Description
FF EGM信号からR波を検知すること:方法1
図8は、本発明の一実施形態による、遠方界電位図からR波を検知するためのアルゴリズムを示す流れ図である。遠方界電位図からR波を検出する際の第1のステップ801は、遠方界電位図信号を入手し、その信号を、デジタルサンプル点のストリームを含む、デジタル形式でサンプリングされた波形に変換することである。これは、ICDによって、又は、たとえば、外部プログラマのような外部処理手段によって実行されることがある。次のステップ802は、遠方界電位図信号の一次導関数を求めることである。たとえば、一次導関数は、所与のFF EGMデジタルサンプルの振幅を入手し、その振幅から、直前のサンプルの振幅を減算し、さらに、その結果をサンプル間の時間間隔で割ることによって求めることができる(結果をサンプル間の時間間隔で割るステップは、連続するサンプル間の時間間隔が通常一定であるので、必要でないこともある)。図8(a)は、FF EGM信号に対応する一次導関数信号401と、本発明の一実施形態に従って後続するステップとをグラフで示す。

本発明の代替の実施形態では、遠方界EGMからR波を検知する方法は、図9に示されるステップに従って実行されることができる。最初のステップ901は再び、FF EGM信号を入手すると共に、その信号を、デジタルサンプル点のストリームを含むデジタル波形に変換することを含む。
オーバーセンシングのタイプを分類する方法
図10は、FF EGMから導出される検知R波を用いて、マーカーチャネルによって示されるような、近接界EGMによる潜在的なオーバーセンシングを解析するための方法を記述する流れ図である。その方法の最初のステップ1001は、FF EGM信号からの検知R波間(ステップ1001)のR−R間隔(すなわち、VS−FF事象間隔)を測定することである(ステップ1002)。VS−FF事象間の間隔のいずれかが、或る所定の判定基準未満(たとえば、約330〜400ミリ秒未満)である場合には(ステップ1003)、心室頻拍が生じているものと見なされ、それ以上のオーバーセンシングの解析は実行されない(ステップ1004)。
Claims (3)
- IMD(埋め込み可能医療デバイス)によって実行される、心腔の内因性脱分極の発生を検出する方法であって、
デジタルサンプル点のストリームを含む、遠方界電位図(FF EGM)信号を入手すること、
前記デジタルサンプル点のストリームのサブセットに振幅変化判定基準を適用することによって、内因性脱分極の可能性がある事象を特定することであって、該サブセットは、
前記ストリーム内の少なくとも3つのデジタルサンプル点を含むこと、
前記特定された内因性脱分極の可能性がある事象から、振幅しきい値判定基準未満の振幅を有する、前記内因性脱分極の可能性がある事象、及び、前の内因性脱分極の直後のブランキング間隔期間内に発生する、前記内因性脱分極の可能性がある事象を外すことによって、第1の、更新された、特定された内因性脱分極の可能性がある事象を生成すること、
前記第1の、更新された、特定された内因性脱分極の可能性がある事象を、前記遠方界電位図(FFEGM)信号からの検知事象としてマークすること
を含み、
前記振幅変化判定基準を適用することは、
サブセットを特定することであって、該サブセットは、
a)前記サブセット内の第1の点から第2の点まで生じる振幅の増加、及び該サブセット内の該第2の点から第3の点まで生じる振幅の増加であって、いずれの増加とも第1の指定された量以上であること、
b)前記第1の点から前記第2の点まで生じる振幅の減少、及び該第2の点から前記第3の点まで生じる振幅の減少であって、いずれの減少とも第2の指定された量以上であること、
c)前記第1の点から前記第3の点まで生じ、第3の指定された量よりも大きな絶対値を有する振幅の変化と
のうちの1つを含む、サブセットを特定すること、
前記特定されたサブセットの前記第1の点を、前記内因性脱分極の可能性がある事象としてマークすること、
最も大きな絶対値を有する、前記内因性脱分極の可能性がある事象それぞれの指定された時間間隔内にあるデジタルサンプル点を、内因性脱分極の可能性があるピーク事象として特定すること、及び
前記デジタルサンプル点のストリーム内の全てのサブセットを解析すること
を含む、
心腔の内因性脱分極の発生を検出する方法。 - IMD(埋め込み可能医療デバイス)によって実行される、心腔の内因性脱分極の発生を検出する方法であって、
デジタルサンプル点のストリームを含む遠方界電位図(FF EGM)信号を入手すること、
前記遠方界電位図(FF EGM)信号から導関数信号を生成すること、
前記遠方界電位図(FF EGM)信号内のピークを指示する、前記導関数信号内のパターンを特定すること、
最小勾配判定基準を満たさない大きさを有する、前記導関数信号内の前記特定されたパターンを、さらに解析する対象から外すこと、
前記導関数信号のゼロクロス(極性変化点)を特定することによって、前記導関数信号の残りの前記特定されたパターン内の極性変化点の近くにおいて発生するような、内因性脱分極の可能性がある事象を特定すること、
振幅しきい値判定基準未満の対応する遠方界電位図(FF EGM)振幅値を有する、
前記内因性脱分極の可能性がある事象を、該内因性脱分極の可能性がある事象としてさらに検討する対象から外すこと、
前の内因性脱分極直後のブランキング期間内に生じる、前記内因性脱分極の可能性がある事象を、内因性脱分極の可能性がある事象としてさらに検討する対象から外すこと、及び
残りの前記内因性脱分極の可能性がある事象を、前記遠方界電位図(FF EGM)信号からの検知事象としてマークすること
を含む、心腔の内因性脱分極の発生を検出する方法。 - 複数の電極を含む、少なくとも1つの埋め込み可能リード線と、
前記少なくとも1つの埋め込み可能リード線に結合された埋め込み可能医療デバイスと、
を備え、
前記埋め込み可能医療デバイスが、
近接界電位図信号、及び、遠方界電位図信号を記憶するように構成されたメモリと、
近接界電位図を得、遠方界電位図を得、前記遠方界電位図信号における各心臓事象を、前記近接界電位図信号における、対応する心臓事象にマッピングし、前記遠方界電位図信号における、対応する心臓事象にマップされない、前記近接界電位図信号における第1の所定の数の心臓事象が、前記遠方界電位図信号における、対応する心臓事象にマップされた前記近接界電位図信号における第2の所定の数の心臓事象の内に発生する場合、リード線の破損を検出する、ように構成されたマイクロプロセッサと、
を含む、
埋め込み可能医療システム。
