Description
本発明の体液増量剤溶液は、4:1〜2:1、より好ましくは約3:1の濃度比でカルシウムイオンおよびマグネシウムイオンを含んでなることが好ましい。
1つの実施形態では、本発明の体液増量剤溶液は、血液量の増量または補給のうち少なくともいずれか一方を行うための代用血液である。さらに別の実施形態では、体液増量剤は血管外液代用物、例えば間質液代用物である。
さらなる態様では、本発明は、血液量減少症または熱傷のうち少なくともいずれか一方の治療における使用、および再灌流傷害の防止または軽減のうち少なくともいずれか一方における使用のための、本明細書中で定義されるような溶液を提供する。
さらなる態様では、本発明は、対象者に対して行なわれる外科的処置の間に組織または臓器のうち少なくともいずれか一方の生理的ホメオスタシスをin situで維持する方法であって、前記組織または臓器を、本明細書中で定義されるような溶液で灌流することを含んでなる方法を提供する。
本明細書中で使用される用語「血液量増量剤」または「血液量増量剤溶液」または「代用血液」は、血液量の補給、増量、または維持における使用が意図されている生理学的液状溶液を意味する。したがってこれらの溶液は、血液量減少症に起因する血液量の損失分の代用における利用が見出される。
本発明は、従来の血液量増量剤が細胞、組織および臓器の生存率および生存力の維持にとって有害であり、従って再灌流傷害の発生に寄与する可能性があるという本発明者の認識から生じている。
本発明の特定の実施形態について、次の実施例を用いて下記に述べる。実施例は本発明の実施形態を例証するために提供されるものであり、どのようにも限定するものと見なすべきものではない。
製剤
以下の調製物においては、エンドトキシンを含まないMilli−Q(登録商標)精製水(米国マサチューセッツ州ミルフォード所在のミリポア社(Millipore Corp))または同等品のASTMタイプI水[比抵抗は25℃でわずか18.0MΩ・cm]を、最初の混合および最後の希釈のいずれにおいても全面的に使用した。以後、用語「精製水」はこの品質の水を示すために用いられる。
1)溶液の構築方法、具体的には;
2)すべての貯蔵溶液および本発明に従って製造される溶液の10倍濃縮ボトルを作製するために上記に指定される精製水を使用すること;
3)本発明の滅菌貯蔵溶液および濃縮物を調製する方法は、オートクレーブ滅菌またはγ線照射を伴うべきではないこと。例えば、滅菌を達成するために溶液を照射すると、グルタミン、グルコース、インスリンおよびチアミンピロリン酸の諸成分が破壊されることになる;
4)すべての10倍貯蔵濃縮物溶液の保存にガラスボトルを使用すること;
5)pH2.4に酸性化して可溶化インスリンを調製することに加え、インスリン材料および貯蔵溶液を−20℃で保存すること;
6)チアミンピロリン酸およびTPPの貯蔵溶液を調製し、暗条件下にて−20℃で保管すること(下記の理由を参照);
7)塩化コリン貯蔵溶液を調製し、−20℃で保管すること;
8)塩化マグネシウム6水化物(すなわち6H2O)を使用すること。これは、無水物塩を使用すると水を吸着し、正確なマグネシウムイオン含量を算出するために使用される重量に誤差を生じるからである。この誤差は、正確なマグネシウムイオンおよびカルシウムイオン濃度という点でクレブス(Krebs)液を誤って作製する共通の理由である。
製造の詳説
1.貯蔵溶液:本発明の溶液の様々な貯蔵用濃縮物、すなわち1×、10×および長期保存用の20×が調製されているが、好ましい貯蔵用濃縮物は、暗条件下3〜8℃で保存するための、精製水を使用した10×濃縮物をろ過滅菌して100mLの密封ボトルに入れたものである。貯蔵溶液は、10×濃縮物の貯蔵溶液100mLを精製水900mLに加え、最終的なpHを20℃で7.22±0.04とするために2.1gの炭酸水素ナトリウムを添加することによって、1×溶液として使用するために再構成される。本発明による溶液の無菌の貯蔵用10×濃縮物はpH4.6±0.2であり、最大で10年間、無菌でかつ沈殿を生じないことが示されている。本発明の溶液の10×貯蔵用濃縮物について推奨される製造時保管寿命は、暗条件下3〜8℃で保管される場合は14か月である。
次の表は、体液増量剤として使用するためのRS−I溶液の組成をまとめたものである。
現時点では、また当分野で一般的な考え方によれば、保存用溶液、灌流用溶液および血液量補給用溶液の製剤は、細胞内または血管外の環境のいずれかの組成を採用するという強い傾向がある。しかしながら、既に本明細書中で述べたように、多くの問題が依然未解決のままである。細胞膜の超微細構造を考慮する際に、本発明者は現行の考え方から離れ、異なる手法、すなわち、細胞膜に直接隣接している環境すなわち間質液相を実際的な人工的方式で模倣し、細胞膜ならびに関連する受容体および酵素部分のホメオスタシスおよび機能的動態を可能な限り維持するように、生理溶液を製剤化する手法をとった。その結果得られた、本明細書中で実施例6および7において実証される、動物種およびヒト由来の単離された細胞、組織および臓器の細胞機能の良好な保存は、この手法の成功を例証している。
動物実験室における外科処置
この処置は、体重100グラムあたり0.1ccのXylocaine(登録商標)+0.2ccのケタミンの筋肉内(IM)注射を用いた完全麻酔誘導後の無菌条件下における正中部下方の腹壁切開で構成された。これに続いて、ラットの大動脈を、24ゲージのangiocath(登録商標)を使用して直視下で切開してカニューレを挿入した。これにより、大量採血ならびに溶液の投与のために直接アクセスすることが可能となる。維持量として、0.2ccのケタミンのIM注射を25分ごとにすべてのラットに行う。実験全体を通じて動物を自然に呼吸させ、呼吸数を常時モニターする。実験の間ラットを平常体温に保つために保温器を使用する。正中部下方の腹壁切開の結果、腸が露出されて腹部の血管が現れるようになる。水分の損失を最小限にするために濡れガーゼ(生理食塩水に浸漬したもの)で腸を覆い包む。実験の完了時には、ラットを麻酔下で屠殺するか、または、大動脈のカテーテル部位の穴を、6−0Prolene(登録商標)の8の字縫合を使用して修復し、腹壁切開を、2層の2−0Vicryl(登録商標)縫合糸を使用して閉鎖する。研究の生存役として選ばれたラットには筋肉内投与で抗生物質を与え、手術後は食物を自由摂取させた。本実施例の処置は、ラット1匹当たり完了までに25〜30分を要する。
同じような週齢(10〜12週)および体重(280〜320g)の19匹のスピローグ・ドーリー(Sprague−Dawley)ラットを実験に使用した。ラットを、乱数割当によって無作為化して3つの治療群すなわち(1)血液量を等体積の生理食塩水で補給(ラット5匹);(2)血液量を、等体積の上記に定義されるようなRS−I溶液で補給(ラット7匹);または(3)失血分の補給なし(ラット7匹)、のうち1つに割り当てた。この研究の第一の終点は呼吸循環停止を裏付けとする動物の死亡とした。
ラットから、30分ごとに2ccずつ連続的に採血した(制御された出血をシミュレート)。各採血の終わりに、0.1mlのヘパリン化溶液(20mlの正常な生理食塩水中に1000ユニット)を注入し、血液凝固を回避するためのヘパリンロックとした。ラットを、上記に定義される3つの治療群のうちの1つに無作為に割り当てた。

である。
生存の定義を、最初の採血から心肺停止が記録されるまでの時間とした。p値が0.05未満(p<0.05)である場合に、差が統計的に有意であるとみなした。値はすべて平均±標準偏差(SD)として表した。解析は、SPSSソフトウェアのバージョン13.0(米国イリノイ州シカゴ所在のSPSS社(SPSS,Inc.))を使用して実施した。
まとめ:蘇生「なし」治療群のラットは、生存期間が最も短く、最小の採血量であった。生存期間および平均採血量は、正常な生理食塩水またはRS−Iのいずれかを添加することによって著しく改善された。生理食塩水と比較してRS−Iは統計上優位の生存期間を示し(p<0.01)、より大量の採血を可能にした(p<0.01)。
RS−Iは、制御された出血性ショックのラットモデルにおける生存期間および失血量に関して、生理食塩水よりも有効な代用血漿であると思われる。
研究の目的
この研究は、静脈内(IV)注入剤として大型動物モデル(ブタ)に投与された場合のRS−Iの安全性を評価するための、試験的研究として設計された。
雌雄両方の、体重27〜35kgの6匹のブタを、この研究治療群に使用した。このブタを動物管理施設に収容し、実験前夜は一晩NPO(絶食)を保った。ブタをそれぞれ個別ケージに収容した。
6匹のブタ全てを1週間観察し、全てが正常な範囲内の行動および食性を示した。
血液の結果を、図4に示し、また図5〜9にグラフで表す。これらの血液結果は、この体重区分のブタの正常範囲内にあった。
1.電解質および生物物理学的パラメータ:
7日間の実験期間の間の血清中電解質または浸透圧重量モル濃度には、ベースライン(「対照」)の値と比較して有意差(p<0.05)は観察されなかった(図4G、表1;図5a〜g)。48時間後には、ブタ番号2および5において、既報の範囲の値へのナトリウムイオンレベルの上昇が観察されたが、第7日までにベースライン値に戻った(図5a)。ブタ番号6だけは実験期間の間に漸進的な上昇を示したがこの場合も、全体的な傾向レベルと同じように、最大値150mmol/Lの範囲内にあった(図4G、表1;図5a)。
2.血清代謝物質:
血清代謝物質のベースライン値はすべて外科的徒手操作の外傷後の第1日目に当然上昇したが、(乳酸を除く)すべてが第7日目までに正常血清の値の範囲内に戻る傾向を示した(図6)。第2日目および第3日目には、乳酸レベルが基準値を下回り(図6)、統計的には有意な傾向を伴わないが乳酸レベルのかなりの変動があった。
外科的徒手操作の外傷後には検査したすべての血清酵素の上昇が認められたが(図7)、(SGPT[ALT];図7b、を除く)すべてが第7日目までに許容可能な血清値まで低下して、心臓、肝臓、肺および腎臓の機能的完全性の復旧を示した。SGPT[ALT]について観察された上昇は、正常血清の値の50パーセンタイルの範囲内にあり、この場合も第7日目までの心臓および肺の機能的完全性を示している群データの範囲内または該データ内で統計的な差はなかった(P<0.26)。
正常血液量(normovolemic)のブタはそれぞれ、1.8〜2.3L(67mL/kg)の推定総血液量を有し、各々のブタに72時間の間に総体積3LのRS−Iを投与した。第7日目まで、RBC数の血液希釈は観察されず、ヘマトクリット(Hct)値およびヘモグロビン(Hg)レベルは正常範囲を維持していた(図8a、b、c)。同様に、リンパ球数は調査した7日間でベースライン値に戻り(図8d)、白血球(WBC)数のみが第7日目までに軽微な有意でない上昇を示した(図8e)。
ブタ類に関する多数の調査研究から血液および血清のプロファイル値における大きな多様性が示されていることは、広く認められているはずである。本研究では、これらの値は検査したブタ類の中では一貫しており、良好な統計的相関を有していた。
特徴的には、量補充療法は、投与される血液補給液の製剤に使用される様々な添加剤の抗原性および/または毒性に関係するようである、炎症症状、アナフィラキシー症状、過剰凝血症状を生じやすい。このことは、得られた血球プロファイルデータの分析からは、RS−I溶液の静脈内投与に当てはまるようには見えない。
a)血清乳酸レベルを維持するためのピルビン酸の代謝(解糖);
b)ATPを生成するための組織乳酸の異化、および
c)グルコースまたはグリコーゲンを形成するための肝臓による再合成(コリ回路)
を最適化する条件が揃っていることである。
ブタに1.0LのRS−Iを3日間毎日IV注入する、上記に詳述した研究は、成功裡に終わった。得られた結果に基づいて結論付けられることは、RS−Iは、静脈内投与される場合、臨床的に許容可能な条件下でブタを飼育するのに明白な安全上の問題点を持たないということである。
方法
心停止ドナー(NHBD)のブタ腎臓を、RS−Iまたは市販の低温保存溶液であるSoltran(登録商標)もしくはUW(ウィスコンシン大学)のうちいずれかの中で「寒冷」(0℃〜4℃)静止(CS)条件下で2時間、あるいは「温暖」(31℃)静止(WS)条件下で保存した。この腎臓をその後、自家血液/乳酸加リンゲル液の50:50灌流混合物を用いて常温条件下で6〜8時間再灌流した。
自家血液を用いた6時間の常温灌流後の摘出ブタ腎臓において測定された機能的パラメータを、以下の表IIIに示すが、表中、「n」は試験した腎臓の数を表している。
方法
哺乳類の組織および臓器調製物の機能的生存能力を、RS−I溶液中で様々な期間について組織/臓器を保管/灌流した後に評価した。生存能力は、様々な機能的指標、例えば細胞膜電位の維持、神経伝達物質の生産量、筋原性、膜受容体感受性、酵素機能、組織学的変化などを使用して評価した。
下記の表IVは、RS−I溶液が様々な哺乳動物およびヒトの組織および臓器調製物の機能的な生存能力を、様々な長さの保存期間(0.3〜10日間)にわたって維持する能力を提示している。
研究の目的
ブタの出血性外傷後の、蘇生(増量)溶液としてのRS−Iの有効性ならびに組織再灌流傷害の低減におけるRS−Iの有効性を、自家血液、または臨床応用に一般に用いられている血液量増量溶液である乳酸加リンゲル液(LR)を用いた補給と比較して調査する。
この研究では、23匹の無作為に選択された非同系の畜豚を、4つの実験群において、すなわち3匹を「シャム」(対照)、6匹を自家血液、6匹をRS−I溶液、6匹をLR溶液の実験群に使用した。標準化された全身麻酔条件下で、すべての血行動態プロファイルを十分計測できるように動物に機器を装備した。その後、ブタの大腿動脈から、MAPが30mmHgに達するまで急速に出血させた。出血は、MAPを30±2mmHgに45分間維持するのに必要なだけ継続させた。出血させた血液を、ACD処理されたバッグに回収し、その正味重量を、出血させた血液量を概算するために使用した。
概略:
生存している3つの実験群の全18匹のブタを1週間観察すると、すべて正常な範囲内の行動および食性を示した。
RS−I群のブタの平均動脈圧(図11を参照)、中心静脈圧、肺動脈閉塞圧の、蘇生の最初の60分間における回復時間が最も速く、自家血液群のブタに匹敵したが、LR群のブタでは最も遅かった。心拍出量の回復は、自家血液群およびLR群のブタのいずれと比較してもRS−I群のブタで有意に速くかつ大きかった(図12を参照)。
術後第7日までの血清中電解質濃度は、LR群のブタにおけるナトリウムイオン濃度の上昇を除いてすべての実験群について正常レベルの範囲内に回復した。アニオンギャップおよび強イオン濃度較差は、術後第7日までにすべての群のブタで正常な範囲内にあった。
術後第7日では、再灌流傷害の証拠は、RS−I群および自家血液群のいずれのブタにおいても肺および肝臓には存在せず、腎臓では軽微であったが、LR群のブタではこれら3種の臓器に顕著かつ広範囲な特徴がみられた(図13を参照)。

- 緩衝化された体液増量剤溶液であって、外科的処置または治療によりヒトまたは動物の身体を治療するための方法に使用される、緩衝化された体液増量剤溶液において、前記緩衝化された体液増量剤溶液は、
i)、N,N−ビス(2−ヒドロキシエチル)−2−アミノエタンスルホン酸と、
ii)5:1〜1:1のモル濃度比のカルシウムイオンおよびマグネシウムイオンと、を含んでなり、
前記緩衝化された体液増量剤溶液は血漿増量剤を含んでいない、緩衝化された体液増量剤溶液。 - 緩衝化された体液増量剤溶液であって、対象者の細胞外液の損失の治療において使用される、緩衝化された体液増量剤溶液において、前記緩衝化された体液増量剤溶液は、

(h)5〜200μmol/Lのアスパラギン酸、
(i)100〜2000μmol/Lのグルタミン、
(j)15〜215μmol/Lのピログルタミン酸、
(k)20〜200μmol/Lのアルギニン、
(l)1〜120nmol/Lのチアミンピロリン酸、
(m)40〜70μmol/LのD−もしくはDLもしくはL−カルニチン、および
(n)5〜200mI.U./Lのブタもしくはヒトのインスリン
のうちの1以上をさらに含んでなる、請求項1〜13のいずれか一項に記載の緩衝化された体液増量剤溶液。 - 抗生物質成分を含んでなる、請求項1〜13のいずれか1項に記載の緩衝化された体液増量剤溶液。
- 抗生物質成分は10〜150mg/Lのクロラムフェニコールである、請求項14に記載の緩衝化された体液増量剤溶液。
- 4〜38℃の温度範囲においてpHが7.05〜7.5である、請求項1〜15のいずれか1項に記載の緩衝化された体液増量剤溶液。
- 前記溶液は代用血液又は血管外液代用物である、請求項1〜16のいずれか一項に記載の緩衝化された体液増量剤溶液。
- 前記溶液は腹膜液代用物である、請求項17に記載の緩衝化された体液増量剤溶液。
- 請求項1〜18のいずれか一項に記載の緩衝化された体液増量剤溶液の濃縮貯蔵溶液であって、該濃縮貯蔵溶液は1〜50倍に濃縮されている、濃縮貯蔵溶液。
- 炭酸水素イオンを含まない、請求項19に記載の濃縮貯蔵溶液。
