Description
癌細胞は、接触阻害の欠失及び非制御的細胞増殖によって特徴付けられる。そのような変化は、自発的に、又は病原性毒素、いわゆる、ゲノムに損傷を与える発癌性物質により引き起こされる。そのような有害物質には、多くの化学物質、タバコの煙のみならず、UV光線も含まれる。更に、遺伝因子は、癌の発症において大きな役割を果たす。非制限的増殖とは別に、他の臓器に「娘腫瘍」(転移)を形成する傾向は、癌細胞の特徴である。転移は、血管又はリンパ管を介して規則的に拡散する。大多数の場合において、癌疾患は、不治であり、致死的である。治療手段は、原発性腫瘍及び転移の外科的除去を目的とする。更に、腫瘍を放射線で治療することが可能である。いわゆる細胞増殖抑制剤、特定のタンパク質若しくは表面マーカーに対する抗体又は免疫調節物質(サイトカイン、インターフェロン)が、急速に増殖する細胞を死滅させること又はアポトーシスを誘発することを試みるために使用される。
DNA修復機構により、細胞は、DNA構造の欠損修飾を排除することができる。DNAにおけるそのような損傷は、DNA複製の際に自発的に又は変異原物質、超高温若しくは電離放射線の影響により生じ得る。DNA損傷は、誤った形態で生じる有糸分裂のDNA複製、もはや合成されない若しくは誤った形態で合成されるタンパク質又は二本鎖切断後に切断される必須染色体領域を生じ得る。細胞の複雑な修復機構が成功しない場合、増殖及び休眠体細胞に蓄積している多数の欠損は、正常な細胞機能が欠損するほどの量まで増加する。生殖細胞において、娘細胞はもはや生存することができず、それは細胞株:細胞又は第二から第三継世代それぞれの不活性化をもたらし、細胞分裂の能力を失わせ、死滅させる。細胞周期の制御内において、制御タンパク質は細胞又はそのDNAが欠損していることを認識し、細胞周期の停止又はプログラム細胞死(アポトーシス)を誘導することができる。
細胞の最も重要な機能は、ゲノムの完全性を維持することである。このため、細胞周期内の全てのプロセスが正確に終了することを確実にする、多様な制御機構が存在する。これらの制御機構は、「チェックポイント [1] 」と呼ばれる。これらは、用語が意味するように十分に画定されたポイントではなく、特定の条件化で開始されうる反応カスケードである。
a.タンパク質発現の変化若しくはスプライシング変異体の発現の変化のいずれかをもたらすか、又は
b.CHK2遺伝子において更なる多型若しくはハロタイプをそれぞれ見出す及び/若しくは確認するのに適している、
CHK2遺伝子のプロモーターにおける機能修飾ゲノム多型を提供すること、
c.一般的な危険性及び疾患の経過を予測するのに適している多型を提供すること、
d.医薬品及び癌療法、特にCHK2阻害剤に対する反応、並びに副作用を一般に予測するのに適している多型を提供すること、
e.他の形態の療法(例えば、放射線、温熱、寒冷)の効果を一般に予測するのに適している多型を提供すること。
CHK2遺伝子のプロモーター領域における3つの多型が知られており、これらは一般に利用可能なデータベースにおいて見出すことができる。これらの3つの多型が検出され、かつ確認されたのは、ヒトのDNA試料の系統的な塩基配列決定によってであった:−7161G>A(rs2236141)、−7235C>G(rs2236142)及び−10532G>A(rs5762767)(図5)。この目的のために、CHK2のプロモーター領域の遺伝子配列をPCR反応により増幅し、サンガーによる方法を使用して塩基配列の決定をした。この目的に必要な方法、例えば、PCR反応に必要となるプライマー対を誘導すること、及び配列決定用プライマーを選択することは、当業者に周知である。この関係において、29塩基対の欠失である新たな多型が見出された(−10649−(−10621)del29、dbSNP IDなし)(図5)。これらのSNPの番号付けは、番号+1が開始コドンATGのヌクレオチドAに割り当てられたものである。番号0は存在しないと理解されるので、番号−1は、開始コドンATGのAの前のヌクレオチドに割り当てられる。
ヒトGHK2遺伝子は、染色体22q12.1(ジーンバンク・アクセション番号:NM_007194)上に位置しており、核で発現する65kDの大きさのタンパク質をコードする。本文脈において、遺伝子は、「CHK2」及び「CHEK2」と称されることが指摘される。以下では、「CHK2」の名称が使用される。遺伝子構造の模式図は図3で見られる。CHK2の活性プロモーター領域の特徴は、既に決定されている。プロモーター配列は、転写因子SP1の多数の結合部位を含有し、その結合は転写活性を増強する。CHK2の正の調節も、CCAATボックスへのNF−Y結合の際に観察された。
CHK2は、DNA損傷後のチェックポイント停止において重要な役割を果たし、かつ様々な腫瘍サプレッサー遺伝子がCHK2の基質であるので、潜在的な腫瘍サプレッサー遺伝子であると考えられている。今まで、散在性腫瘍、例えば、結腸直腸腫瘍、肺癌、前立腺癌、乳癌を有する一部の患者において、並びに複数腫瘍表現型であるリー・フラウメニ症候群(Li-Fraumeni syndrome)を有する患者において、この遺伝子における体細胞変異を検出することが可能であった。一塩基多型(SNP)と対照的に、これらの変異は、例えば、関連する患者の抹消血細胞では見出されない。
今まで、幾つかのケースにおいて、異なる腫瘍疾患によるCHK2における遺伝子変化を検出すること及びこれらを疾患の発症の危険性と関連づけることが可能であった。これらは、頻繁な遺伝子変異体ではないが、希な変異であり(頻度<1%)、健康な対照群よりも患者の大集団において多く検出され得た。現在まで、これらの希な変異のみならず、頻繁に発生するSNPも解析した研究は、1つしか発表されていない。この関係で、プロモーター多型−7161G>A(rs223641)も、乳癌の発症の危険性に関して解析された。しかしながら、関連性を検出することはできなかった(ベイネス(Baynes)ら、ATM、BRCA1、BRCA2、CHEK2及びTP53癌感受性遺伝子における一般的な変異は乳癌の危険性を増大する可能性は低い(Common variants in the ATM, BRCA1, BRCA2, CHEK2 and TP53 cancer susceptibility genes are unlikely to increase breast cancer risk.)、2007、ブリースト・キャンサー・リサーチ(Breast Cancer Res) 9:R27)。
チェックポイントが多くの調節カスケードに関与しているので、それらは制癌剤の適した標的である。チェックポイントタンパク質の特定の性質がこのことを説明する:(1)チェックポイントの複雑なシグナル伝達系は、攻撃のための複数の標的を提供する、(2)健常な細胞において、チェックポイントのいくつかは重要性が低いと考えられ、それは阻害剤の毒性を著しく低減する、(3)欠損チェックポイントの回復は、細胞増殖の減速をもたらす場合がある、(4)シグナル伝達系としてのチェックポイントは、中断され得る適応に付される、及び(5)損なわれたチェックポイントの回復は、癌細胞のアポトーシス速度を増加し、したがって特定の物質に対する感受性を増加し得る。
しかしながら、CHK2活性化剤は癌療法において代替案となり得る。CHK2は、腫瘍形成の抑制において役割を果たし、その活性化は、DNA損傷剤が存在しない場合、腫瘍細胞が増殖状態から離れ、かつ死滅することを誘導することができる。更に、これは、強力なG2停止を誘導する。活性化戦略は、阻害剤の短所を補うことができる場合があり、腫瘍は非常に異質であり、このため、1つのシグナルカスケードのみを不活性化することが必ずしも十分と言うわけではない。多くの種類の腫瘍細胞に共通する維持された細胞周期機構の過剰活性化によって、この異質性は中和させることができる。
このために、白人の異なるDNA試料の遺伝子型を決定した。結果を以下の表に示す。
1.特定の表現型(例えば、細胞特性、疾患の状態、疾患の経過、医薬組成物に対する反応)について、多型−7161G>A、−7235C>G、−10532G>A及び−10649−(−10621)del29との関連性が確立され、これらの関連性は、それぞれの遺伝子型に対して個別に、又はハロタイプの全ての順列を使用して、確立することができる。
2.CHK2又は隣接遺伝子において新たに検出された遺伝子変化に関して、既に存在する関連性が、上記遺伝子型又はハロタイプを用いることにより増強又は低減されるかを試験する。

どの機能修飾がCHK2遺伝子におけるプロモーター多型に寄与しうるかについての分析を実施した。プロモーターの遺伝子型及びハロタイプそれぞれに依存したCHK2タンパク質の選択的スプライシング、組織特異的発現又は過剰発現との相関が考えられる。この関係において、先ず、観察されたヌクレオチド置換が転写因子の結合に影響し得るかを見出すために、コンピュータープログラムを使用して解析を実施した。転写因子は、特定のコンセンサス配列に結合し、かつプロモーター活性を増加又は減少させる場合があり、その結果、このことは遺伝子の転写の増加又は低減をもたらし、コードするタンパク質の発現が増加又は低減する。図7に示されるように、全ての上記プロモーターSNPは、異なる転写因子(例えば、p53、NF−kB又はMef2)の結合部位のコンセンサス配列に位置しており、その結合は多型により損なわれる場合がある。特定の遺伝子型の発生は、これらの結合部位が欠失する、又はこれらがコンセンサス配列の変化により新たに形成されるという作用を有する。この作用の実験分析のために、いわゆるEMSA(電気泳動度移動アッセイ;electrophoretic mobility shift assay)を実施した。このアッセイでは、関連する多型を含有する短核酸セグメントを、細胞核抽出物と共にインキュベートする。次に、これらの抽出物に含有される転写因子タンパク質が、異なる強度で核酸セグメントと結合する。続いて、DNAへの結合を、X線フィルムを使用して可視化する。強力な結合は強いバンドを生じる。図8は、−7235C>G多型のC又はG対立遺伝子のいずれかを含有する特定の構築物によるこのアッセイの結果を示す。C−構築物のバンドの存在は、この領域への転写因子の結合を示す。G−構築物はこのバンドを有さず、これはこの対立遺伝子に結合する転写因子がないことを示す。特定のオリゴヌクレオチドによるバンド強度の減少は、結合転写因子が特異的に結合することを示す。図9は、−10649−(−10621)del29多型の挿入対立遺伝子又は欠失対立遺伝子のいずれかを含有する特定の構築物によるこのアッセイの結果を示す。対立遺伝子は両方とも、第2対立遺伝子が競合し得る転写因子の結合をもたらす。しかしながら、この場合では転写因子は異なる。図10は、欠失対立遺伝子への転写因子の結合が、欠失対立遺伝子におけるものよりも増加したプロモーター活性をもたらすことを更に示す。
細胞周期の調節におけるチェックポイントキナーゼ2の主要な役割によって、本発明の必須の主題は、CHK2における遺伝子変化を用いることにより一般的な疾患の危険性及び疾患の経過を予測する可能性である。
1.偏在的に発現するタンパク質をコードする遺伝子における遺伝子変化は、多様な疾患及び疾患の多様な危険性に影響し、或いはそれらの原因となること、並びに
2.チェックポイントタンパク質は、人体におけるゲノム完全性の維持のための複雑なネットワークの一部であること。
本発明の意義の範囲内にある薬理遺伝学は、医薬品の効能、医薬品の有効性及び性能、並びに望ましくない作用の発生の診断に関する。医薬品の効能及び/又は望ましくない副作用の発生の定義において、化学的に定義された製品の特定物質の特性に加えて、多様なパラメーターが用いられる。2つの重要なパラメーターである達成可能な血漿濃度及び達成可能な血漿半減期は、医薬品の効能若しくは無効能又は望ましくない作用の発生をかなりの程度まで決定する。血漿半減期の値は、とりわけ、特定の医薬品が肝臓又は身体の他の臓器において有効又は無効な代謝産物に代謝され、かつそれらが身体から排除される速度によって決定されるが、ここで排除は、腎臓、呼気、汗、精液、糞便又は他の身体分泌物を介して発生し得る。更に、経口投与の場合、効能は、いわゆる「初回通過効果」("first-pass effect")により制限されるが、その理由は、腸による医薬品の吸収の後、肝臓において特定量の医薬品が無効な代謝産物に代謝されるからである。
遺伝子不安定性は全ての腫瘍において特徴的であり、腫瘍形成、進行及び医薬品に対する抵抗性の発生においても役割を果たす。大部分の腫瘍細胞は、それら腫瘍細胞に生存優位性をもたらす欠損G1−Sチェックポイントを有する。しかしながら、この欠損は、ゲノム完全性を脅かす刺激が存在する場合、腫瘍細胞がG2チェックポイントに高度に依存することを意味する。CHK2は、DNA損傷が生じる場合、G1チェックポイントの維持に関与する。したがって、CHK2の活性化、ゆえにG1チェックポイントの回復は、治療抵抗性を回避する可能性をもたらす。CHK2の欠失は、放射線に対する抵抗性を引き起こすことが既に示されている。この抵抗性をCHK2活性化剤により逆転し得る。CHK2もG2チェックポイントに影響を及ぼすので、その阻害、及びしたがってG2チェックポイントの脱活性化は、遺伝毒性物質により引き起こされるDNA損傷及び変化が蓄積し得ること、及びそれらは腫瘍細胞の死を引き起こすと言うことの可能性を提供することができる。
化学療法は、その損傷効果が特定の癌細胞に対して可能な限り正確に標的化されており、且つそれらを死滅させ、又はそれらの増殖を阻害する物質を使用する。特定の用量の細胞増殖抑制剤は、進行する療法によって不変のままでいる標的細胞の特定の割合しか死滅させることができない。このため、腫瘍がもはや検出されない場合でも、化学療法を治療の期間中に縮小してはならない。むしろ、縮小した治療は、耐性腫瘍細胞クローンを選択してしまうことが予想される。化学療法は、短い間隔で適用され、ほぼ全ての場合において、2つ以上の細胞増殖抑制剤が、効能を増加するために組み合わされる。このため、療法は、共通毒性基準(Common Toxicity Criteria)に従って分類される副作用も引き起こす。これらの基準には、白血球及び血小板の数、嘔気、嘔吐、下痢及び口内炎が含まれる。

結腸直腸癌の場合における−7235C>G多型と生存との相関関係
−7235C>G多型の遺伝子型と結腸直腸癌を有する患者の生存との間に相関関係があるかを更に調査した。これを以下の図20に示す。
OR CG対CC=3.044(95%CI=1.246〜7.435)p=0.012;
OR GG+CG対CC=2.7(95%CI=1.2〜6.4)p=0.024。
以下の危険度が計算され得る:
OR A対G:1.613(95%CI=1.088〜2.393);p=0.021;
OR AA対GG:3.890(95%CI=1.229〜12.31);p=0.019;
OR AA+AG対GG:3.73(95%CI=1.2〜11.8);p=0.021。
−7235C>G多型に依存するグリア芽細胞腫を有する患者の生存のコックス回帰
以下の表は、全切除後のグリア芽細胞腫患者の腫瘍原因死の多変量コックス回帰モデルの結果を示す(HR=危険率(ハザード比)、*=基準、CI=信頼区間、ED=一次診断〔ドイツ語:最初の診断(Erstdiagnose)〕、m=男性、w=女性〔ドイツ語::女性(weiblich)〕)。
グリア芽細胞腫の場合における−10621del29多型及び生存に関して

Claims (6)
- 癌疾患の危険性、又は該疾患の進行を予測するためのインビトロ方法であって、患者の試料においてヒト染色体22q12.1上のCHK2遺伝子のプロモーター領域で1つ以上の遺伝子変化が探索され、かつ遺伝子変化は多型−7161G>A、多型−7235C>G、多型−10532G>A及び多型−10649−(−10621)del29から選択されること、及び前記多型−7161G>Aおよび多型−7235C>Gの少なくとも1つは、結腸癌発症の危険性又は進行の予測に用いられ、前記多型−7161G>A、多型−7235C>G、および多型−10649−(−10621)del29の少なくとも1つは、グリア芽細胞腫発症の危険性又は進行の予測に用いられ、前記多型−10649−(−10621)del29は、乳がん発症の危険性又は進行の予測に用いられ、前記−7235C>Gは、喉頭癌発症の危険性又は進行の予測に用いられることを特徴とする、上記方法。
- 患者の試料において、−7161G>A、−7235C>G、−10532G>A及び−10649−(−10621)del29の多型のうちの1つ、2つ、3つ、又は4つが探索されることを特徴とする、請求項1に記載の方法。
- 前記多型−7161G>A又は多型−7235C>Gは、結腸癌発症の危険性又は進行の予測に用いられることを特徴とする、請求項1に記載の方法。
- 前記多型−7161G>A、多型−7235C>G、又は多型−10649−(−10621)del29は、グリア芽細胞腫発症の危険性又は進行の予測に用いられることを特徴とする、請求項1に記載の方法。
- 前記多型−10649−(−10621)del29は、乳がん発症の危険性又は進行の予測に用いられることを特徴とする、請求項1に記載の方法。
- 前記−7235C>Gは、喉頭癌発症の危険性又は進行の予測に用いられることを特徴とする請求項1に記載の方法。
