本出願は、2006年1月5日付け出願に係る「自己監視データから糖尿病における血糖変動性を評価するための方法、装置及びコンピュータ・プログラム・プロダクト」と題する米国出願第60/756,340号並びに2006年3月29日付け出願に係る「自己監視データから糖尿病における血糖変動性を評価するための方法、装置及びコンピュータ・プログラム・プロダクト」と題する米国出願第60/786,944号に対する優先権を主張し、これら出願の内容全体が、本明細書中において参考として組み込まれる。
本明細書中に記述されている研究は、米国国立衛生研究所により授けられた政府援助No.R01 DK51562により支援されている。米国政府は、本発明に所定の権利を有する。
本システムは、一般には、ブドウ糖監視技術に関し、特に、低及び高血糖のリスク評価に関するものである。
糖尿病は複雑な病気であり、分子から人間行動に至るまでのバイオシステム組織の全水準に作用するメカニズムに起因し、かつ、そのメカニズムによって、それらの進展の中で決定付けられる、共通の最終要素である高血糖によって特徴付けられる。真性糖尿病は2つの主要な型がある。1型(T1DM)は、インスリンを生産する膵臓ベータ細胞の自己免疫破壊によって生じ、2型(T2DM)は、インスリン分泌欠乏の進展と相まったインスリン活動不全(インスリン抵抗性)によって生じる。現在、米国では、2,000万人の人達が糖尿病によって苦しめられており、現在でも糖尿病の蔓延は増加し続けている。糖尿病のリスクとコスト(年間100億ドル超)が糖尿病による四つの主要区域:成人失明に繋がる原因となる網膜疾患、全ての腎不全体の半分にあたる腎臓病、毎年82,000を越える切断手術となる傾向にある神経障害、及び、糖尿病を除いた心血管疾患以外に比べ二〜四倍多く見られる心血管疾患(CVD)、における慢性合併症によって発生している。糖尿病における心血管疾患も、より病的で、より致死的で、例えばバイパス手術やステントといった近代診療による利益が少ない。プラス面での特徴を考えると、インスリン及び/又は経口薬による集中治療は、心血管疾患病(27)を含めて、T1DM(60)及びT2DM(62)両方のほとんどの合併症を顕著に減少させる。今では古い話ではあるが、DIGAMI研究は、急性心筋梗塞の予兆のある糖尿病患者に対する厳格な代謝管理の利益を確証した(24)。本研究の続報であるDIGAMI-2(51)は、十分に管理された糖尿病心血管の結果が、非糖尿病集団におけるそれと類似することを示すことで、積極的な代謝管理の重要な利益を確証した。
従来のBG変動性に係る統計的演算は、SMBG測定値の標準偏差(SD)の計算及び/又は低血糖(BGが3.9mmol/l若しくは70mg/dl、又は他の閾値未満)及び高血糖(BGが10mmol/l若しくは180mg/dl、又は他の閾値超)の集計を含む。これらの従来手法に加え、様々な変動性尺度が過去に紹介されている:
M値−旧来の変動性尺度(1965年に導入された)で、二つの版があり、BG範囲の補正を行わないものと、補正を行うもの(57);
MAGE−1970年に導入された血糖変動の平均変動値(Mean Amplitude of Glucose Excursions)
信頼性指標(LI)−最近(2004年)報告され、1型糖尿病における重症低血糖及びブドウ糖変動に対する評価を意図した指数(53)。
−低血糖及び高血糖の範囲に、同様の強調が置かれると思われる;
−正常BG範囲(3.9〜10mmol/L又は70〜180mg/dl)の重要度が下がるだろう、したがって、正常範囲内に含まれる変動性はより重要性が下がり、この範囲外に逸脱するであろう。このことは、著しいBGばらつきの臨床的重要性と、正常範囲内のばらつきは一般的に無害であるという臨床的理解とに対応している;
−著しい低血糖及び高血糖への逸脱は、徐々に増加するリスクの値を得ると思われ、これは、著しい低血糖又は高血糖が患者に対してより高リスクをもたらすという臨床的印象に対応する。この特徴点は、これらの条件の範囲を考慮していない低血糖又は高血糖の発症の伝統的な単純集計に対して、BGリスク・スペースを対照させる。
・範囲を評価することができ、かつ、異常なBG変動リスクを予測することができるインテリジェントデータ解釈コンポーネントを導入することで、範囲の評価、異常なBG変動リスクの予測、同様の特性によって将来のSMBGデバイスとなりうる既存のSMBGデバイスの強化;
・同様の特性による、糖尿病管理を補助することを目的とした携帯デバイス(携帯端末、PDA)の強化;
・同様の特性による、SMBGデータを回収するソフトウェアの強化−このようなソフトウェアは、実質的には家庭用BG監視デバイスの全製造業者によって製造され、患者及びヘルスケア機関によって、SMBGデータ解釈のために習慣的に用いられている。該ソフトウェアは、患者の個人コンピュータに備えられることができ、又はインターネット・ポータルを経由して使用されることができる;
・糖尿病の様々な治療(インスリンや、プラムリンチド(pramlintide)及びエキセナチド(exenatide)といった変動性低減剤)の有効性評価;
・新しいインスリン運搬デバイス(インスリンポンプ)、又は未来型閉ループ糖尿病管理システムの有効性の評価
BGレベルは米国ではmg/dlで、他のほとんどの国ではmmol/L(又はmM)で測定される。二つのスケールは、18mg/dl=1mMとして直接関連している。ほとんどのBG基準メーターの全ての範囲は1.1〜33.3mMであり、これは、全ての観察測定値を実質的にカバーしているものと考えられる。糖尿病の人に対する目標BG範囲は、3.9〜10mMと考えられる。低血糖は、BGが3.9mM未満と定義され、高血糖は、BGが10mM超と定義される。このスケールは数値的に非対称であり−高血糖範囲(10〜33.3mM)が低血糖範囲(1.1〜3.9mM)よりも広く、正常血糖範囲(3.9〜10mM)は、本スケール内において中心となっていない。我々は、一般的2パラメータ分析的フォームを有する、BG範囲[1.1,33.3]で定義される連続関数であるf(BG)変換を導入することでこの非対称を修正した:

これらは、パラメータαへの単一非線型方程式へと容易に低下させることができる。α>0の制限のもと数値的に解くと、α=1.026,β=1.861,γ=1.794となる。仮にBGをmg/dl単位で測定する場合には、α=1.084,β=5.381,γ=1.509となる。

ここで、n1,n2,・・・,nMは、個々に3以上であることが好ましい。観察日数Mは、約7〜14(約1〜2週間)又は約42(6週間)超であり得るが、約14〜42(2〜6週間付近)が好ましく、特に、28(約4週間)が好ましい。観察の総数は約30から75、又は約75より多くすることができるが、約75の血糖データ点が望ましい。
したがって、平均日常リスク範囲及び平均日常BG範囲は次のように定義される:
ブドウ糖変動の評価において類似の結果を導くであろう様々な摂動が、ADRRを計算する公式について可能である。例えば、BGリスク関数及びBGリスク・スペースは、二乗ではなく、絶対値を基礎として定義することができ、これは、極端なBG変動に対する重みを下げて、r(BG)=10.abs[f(BG)]のように変わる。一般的には、ADRRの可能な変形例は次のように分類できる。

ADRRは、先に行われたLifeScan委託研究(LifeScan-sponsored studies)(第二章)におけるN=177の参加者についての記録データの再解析を用いて初めて開発され、それらの90名が1型糖尿病であり、残りが2型糖尿病であった。これら対象の人口統計学特性を表1に示す:
本研究では、参加者は4月間SMBGを実施した。様々な変動性尺度が、決められた手順によるSMBGの最初の月から計算され、そして、その後の3月間の調査において記録された極端な低血糖及び高血糖の事象の数に相関付けられた。この初期開発の間にテストされた尺度は:

LifeScan(対象N=355)で個別に収集した大量のデータセットを用い、前セクションで示した血糖コントロール及びブドウ糖変動の全基本尺度に対して、及び文献で報告された全ての公知変動尺度に対して、ADRR及びその標準偏差(SDRR)は広い範囲でテストされた。表3は、検証データセットへの参加者の人口統計的特徴を示す。

図7は、デバイス又はソフトウェアにおいて実行される「変動性ゲージ」の生成に使われうるADRRの可能な表示を示す。該表示は、正確に同一の平均血糖を有するが全く異なるブドウ糖変動性を有する二人の対象のADRR値によって説明される。
1)ADRRは、ブドウ糖変動性についての最も優れた総合尺度である;
2)ADRRは、様々な計画対象期間に亘る、将来の極端な低血糖及び高血糖の事象の両方について予兆となり、それらの分散の50%近くを占める;
3)低血糖及び高血糖の範囲に分かれて、LBGI及びHBGIは、最も優れた変動性尺度の中で堅実である。LBGI及びHBGIが相互補完的となるように設計されているため、LBGI及びHBGIの組み合わせは、総合ブドウ糖変動の表現として、表4A、4B及び4Cに示す他の手法の組み合わせよりも優れている;
4)ADRRは、LBGI及びHBGIの組み合わせ同様、平均血糖から独立性を有して極端なブドウ糖変動の予測性に対する貢献を有している。実際、将来の極度な発症に対する主要な責任は、平均血糖ではなく、ブドウ糖変動性によって運ばれる。

イントロダクション:
第四段階のデータ収集は、研究責任者をボリス・コバチェフ(Boris Kovatchev)とする大がかりな米国国立衛生研究所助成研究(NIH-funded study)NIDDK R01 DK51562号、「IDDMの生物学的挙動及び制御」の継続、特に、高インスリン血症正常血糖クランプと、これに続く、目標基準40mg/dlの低血糖への低下の期間中に、T1DMの成人の代謝パラメータを調査した病院検査室研究(HIC#10714)からのデータの継続であった。本研究では、T1DMの39人に対する代謝要求及び逆調整データが収集された。各人の個人代謝要求(IMD)の計算には、運動及び低血糖といった非定常状態を考慮するためにBKによって修正された古典的なデフロンゾ(DeFronzo)のクランプ方程式を用いた。逆調整の応答は、ベースラインにおける、またその後、低血糖への低下中における10分ごとにおけるエピネフリンレベルの直接計測によって評価された。
(1)ベースラインと追跡調査の評価との間の経過時間が1年超であるとしても、SMBG由来のパラメータであるLBGI、HBGI及びADRRは、ほとんどの対象者にとって、時間に対して信頼性がある;
(2)SMBG由来のパラメータ、特にADRRは、病院内で評価された代謝要求(IMD)及び逆調整応答(Epi)を良好に予測する:正常血糖期間における、ベースラインADRRと対象者のIMD(mg/kg/minuteにおける)との間の相関は、0.59、p=0.002であった;低血糖が引き起こされている期間における、ADRRのピーク・エピネフリン・リリースに対する相関関係は-0.52、p=0.008であった。このように、対象者の自然な環境において測定される、増加したブドウ糖変動性は、病院検査室研究で測定される、増加したブドウ糖接種及び減少した逆調整応答との両方と関連している。この結果は、ボーラス計算機を生成するために、第四段階の全ての目的の観点から、とても重要である:ADRRは、「最高基準」の実験室評価によって測定されたのと同様に、インスリン感受性及び逆調整能力の両方についての現場での相関として使えることになる。ブドウ糖変動性の他の何れの尺度も、これらの病院パラメータとの、統計的な重要な関係を十分に達成することができず、このことが、リスク・スペースにおけるBG変動性の測定の重要性、及びADRRの優位性を強調する。

前記平均日常リスク範囲及び前記平均日常リスク範囲が分類される前記少なくとも四つのカテゴリのうちの一つを表示する表示モジュールと、
を備えるシステム。 - コンピュータ・システム内の少なくとも一つのプロセッサに患者の血糖変動性の測定ができるようにする、コンピュータ実行可能命令が格納された非一時的なコンピュータ可読記録媒体内に記録されたコンピュータ・プログラムであって、
前記コンピュータ実行可能命令によって、前記プロセッサが、
前記患者の血液中の糖濃度値を表す複数の自己監視血糖データポイントの取得であって、前記複数の自己監視血糖データポイントが少なくとも一日期間に亘る取得と、
前記複数の自己監視血糖データポイントの、血糖範囲から変換範囲への、変換関数に従った変換であって、前記変換範囲は−√10及び√10をそれぞれその最小及び最大値とし、前記変換関数を、

ここで、BGは血糖値であり、BGがmM単位で測定された場合は(α、β、γ)=(1.026,1.861,1.794)であり、BGがmg/dl単位で測定された場合は(α、β、γ)=(1.084,5.381,1.509)とする変換と、
BGリスク・スペースの定義であって、前記BGリスク・スペースを、

それぞれの日に対する前記複数の自己監視血糖データポイントに対する最大高血糖リスク値の次式による演算と、
平均日常リスク範囲の次式による演算と、
前記平均日常リスク範囲を少なくとも四つのカテゴリとする分類であって、該少なくとも四つのカテゴリは、低、低−中、中−高、及び高を含み、前記低リスクのカテゴリは、0と20の間のリスク範囲に相当し、前記低−中リスクのカテゴリは、20と30の間のリスク範囲に相当し、前記中−高リスクのカテゴリは、30と40の間のリスク範囲に相当し、前記高リスクのカテゴリは、40より多いリスク範囲に相当する分類と、
前記平均日常リスク範囲及び前記平均日常リスク範囲が分類される前記少なくとも四つのカテゴリのうちの一つの表示と、
を実行するコンピュータ・プログラム。 - 患者の血糖変動性を測定するためのコンピュータ実装方法であって、
コンピュータにおいて、前記患者の血液中の糖濃度値を表す複数の血糖データポイントを取得するステップと、
前記コンピュータにおいて、所定継続期間の期間内における複数の血糖データポイントのそれぞれに基づいた複数のリスク範囲を演算するステップと、
前記コンピュータにおいて、演算された前記複数のリスク範囲に基づく、少なくとも一つの平均所定継続期間血糖リスク範囲を演算するステップと、
ユーザとの通信のために、前記少なくとも一つの平均所定継続期間血糖リスク範囲を出力装置に出力するステップと、
を備える方法。
- 所定継続期間は、前記少なくとも一つの平均所定継続期間血糖リスク範囲が平均日常血糖リスク範囲を提供する日である請求項87に記載の方法。
- 患者の血糖変動性を決定するためのシステムであって、
前記患者の血液中の糖濃度値を表す複数の血糖データポイントを取得する取得モジュールと、
所定継続期間の期間内に複数の血糖データポイントのそれぞれに基づいて複数のリスク範囲を演算し、演算された前記複数のリスク範囲に基づく、少なくとも一つの平均所定継続期間血糖リスク範囲を演算するようにプログラムされたプロセッサと、
を備えるシステム。 - 所定継続期間は、前記少なくとも一つの平均所定継続期間血糖リスク範囲が平均日常血糖リスク範囲を提供する日である請求項90に記載のシステム。
- コンピュータ・システム内の少なくとも一つのプロセッサに患者の血糖変動性の測定ができるようにする、コンピュータ実行可能命令が格納された非一時的なコンピュータ可読記録媒体内に記録されたコンピュータ・プログラムであって、
前記コンピュータ実行可能命令によって、前記プロセッサが、
前記患者の血液中の糖濃度値を表す複数の血糖データポイントの取得と、
所定継続期間の期間内に複数の血糖データポイントのそれぞれに基づいた複数のリスク範囲の演算と、
演算された前記複数のリスク範囲に基づく、少なくとも一つの平均所定継続期間血糖リスク範囲の演算と、
を実行するコンピュータ・プログラム。 - 所定継続期間は、前記少なくとも一つの平均所定継続期間血糖リスク範囲が平均日常血糖リスク範囲を提供する日である請求項93に記載のコンピュータ・プログラム。
