Description
本出願は、2007年1月23日に出願された米国暫定特許出願シリアルナンバー60/881,907の優先権を主張し、この全体的な内容は、本書に参照によって盛り込まれる。
本実施例では、回転角度0°で、PEEPが、予期される肺胞の閉鎖圧以上に調整される(肺疾患に依存して、15乃至25cmH2Oの間)。PIPは、十分な換気を確実にするようPEEPより実質的に高く設定される。
本例示的な実施例では、リクルートメントマニューバの後、回転角が増加するにつれ、PEEPを連続的に減少させる。人工換気される肺の状態が連続的に記録される。回転角度0°の所定のPEEPで、PEEPを開始して、最大回転角度でPEEPが1乃至2cmH2O減少しうるようにPEEPを減少させる(手順1)。肺胞虚脱の信号が、上述した信号のいずれにおいても何も起こらない場合、PEEPレベルが記録され、0°のときの先の設定まで連続的に増加しうる。他方の側へ患者を回転している間、PEEPは同様の方法で減少する(手順2)。肺胞虚脱の信号が、上述した信号のいずれにおいても何も起こらない場合、PEEPレベルがこの値で保持されて、患者は0°まで回転して戻る。
この例示的な実施例では、各側のPEEP虚脱圧を決定した後、PEEPが各側で必要とされるレベル以下に決して落ちないことを確実にしつつ、PEEPは回転が進むに伴い連続的に調整されうる。
Claims (15)
- 治療を最適化するシステムであって、当該システムが:
第1の位置及び第2の位置に位置するよう構成される支持面と;
肺を人工的に換気するよう構成される人工呼吸器と;
N又はそれ以上の生理的パラメータ、並びにM又はそれ以上の換気パラメータ及び位置パラメータの値(N、Mは1又はそれ以上の整数)を得るよう構成される測定装置と;
前記N又はそれ以上の生理的パラメータ、並びにM又はそれ以上の換気パラメータ及び位置パラメータの値を分析システムへ送信するよう構成されるフィードバックシステムであって、前記分析システムは、前記N又はそれ以上の生理的パラメータを利益とし、前記M又はそれ以上の換気パラメータ及び位置パラメータを費用として設定されたN×M次元の費用関数を計算するよう構成されるフィードバックシステムと;
前記費用関数を最小にすべく前記M又はそれ以上の換気パラメータ及び位置パラメータを調節するよう構成される制御システムとを具えることを特徴とするシステム。 - 請求項1に記載のシステムにおいて、前記支持面がその長軸周りに回転可能であることを特徴とするシステム。
- 請求項1に記載のシステムにおいて、前記支持面が調節可能な空気袋を具えることを特徴とするシステム。
- 請求項1に記載のシステムにおいて、前記制御システムが、前記費用関数を最小にするように、前記M又はそれ以上の換気パラメータ及び前記位置パラメータを自動的に調節することを特徴とするシステム。
- 請求項1に記載のシステムにおいて、前記制御システムは、介護者による手動調節と、前記費用関数が最小であるように前記M又はそれ以上の換気パラメータ及び位置パラメータが調節されるときを示す可聴又は可視インジケータとを具えることを特徴とするシステム。
- 請求項1に記載のシステムにおいて、前記N又はそれ以上の生理的パラメータが、呼吸パラメータを含むことを特徴とするシステム。
- 請求項1に記載のシステムにおいて、前記N又はそれ以上の生理的パラメータが、肺メカニクス及び直列死腔の測定値を含むことを特徴とするシステム。
- 請求項1に記載のシステムにおいて、前記N又はそれ以上の生理的パラメータが、直接VO2、paO2、および肺メカニクスの測定値のいずれか一つを含むことを特徴とするシステム。
- 請求項1に記載のシステムにおいて、前記N又はそれ以上の生理的パラメータが、呼気及び吸気の圧力体積曲線の上側及び下側の変曲点と、最大圧力体積コンプライアンス(Pmax)点での気道圧とのうちの1又はそれ以上を含むことを特徴とするシステム。
- 請求項1に記載のシステムにおいて、前記N又はそれ以上の生理的パラメータが、血行力学パラメータを含むことを特徴とするシステム。
- 請求項1に記載のシステムにおいて、前記N又はそれ以上の生理的パラメータが、DO2、間接VO2、SpO2、侵襲性心拍出量、心臓の1回仕事量、1回拍出量、右心端拡張期容積、肺血管抵抗、肺毛細血管圧、肺血管コンプライアンス、O2除去率、Qs/Qtシャント分数、及び肺血管外水分量測定値のうちの1又はそれ以上を含むことを特徴とする方法。
- 請求項1に記載のシステムにおいて、前記Nまたはそれ以上の生理的パラメータが、画像データを含むことを特徴とするシステム。
- 請求項12に記載のシステムにおいて、前記画像データが、電気インピーダンス断層撮影(ETI)データ及びコンピュータ断層撮影(CT)データのうちの1またはそれ以上を含むことを特徴とするシステム。
- 請求項1に記載のシステムにおいて、前記M又はそれ以上の位置パラメータが、ピッチ測定値を含むことを特徴とするシステム。
- 請求項1に記載のシステムにおいて、前記M又はそれ以上の位置パラメータが、回転位置測定値を含むことを特徴とするシステム。
