、異常な腸上皮細胞(IEC)押し出しであり得る。
本発明は一実施形態において、患者における過敏性腸症候群(IBS)もしくは炎症性腸疾患(IBD)を検出するための方法であって、該方法は、(a)患者の腸上皮細胞、中咽頭上皮細胞、もしくは口腔上皮細胞を、カスパーゼ−1インヒビターに結合体化した検出可能なマーカーを有するプローブで染色する工程、および(b)該染色された腸上皮細胞、中咽頭上皮細胞、もしくは口腔上皮細胞を、該患者の腸上皮細胞、中咽頭上皮細胞、もしくは口腔上皮細胞と関連したカスパーゼ−1の正常以上のレベルの証拠として、それぞれ正常な個体に由来する同様に染色された腸上皮細胞、中咽頭上皮細胞、もしくは口腔上皮細胞と比較して上昇したレベルの検出可能なマーカーの存在について試験する工程による、方法を包含する。
特定の実施形態では、例えば以下が提供される:
(項目1)
患者における過敏性腸症候群(IBS)もしくは炎症性腸疾患(IBD)を検出するための方法であって、該方法は、
(a)患者の腸上皮細胞、中咽頭上皮細胞、もしくは口腔上皮細胞を、カスパーゼ−1インヒビターに結合体化した検出可能なマーカーを有するプローブで染色する工程、および
(b)該染色された腸上皮細胞、中咽頭上皮細胞、もしくは口腔上皮細胞を、該患者の腸上皮細胞、中咽頭上皮細胞、もしくは口腔上皮細胞と関連したカスパーゼ−1の正常以上のレベルの証拠として、それぞれ正常な個体に由来する同様に染色された腸上皮細胞、中咽頭上皮細胞、もしくは口腔上皮細胞と比較して上昇したレベルの検出可能なマーカーの存在について試験する工程

(c)ここで該患者の腸上皮細胞、中咽頭上皮細胞、もしくは口腔上皮細胞におけるカスパーゼ−1の上昇したレベルは、該患者における過敏性腸症候群(IBS)もしくは炎症性腸疾患(IBD)と関連する細胞バリア機能不全の指標である、

(項目2)
前記染色する工程は、(i)前記患者から、生検もしくは吸引によって患者の腸上皮細胞を得る工程、および(ii)該細胞をインビトロで染色する工程を包含する、項目1に記載の方法。
(項目3)
前記染色する工程は、(i)前記患者から、歯科生検によって中咽頭上皮細胞を得る工程、および(ii)該細胞をインビトロで染色する工程を包含する、項目1に記載の方法。

A.カスパーゼ−1染色によって細胞バリア機能不全を検出する方法
A1.カスパーゼ−1媒介性IEC押し出しは、上皮単層の破れ目を生じる
インビボでの腸の透過性に対するカスパーゼ−1誘発性IEC押し出しの効果を理解するために、本発明者らは、129/SvEv(WT)マウスと比較して、(上皮ギャップの増大した密度によって測定される場合の)IL−10 KOにおいて認められた増大した細胞押し出しが、増大したカスパーゼ−1活性化に起因するか否かを最初に試験した。IL−10 KOマウスの小腸における活性カスパーゼ−1発現の増大は、ウェスタンブロット分析(図3a)で認められ、ELISAによる増大した活性IL−1β発現(図3b)とともに確認された。IL−10 KOにおける低下した細胞増殖が、観察された上皮ギャップ密度の差異に寄与したか否かを決定するために、本発明者らは、2種類のマウス系統に由来する腸サンプルをPCNAで染色した。IL−10 KOは、WTマウスと比較して、細胞増殖において38%低下を有した(図3cおよび図3d)。
IECのカスパーゼ−1活性化がヒトにおける細胞押し出しの主要な機構を代表するか否かを調査するために、本発明者らは、IBD患者および無症候性コントロール患者の正常に見える回腸末端部から、粘膜生検物および管腔吸引物を集めた。粘膜生検サンプルを、FLICA−1および3&7で染色して、活性化カスパーゼ−1染色(ピロトーシス性)もしくはカスパーゼ−3&7染色(アポトーシス性)に対して陽性のIECを同定した(図6a)。コントロールにおける陽性染色されたカスパーゼ−1細胞 対 カスパーゼ−3&7細胞の比は、1.16:1であった;これは、IBD患者では1.7:1に増大していた(図6b)。管腔吸引物の分析に関しては、コントロール患者は、細胞学的ブロック(cytology block)調製のための材料を不十分にしか有しなかった。IBD患者では、細胞学的ブロックで見られた有核細胞の総数は、12〜155個の細胞に及び、IECは、上記細胞のうちの41〜100%を占めた(図6c)。本発明者らは、フィルター上に集められた管腔吸引物中の押し出された細胞の総数を定量した:コントロール患者と比較してIBD患者由来の管腔吸引物では、有意により多い細胞数が認められた(図6d)。押し出された細胞および細胞破片を、活性化カスパーゼ−1および3&7に関してFLICAで染色した。FLICA染色した管腔吸引物の画像を、組織サンプルに使用される等級付けスケールと同様に、2つの膜上に存在する細胞および細胞破片のカスパーゼ染色の強度に基づいてスコア付けした。各画像に、染色強度および染色された細胞もしくは細胞破片の数に応じて、スコア0〜4を割り当てた。このスコア付けシステムを使用して、コントロールにおける陽性染色されたカスパーゼ−1細胞 対 カスパーゼ−3&7細胞の比は、約1:1であった。これは、IBD患者では2:1へと増大した(図6e)。
OECのカスパーゼ−1活性化がクローン病の診断であるか否かを決定するために、本発明者らは、標準的手順を使用して、正常個体およびクローン病個体の中咽頭領域の歯科生検物を得た。生検で得られた上皮細胞を、上記のように、カスパーゼ−1マーカーを用いてインビトロで染色し、蛍光顕微鏡法によって検鏡して、カスパーゼ−1レベルを決定した。図11における棒グラフから認められるように、クローン病患者におけるカスパーゼ(capase)−1レベルは、正常レベルの約2倍に上昇した。
合計35名の患者(IBSを有する17名および18名のコントロール)を研究に採用し、IBSを有すると考えられた1名の患者を、大腸生検での顕微鏡的大腸炎(microscopic colitis)の存在に起因して、さらなる分析から排除した。ベースラインの患者特徴を、表1に示す。16名のIBS患者の平均年齢は、42.8±18.5歳であった。7名の女性患者および9名の男性患者がいた。コントロール患者(n=18)は、平均年齢47.4±10.1歳であり、10名の女性患者および8名の男性患者がいた。コントロールにおける結腸内視鏡検査の適応症は、結腸直腸がんスクリーニング(n=9)および直腸出血もしくは便潜血検査陽性(n=9)であった。IBS群は、12名の下痢型IBS患者および4名の便秘型IBSを含んでいた。彼らの症状の他の原因の評価に関しては、本発明者らは、全ての患者に対して詳細な病歴を訊いて、ラクトース/フルクトース不耐症を排除した。1名の下痢型IBS患者を除く他のすべての患者については、血清抗体(抗tTGもしくは抗筋内膜抗体)もしくは生検を伴うEGDを行って、セリアック病を除外した。血清検査もEGDも行わなかった1名の患者は、セリアック病の低リスク群にいた。2名の患者を除く他のすべての患者については、血清TSHをチェックして、彼らの症状の原因として甲状腺機能障害を排除した。通常の大腸内視鏡検査は、IBS患者およびコントロール患者の両方における最も一般的な内視鏡検査所見であった。他の所見は、ポリープ(n=8)、憩室症(n=4)および痔(n=8)であった。全てのIBS患者およびコントロールで行った回腸末端部および結腸のランダム生検は、正常範囲内であった。計数において使用した3連続の視野でのコントロール患者およびIBS患者の代表的なpCLE画像を、図2に示す。
マウス
少なくとも10世代にわたって本発明者らの動物施設で交配した、24〜28週齢の間のIL−10 KOマウス(Jackson Laboratories, Bar Harbor, Maine)およびバックグラウンド129/SvEvマウス(Taconic Farms Inc. Cambridge City, Indiana)を、全ての実験に使用した。マウスを従来の明暗サイクルの飼育施設の中で飼育した。動物プロトコルは、University of AlbertaのAnimal Care and Use Committee for Health Sciencesによって承認された。
研究プロトコルは、University of Albertaのthe Human Ethics Research Review Boardによって検討および承認され、その研究は、ClinicalTrial.Gov(NCT00988273)で登録された。結腸内視鏡検査を受ける患者は、上記研究に参加する書面によるインフォームドコンセントを提供した。結腸内視鏡検査を受けるIBD患者(N=11、6人のクローン病患者、5人の潰瘍性大腸炎患者)および無症候性コントロール患者(N=8)において、正常に見える回腸末端部からの管腔吸引物を、以前に記載された方法7を使用して0.9% NaCl溶液で腸表面を穏やかに洗浄した後に集め、直ぐに(<15分)分析した。細胞学的ブロックを、食塩水洗浄後に集めた25mLの管腔吸引物から調製し、上皮細胞もしくは免疫細胞の形態同定のために、ヘマトキシリンとエオシンで染色した。FLICA染色のために、吸引液のうちの5mLに由来する細胞を、5.0μM 孔サイズを有する25mmポリカーボネートMembra−fil Nucleopore膜(Whatman, GE Healthcare Life Sciences, Piscataway, NJ)上に、真空濾過を使用して固定化し、0.5%(w/v) BSAを含むさらに20mLのPBS(pH7.4)の濾過によって洗浄した。蛍光活性な部位特異的不可逆的インヒビター特異的活性化カスパーゼ−1およびカスパーゼ−3&7(カルボキシフルオレセインFLICAアポトーシス検出キット;Immunochemistry Technologies LLC, Bloomington, MN)を使用して、Nucleopore膜上で吸引した細胞を直接染色した。固定化した吸引細胞を有する膜を、半分にカットし、FAM−YVAD−FMK(FLICA−1)染料の1:700希釈物で染色して、活性化カスパーゼ−1を検出するか、またはFAM−DEVD−FMK(FLICA 3&7)染料の1:700希釈物で染色して、活性化カスパーゼ−3&7を検出した。正常に見える回腸末端部に由来する4つの粘膜生検サンプルを、分析のために得(コントロール N=3, IBD N=3)、2つの生検サンプルを、液体窒素の中に入れ、サイトカインアッセイに使用するまで−80℃で貯蔵した。2つの生検サンプルを、OCT(Tissue−Tek, Torrence, CA)の中に包埋し、液体窒素の中に入れ、切片を調製するまで−80℃で貯蔵した。
ニゲリシン(Invitrogen, Burlington, ON)、Ac−YVAD−CMK(Alexis Biochemicals, Farmingdale, NY)、種々の直径(0.5〜6μm)のFluoresbriteTM Yellow Green Carboxylate Microspheres(Polysciences Inc, Warrington, PA)を購入した。IV型ピリ線毛を、Pseudomonas aeruginosa K株から以前に記載された方法で調製し、SDS−PAGEによる純度、アシアロ−GM1に結合するがGM1には結合しない能力、およびステンレス鋼に結合する能力に関して特徴付けした。上記ピリ線毛調製物は、銀染色したSDS−PAGEゲル上で検出可能でなかった少量のP.aeruginosa血清型05 LPSを含んだ。Escherichia coli TMW2.497は、Dr. M. Gantzleの厚意であったプラスミドpQBI−63上に緑色蛍光タンパク質(GFP)をコードする遺伝子を有するE.coli JM109誘導体であった。
T84ヒト結腸がん上皮細胞を、ダルベッコ最小必須培地(DMEM)/F−12、10% (v/v) 熱非働化ウシ胎仔血清(FBS)、1%(w/v) ペニシリン−ストレプトマイシン中で、組織培養プレート(10cm)で維持した。その細胞をTranswells(2×105 細胞/ウェル、6.5mm直径; 0.4μm孔サイズ; Corning Life Sciences, Tewksbury, MA)にプレートし、研究のために頂端細胞接着複合体(apical junctional complex)(経上皮抵抗>2,000Ω・cm2によって示される場合)の発生まで増殖させた。カスパーゼ−1阻害実験に関しては、ニゲリシン処理前に、組織培養培地を除去し、50□M カスパーゼ−1インヒビター(Ac−YVAD−CMK)を含む新たな培地を導入した。ニゲリシン(10、25、50□M)を、Transwellの頂端側および基底側の両方に添加した。経上皮抵抗(TER)を、Millicell−ERS Voltmeterおよび箸型電極(Millipore, Bedford, MA)を使用して、ニゲリシン処理の前および処理3時間後に測定した。ミクロスフェアおよびE.coli実験のために、ニゲリシンとともに一晩インキュベートした後、107/mlの1μm ミクロスフェアもしくは109/ml E.coli TMW2.497を、Transwellの頂端側に添加した。ミクロスフェアもしくはE.coliとともにインキュベートして1時間後、細胞を冷メタノール中で5分間固定した。次いで、細胞を0.2%(v/v) Triton X−100で15分間透過性にし、0.2% (v/v) ヤギ血清および1%(w/v) BSAを含むPBS中で1時間ブロックした。
ヒト生検サンプルおよび齧歯類回腸組織を、溶解緩衝液(0.01M PBS、0.5% (v/v) Tween 20、およびHaltプロテアーゼインヒビター(ジメチルスルホキシドおよび4−(2−アミノエチル)−ベンゼンスルホニルフルオリド(Thermo Scientific, Pittsburgh, PA)を含む))中、氷の上でタンパク質抽出のためにホモジナイズした。タンパク質含有上清を、13,000gで30分間、4℃での遠心分離によって分離し、分析まで−70℃で貯蔵した。
ヒトサンプル由来の活性IL−1βの濃度を、Human IL−1β Ultra−Sensitive Kit(Meso Scale Discovery, Gaithersburg, MD)で測定した。マウス腸組織における活性IL−1β発現を、Mouse ProInflammatory 7−Plex Ultra−Sensitive Kit(Meso Scale Discovery, Gaithersburg, MD)で測定した。得られたサイトカインを、ビシンコニン酸アッセイ(Pierce, Rockford, IL)によって決定される場合の各個々のサンプルの総タンパク質含有量に対して正規化した。
ヒト生検組織、マウス回腸粘膜をこすり取ったものおよびT84細胞を、プロテアーゼインヒビターを含むM−PER Mammalian Protein Extraction Reagent(Thermo Scientific, Pittsburgh, PA)中に溶解した。総細胞溶解物(サンプルに対して正規化した50μg タンパク質)を、15% SDS-PAGEゲルに載せ、その後、ニトロセルロース膜へのタンパク質の電気泳動転写を行った。膜をODYSSEYブロッキング緩衝液(Infrared Imaging System, Marysville, OH)で1時間、室温(RT)においてブロックし、ローディングコントロールとして働くβ−アクチン抗体(Cell Signaling Technology, Danvers, MA)とともに、4℃において一晩、IL−1β抗体(Cell Signaling Technology, Danvers, MA)もしくはカスパーゼ−1抗体(Abcam, Cambridge, MA)でプローブした。洗浄後、膜を蛍光二次抗体とともに1時間RTにおいてインキュベートし、LI−COR Odyssey*(Infrared Imaging System, Marysville, OH)によって分析した。
カスパーゼ−1活性化および透過性実験からの細胞培養サンプルを、冷メタノール中で5分間固定し、ウサギ抗ZO−1一次抗体(Invitrogen, Burlington, ON)とともに一晩、4℃においてインキュベートした。洗浄後、細胞を、カスパーゼ−1活性化に関するFLICA−1染料もしくはヤギ抗ウサギIgG Alex546抗体(Invitrogen, Burlington, ON)いずれかの1:150希釈物とともにインキュベートし、DAPIで対比染色した。次いで、単層を支持している膜を切り出し、スライドガラス(DakoCytomation Mounting Medium, Carpentaria, CA)に載せた。凍結ヒト生検サンプルを、5μmの切片にし、風乾し、アセトン固定し、その後、活性化カスパーゼ1に関してはFLICA−1の1:50希釈物で、および活性化カスパーゼ−3&7に関してはFLICA 3&7(Immunochemistry Technologies LLC, Bloomington, MN)の1:50希釈物で染色した。次いで、切片を4% パラホルムアルデヒドで15分間、RTにおいて後固定し、F−アクチンに関してはローダミン−ファロイジン(Invitrogen, Burlington, ON)で、および核に関してはDAPIで染色した。
マウス回腸末端部組織を、以前に公開された方法を使用して、ウサギ抗PCNA抗体(Abcam, Cambridge, MA)で染色した。PCNAに関する染色後に、その切片をDAPIで染色し、Zeiss倒立顕微鏡法(Zeiss, Toronto, Ontario)で画像化した。PCNA陽性細胞を、最低限でも動物1匹あたり5つのピリ絨毛において2名の盲検状態の評価者に計数させた。
インビボ透過性を、FITC−デキストランの透過、蛍光ミクロスフェアおよび細菌移動研究で評価した。デキストラン研究のために、自由に水を利用させて一晩絶食させた後、マウスに0.6μg/kg FITC−デキストラン(FD−4, 4kD; Sigma Aldrich, St. Louis, MO)を強制投与した。心臓穿刺後4時間で血液サンプルを集め、血清を1,957×gで4℃において20分間遠心分離にかけた。上清の蛍光発光を、Typhoon Variable Mode Imager(GE Healthcare, Piscataway, NJ)で488nmレーザーを使用して測定した。
マウス回腸の共焦点レーザー内視鏡検査および上皮ギャップ密度の決定のためのホールマウントマウス腸組織の共焦点顕微鏡検査を、以前に記載された方法を使用して行った。細胞培養、ヒトおよびマウスの腸スライドを、スピニングディスク型共焦点顕微鏡(Quorum Technologies Inc, Guelph, ON)を使用して、以前に記載された方法を使用して画像化した。
コントロールおよびニゲリシン処理したT84細胞を、0.1M カコジル酸−HCl(pH7.4)で一晩4℃において緩衝化した2%(v/v) グルタルアルデヒドを用いて固定化した。固定の後、それらをカコジル酸緩衝液中で洗浄し、1%(w/v) 四酸化オスミウム中で2時間後固定し、次いで、カコジル酸緩衝液で再洗浄した。段階的な一連のエタノール濃度で脱水した後、標本をプロピレンオキシドのいくつかの洗浄液中に入れ、その後、エポキシ樹脂(EPON 12)に包埋した。超薄切片を、酢酸ウラシルおよびクエン酸鉛でコントラストを上げ、加速電圧60kVでHitashi 7650透過型電子顕微鏡で検鏡した。視野を記録し、1000〜4800の最終倍率でプリントし、カーボン格子レプリカの助けを借りて較正した。
GraphPad(La Jolla, CA) Prism 4によって計算したWilcoxonランクサム検定を使用して、サンプルを比較した。0.05未満の両側P値を、有意とみなした。多重比較のためにBonferroni調整を行った。
方法
これは、ClinicalTrial.Gov (NCT00988273)で登録された前向きコホート研究であった。研究プロトコルは、University of AlbertaのHuman Ethics Research Review Boardによって検討および承認された。研究群は、Rome III基準に基づいて、IBSと一致する症状を有する患者からなった。コントロール群は、他の適応症について結腸内視鏡検査を受けており、IBSの症状、最も一般的には、結腸直腸がんスクリーニングおよび便潜血検査陽性を有しない患者からなった。研究の包含基準は、以下であった:18歳齢より上で書面によるインフォームドコンセントを与える能力がある患者。排除基準は、以下を含んだ:フルオレセインもしくは貝類に対するアレルギーのあることが既知、腎臓機能が損なわれている(血清クレアチニンが1.5mg/dLを超える)、および妊娠もしくは授乳している患者。全ての患者は、研究参加への書面によるインフォームドコンセントを与えた。患者の人口統計学、病歴、身体検査所見、および内視鏡検査所見を、前向きデータベースに記録した。
サンプルサイズ計算:
サンプルサイズ計算を、本発明者らの以前の研究24からの無症候性患者およびIBS患者の上皮ギャップ密度データに基づいて行った。10ギャップ/1000細胞という平均ギャップ密度の差異および10ギャップ/1000細胞という標準偏差を想定すると、合計32名の患者(1群あたり16名)が、タイプI誤差(α)0.05で80% 検出力を達成するために必要とされる。ノンパラメトリック法が使用されると予測されたので、患者登録を、合計35名の患者に対して約10%増やした。

- 患者における過敏性腸症候群(IBS)もしくは炎症性腸疾患(IBD)の検出を補助するための方法であって、該方法は、
