本実施形態にかかる褥瘡防止サポータを装着するには、筒型クッション3と一体に固定されたクッションカバー2の近位側開口7aに足先101を挿入し、クッションカバーの内腔に下肢を挿入する。このとき、クッションカバー2の緊締部4を利用者の足首103等に位置するようにし、大体の位置を固定する。上記のように緊締部4は伸縮力が比較的強く、ある程度の緊締力を有するため、下肢100の細い部分である足首103に配置することで、一体化された筒型クッション3の位置ずれを起こしにくく装着される。また、緊締部4から、ふくらはぎ部分を介して近位側端部6への伸縮性の変化は、漸増減的に作成することによって、ふくらはぎへの抗浮腫効果を調整することができる。

本実施形態にかかる褥瘡防止サポータ1によれば、図5及び図6に示すように、仰向けの状態において踵部が床面102aから浮いた状態に配置され、踵部分の褥瘡を防止することができる。また、寝返りなどで側臥位の状態になった場合でも、踝108の部分が床面から浮き上がった状態となっているため、踝108の部分の褥瘡も防止することができる。同時に褥瘡防止サポータは、下腿部の内外上下面を適度な着圧にて覆うため、局所的な過度圧迫は起こり難く、膝以下の下肢荷重はふくらはぎ部分に圧分散して受け止められることとなり、褥瘡予防効果が達せられる。さらに側臥位でも腓骨頭106は浮上しており、腓骨神経107の圧迫は回避され、腓骨神経麻痺も予防される。

図7は、本発明の第2実施形態にかかる褥瘡防止サポータの構成を模式的に示す断面図である。本実施形態にかかる褥瘡防止サポータ1aは、第1実施形態にかかる褥瘡防止サポータ1と共通する構成を有するので、以下、構成の違いを中心に説明する。

前述の第1実施形態にかかる褥瘡防止サポータ1は、踵102の部分が筒型クッション3から露出した状態に装着され、踵102が浮いた状態で保持されるが、当該姿勢は膝関節が伸展傾向の状態である場合に好適に使用される。それに対し、本実施形態にかかる筒型クッション23は、膝関節屈曲拘縮においても対応できるように、踵102をクッション23内に収納した状態に装着し、踵が床面に直接接触しないようにしたものである。

本実施形態における筒型クッション23は、クッションカバー2との取り付け位置を若干下方向にずらして足首よりも末梢部の踵102を筒型クッション23の凹部内に配置するように取り付ける。このように構成することにより、踵102の部分にクッションが配置され、褥瘡を防止することができると共に、膝関節がやや屈曲位の状態であっても、下腿104の部分が下支えされ、踵102に圧力が集中的に加わることなく分散され、褥瘡を防止することができる。なお、凹部24をやや深くすることで、踵102の底部に直接圧が加わらない様な形状とすることも可能である。

また、筒型クッション23の外部に位置するクッションカバー2の両側端部5,6は、筒型クッションの外表面側へ折り返されて両端がオーバーラップするように配置して筒型クッションに固定しても良い。また、両側端部5,6同士は、それぞれ設けられた面ファスナー12により連結されて、クッションカバー2が筒型クッション23から位置ずれしないようになっている。なお、両側端部5,6同士を固定する手段としては、面ファスナー12に限定されるものではなく、直接両者を縫製してもよいし、ボタン、フックなど公知の手段を用いることが可能である。
