Claims (12)
- 気体の流れが患者に送達される人工呼吸器−患者インターフェースと、
患者換気システムの1つ以上の患者−人工呼吸器特性の測定値または推定値を受けるようになっている患者モデル推定器であって、患者換気システムが患者の呼吸サブシステムならびに吸気および呼気付属品を含む、患者モデル推定器が、
(i)運動方程式、および臨床的に観察された患者自発筋圧を近似する1つ以上の周期または準周期関数に基づいて、運動方程式と、呼気および吸気に関する1つ以上の時間パラメータの関数として表される吸気相のモデルおよび呼気相のモデルとの組合せに基づく1つ以上の方程式である、患者換気システムの吸気相および呼気相を予測する呼吸予測モデルを確立すること、および
(ii)受信した1つ以上の測定または推定特性に基づいて、呼吸筋努力値を抽出するために呼吸予測モデルを解くこと、によって患者の呼吸筋努力の定量化を実行するようになっている、患者モデル推定器と、
呼吸筋努力値、または呼吸筋努力値に基づいて導かれる1つ以上のその他の呼吸パラメータに基づいて、患者への気体の流れの送達の様々な態様を制御するようになっている制御装置と、を含む人工呼吸器システム。 - 臨床的に観察された患者自発筋圧を近似する1つ以上の周期または準周期関数が、臨床的に観察された吸気筋圧を近似する周期または準周期関数を含み、呼吸筋努力値が、患者によって生じる吸気筋努力の推定値を含む、請求項1に記載の人工呼吸器システム。
- 臨床的に観察された患者自発筋圧を近似する1つ以上の周期または準周期関数が、臨床的に観察された呼気筋圧を近似する周期または準周期関数を含み、呼吸筋努力値が、患者によって生じる呼気筋努力の推定値を含む、請求項1に記載の人工呼吸器システム。
- 呼吸予測モデルが、患者の複数の呼吸周期に有効であると見なされ、方法が、患者の呼吸周期の間に所定の時間窓で呼吸予測モデルを定期的に再確立、更新、または最適化するステップをさらに含む、請求項3に記載の人工呼吸器システム。
- 前記呼吸筋努力値を抽出するために呼吸予測モデルを解くことが、呼吸予測モデルの確立に続く複数の呼吸周期のうちの1つの呼吸周期の間に呼吸予測モデルを解くこと、ならびに測定によって導入された時間遅延および代理現象による筋肉活動の間接的な兆候を考慮するために呼吸筋努力値を修正することを含む、請求項6に記載の人工呼吸器システム。
- 前記呼吸筋努力値を抽出するために呼吸予測モデルを解くことが、運動方程式の導出パラメータを最適化することを含む、請求項1に記載の人工呼吸器システム。
- 患者モデル推定器がさらに、患者換気システムの瞬時リーク流値を判定するようになっており、呼吸予測モデルを解くことがさらに瞬時リーク流値に基づく、請求項1に記載の人工呼吸器システム。
